26.07.14. On further investigation of site – In rainy atmosphere the land was

Wire LT Line pole for Residence purpose – G.I. earth wire of pole was broken

and stick with stay wire - Buffalow comes in contact with said stay wire and got

ckts. in different directions. Due to the cyclone 3nos of LT poles were broken

from one of the ckt.,due to Nim tree fallen on the said line & LT cond. Were

also broken & lying on the ground nr. the house of shri Pankajbhai K. Prajapati

where the victimise she buffalo was tied up in front of the house.The power

supply from the said TC was disconnected by removing the D.O. fuse from

X'mer center by line staff persons & feeder was also under the fault since the

cyclone.On dtd:12.5.14 after attending the PF, the feeder was charged & power

supply has been restored.It has been came to known that the DO fuses of said

x'mer,which were removed earlier have been re installed by some unknown

person,due to that , on charging the feeder the broken cond. lying on the ground

were also became live & at the same time the victimise she buffalo which was

tied near the broken LT line cond. came into contact & got electic shock and

died.

The site for Non-fatal Human accident of Shri Kharadi Laxmanbhai Sardarbhai

At ,Village : Limb ,Ta:- Bayad was visited on Dt.:-22.04.14. By D.E. Talod-2

Subdivision. On Inspection of Site Mr. L.S.Kharadi who was doing the jumpering work

for release of new Ag connection on 11 kv Limb Ag Feeder for which Line clear was

already taken by Mr. L.S.Kharadi (E.A) through telephonic msg in Aambaliyara

substation by J.E. Talod-2 Subdivision. While the work of tapping jumper of patel

chandubhai ranchodbhai on existing t/c structure one by one phase jumper was given by

providing separate phase earthing and after completion of jumpering work he had

removed the separate earthings. Suddenly he saw some part of excess conductor was on

the middle phase of a line and while process of removal of excess part of conductor due

to Electric spark he met with an Electrical accident and fallen down. Meanwhile no

tripping was recorded in substation from crossing of any JGY and AG feeders and

crossing of HT-HT, HT-LT and LT-LT is within limit.

Due to fan short of nearer house of shri thakor ajmalji shankarji consumer

no.22522/10011/4 & fan is fitted on metal pipe with senting wire so current pass

throuh senting wire to pipe & to shed of metal above it & pass through metal

shed of house of thakor isaben narayanji touching with nearer house metal

shed. A buffalo was resided below the metal shed and hence current was

flowing through this metal shed to the supporting angle of this metal shed. When

thakor ishaben narayanji removed buffalos from chain buffalos got in contact

with supporting angle & got electrocuted and died at the site. Also shock

18:20 and since then feeder was kept off till 19.00 hour due to cyclone but after

cyclone is over feeder was make ON from sub station and though two conductor

was broken and fallen on roadside, so one 407 truck which was passing nearer

to these are came in contact with the 11 KV line conductor. Also three victims

were coming on one bike behind the truck, they some how came in contact with

the truck or live conductor and got electrocuted.

pole& due to shortage of binding wire third jumper is not given by Shri K.A.Patel

. At that time Shri K.A. Patel got Electric shock by returned power on tapping

pole & he lost his balance & he fell down on the ground .In Kansa NA area

Sharthi Duplex is located & building work is under construction & a lighting

temporary connection is given to Sharthi Duplex on named Shri Vishalkumar

Bharatbhai Patel from 11 kv Rajgadh feeder .On north side of Sharthi Duplex

Kansa AG area is located& opposite direction of Sharthi Duplex a Panchamrut

society is located & a temporary connection is given in Panchamrut society by

Visnagar Town office on 11 kv City -3 feeder emanating from 132 kv Visnagar

s/s named Patel Bharatbhai Bhikhabhai.

In Sharthi Duplex party had done the underground wiring from Panchamrut

society by unauthorised flexible wire to get the 24 hrs power from Panchamrut

society. He had done the illegal connection by flexible wire from Panchamrut

society to Sharthi Duplex. On location of Sharthi Duplex two nos MCCB provided

to get the illegal power& at the time of accident someone had start the illegal

power from 2nd source of power of City -3 feeder & Shri K.A.Patel got the

returned power of 11 kv City-3 feeder& got electric shock. He is injured , on

taken up on hand by this office. Contaractor Shri. H B Patel had already erected

two Nos. of RSJ across the canal but due to recent heavy rain the work could

not completed due to water logging near RSJ Girders.Mean while on

date:20/07/2014 all of sudden victim had came in contact with 11 KV Khasta

feeder and got electroculated by induction of line. As per our observation there

is no regular way of tresspassers and it is not clear that why victim had climbed

up on the dump.

However it is to report that the victim got electoculated by induction of 11 KV

line.On investigation it was found that there was no any earth fault (T.T) on the

feeder at the time of accident. . For the sake of safety the line is shifted on newly

erected RSJ Girders on date:22/07/2014 to prevent any such types of accidents

in future by departmental staff.

attending switch complain & LT Maintanence work of Navanagar AG fdr. That

fdr LC was already taken by P.F.Chavada ALM at 10.:50 AM. Earthing of 11 KV

line was already done on near by AG TC of this fdr from S\S side. Victim was

climbed on AB Switch structure to check & repair jumpers and then after Victim

climbed on road side pole to repaird third phase jumper. At that time while victim

touched the AB Switch part suddenly he fell down to the ground & after few

minutes he died. Sytoms of burning found in hands & legs which shows electrical

current might be passed through his body. The detailed reason for electrical

current was found on dtd.4.3.15 by deputing team of engineers & line staff and it

is concluded that victim was died due to electrical current coming from

consumer doing theft of electricity or changing over the supply

of outgoint wire was break hence the flexible wire opened . The opened flexible

wire came in contact with iron pipe which is used as support for metal roof of

informed on dtd.05/10/2014 by Shri R.J.Patel, SE(T),R&C Office, UGVCL that

the fatal human outsider accident has occurred to his nephew named Sarojben

accident location was inspected immediately with him and line staff @ 3:30 pm.

It was found out that an electric connection has made for supplying table fan with

14 mtr long flexible 1/18 swg yellow coloured wire having no pin plug on one end

& other end faulty PVC two pin plug having male contact provided. An open

ended wire was directly connected with the plug point of the switch board. It may

be assumed that the victim has tried to connect table fan’s two pin plug having

female contact with the male pin plug of the extended flexible 1/18 swg yellow

coloured wire. While making connection one terminal of pin plug came in contact

with the palm of the victim and she got electrocuted.

incidence by realative of the victim nor the police station. This incidence has

location no. 62 near by village Rajpur(Navavas) due to some unknown reason.

There was no any tripping noted on 11 KV Davol Ag feeder of 66KV Satlasana

Sub-Station. We have visited the site on dated 21.01.2015 & we have not

noticed any evidence related to this type of accident either on pole or on

conductor. We have recieved the PM Report and inquest panchnama on dated

30.01.2015 from satlasana police station.

While carrying out restringing work on 11 KV Radhu Ag fdr,due to line crossing

of 11 KV Pansoli Ind Fdr.cond touched to this fdr,line staff working on different

poles at that time elecrocuted.Shri S K Prajapati VS EA died & other three were

rescued to the hospital.Earthing was provided on one side but not on the other

side of the Ag fdr. Hence the accident took place ,clearancebetweenside wire of

11 kv Pansholi fdr & top wire of 11 KV Radhu ag feeder measured was 0.7 mtr.

erected from 11 KV Bholad JGY feeder.

On date 22.11.14, Shri J G Rathava LI, Shri B.S Rathod LM & Shri M L Rathod

EA, were deputed to release above connection.

Shri B.S Rathod LM has taken Line clear of 11 KV Bholad JGY feeder through

his mobile from Koth S/S. But while charging the line Shri M .L Rathod got

electric shock.

On investigation it is found that on duty operator Shri S M vora has by mistake

switched off 11 KV Koth JGY Feeder instead of 11 KV Bholad feeder, hence

this accident took place.
