taken. Loc between 34&35 one 11 kv Goral feeder was crossing. Clearance

between these two feeders was about half feet, whily carrying out tree cutting tree

branch fallen on Akshar JGY feeder due to that conductor jumped and touch with

Goral AG feeder. Hence, return power came momentarily on Akshar JGY feeder.

cable is laid down from resident to cattke shed over head for ltg. purpose the

phage wire's inulation is damage where it touch with cattle shed iron roof sheet.

the wooden stick fallen on the ten wire fencing of the farm. the red phase touches

the top most wire of fencing and current is distributed through whole fencing area.

A single phase two wire LT line was there on 10 mtr PSC Pole just near the

Kharna-Amarpura bus stand over the road crossing of Gozariya-Gandhinagar

Highway. Also 11 KV Boru Ag Dom feeder emanating from 66 KV boru s/s was

crossing the single phase two wire LT line. On dated 19/6/12, a programme was

given to remove the 8 mtr nacked psc pole just near the 10 mtr psc pole at Kharna-

Amarpura bus stand. On the said 10 mtr psc pole two service wire was connected.

To remove the nacked pole, the service wire was removed. After removing nacked

pole victim shri A K Asari climbed on 10 mtr psc pole to reconeect the service wire.

The ground staff had binded the single phase service with 2 core service wire and

gave to Shri A.K.Asari . He had hold the service and tried to bind with 10 meter

PSC pole. During Binding work jerk took place and GI wire of single phase service

touched to conductor of 11 KV Boru AG feeder and victim felt down and met with

non-fatal electrical accident.

According to the details, the accident took place on Dt. 11/7/12 at 11:30 AM

approx. The Victim Shri. H.S.Rathod went to Shri.Patel Jayntibhai Ambaram's

house at Soneripura for attending No Power Complaint. He checked the

connection and found no supply at the out going terminals of the meter. Revealing

the service might be faulty, he climbed on the roof of house. He checked the

service line and found it broken somewhere under the insulation. He detected the

exact place of breakage and cut the cable skillfully, to prepare for the new joint. He

started applying joint and made a first joint at the neutral. While providing Lt tap on

the joint to the phase - which was live at the time,- suddenly and accidentally, he

touched the live part of the phase wire. As he was standing on the iron sheet roof,

he received electrical shock and fell down there only, shouting for the help.

On dated 13.06.12 at about 11:50 PM, there was a heavy rain and this open joint

touches to the tube light patti and so it became live. Also the bottom surface of the

pan parlour was become wet due to heavy rain and suddenly victim's head came

in contact with this live patti and so victim got electric shock and died.

The site is on Ajol-Lodra Road –Right-hand side. The scheme is under

construction. He has made plotting & houses are being constructed. He has

developed common Amenities like internal road, garden, st. light, C. C .TV

Camera etc. The victim was doing the work of painting on Tabular poles which are

erected for C C.TV Camera on the side of main internal road. The victim was doing

this work using sliding ladder. To get proper height he had tied four bamboos on

four legs of ladder and put this structure on the tractor trolley. The victim has

climbed up on trolley and holds the structure. While driver was driving the tractor.

One 11 KV Veda Ag. Feeder passes through these primacies and cross the

internal road. As soon as, the tractor along with trolly passed under our electric

line, the top portion of structure touched the 11 KV line & victim meet with fatal

electrical accident. Here it is to inform that total height of the moving structured

with tractor was measured about 22 ft. Distance between lower condition of 11 KV

line & RCC road is 19 ft 4 inch. The RCC road is newly constructed with refilling of

earth about 1 ft from original ground level.

Ramsibhai Raval in the premises of Shri Raval Rameshbhai Vithalbhai. As per the

Raval Rameshbhai Vithalbhai statement the accident occured while fixing the bulb

on holder & died. While site inspection no any leakage of current or any sign of

electrical accident has been noted. The Dy Engr has visited the doctor of PHC

Lanva who has carried out the post mortom The doctor has personnelly told that

he is not be able to say any thing about perfect cause of death hence he has send

some sample to FSL for further investigation for detail report of cause of accident.

On Dated 04.05.2012, Heavy wind flow passed through Dhandhuka at 6.00 pm.

Due to it 11 kv feeders were tripped. Among these feeders, 11 kv zanzarka feeder

was also tripped. Shri R.S.Ganderi (LI), Shri R.K.Ninama (v/s Helper),Shri

H.L.Pandor(v/s Helper),Shri P.S.Parmar(v/s Helper), went to attend fault on 11 kv

zanzarka feeder. First they cut the 1st AB switch on Limbadi road and charged

zanzarka feeder from S/S and after testing at AB switch they found one phase was

not coming.So, they were making patrolling from first AB swith to towards jaliya

S/S.While petrolling they found that one zamper was burnt on 42' gurder pole,

which is shackle pole in the compound of D.A.ENGLISH medium highschool near

S/S. Also two nos of feeders are passing on these shackle pole. The one burnt

jumpure found on upper side feeder of shackle pole, which was 11kv Zanzarka

feeder. All of them discussed about taking line clear of two feeders to repaire burnt

jumper. Shri R.S.Ganderi took line clears of 11 kv Zanzarka feeder & 11 kv Rojka

feeder on mobile phone. Actually, upper feeder is 11 kv zanzarka feeder and lower

is 11 kv zankhi feeder of 66 kv jaliya S/S. Shri P.S.parmar climed on shackle pole

for repairing of jumper & falled down. immediately sent him to RMS hospital and

then shifted to CIMS ahmedabad.

On dated 11.06.12 ,a dumper truck GJ-1 BV 1924 came with sand filled trolly at

the 37/1,Ashawmegh ind.Estate in the premises of HT connection Sun Electro

Grovers,HT 18504 to unload the sand as the construction work inside the factory is

going on at 10.00 AM.After unloading the sand using hydraulic system,the driver

tried to drive his truck in forward direction towards gate keeping the hydraulic

system rod in upholding position so the upper part of the trolly trepped in one live

conductor of 11KV Satluj feeder.Then driver of the truck jumped from the truck

while cleaner of the truck tried to come down by keeping one foot on ground & one

hand with handle of cabin of the truck.The electric current passed from conductor

to truck body to the victim's body and to ground.At the same time the

cleaner(victim) got electrocuted and at the same time feeder was tripped at 10:10

AM.Victim died at 10:55 after hospitalization during treatment.

and due to leakage current on MSP(metal box) the victim got electrocuted and

died.

conductor at this location having very low hight from ground and the driver

(Pravinbhai G Bharvad) has seen the conductor but anyhow the vehicle came in

contact with live conductor and instantly the vehicle stopped & driver could not

restart the vehicle. So victim & other 4 person tried to came down from the vehicle

but victim could not balance his body while came down & his left hand touched the

dala (rear part of vehicle) & fallen on the ground & got electrocuted. One of them

named Jigneshbhai R Tadvi tried to save the victim but he himself got electrocuted

but found ok later. The other 4 persons came down safely.

100 kva transformer center & 3 phase Power supply schedule is 23.15 to 9.15. AG premises (ordi) is

about 8to9 meter away from transformer center . On 1-10-2012 in morning at about 8.00 hrs Shri

Mahendrabhai Manilal Patel bore operator is to start the motor by starter for doing piyat in AG land.

By site inspection & investigation No body was present in AG premises (ordi) at the time of incident

but looking to the site Shri Mahendrabhai Manibhai Patel came in AG primises( Ordi) for reparing or

to start the starter. Window cover of starter is found open & black spots found on below side of starter

window cover it may be Shri Mahendrabhai Manibhai came in contact of Live wire & he got electric

shock& he fell down on the ground& thus Fatal electrical accident occured.By site verification incident

victim had holded on end of the freshly cut neem tree branch and the other leafy end of the branch

As per the statement of victim's father, his son came from the school on dtd. 26/09/2012 about 15:00

hrs. and touched the switch of light and he fell down on the floor and died. During investigation, an

open switch board was found in victim's house. Also there was no any leakage current and any sign of

burning spot on switch board or on wall of the house. So it was the suspicious case for electrical

accident and we can only conclude the cause of the death after recieving the PM report. On reciept of

the PM report on dtd. 10/10/2012, it was concluded that the death has occured due to cardiac arrest

due to electric shock.

to earth. Victim tried to reinstall the water pipe connection, and due to leakage current, he got shock.

Then victim was transferred to Hospital where he got died. Morever, ON-OFF switch of switch board

to which the plug of motor was connected was not working properly, means even in OFF position,

phase was ON.

Bhikhaji Thakor conn no 22612/00342/8 given by 2 core 2.5mm2 yellow colour pvc cable . A house of

Shri Somaji Bhikhaji Thakor & a house of Shri Motiji Madhaji Thakor is located near to each other .

There is no Lighting connection in Motiji Madhaji Thakor's house. A illegal connection was taken from

Somaji BhikhajiThakor's house by flexible wire approximate 10 mtr. 12 watt lamp was connected. The

flexible wire was jointed& broken & comes from the door in his house. It is asummed that Shri Motiji

Madhaji open the door & by mistake he touch the broken flexible wire & he got the electric shock & fell

down on ground .There is no any Eye wittness at the time of accident .Near by people Shri Jehaji

Dhulaji Thakor might have seen the accident & they made a notice of it then theycame & call 108 &

brought him at Vadnagar Civil hospital & Doctor has declared he is died & sends the dead body for

Post mortem. During visiting the place of accident panchnama done. For An illlegal connection given

by Somaji Bhikaji Thakor a pannelty bill isssued to him.We have checked the UGVCL installation with

line tester that there is no any leakage current on MMB, or on wall at consumer premises. There is no

any spot of burns at consumer premises or on door

smt. Jyotsnaben gopalbhai prajapati ( outsider) aged approx. 40 years,at 27 of k.b. complex society

b/h vishwas hotel, sidhpur highway was carrying approx. 22 feet long iron box pipe for some work.

While she was lifting/carrying the iron box pipe near compound wall of height approx. Five feet of her

residence, due to approx 22 feet length of that pipe, one side end of the iron box pipe was out of the

wall whose top side touched to 11kv ankvi ag. Feeder, which is passing out side the compound wall

approx. Five feet away from 11kv line and ground clearance of that 11kv line is approx. 19 feet.

Electric current passed through the iron box pipe & she got electoculated. Her husband shri

gopalbhai kantilal prajapati (outsider) aged about 42 years old rushed to save her & he also got

electoculated. Thus couple got electoculated & died on the spot. It is found that there was tripping on

11kv ag. Ankvi feeder at 8:42 a.m. & spark spots were found on the top end of the iron box pipe.
