BACKGROUND

The Nat ional I n s t i t u t e of Allergy and I n f e c t i o u s Diseases ( N I A I D ) announces t h e a v a i l a b i l i t y of a Request f o r Applicat ions (RFA) f o r AIDS C l i n i c a l T r i a l s Un i t s (ACTUS). The purpose of t h i s RFA is t o recompete t h e a d u l t component of t h e AIDS C l i n i c a l T r i a l Group (ACTG) by s o l i c i t i n g a p p l i c a t i o n s from e x i s t i n gACTUs and from new a p p l i c a n t s . Th i s i n i t i a t i v e w i l l u t i l i z e t h e coope ra t ive agreement. The a d u l t component of t h e ACTG is a network of 32 domestic biomedical r e s e a r c h i n s t i t u t i o n s t h a t , i n aggregate , has t h e c a p a b i l i t i e s t o develop new t h e r a p e u t i c i n t e r v e n t i o n s from i n i t i a l c l i n i c a l t r i a l s i n human s u b j e c t s t o t h e i r f i n a l approval by t h e Food and Drug Adminis t ra t ion. The r e sea rch o b j e c t i v e s of t h e ACTG are t o e v a l u a t e t h e s a f e t y and e f f i c a c y of t h e r a p e u t i c i n t e r v e n t i o n s f o r t h e t r ea tmen t of Human Immunodeficiency Virus ( H I V ) i n f e c t i o n , acquired immunodeficiency syndrome (AIDS), and a s s o c i a t e d o p p o r t u n i s t i c c o n d i t i o n s .

This RFA, Recompetition of t h e Adult AIDS C l i n i c a l T r i a l s Un i t s , i s r e l a t e d t o t h e p r i o r i t y area of H I V i n f e c t i o n . OBJECTIVES

It is t h e i n t e n t of t h e recompeti t ion t o maintain a cooperat ive network of a d u l t ACTUs t h a t has t h e c a p a b i l i t y t o conduct a l l phases of t h e r a p e u t i cc l i n i c a l eva lua t ion t o i d e n t i f y new t h e r a p i e s and t o f a c i l i t a t e t h e t r a n s f e r of i n t e r v e n t i o n s from t h e r e sea rch s e t t i n g t o r o u t i n e p a t i e n t care. Emphasisw i l l be placed on t h e i n c l u s i o n of women, m i n o r i t i e s and substance abuse r s i n t h e c l i n i c a l s t u d i e s so t h a t t h e p a t i e n t s p a r t i c i p a t i n g i n t h e c l i n i c a l t r i a l s network r e f l e c t t h e demographic f e a t u r e s of t h e n a t i o n a l H I V epidemic. This network w i l l be composed of i n v e s t i g a t o r s from a v a r i e t y of d i s c i p l i n e s with demonstrated e x p e r t i s e and experience i n conducting c l i n i c a l t r i a l s e s p e c i a l l y among H I V i n f e c t e d p a t i e n t s . The r e s e a r c h o b j e c t i v e s w i l l be accomplishedthrough t h e development of p r o t o c o l s , a c c r u a l of p a t i e n t s t o ACTG p r o t o c o l s ,

submission, a n a l y s i s and p u b l i c a t i o n of d a t a , and by p a r t i c i p a t i o n of t h e ACTU i n v e s t i g a t o r s i n ACTG committees. The r e sea rch agenda and p r i o r i t i e s w i l l be e s t a b l i s h e d by t h e ACTG Executive Committee whose members include t h e Associate D i rec to r of t h e Treatment Research Program, DAIDS. S c i e n t i f i c areas of h ighes t p r i o r i t y inc lude a n t i r e t r o v i r a l therapy and t r ea tmen t of o p p o r t u n i s t i c i n f e c t i o n s a s s o c i a t e d with H I V i n f e c t i o n . S t u d i e s of HIV-associated malignancies and neuro log ica l complicat ions w i l l a l s o be emphasized. The ACTG committees ( S c i e n t i f i c , Resource, and Executive) are composed of i n v e s t i g a t o r s from t h e ACTUs, a N I A I D s t a f f r e p r e s e n t a t i v e , and r e p r e s e n t a t i v e s from p a t i e n t advocacy groups. I n o rde r t o achieve t h e s e o b j e c t i v e s , supplementary r e sources w i l l cont inue t o be provided t o t h e ACTU P r i n c i p a l I n v e s t i g a t o r s by t h e N I A I D through o t h e r funding mechanisms and include: ( 1 ) a S t a t i s t i c a l and Data Analysis Center t h a t assists i n p ro toco ldesign and func t ions as a c e n t r a l d a t a management c e n t e r , ( 2 ) an OperationsOf f i ce t h a t provides t e c h n i c a l and l o g i s t i c a l support f o r t h e development and implementation of p r o t o c o l s , (3) C l i n i c a l S i t e Monitoring t o ensure t h e q u a l i t y of d a t a and conformance with r e g u l a t o r y requirements, and (4) a C l i n i c a l Research Products Repository t h a t provides f o r t h e r e c e i p t ,inventory, and d i s t r i b u t i o n of i n v e s t i g a t i o n a l drugs.

The RFA con ta ins information r e l a t e d to both r equ i r ed and o p t i o n a l components.A l l a p p l i c a n t s must apply f o r t h e ACTU c l i n i c a l co re funding t o be e l i g i b l e t o apply f o r o p t i o n a l components. The c l i n i c a l co re funding w i l l provide t h e r e sources r e q u i r e d t o maintain an i n f r a s t r u c t u r e s u f f i c i e n t t o accrue an agreed upon minimum annual number of new p a t i e n t s on t o p r o t o c o l s . The c l i n i c a l co re funds a l s o w i l l support l a b o r a t o r y c o s t s f o r p ro toco l mandated immunophenotyping. Applicants f o r t h e ACTU c l i n i c a l co re funding w i l l be e l i g i b l e t o apply f o r o p t i o n a l components t h a t include: ( 1 ) p e d i a t r i cc l i n i c a l t r i a l s [ l i m i t e d t o a d u l t ACTUs t h a t have an e x i s t i n g p e d i a t r i ccomponent], ( 2 ) v i ro logy co re l a b o r a t o r y support f o r p ro toco l mandated t e s t i n g , (3) pharmacology co re l a b o r a t o r y support f o r p ro toco l mandated t e s t i n g , (4) developmental r e sea rch i n v i ro logy [ l i m i t e d t o a p p l i c a n t s who apply f o r and are awarded funds f o r (211, ( 5 ) developmental r e sea rch i n pharmacology [ l i m i t e d t o a p p l i c a n t s who apply f o r and are awarded funds f o r (311, ( 6 ) developmental r e sea rch i n immunology, and ( 7 ) developmental r e sea rch i n o t h e r areas of microbiology. An a p p l i c a n t must r e c e i v e an award f o r t h e ACTU c l i n i c a l co re t o be e l i g i b l e f o r funding of t h e o p t i o n a l components.However, funding p r i o r i t i e s f o r t h e o p t i o n a l components w i l l be e s t a b l i s h e d independent of t h e p r i o r i t i e s f o r t h e ACTU c l i n i c a l co re awards. Fundingp r i o r i t i e s f o r t h e ACTU c l i n i c a l co re w i l l no t be a f f e c t e d by t h e presence o r absence of o p t i o n a l components.

SPECIAL INSTRUCTIONS FOR THE INCLUSION OF WOMEN AND MINORITIES I N CLINICAL RESEARCH STUDIES

The fol lowing is a b r i e f s ta tement of t h e N I H and ADAMHA p o l i c y r ega rd ing t h e i n c l u s i o n of women and m i n o r i t i e s i n s tudy popu la t ions . The i n c l u s i o n of women and m i n o r i t i e s should be addressed i n a p p l i c a t i o n s submitted i n response t o t h i s RFA.

For p r o j e c t s involving c l i n i c a l r e s e a r c h , N I H r e q u i r e s a p p l i c a n t s t o g ives p e c i a l a t t e n t i o n t o t h e i n c l u s i o n of women and m i n o r i t i e s i n s tudypopulat ions. I f women o r m i n o r i t i e s are no t included o r adequatelyrep resen ted i n t h e s tudy populat ions f o r c l in i ca l s t u d i e s , a s p e c i f i cj u s t i f i c a t i o n must be provided. Applicat ion without such documentation w i l l no t be accepted f o r review.

MECHANISM OF SUPPORT

Awards w i l l be made as cooperat ive agreements ( U O I ) . The coope ra t ive agreement funding instrument d i f f e r s from t h e t r a d i t i o n a l r e sea rch g r a n t i n t h a t t h e Government awarding component ( N I A I D ) a n t i c i p a t e s s u b s t a n t i a l programmatic involvement du r ing t h e performance. The n a t u r e of N I A I D s t a f f a s s i s t a n c e is desc r ibed i n t h e RFA. However, a p p l i c a n t s must d e f i n e t h e i r own o b j e c t i v e s i n accord with i n d i v i d u a l i n t e r e s t s and approaches t o conductingt h e r e sea rch . The N I A I D a n t i c i p a t e s t h a t $48,000,000 w i l l be a v a i l a b l e f o r t h e i n i t i a l year of funding a p p l i c a t i o n s i n response t o t h i s RFA and t h a t approximately 20-25 a p p l i c a t i o n s w i l l be funded. The award of g r a n t s pursuant t o t h i s RFA is cont ingent on t h e cont inuing a v a i l a b i l i t y of funds f o r t h i s purpose and on t h e r e c e i p t of a s u f f i c i e n t number of a p p l i c a t i o n s of highs c i e n t i f i c m e r i t .

The Alcohol, Drug Abuse, and Mental Health Adminis t ra t ion (ADAMHA) is announcing t h e t h i r d year of t h e ADAMHA S m a l l Ins t rumentat ion Program (ASIP).Th i s program was reauthor ized by Congress f o r FY 1991 i n Sec t ion 501(m) of t h e Publ ic Health Serv ice Act, as added t o by P.L. 100-690, i n response to f i n d i n g s t h a t much of t h e r e sea rch ins t rumenta t ion i n t h e Nation's p r i n c i p a lu n i v e r s i t i e s is e i t h e r obso le t e o r poorly maintained. These f ind ings ,documented i n s e v e r a l r e p o r t s , i d e n t i f i e d t h e need f o r upgrading equipmentc u r r e n t l y i n use . The most s i g n i f i c a n t need w a s f o r r e l a t i v e l y low-cost p i eces of equipment. To address t h i s problem, ADAMHA e s t a b l i s h e d t h e Small Ins t rumenta t ion Program i n FY 1989. Awards are made under a u t h o r i t y of T i t l e s I11 and of t h e PHS A c t as amended. Funds w i l l be provided t o r e sea rch - in t ens ive i n s t i t u t i o n s c u r r e n t l y r ece iv ing ADAMHA r e sea rch suppor t .The ASIP is no t intended t o r ep lace r e q u e s t s f o r equipment i n a p p l i c a t i o n s f o r i nd iv idua l r e sea rch p r o j e c t s . Rather , it is intended t o he lp fund items of equipment t h a t are d i f f i c u l t t o j u s t i f y wi th in t h e contex t of an ind iv idua l r e sea rch p r o j e c t but t h a t w i l l upgrade t h e i n s t i t u t i o n ' s r e sea rch i n f r a s t r u c t u r e .

The ADAMHA program has a similar purpose t o t h e Nat ional I n s t i t u t e s of Health Small Ins t rumenta t ion Program but w i l l ope ra t e s e p a r a t e l y and under s l i g h t l yd i f f e r e n t gu ide l ines because of d i f f e r e n c e s i n t h e i n f r a s t r u c t u r e supportmechanisms a v a i l a b l e t o t h e two agencies .

The ADAMHA program w i l l be funded i n FY 1991 a t $2,402,000. The programd provides awards ranging from $20,000 t o $60,000 t o e l i g i b l e i n s t i t u t i o n s .

E l i g i b l e i n s t i t u t i o n s are those t h a t had f i v e o r more a c t i v e ADAMHA r e sea rch g r a n t s awarded i n FY 1990. The awards must be from t h e fol lowing types of r e sea rch mechanisms: R01, R03, R29, and R37. The amount f o r which an

Each e l i g i b l e i n s t i t u t i o n may submit ONLY ONE a p p l i c a t i o n t h a t i nco rpora t e s a l l appropr i a t e equipment r e q u e s t s from t h a t i n s t i t u t i o n . Thus, it is e s s e n t i a l t h a t i n s t i t u t i o n a l o f f i c i a l s p u b l i c i z e t h e a v a i l a b i l i t y of ASIP funds so t h a t ADAMHA-supported i n v e s t i g a t o r s i n need of small r e sea rch instruments are provided t h e opportuni ty t o i n d i c a t e t h e i r needs f o r such equipment t o t h e appropr i a t e i n s t i t u t i o n a l o f f i c i a l .

The equipment requested must be a v a i l a b l e f o r use by more than one p r o j e c t e i t h e r c u r r e n t l y o r i n t h e f u t u r e . The primary u s e r ( s ) of t h e equipment must be one o r more P r i n c i p a l I n v e s t i g a t o r s of a c t i v e ADAMHA-supported r e s e a r c h g r a n t s , and t h e s p e c i f i c p r o j e c t s must be c i t e d i n t h e a p p l i c a t i o n . No i n d i r e c t c o s t s w i l l be provided and t h e r e w i l l be no f u t u r e yea r funding commitment. The requested funds may be f o r f u l l o r p a r t i a l support of one o r more p i e c e s of equipment I n no case, however, can t h e t o t a l purchase p r i c e of a requested p i e c e of equipment be less than $5 ,000 o r more t h a n $100,000r e g a r d l e s s of t h e sou rce ( s1 of funding. I f t h e t o t a l d o l l a r amount of proposed eguipment purchases exceeds t h e amount f o r which t h e i n s t i t u t i o n is e l i g i b l e , a s ta tement must be submitted i n d i c a t i n g t h e i n s t i t u t i o n w i l l provide t h e d i f f e r e n c e . Support from t h i s psogram cannot be used t o purchase items exceeding $100,000 i n c o s t even i f c o s t s are sha red . The equipmentpurchased must be t h e same as t h a t s p e c i f i e d i n t h e ASlP a p p l i c a t i o n .

Applicat ions must be r ece ived by June 21, 1991, and l e t t e r s of i n t e n t should be r ece ived by May 20, 1991. Deta i l ed a p p l i c a t i o n procedures have been s e n t t o e l i g i b l e i n s t i t u t i o n s . Applicat ions w i l l be pee r reviewed by a s i n g l e ADAMHA-wide committee. The review c r i t e r i a are: Degree of adherence t o t h e terms of t h e l e t t e r of e l i g i b i l i t y and adequacy of t h e j u s t i f i c a t i o n provided f o r t h e equipment r eques t ed . The reviewers w i l l determine whether t h e a p p l i c a t i o n is recommended f o r approval ; no p r i o r i t y sco res w i l l be voted. Applicat ions w i l l be assi ned t o i n d i v i d u a l ADAMHA I n s t i t u t e s f o r cons ide ra t ion by t h e i r Nat ional Advisory Counci ls and for funding. The I n s t i t u t e s expect t o make t h e awards i n September.
