Registration deadline Complete the registration spreadsheet at the end of this document and return it via email to izudin.issa@ufv.ca no later than Friday, Jan. 18, 2019. This preliminary roster list will give the tournament directors an idea of how many athletes to expect.

The final roster (no new names from preliminary roster) must be submitted to the same email addresses no later than Friday, Jan. 25 at 6 p.m.

All athletes/coaches must be registered members of their provincial association.

Rules U SPORTS rules will be in effect

Registration fees $30 per U SPORTS varsity athlete, $40 per non-varsity athlete.

Fees can be paid by cheque, made payable to UFV Athletics, and submitted on-site during team check-in. University teams can also opt to be invoiced.

Waivers and liability All non-U SPORTS/Canada West participants must arrive with payment as well as a signed waiver (included in this document). Athletes age 18 and under must have parent/guardian consent.

Medical services provided

• The host therapist/student therapist will discuss the venue’s emergency action protocol with the visiting medical staff prior to competition. • A student therapist and Certified Athletic Therapist will be on site during all scheduled Canada West conference competitions.• All Canada West schools are required to indicate whether they are bringing a student therapist to the event. Teams which do not have a therapist travelling may make arrangements for a designated student therapist through UFV. Please make arrangements at least one week prior to arrival. Teams will be billed ac-cording to the Canada West Universities Athletic Association Operations Manu-al, Section 14: Medical Support Services Guidelines. Cost is $150 for four hours, or $300 for four-plus hours. If a team brings less than four athletes they will not be charged.• The visiting team must provide their own supplies, otherwise supplies used by the host university will be charged back to the visiting school.• A doctor will be on-site for all Canada West wrestling events hosted by the University of the Fraser Valley.

Emergency equipment Emergency equipment will be provided at the venue site and the host therapist will review the location of supplies prior to the event:• Emergency telephone • AED• Oxygen• Coolers with ice and ice bags• Crutches• Splints• Water

Athletic therapy clinic • The athletic therapy clinic is available for the use of visiting teams upon arrival to the athletic facility. This is located in the Envision Athletic Centre between the South and North Gyms (room E16). • Upon arrival, the host therapist will direct the visiting team medical personnel to the appropriate location.

Physician services • Where non-emergent physician assistance is requested by a visiting team, the head therapist will facilitate contact between the host and visiting team physi-cians as early as possible. • After-hours walk-in clinic: Bakerview Medical Clinic - 604-853-9853 14-33324 South Fraser Way Abbotsford, B.C., V2S 2B4 Hours: Mon-Fri, 5:30 p.m. to 9 p.m.; Sat, Sun and holidays, 9 a.m. to 9 p.m.

Therapy services • Limited therapy access for visiting athletes. An appropriate referral outlining the injury description and treatment plan are required for service. By appoint-ment only. • Usage of the clinic will be granted to Certified Athletic Therapists or Licensed Physiotherapists based on clinic availability.

Hospital Should emergency transport to the Abbotsford Regional Hospital (32900 Mar-shall Road, Abbotsford, B.C., 604-851-4700) be required, the University of the Fraser Valley will not be responsible for any charges associated with emergency transportation of visiting athletes.
