UNIVERSITY OF PERPETUAL HELP SYSTEM, DALTA Doctor/Brigadier General Antonio Laperal Tamayo, AFP, FPCHA, Ph. D., and his wife, Daisy Moran Tamayo, RN, MAN, Ph. D., who shared the same passion for education and healthcare, founded the University Of Perpetual Help System DALTA, on February 5, 1975. The founding of the university is marked by a remarkable history of family ancestry and legacy, vision, and the abiding faith of the founders in the decisive role of education and healthcare in raising the quality of life of the people, and bringing about substantive social, economic, spiritual, and political transformation. The term “DALTA” in the name of the university is the acronym for Daisy Antonio Laperal Tamayo, the conjugal founders. This was followed by the founding of the third Campus at Perpetual Help School of Laguna, at Biñan, in l976, and its medical school, the Perpetual Help School of Laguna Foundation. GENERAL ANTONIO L TAMAYO’S TRAINING IN CORPORATE GOVERNANCE

On February 5, 1975, just seven years after his parents opened the Perpetual Help College and Hospital in Manila,

In 1976, one year after opening of the PHCR, they established the Perpetual Help Medical Center (PHMC) about 200

meters from the University. Through the years, the PHMC metamorphosed into the University of Perpetual Help Rizal

Medical Center (UPHRMC). Today, its amended official registered and business name is University of Perpetual

Help DALTA Medical Center (UPHDMC).

On April 20, 1997, the Commission on Higher Education (CHED), in its En Banc Resolution No. R 95-97,

Series of l997, granted university status to the University of Perpetual Help System Dalta (UPHSD) at Las

Piñas, and the University of Perpetual Help System Jonelta (UPHSJ) at Biñan, as a single unit under

the name “UNIVERSITY OF PERPETUAL HELP SYSTEM”, with all the privileges, entitlements, and

responsibilities appurtenant thereto, effective Collegiate Year (CY) 1997-1998.

“Our young can only be assured of a better tomorrow by building the right kind of Character today” By these words

and through the UPHD academic programs did the Man of Vision, Dr. Antonio Laperal Tamayo hone Filipinos to

become a People of Mission and Action. Then known as the University of Perpetual Help Rizal, the Las Piñas

campus of the University of Perpetual Help DALTA (UPHD) was founded in 1975 by Dr. Antonio L. Tamayo, and

co-founded by his wife Dr. Daisy M. Tamayo and friend Mr. Ernesto Palanca Crisostomo. The founder’s passion to

fulfill his nationalistic dreams was mainly directed to promoting a better quality of life for all Filipinos through the

school’s course offerings.

Vision

The University of Perpetual Help System DALTA shall emerge as a premier university in the Philippines. It shall

provide a venue for the pursuit of excellence in academics, technology, and research through community

partnership.

The University shall take the role of a catalyst for human development. It shall inculcate Christian values -

Catholic in doctrine as a way of strengthening the moral fiber of the Filipino - a people who are “Helpers of God”,

proud of their race and prepared for the exemplary global participation in the arts, sciences, humanities, and

business.

It foresees the Filipino people enjoying a quality of life in abundance, living in peace, and building a nation that the

next generation will nourish, cherish, and value.

The UNIVERSITY OF PERPETUAL HELP SYSTEM DALTA is dedicated to the development of the Filipino as a

leader. It aims to graduate dynamic students who are physically, intellectually, socially and spiritually

committed to the achievement of the highest quality of life.

Educational Programs:

'Doctor of Medicine’ (MD) from the Philippines is the only recognized primary qualification by any medical

council. It is the only recognized medical qualification as a medical practitioner of the Philippines.

Students who have 10+2 - A Level / CBSE / FSc / HSC / HSEB / STPM will get partial credit exemption of

about 80 credit units from the Bachelors Degree. They will study for additional 80 - 90 credit units in the

Accelerated Bachelors Program. Mean while they will also study and get trained to take NMAT as required by

CHED. Students just need to pass the exam. It is only to determine the percentile rate. Then they will enroll

'Doctor of Medicine’ (MD) program. The total duration for BSc / BA and Doctor of Medicine (M.D.) will be for

5.5 years and on completion the students will be awarded BA Psychology or BSc in Biology and Doctor of

Medicine (M.D.), which will be equivalent to MBBS.

Students passing Grade 10 - O Level / SLC / SSC equivalent will do 3.5 Years of Pre - Medical (Any Allied

Medical Courses - BSc) + 4 Years of Doctor of Medicine (MD). The total duration for completing Doctor of

Medicine (M.D.) will be for 7.5 years and on completion the students will be awarded BSc in Biology / Medical

Technology and Doctor of Medicine (M.D.) which will be equivalent to MBBS.

Eligibility For Medical Courses:

For 10 / 10 + 2 equivalent 50% marks in Physics, Chemistry, Biology. 50% in Math and English is also

preferred.

NOTE: Eligibility Certificate and screening test are required for MCI registration to practice in India. Students have

to get eligibility certificate from MCI to study abroad. Our Graduates are eligible to appear for MCI screening

test.

Historical Perspective

The University of Perpetual Help Rizal JONELTA Foundation School Medicine was established in June 1996, as

an extension school of the Perpetual Help School of Laguna for Medicine and Allied Health Science and was

thus known as the Perpetual Help College of Medicine- Rizal (Extension School,) with Dr. Aretas P. Singson-

Alday as its Founding Dean. On April 20, 1997 by virtue of Resolution No. R95-57 series of 1997 of the

Commission on higher Education, the Perpetual Help System Campus in Laguna and Rizal became the University

of Perpetual Help Rizal College of Medicine.

Through the efforts of its President, Dr./ BGEN Antonio L. Tamayo and wife, Daisy and its founding Dean,

Aretas P. Singson-Alday, MD the Medical institution was granted autonomous status by the Commission on Higher

Education on November 7, 1997 and became the University of Perpetual Help Rizal Jonelta Foundation School of

Medicine.

UPHSD - Jonelta Foundation School of Medicine is recognized by Commission on Higher Education and Development (CHED), World Health Organization (WHO) and listed in International Medical Education Directory (IMED), Education Commission For Foreign Medical Graduates (ECFMG). It is also recognized by Bangladesh Medical & Dental Council (BMDC), Indonesian Medical Council (IMC), Malaysian Medical Council (MMC), Maldives Medical Council (MMC), Medical Council of India (MCI), Pakistan Medical & Dental Council (PMDC), The Medical Council of Thailand (MCT) and other medical councils upon passing the required Medical Qualifying Exams or Screening Tests.

The University of Perpetual Help Rizal JONELTA Foundation School of Medicine is dedicated to developing a

General Objectives/Goals 1. To develop the complete physician, instilled with values geared towards health promotion.

2. To enhance faculty development through sustained and relevant learning programs cascaded to the student.

CURRICULUM

The four-year medical course generally progresses from the study of the normal to the abnormal human being. Most

of the subjects are yearly courses which are taken up for two semesters.

The first year covers the normal Human Anatomy, Biochemistry, Physiology, Preventive Medicine &

Community health, Psychiatry and Medical Practice I. Class lectures are delivered mostly through computers using

LCD projectors complemented by slide/ film showing, laboratory work and group discussions. Pedagogy and

Bioethics are incorporated in Principles of Medical Practice I. An introduction to medical Informatics is

incorporated in Preventive Medicine and Community Health before the biostatistics session. Problem-based

learning is gradually introduces in certain topics in Biochemistry and Physiology.

The second year deals with Pathology, Microbiology, Parasitology, Pharmacology, Preventive Medicine and

Community Health, Psychiatry II and Medical Economics plus introductory courses in clinical subjects. In small

groups, students begin to meet patients in actual hospital setting where they are introduced to history taking and

physical examination.

The third year involves didactic lectures on common diseases encountered in clinical practice as well as

interesting or rare conditions. Pathophysiology is reviewed while clinical features, diagnostic approaches and

principles of management are emphasized.

The fourth year or clinical clerkship is mainly spent in actual patient care with the JONELTA section of the

University of Perpetual DALTA Medical Center serving as the clinical laboratory. Hospital rounds and group

discussions give the student a good perspective on bedside manners, diagnosis and management. The clerks

rotate through the different clinical departments for 48 weeks with periodic evaluation.

International Student Admission Guidelines

International Student applicants are nationals of a country other than the Philippines holding a foreign passport and

who have been issued a valid Student Visa or Special Study Permit or Permanent Residency Visa by the

appropriate Philippine agency. It also includes those who claim dual citizenship.

Initial Requirements:

1. Filled up Application form.

2. Copy of Passport (pages with the photograph, date of birth and passport validity).

3. Certificate of 10+2 Higher Secondary Education.

4. Transcripts of 10+2 Higher Secondary Education.

5. One (1) 2” by 2” photo of the applicant PROTOCOL FOR FOREIGN STUDENTS SEEKING ADMISSION IN THE UNDERGRADUATE PROGRAM

I. PRIOR TO ARRIVAL IN THE PHILIPPINES

applicant’s country of origin or legal residence.

2. Data page of student’s passport showing date and place of birth.

B. Upon receipt of scholastic records and date page of the student’s passport:

1. Forward scholastic records to DEAN concerned for evaluation

2. Endorse the evaluation to the Registrar for issuance of Conditional Offer Letter

C. Send Conditional Offer Letter to the applicant or to the representative.

D. Applicant should pay the Admission Fee $300 to Coordinator's Office.

E. Applicant will apply for tourist visa, if allowed by the Philippine Embassy / Consular Office in the student’s

country of origin after receipt of Conditional Offer Letter.

RPR test (Rapid Plasma Ragin Test)

1. A passport with a validity period of six (6) months beyond the intended period of stay in the Philippines and a photocopy of the

passport page with name and photograph of applicant.

2. Duly-accomplished Non-Immigrant Visa Application Form FA-Form No. 2-A;

3. One (1) 2” by 2” photo of the applicant;

4. Photocopy of your roundtrip airline ticket OR ticket booking confirmation;

5. Proof of means of financial income or support i.e. bank statement, employment certificate; business registration; -

NOTES:

1) Students will get a Bachelor of Science in Biology Degree after successful completion of Pre-Medical course. Then they will have to sit for National Medical Acceptance Test after which they will proceed to Doctor of Medicine (MD) course for another 4years. Thus they will receive Dual Degree.

2) All Amounts Shown are in Philippine Pesos OR US Dollars. All tuition fees MUST be paid in Philippines Pesos OR US Dollars at university account ONLY. 1 USD = 45 PHP. 1 USD = 60 INR. 1 PHP = 1.40 INR. (As of May 2014). Dollar Rates used for conversion purposes it may fluctuate.

3) All accommodation is in Private Apartments / Bare Rooms near the university campus. Rental rates vary from P8,000 ($190) / Month - P 12,000 ($280) good for 3 - 4 Students. 2 Months Deposit and 1 Month Advance required. Students will have to buy their own furniture and other household accessories.

4) Students who require accommodation in advance must send us the 2 Months Deposit at least 1 month in advance for us to arrange the room in a group.

5) Students shall bear all costs and expenses for 9A (Tourist Visa) extension (if required approx. $200) and conversion

to 9F (Student Visa) as deemed payable to the Bureau of Immigration, Philippines.

6) The costs above are the minimum required for studies. All expenses may vary as per actual cost & needs.

Step 1:

a. Submit Completed Application form to Overseas Education World. You can download the application form in download section from here www.overseaseducationworld.com b. Rs 40,000 (Forty Thousand) Demand Draft. c. Photocopy of class 10 & 12 pass certificate d. Photocopy of class 10 & 12 mark sheet e. Photocopy of Passport (if available – this can be submitted later)

Ensure that your application reaches before 15th

of September of the year of admission.

Notes:

i. Students who have a valid passport should file for a “police clearance certificate” for Philippines (PCC for Philippines) from the passport office or can apply online also. ii. Students who do not have a passport should first apply for a passport and then apply for a PCC for Philippines letter from the passport office. iii. Students should open a Bank Account and get an international debit card issued in their name. iv. The Passport should be valid for at least 1 year

Get 10 passport size photos made of the face with a white background – size is: 2 inch * 2 inch. These photo should be less

than 6 months old.

Submit Before 10th

October of Year of Admission

1. Submit for Documentation By Demand Draft – payable to “Overseas Education World” and payable in Kolkata. 2. TWO Copies of Bank statement to be stamped & signed by the Bank for the last 6 months of the student (original Bank stamp on the statement) 3. Original Passport 4. Original 10th Certificate (duly attested by the State Government and also by the Philippine Consulate) 5. Original 12th Certificate (duly attested by the State Government and also by the Philippine Consulate) 6. Original Police Clearance Certificate (duly attested by the Philippine Consulate) 7. Original Bank Balance Certificate from the Sponsor of the student for about 3-4 lakhs

8. Affidavit of Support from the Sponsor

Step 3:

- Submit fee for visa by Demand Draft – payable to “Overseas Education World” and payable at Kolkata. - We will process your visa work

Step 4:

- Submit our Last processing fee by demand draft or cash in hand in Airport before leaving India. - Fly to Philippines in November 1
