    Investigation

    We have hired an independent firm toinvestigate the incident for the rootcauses of the accidents.

    FindingsThe crate was improperly erected. Bolts thatholt the turret to the vertical base were nottorqued to the proper specifications.Operator failed to do a daily check onequipment.Crane was over stressed lifting a load thatwas to heavy.

    Training

    We have contacted a crane trainingcompany. CraneTech is going to visit outcrane sites.CraneTech is going to give instruction to all

    operators and all operators will be certified under

    NCCCO (National Commission for the Certificationof Crane Operators) regardless if they already havetheir certificate.They will provide training to our operators and

    riggers.They will be visiting all of our active sites and

    provide site specific training.CraneTech is also creating a safety check list. This

    is a daily operator checklist to be competed daily.Also to be signed off by the site manager.

    Initially, Training is going to cost about 3thousand dollars per site.The checklist is going to cost about 500 dollarsfor its creation.

    Executive Summary

    Our company was able to incorporate investigation,training, public/customer relations, legal language, andemployee relations into our six week solution. We believethat the implementation of our plan will be extremely

    effective and will eventually benefit our company.

    We were able to overcome our organizational barriers andwe will continue to focus on the future of ourimplementation of our plan.

    Our company has been able to maintain our operatingprinciples throughout our problem solving journey and weplan to keep our operating principles one of our mainpriorities throughout our implementation of our plan.

    Our independent investigation firm reachedtheir conclusion on the primary causes of our

    crane accidents.

    We immediately started planning and decidedto have Cranetech visit our crane sites.Instruction will be given to all operatorswithin our company from this point forward.
