18F-FDG-PET is useful in initial staging and follow-up of malignancies. Unusual sites of FDG uptake or CT finding may indicate second primary malignancy instead of metastasis. Here, a lung cancer in a patient with recently diagnosed myofibroblastic tumor on biopsy was presented. The final confirmation of cancer was based on pathologic report.
