Introductory Statement / DefinitionsThis Health and Safety Plan addresses the needs of the on-site workersc o n t r a c t e d by Merr imac Cons t ru c t i on at the Eau Cla ire Munic ipa l Wel l F i e ld .The Plan has been deve l oped per requ i r ement s of the Army Corp s of Eng ineer sSpec if icat ion, Sect ion 0 1004 , amendment 0004 (86-B-0001 ) .A comp l e t e descr ip t ion of the s ite may be found in the above s i tedspecif icat ion, section 01004, page 1 .Ner imac cons truc t ion will perform shallow excavat ion on the Eau Cla i re s itefor erect ion of a pump building and concre te pad for instal lat ion ofaeration towers on the site. In the event that the Cont ra c to r uncovers an area of s ign if icant soilcon tam inan t s , based on air monitor ing data and visual ob s e rva t i on s , the TCTsite safety off icer will inform all on-site workers to leave the siteimmediate ly. Re-entry of workers will not be permitted until the extent ofcontaminat ion has been properly defined by the USACE.Heat stress monitoring will be performed only if ambient temperatures"arrant such act ions. Workers in level D protect ion (essential ly a work

iform) in above ground work should not encounter heat stress condit ionsgreater than those typical on any construct ion s ite. If level C protect ionbecomes necessary, heat stress monitoring will be performed accord ing toNIOSH recommendat ions.

Medical ProgramAt least 5 days prior to commencement of work, the Contractor shall providethe following written certification for each on-site worker by anOccupat ional Health Physician to the USAGE:- each on-site worker has undergone a medical examination- each on-site worker is able to perform their work assignments• each on-site worker has the ability to use respiratory protect ionThe fol lowing items const i tute minimum medical survei l lance requ i r emen t s :- med ica l h i s tory- general physical- pulmonary function (FVC and FEV 1 .0 )- electrocardiogram- blood profile (20 channel)- CBD- urinalysis- chest x-ray (number at discretion of attend ing physician)- visual acuity- otoscopic exam- tetanus

VII. Environmental MonitoringA d i rec t-read ing air monitor ing dev ice sens it ive to ch lor inatedhydrocarbons ( such as those listed on page 2) shall be con t i nuous l yoperated on site by the Site Safety Off i cer , downwind of excavat ion and/orwater piping disruption act iv it ies *The Con t r a c t o r shall prov ide and install a wind-d i rec t ion ind i cator on-site, visible to all on-site workers* The Site Safety Off i cer shall makerecommendat ions to on-site workers regarding appropriate location of workre lat ive to excavat i on and water piping d i s rupt ion .A record of all air mon i tor ing performed on-s i te shall be prov i ded to theContractor upon completion of activit ies.

Remove inner boots , splash suit and gloves; deposit in separate-container lined with plast ic .6. Remove facep iece and depos i t on plastic sheet .7. Wash hands & face and shower as soon as poss ible .At complet ion of the ent ire Contrac tor pro jec t , if soil contaminat ion hasbeen de term ined to be present , al l non-wooden tools and heavy equ ipmentshall be washed under high pressure water * Additional decontaminat ion mayke necessary using steam, at the discretion of the Scientific Coordinators ,

^ased on informat ion supplied by the USAGE. Under normal workingcondit ions on-site, that is, contacting well system water at stated maximallevels, no decontamination other than air drying should be necessary.IX. General Safety PracticesThe following general safety practices will be applicable as a minimun forall on-site activities for the duration of the Contractor ' s work:1 . Eat i ng , dr ink ing, chewing gum or tabacco and smoking are proh ib i ted .2. Hands and face should be thoroughly washed upon leaving the work areaand before eating, drinking or other activities.3. No excess ive facial hair, which would interfere with sat i s factoryf i tt ing of resp i rators , is al lowed on workers requir ing resp i ra toryprotect ive equ ipment *A. Storing utensils, food or food containers while on-site is prohib ited.

X. Energency Response Procedures and Contingency Measures

Cont ingency plans for emergencies are as follows:- Genera l Informat ion

- The Site Safety Officer shall des ignate an appropr iate locat ion for afirst aid/emergency station to be equipped with the following equipment andinformation (suppl ied by the contractor ) :

- 16-un i t first aid kit- emergency deluge shower- class ABC fire extinguisher- list of emergency telephone numbers

- physician or medical facility- Eau Claire Fire Dep t .- Eau Claire Pol ice Dep t .- Local Ambu l an c e- Local Poison Control Center- Region V EPA, ERT, ICDM- Wiscons in State Spill Con t ra c t o r

- full face resp irators with organic vapor car t r idges- chemical res istant clothing- chemical resistant gloves- eye protection

• the Cont ra c to r shall provide the Site Safe ty Off i cer and the Team-reader with portab le air horns . Two long blasts shall signal immediateevacuation of the site, regardless of the reason for evacuat ion .

- the Site Safety Officer shall travel the shortest route to the nearesthospital or medica l faci l ity. A map with d irect ions shall be posted on-site at the first aid s ta t ion , to be made avai lable by the Con t r a c t o r .

- the Scientific Coordinator shall notify appropriate local, State andral emergency agencies of the work to be performed, as well asant ic ipated hazards on the work site.

- Fires and Severe Storms- a class ABC fire extinguisher will be maintained on site at all times- te lephone and/or radio contact will be maintained with the local firedepar tmen t- at least two access routes for emergency vehicles will be maintained by

the contractor throughout the duration of the project- all on-site workers will immediately leave the site, if life-threaten ing cond i t ions ex i s t . Appropr ia te ly trained f iref ightersuti l iz ing se l f-conta ined breath ing apparat i may be necessary forext ingu ish ing major fires

SAFETY AS;D mm PROTEM ON m JOBThe Williams-Steip.er Occupational Safety and Health Act of 1970 provides jobsafety and health pi election for workers. The purpose of the Federal law is to assuresafe and healthful working conditions throughout the Nation.

The U.S. Department of labor has primary responsibility for administering theAct. The Department issues job safety and health standards, and employers andemployees are required to comply with these standards.

BY LAW: SAFETY ON THE JOB IS EVERYBODY'S RESPONSIBILITY!EMPLOYERS: The Williams-Stciecr Act requires that each employer

furnish his employees a place of employment free fromrecognized hazards that might cause serious injury ordeath; and the Act further requires that employerscomply with the specific safety and health standardsissued by the Department of Labor.

COMPLIANCE WITH SAFETY AND HEALTH REQUIREMENTSTo ensure compliance with safety and health requirements, the U.S. Departmentof Labor conducts periodic job-site inspections. The inspections are conductedby trained safety and health compliance officers. The law requires that an author-ized representative of the employer and a representative of the workers be givenan opportunity to accompany the inspector for the purpose of aiding the inspection.Workers also have the right to notify the Department of Labor and request an in-spection if they believe th,jt unsafe and unheaithfu! conditions exist at their work-site. In addition, employees have the right to bring unsafe conditions to theattention of the safety and health compliance officer making the inspection Ifupon inspection the Department of Labor believes that the Act has been violated,a citation of violation and a proposed penalty is issued to the employer.

Citations of violation issued by the Department of Labor must be prominently dis-played at or near the place of violation.

The Act provides for mandatory penalties of up to $1,000 for each serious violationand for optional penalties of up to $1,000 for each non-serious violation. Penaltiesol up to $1 .000 are required for each day during which an employer fails to correcta violation within the period set in the citation. Also, any employer who willfullyor repeatedly violates the Act is to be assessed civil penalties of not more than$10,000 for each violation.

Criminal penalties are also provided for in the Act. Any willful violation resultingin death of an employee, upon conviction, is punishable by a fine of not more than$10,000 or by imprisonment for not more than six months, or by both. Convic-tion of an employer after a first conviction doubles these maximum penalties.

The Act provides that employees may not be discharged or discriminated againstIn any way lor filing safety and health complaints or otherwise exercising theirrights under the Act.
