Due to limited health services in the rural areas of Uganda many people are not able to access

the treatment that they need. For poor people just raising funds for the transport to Kampala

and the national hospital Mulago is a challenge, let alone paying for the investigations needed

to diagnose their condition and then to start treatment. This often entails many appointments

coming back or long stays in the capital. The result is that diseases which actually can be

treated quite easily are left to develop the full course of disease causing immense suffering for

the person and for that family.

Realizing that this was the situation for many of our clients, Rays of Hope Hospice Jinja started

in the first half of 2016 a special programme designed to help people with life threatening

disease to get access to diagnostic investigations and treatment if needed. Rays of Hope

Hospice Jinja will assist with transport, investigations and treatment and even upkeep, but if

the family can afford to pay part of the expenses, we will cost share.

We have developed very good relationship and collaboration with Transport for Ugandan Sick

Children (TUSC) , who will help transport those who cannot use public transport or bring bigger

groups when we have many who need to go to Mulago National Referral Hospital in Kampala

(85 km).

We are also working very closely together with Kawempe Home Care, who in August 2016 started a hostel for children who were getting investigations and treatment for cancer at Mulago. The children can stay and eat for free with their care takers, and they are helped to go to scheduled appointments.

These two partnerships have not only kept the overall treatment costs down, they have made it possible for patients who otherwise never would get treatment as they can’t find their way to and around Kampala, or be able to stay for the long time, often months, it takes to get through the investigations and treatment. Another important link in the chain of supporting and helping our patients referred to Mulago is

a volunteer, Zaina, who for years have been helping clients get through the maze of Mulago.

She knows how to find the right doors to knock on to get attention, moreover, through her we

can also organize payments for tests, medicines or other support, which the clients need.

In September 2016, Rays of Hope Hospice Jinja submitted a proposal to Rotary Holstebro,

Denmark for funds to pay for treatment of five of our palliative care clients requesting UGX

10,000,000 (US $ 2775 or DKK 20,000) to cover their treatment costs.

Due to the collaborating partners, in particular Kawempe Home Care Hostel, TUSC, and Zaina,

the volunteer at Mulago , we have been able to cut our expenses much and thus help many

more clients than expected. So instead of helping only the five clients, a total of 25 clients have

benefited from the help of Holstebro Rotary.

The treatment programme of Rays of Hope Hospice Jinja is of high priority for us, and we are

working diligently to ensure continued funding.

Profile of clients supported and outcome of treatment during the period Oct.2016 – March

BRIEF HISTORY OF PATIENTS ON HOSPICE JINJA TREATMENT PROGRAM

SUPPORTED BY ROTARY HOLSTEBRO, DENMARK OCT 2016 – APRIL 2017

1. Nabuduwa Beatrice is a 27 year old woman, a widow and mother of four children. She is also a

person with albinism, and she is HIV pos on ARV treatment.

She came to Rays of Hope Hospice Jinja in February 2016 with a small skin sore on the side of the neck.

It grew very fast and a skin biopsy done in April 2016 showed squamous cell carcinoma. Beatrice was

helped to get full diagnostic tests, operation, upkeep and follow up. She is now fully treated, and she is

continuing to do well on her AIDS treatment.

Total cost: transport, diagnosis and treatment: UGX440,500 (US$ 122 or DKK 851)

2 WILSON KASSADHA

Wilson Kassadha joined Rays of Hope Hospice Jinja in 2013, when he was 70 years old. Since 1992 he

has had a swelling in lower left jaw. In 2013 a biopsy showed Fibrous Dysplasia, he was suggested

operation, but did have the financial resources.

As the swelling grew, it became increasingly difficult for Wilson to eat, and he was grateful when Rays of

Hope Hospice Jinja could offer him assistance with the operation he had wanted for long time, but was

not able to afford.

Wilson Kassadha was admitted at Mulago Hospital end of January where he underwent pre-operative

investigations and was operated 17 February, 2017. During operations he went into cardiac distress and

was kept in intensive care unit for 5 days after the operation. Wilson seemed to be improving, but in

mid-March, while still admitted in hospital, he developed what sounds like a pneumonia and he died on

20 march, 2017. May his soul rest in peace.

3. AISHA NANTALE

Aisha is 26 years old, married with a child of 9 years. She was diagnosed with HIV/AIDS in August

2014 and was started on ARVs. In March 2015 she started developing condylomas (genitals

warts). Aisha joined Rays of Hope Hospice Jinja in July 2015 due to very extensive masses of

genital warts. They have now spread further to cover the whole of genital and rectal area.

With the help of Rotary Aisha was admitted at Mulago Hospital and was given locally applied

treatment for the condylomas, but when it came to surgery, Aisha lost courage and refused

treatment. She was later taken to a senior gynecologist, but she is still undecided. The

condylomas are still massive, but due to local treatment, she does no longer need morphine for

pain control and she is able to work around the house.

4. ALABYE JOHN

John Alabye is a 53 year old from Kamuli district diagnosed with HIV/AIDS in May 2015 and

Initiated on ART in September 2015, but later he developed lesions on his right lower limb.

He was abandoned by his wife with 6 school going children and in a poor social setting. He was

supported to do a biopsy on the lower limb lesions, and nodular Kaposi’s Sarcoma was revealed

on histology and he was started on chemotherapy, which he is finalizing.

During his treatment for the cancer he was also diagnosed with TB, which presently is being

treated. He is also taking his HIV/AIDS medicine well and is now much better on the way back to

health

5. ALEX MUKASA

Alex , 35 years, was well until February 2016, where he

started to get swelling of the left foot and leg. He went to

local hospital and was diagnosed with HIV/AIDS and Kaposi’s

Sarcoma. He was started on ARVs and referred for treatment

at Mulago, but did not go – due to lack of money and also of

fear, as his brother recently died there.

Alex joined Rays of Hope Hospice Jinja in August 2016, when

he was in severe pain due to the cancer. His condition

deteriorated very fast, and he developed disseminated

Kaposi’s Sarcoma involving the lungs leaving him to breathe

with great difficulty. The photo above was taken on Hospice

home visit, when the condition was very serious. He was given

transport money so the following day he went to Mulago, was admitted and started on treatment. Alex

is now finishing his chemotherapy and he is doing very well. His mother who did not want him to go to

Mulago as she has already lost one son there, is ecstatic over his restoration of health.

6. BIJABIWE ROSE

May 2016

Rose was 14 years old, when she first came to Rays of Hope Hospice Jinja in October 2015. She

had attended Kiyunga HC IV in Luuka district where she was referred to Mulago Hospital for

biopsy, but did not go due to financial constraints. Hospice Jinja supported her to go for biopsy

and further investigations in Mulago. Biopsy results confirmed Hodgkin’s disease, and now she

receiving chemotherapy from Uganda Cancer Institute at Mulago. While receiving treatment she

is staying at the Kawempe Home Care children’s hostel.

Now Rose needs pre-operative investigations and hopefully she can be fully treated. No doubt

she is now in a much better shape and she is happy!

Total cost to date: UGX 948,000 ( US$ 262 or DKK 1827) of which 250,000 was covered by another

donor before Rotary came in. Rotary has supported with UGX 698,000 (US$ 193 or DKK 1345)

7. MAFABI SULAIMAN

He is a 45 old man with albinism who developed a swelling on his lateral side of left eye 30 years

ago, it became an ulcer, later became a big wound and involved the left eye. He was taken to

Kenya for radiotherapy. The wound healed and left eyeball was closed inside during the healing

process.

In October 2016 presented with a small swelling the temporal side of the left eye, it was

discharging pus and blood. He didn’t have money to go for further testing and management.

Sulaiman was supported to go for tests and biopsy in Mulago Hospital. The biopsy results

revealed benign swelling, which was managed by surgery. He has improved and is doing well

with his family.

Total costs for transport, diagnosis and treatment: UGX 155,000 (US$ 42 or DDK 299)

8. WAISWA MOSES

Moses is a 14 year old boy, the youngest of five children. The father died. He has been on ARVs since he was 7 years old. In May 2016 he developed swelling of lower limbs and abdominal pains. Clinically he had Kaposi ’s sarcoma. Rays of Hope Hospice Jinja referred him for treatment at Mulago for further testing and treatment. Moses is now on chemotherapy and all the time while being tested and treated he is staying with his mother at Kawempe Home Care Hostel for children in treatment for cancer. Total cost for tests, treatment and transport UGX 84,000 (US$ 23 or DDK 162)

9. KALOGO MUSAMIN

Kalego is 20 year old, the 6th child of nine siblings. He joined Rays of Hope Hospice Jinja in July

2016 and the months prior he had headaches and was losing vision on left eye and the eye

started swelling. In August he developed paralysis of left side, but recovered and paralysis

improved in October. Kalengo was referred to Mulago neurological department in January 2017.

The findings so far have indicated a tumor/cyst behind the eye. He is still undergoing detailed

investigations before treatment can be started. Meanwhile, he is doing well at Kawempe Home

Care Hostel for young cancer patients.

10. BUKUTENDERA AIDA

Aidah is a 40 year old mother of five boys, aged 17 to 6 years. She joined Rays of Hope in

August 2016, when she had a fungating breast tumor with a lot of pain. The wound of the

tumor was at times bleeding profusely, and she was getting increasingly weak. She didn’t

have money for further management, so was supported to go for palliative mastectomy in

Jinja Hospital.. She is still undergoing various investigations before further management in

Uganda Cancer Institute – Mulago.

Total cost for transport, tests and operations: UGC 303,000 (US $ 84 or DKK 584)

( Aida does not wish to have her photo taken ).

11. ALI KIIRE

Ali Kiire is a 42 year old man with 11 children aged (20-2 ½ years) from three different

marriages.

For the past 9 years he has an ever increasing swelling of left leg over the calf muscle. In

2014 he was diagnosed with HIV/AIDS and started ARVs in Dec. 2014.

Ali Kiire became a patient with Rays of Hope Hospice Jinja in October 2015, when he had a

painful swelling of left calf muscle. An X-ray has suggested a tumor (Rhabdomyosarcoma).

Over the following months there was a severe increase in size and Ali was supported to go

to Mulago for biopsy. We are still awaiting the results, so he can be helped with further

treatment.

12. FATINA NABIRYE

Fatina was 17 years old when she joined Rays of

Hope Hospice Jinja in February 2016. She is the 1st

of 6 siblings. She stopped schooling in Primary 7 .

Fatina was diagnosed with HIV/AIDS in July 2015,

when her CD4 count was 98. She was started on

ARVs, but adherence was very poor. In March

2016, she was diagnosed with Kaposi’s Sarcoma

and was started on chemotherapy. A few months

into the treatment she disappeared and we were

only able to find her February 2017. She is now at

the Kawempe Home Care Hostel while she is

continuing her treatment. Her CD4 count is now 5

and she will be changed to 2nd line ARV treatment.

At Kawempe Hostel she is given the best possible

support for continuing her treatment and will be

trained in adherence to her treatment,

13. SAMUEL MUTUMBA

Samuel is a 46 year old who joined rays of Hope Hospice Jinja in June 2016. He has for the past

two years had pain in the mouth and jaws when eating and excessive salvation. A biopsy from

December 2015 indicated Infected Hemangioma ( a benign condition).

Samuel is a father of 5 children.

Samuel went for biopsy in November, 2016, which showed squamous cell carcinoma. After

further investigations he started chemotherapy in February 2017. He is still very weak and

worried for the future.

Total cost to date: transport, investigations, chemotherapy UGX 1,032,800 ( US $ 285 or DKK

1987) Rotary has paid US$ 187 or DKK 1303

14. WATETA MUZAMIR

Wateta joined Rays of Hope Hospice Jinja in 2012, when he was 12 years old. He had earlier

been diagnosed and treated for Burkett’s Lymphoma. Following the treatment he lost his sight

and was left with a tracheostomy tube in place.

I April 2016 his tube was removed and the opening closed. He has since been able to breathe

well without it. The reason for his loss of vision is unknown and he had never been seen by a

specialist . Wateta was therefore referred to Mulago Hospital for evaluation of possibility of

treatment for his blindness. Unfortunately, there is no treatment for him. In spite of this

disappointing result, Wateta is very pleased with knowing what to expect for the future.

15. FLORENCE NAKASANGO

Florence is 10 years old. She is the 2nd of three children, and her mother is a widow. In Oct.2014 she

developed a growth on upper eyelid of her left eye. She was operated in April 2015, and the growth was

removed. Biopsy showed no malignancy.

Six months later the growth re-appeared. Not only could she hardly look out that eye, she was also

teased badly in school. Florence was referred for examination and treatment at Mulago Hospital. The

biopsy showed that it was not a cancer, but a non-malignant pyogenic granuloma.

Within a very short time the growth reappeared and a new biopsy showed malignant tumor (spindle cell

carcinoma, possibly rhabdomyosarcoma), and she was referred to Mulago Cancer Institute for

chemotherapy.

16. NABIRYE EVA

is a 26 year old woman and mother of two children. In June 2015 she found a small, hard swelling in her right breast. She managed to get an ultrasound and a biopsy, and in May 2016 she was diagnosed with breast cancer. She was advised from the Cancer Institute to get operation (mastectomy) and receive chemotherapy but she didn’t have funds to do so. She was helped to do all investigations, operation and chemotherapy. She is still on follow-up, but all tests look very Eva is now doing very well and she is a central volunteer with Rays of Hope Hospice Jinja in her home area, doing a great job among our clients. Total cost: for investigations, chemotherapy, operation and follow-up: UGX 1,471,000 (US$ 406 or DKK 2827). Rotary has paid US $ 24 or 163 DKK for one chemotherapy and some follow-up tests.

17. BISSO ISAAC

He is 18 year old boy from Kaliro district with a severely disabled mother. He was

diagnosed with low grade fibrosarcoma in 2014 after developing a swelling in his right

mandible and surgery was done. In 2016, he developed a painful swelling at the right

lateral side of base of the tongue growing upwards.

Isaac’s social status is very difficult as his is the only son of a severely handicapped

widow ( in photo below, sitting on the ground in center). Isaac has never been to school –

he is a peasant farmer.

Isaac has undergone several investigations and the results are not good. The cancer (low

grade fibro sarcoma) is now breaking through the roof of the mouth and treatment is not

possible. We shall follow Isaac with palliative care.

18 FAIZAL KALGURAVU

Faizal was diagnosed with Kaposi ’s sarcoma (endemic) in 2009 and was referred to Mulago

Cancer Institute for treatment. Due to financial constraints he did not go. Faizal joined Rays of

Hope Hospice Jinja in 2012 when he was 20 years old due to severe pain, which was treated

with morphine. Over the years the condition deteriorated until also the internal organs (lungs

and intestines) were affected. Rays of Hope Hospice Jinja supported transport, tests and

treatment, which was started in March 2016.

Faizal is now fully treated and all he wants is to start a small business and get married.

Total costs: UGX 268,800 (US $ 74 or DKK 518). Rotary has supported with DKK 317

February 2016 – before treatment started

October 2016 – half way through chemo therapy together with Eva. Both undergoing chemotherapy at

the time. Note Faizal’s right hand is still very big. Faizal, November 2016

start a business and to get married.

19 NAMULONDO AISHA Aisha was a 26 year old woman with one child. She was divorced from her husband. She presented to Hospice team in May 2016 with history of acute epigastric pain, chest pain and failure swallow solid foods for six months. She gradually became wasted, xray and scans were done, but no clear diagnosis was given. Later in September 2016 she was supported to go for further investigations in Mulago. Biopsy was done and after histology Cancer of the Esophagus was revealed and after few weeks she passed on. May her soul rest in peace. Total cost of transport, investigations: UGX 315,000 ( US$ 87 or DKK 608)

20. HARRIET KALENDE

Harriet, a 35 year old woman, joined Rays of Hope Hospice Jinja in April 2016. She was diagnosed with HIV/AIDS in 2013, but had very poor adherence. Her first husband, with whom she had 2 children, died, and her second husband abandoned her when she got sick. She had 4 children with him, the youngest 10 months. One child died.

When we came in contact with Harriet, she was very weak with multiple AIDS related opportunistic infection. She was changed to 2nd line ARV medication, and we helped her with food support and a small monthly money support.

Harriet had little support in her family, so we admitted her to St. Francis Health Care Centre with whom we have a very close relationship with in terms of providing support for children failing on ARVs. Harriet did improve significantly, but after Christmas, when she could not go home, and nobody would come to visit her from the family, she lost hope. Eventually, on 25 January, she demanded to be discharged and was taken home. Harriet died three weeks later. May her soul rest in peace.

21. MUTAGYAS BALIKOWA

Balikowa is a 20 year old man with albinism, no. 4 of 10 siblings. Three years he started to get a tumor on right temporal area. Another tumour started a year later on the left side of his face. Mutagyas has never been evaluated by any health center before coming to Hospice Jinja, where he had a big, oozing tmour on the right side of his face and a smaller, crusting tumour on the left side. Mutagyas was referred to Mulago Hospital and he was operated on both sides of his face with skin grafting from his thigh. While attending treatment from Mulago he is staying at the Kawempe Home Care Hostel. Mutagyas is doing very well and the wounds are healing. Presently he is just waiting for the final biopsy results, however, the doctors are very hopeful that he is fully treated and now can focus on prevention of further skin cancers. 22. HARRIET NEWUMBE

Harriet is a 42 year old woman, mother of three children, one child died. She presented to

Hospice with cancer of the eye socket. She was referred to Mulago Hospital and we assisted her with

funds for the chemotherapy. While under treatment Harriet passed away at Mulago Hospital. May her

soul rest in peace.

Total cost for chemotherapy and medicines: UGX 250,000 (US $ 69 or DKK 479)

23. MUSA KYOZIRA

Musa is a 9 year old boy with clinical diagnosis of Filariasis. He has severe chronic edema of left

leg with oozing wound. He also has severe edema of scrotum and of left arm and right side of face. His

parents are very poor and he has never been treated. Musa is not going to school as he is being teased.

Musa was referred to Jinja Regional Hospital, but will need further treatment, which we are presently

organizing.

Total cost to date: UGX 20,000 (US$ 5 or DKK 38)

24 BABY OF JACQUELINE

Mandala Moses is a 10 year old orphan living with his grandmother . Moses has HIV/AIDS and he

needs assistance in adherence. The grandmother has 8 grandchildren living with her as their parents,

her children all died. One of them is Jaqueline, who is now a very young mother. We couldn’t help

noticing that her baby was born with clubfeet. The baby is now 18 months and has never received

treatment. The baby was referred to CoRSU Hospital, where they provide subsidized treatment have a

specialist orthopedic unit and treatment is heavily subsidized. The baby and mother is still admitted in

the hospital.

Total cost to date: UGX 542,000 (US$ 150 or DKK 1,040)

25 NAKINTU EVA

Eva is a 28 year old mother of one child living with

HIV/AIDS. She is herself number 2 of 12 children, where

two sisters died. She was diagnoses with HIV/AIDs in

November 2015, was diagnosed and treated for TB in early

2016, but only started on HIV/AIDS medicine in September

2016 where her immune system was very low (CD4 13). Eva

started developing Kaposi Sarcoma on both legs in mid-

2016.

She was started on chemotherapy from Mulago end

of 2016, but with little response and they were proposing

changing the chemotherapy combination.

Eva came to Hospice Jinja in November 2016 with

severe pain and big wounds in her legs. We could help her

with pain, management of the wounds and adherence

support, but when the parents had difficulties continuing

going for treatment at Mulago we were able and happy to

help. Until now Eva has only received one dose of the new

chemotherapy combination, but she is already seeing a

noticeable improvement, and we shall of course continue

our support.
