This is a reminder notice that our next, California Joint Safety Committee Meeting has been set for Tuesday, April 4, 2017 at 1:00pm in the North Classroom. The Meeting will be held at our Cal-Nevada JATC – Riverside Training Center, located at 9846 Limonite Ave., Riverside, California. The Meeting Minutes from our last Safety Meeting held on December 7, 2016 are attached for your review. Below are the Meeting dates and locations for the remaining 2017 Safety Meetings. 2017 Quarterly Safety Meeting Dates and Locations:

Meeting called to order by Chairman Armstrong at 1:00pm. Chairman Armstrong welcomed the group and had everyone introduce themselves. Previous Minutes: M/S/C to approve the Meeting Minutes of the Joint Safety Committee Meeting held on August 30, 2016. Review of Accidents & Incidents: The updated Accident & Incident Report is attached hereto as Exhibit A. Local 1245 -Northern California: as reported by Chairman Armstrong: Chairman Armstrong discussed a fatality with one of PG&E tree contractors where a tree worker was crushed by 150ft tree that fell on him. Chairman Armstrong noted he didn’t have any Outside Line Contractor accidents or incidents to report beyond what the Contractor’s will report on today and those incidents are set forth below in the Accident & Incident Report attached hereto as Exhibit A. Local 47 -Southern California: as reported by Rod Peterson: Mr. Peterson noted he didn’t have any Contractor accidents or incidents to report beyond what the Contractor’s will report on today and those incidents are set forth below in the Accident & Incident Report attached hereto as Exhibit A. Contractor’s Reports: The following Contractors noted they had no accidents or incidents to report and discuss today. Black & Veatch
International Line Builders Mountain Power Petrelli The other Contractor’s present, reported on the accidents and incidents set forth in the attached Exhibit A that occurred during the 1st, 2nd, 3rd & 4 quarter(s) of 2016. Attached hereto as Exhibit’s B & C are a couple of Safety Bulletins issued by SCE. JATC Reports: Randy Tomlin reported for the JATC and noted they had no apprentice accidents/incidents to be reported or discussed that were not previously covered.

Old Business:
1. Secretary Weaver gave an update on the Electrical Industry Certifications Association [EICA] and the status of where we are in the process of obtaining ANSI accreditation for our own Industry Crane Certification/Training Program and noted that we are making good progress and should be filing for ANSI accreditation in the 1st quarter of 2017.
2. Chairman Armstrong and others discussed the updates to the Red Safety Book that need to
be made as a result of Federal OSHA’s updated Final Rule 29 CFR 1910.269 & 29 CFR 1926, Subpart V and California OSHA’s response and possible additions to this rule. Mr. Armstrong noted that the Red-Book Subcommittee had not met yet but would be meeting soon. It was noted that Cal-OSHA would most likely not have their final rule completed until the 1st quarter of 2017 or 2nd quarter 2017 and maybe not until late 2017. For the record the Red Book Subcommittee is composed of the following 8 individuals from Labor and Management:

3. It was noted that we are currently having more Red Safety Books printed given the delay in Cal-OSHA publishing their final rule.
4. The benefits we are seeing from our new Foreman Training class were discussed.
5. It was noted that Customer’s in general our getting more involved with the Contractors safety issues and it was agreed that this is only going to become a more prevalent practice.
New Business:
1. Mr. Steve Roberts discussed the practice of using multiple bucket trucks with one guy in the bucket truck when working on energized circuits. Close proximity and hot apprentices in a bucket by themselves was also discussed. Mr. Roberts noted that the use of 2 bucket trucks with one employee in each bucket seems to be a practice that is becoming more common. Director Mendez noted that the JATC Committee is looking into this practice and specifically the issue of a hot apprentice in a bucket by themselves. A general discussion followed.
2. Mr. Ward Andrews noted that they have been doing a lot of research on causes of incidents
and Situational Awareness is the root cause of a majority of their past incidents. A general discussion followed.
3. Secretary Weaver discussed the notices that are sent out when a JL loses his/her right to supervise an apprentice from the Cal-Nevada or other outside JATC program. A general discussion followed.

SignificantInjury-On1/29/2016,District:Redlands,Whileintheprocessofremovingabushtouncoverafaultedundergroundcable,acontractworkersustainedafaciallaceration.Theworkerwaspullingthebrushwhileanotherwaspryingontheshovel.Theshovelhandlebrokeandcameintocontactwiththefirstworker'sjaw.Theworkersstoppedwork,securedthearea,andtransportedtheinjuredworkertourgentcare.

SignificantInjury-OnMarch5,2016,City:SantaAna,Acontractworkerwashandexcavatingtoexposeandremoveburiedrebartofacilitateinstallationofa3'x5"interceptpullbox.Theworkerwaspushingdownonashovelwithhisrightfootwhenhelosthisfooting.Whileattemptingtoregainhisbalance,hesteppedonapieceofrebarprotrudingfromthegroundwithhisleftfoot.Therebarpenetratedthebottomofhisbootandthebottomofhisfoot.Theworkerinitiallykeptworkingsincehedidn'texperiencemuchpain.Aftertheworkshiftendedhedecidedtoseekmedicalattentionforasmallpuncturewoundinthebottomofhisleftfoot.

SignificantInjury-OnApril4,2016,District:Covina,Acontractorwassteppingfromanoutsideexcavationontograderings,whichwere6incheslowerandfelthiskneegivewayashedescended.HesoughtmedicalattentionandwasreferredforanMRI.Thecontractordidnotfollowasafeworkpracticebysteppingontotheplatformversusthegraderingsthatresultedinpossiblehyper-extensionofhisrightknee.Itisrecommendedthatasecuredplywoodbridgebesettoensureasafewalkwaybecreatedbetweenshiftedgrades.

SignificantInjury-OnApril22,2016,District:Valencia,AtruckdriverforacivilcontractorwaslockingupthejobsitePorta-Pottydoorbeforeleavingthejobsite.Whileliftingthedoortoplaceapadlockinthemisalignedflanges,theworkerstrainedhisleftarm.Afterseeingthedoctorafewdayslater,theworkerwasdiagnosedwithastrainedleftbicepandstrainedelbow.ThemisalignedlockflangesonthePorta-Pottydoorrequiredtheworkertoneedtoliftthedoorinordertosecureit.Thisactioncausedtheworkertostrainhisharm.ThePorta-Pottywasreplacedandtheincidentwasreviewedwiththeothercrews.

SignificantInjury-OnApril27,2016,District:Covina,Acontractworkertrippedoverasmalldeclineinthegroundlevelasheexaminedaclearancebetweenatreeandconductor.Thetripcausedhimtopullhiscalfmuscle.Heimmediatelynotifiedhissupervisorandtheyagreedthathewouldgohomeandresthisleg.Ifhislegdidn'timprove,theworkwouldgetinexaminedbyadoctor.Ultimately,hemadeavisittothedoctorandisnowperformingtroubleorderinspectionstofivehislegmuscletheappropriatetimetoheel.

SignigicantInjury-OnApril29,2016,District:Saddleback,Acontractworkerwasremovingashoringjackfromatrenchwhenhelosthisfooting.Theshoresliddownandintoatrench,pinchinghisrightringfingerbetweentheshoringtoolandasteelplate.Thepinchcausedasmalllacerationthatrequiredfourstitches.Theforemanappliedpressureandtookthelaborertoanurgentcarefacility.Inthiscasetheworkerwasnotinthecorrectpositiontopullthejackfromthetrench,causinghimtolosehisbalanceanddropthejackbackintothetrench.

SignificantInjury-OnMay18,2016,District:SanBernardino,OnWednesdayevening,May18,alinemansustainedseriousinjuriestohishandswhileworkingonanoverheadtoundergroundconversionprojectontheBarracks12kVoutofFortIrwinSubstationinSanBernardinoCounty.Theinjuredlinemanwaspartofafive-manelectricalcrew.Preliminaryinformationindicatesthatwhilethelinemanandaseventh-stepapprenticewereinthebucketinstallingpotheadsonanewprimaryriserrun,thelinemanmadeelectricalcontactwiththeoverheadconductor.HewastakentotheFortIrwinBaseHospitalandthenflowntoArrowheadRegionalMedicalCenterinColtonwhereheisbeingtreated.Hiswifeiswithhimandheisingoodspirits.Wewishhimaspeedyrecovery.Theinvestigationoftheiincidentisunderwayandwewillpassalonginformationthatwelearnfromthefindings.Theyhavesharedthateachofthethreephasesonthepotheadsideofthearmdidhavecover("guts")installed.

SignificantInjury-OnJune28,2016,District:Kernville,Acontractcrewweretaskedwithdiggingnewpoleholesforfire-damagedpolesinLakeIsabella.Whilehikingupahilltohandofftoolsforanothercrewaworkerbegantoexperiencechestpains.Theforemanescortedhimtotheon-siteambulanceforevaluation.Theparamediconsitedidathoroughevaluationandconfirmedtherewerenoheatorheart-relatedissuesbeyondthepainsymptoms.Itwasrecommendedthattheworkerstayhydratedandrestwhenneeded.
SignificantInjury-OnJune20,2016,District:Afourmancrewreplacingarearpropertylinepole.6thStepApprenticewasinthebucketofaknuckleboombuckettruckthatwaspositionedinadriveway.Hewasguidingapoledownthroughtheexistinglines.Thepolewassuspendedunderacranethatwaspositionedoutinthestreet.Foranunknownreason,thecranetippedover.Theboomofthecranemadecontactwiththeupperstageofthebuckettruckboomandbrokeit.The6thStepapprenticewastossedourofthebucketbutrestrainedbyhisharnessandlanyard.6thStepApprenticesufferedinjuriestohisheadandneck(itwasreporttomebyanothercrewmember,that6thStepApprenticehithisheadonthestuccowallofthehousenearthetruck)6thStepApprenticewashangingabouttenfeetoffoftheground,conscious,butbleedingfromascalpwound.Thecrewusedaladderthattheyleanedagainstthehousetorescue6thStepApprenticefromtheharnessandgethimtotheground.6thStepApprenticewasimmediatelytransportedtothehospital.Basedoffofourinterviewswiththecrew,theContractordiscoveredthis.TheGFhadaverydetailedprogramwithdistancetotheloadpick(76feet),loadchartconfirmingwiththefulloutriggersinplaceandthattherewasveryrespectfulsafetycushion.(upto90feet)theforeman,whohasthecranecertificationtobeoperatingour38Tcrane,waspushingouttheoutriggerswhiletheapprenticewasattachingtheoutriggerpadsalongwiththewoodpads.Ontheloadsidefrontoutrigger,theforemandidn'tlikewherehisoutriggerwasgoingtolandwhichwasontopofthe1/2inchanglerailwhichprovideswheelguidanceforthehomeownersirongate.Atthispointheelectedtopintheoutriggeratmidspanandcalleditgood.Iftheforemanhadlookedatthechartforthehalfriggershewouldhaverealizedthatthetruckwouldnotmakethepick.Infact,thetruckwouldhavetobemoved30feetclosertothepick,usingthehalfriggerchart.Thestreetwasclosedoffandthetruckcouldhavebeenmovedamere3feettowardthestreetandthepickcouldhavebeencompletedwithoutincident.TheContractorcalledforamandatorysafetystanddowntogooverthesefactsandotherpertinentcraneinformationbeforereturningtowork.Wealsoincludedthattherewillnotbeanymid-spanoutriggerpickswithouttwoadditionalsignaturesfromcertifiedcranecertificationholdersverifyingthecalculationsaregood.

SignificantInjury-OnOctober24,2016,DistrictPalmSprings,APalmSpringsE-CrewwastaskedwithrepairingablowntapatapotheadpolewhileenergizedontheTram33kVoutoftheGarnetSubstation.ThecrewwasmadeupofanUpgradeForeman,twoLinemen,andoneGroundman.Atroughly21:30,Lineman#1andLineman#2wereinadoublebuckettruck,hadcompletedreplacingthetapviahotsticks,andwerepreparingtoinstallthesecondoneboltonthetapwhenaflashoccurredlockingoutthecircuit.Thetwolinemenstoppedtoaccesstheirphysicalconditionalongwiththeconditionofthepole,thendescendedtotheground.Lineman#1,whowascompletingthehotstickwork,sufferedwhatinitiallyseemstobeaminorburntotherightwrist(areabetweensleeveandglove),aswellas,asunburnlikefeelalongtherightsideofhisface.TheGroundmanappliedburngeltothewristwhiletheFieldSupervisorcalled911.Lineman#1wastransportedtoDesertRegionalHospitalinPalmSpringsviaambulance.Hewasevaluated,treatedforminorburns,andreleasedat24:00.Lineman#2didnotsufferanyphysicalinjurybutwasshakenup.Atthispoint,webelievetheLightningArrestorfailedcausingtheflash.TheL/A'swerethenewerPolymertype.

SignificantInjury-OnDecember21,2016,Atapproximately1800hrsacontractlinecrewwasinthematerialyardloadingpolesontoalowboystyletrailerwitha3060linetruck(operatedbyaJourneymanLineman).ThefivepersoncrewwaspreparingtochangeoutthreepolesaspartofanoutagethateveninginLongBeach,CA.Whileloadingthefirstpole(90’Class1DouglasFir)thewinchlinefaileddroppingthepole3to5’intothebedofthetrailer.ThepolebouncedandstruckanApprenticeLinemanwhowasstandingofftothesideofthetrailer,waitingtounhookrigging.Acrewmembercalled911andemergencymedicalservicestransportedtheemployeetothehospitalforobservation.Thehospitalreleasedtheemployeetofullduty,withaminorcontusion.Notreatmentormedicationwasadministered.Thecaseisclassifiedasevaluationonly/non-recordable.

CCCI-OnJanuary15,2016,District:Covina,Acontractorcrewwastaskedwithreplacingadeterioratedpole.ThePoleconfigurationwasafour-wire,1/0doubledead-end.Thecrewusedalinetruckwithacrossarm,securedtohotarmstospreadthe4-wireprimaryinordertosetthenewpole.Thecrewsetthepoleandbegantotransferwire.Thecrewstartedwiththeneutralfirst,andafteritwasdead-endedtotheeast,thelinemanworkingtheneutraldead-endgoingtothewestcameoffhishoist.Thejumperwireranthroughthedead-endshoe,causingtheneutraltosagdownintothefieldphase200feetnorthofthestructure,whichinturncausedthewiretoburntotheground,lockingouttheLimber12kV.ThecrewquicklysecuredtheareaandimmediatelynotifiedtheDOCandswithchingcenterofanunplannedoutage.Thecrewwasabletorestoretheloadseveralhourslater.

CCCI-OnJanuary25,2016,DistrictMontebello,Acontractorcrewwastaskedwithreplacingadeterioratedpole.Thecrewwasunabletogainaccessintotheadjacentcommercialpropertywherethepadmounttransformerwaslocated.Theforemancalledhisgeneralforeman,whomadecontactwiththebusiness'manager.Themanagerstatednoonecouldcomeouttogainaccess.Thecrewthenusedaloadbusttooltoopenthecutouts.Uponopeningcutout1,cutout2openedduetoablownfuse,causingtheJoslyn16kVtolockout.Noonewasinjuredduringthelockout.Theincidentisbeinginvestigatedtodeterminewhycutout2opened.

CCCI-OnFebruary2,2016,DistrictCovina,Fourcrewswereworkingatvariouslocationson1stAvenuejusteastofHamiltoninthecityofPomona.AsinglearmbrokeatthetopofthepolemakingcontactwiththecenterphaseandlockingouttheHambone12kVoutofGaneshaSubstations.Atthetimeofthearmbreakthereweretwocrewsworkingthefirstandthesecondpolestotheeastofthepolewherethearmbroke.Thepoleconfirgurationisadoublecircuitverticalconstruction:theroadsidecircuitistheParcells,whichwasde-energizedandgrounded,andtheotherroadsideistheHambone12kV,whichwasholdingNoTestordersatthetimeofthearmbreak.Theworkordercalledtoreinsulatetheroadsidepinandglassonthepolewheretheincidentoccurred.Therewerenoinjuries.TheforemanthatwasholdingNoTestorderscalledtheSubstation.Troublemenweresentout,andtheforemantookaclearanceontheHambone12kVandmaderepairs.Thetoparmoftheadjacentstructure(wherethefailureoccurred)wasdeterioratedbywoodpeckers.Thisissuewasnotevidentwhenperformingavisualinspection.Crewshadnotperformedanyworkonthisstructurepriortofailure.

CCCI-OnFebruary5,2016,District:Menifee,Acontractorcrewwasintheprocessofreplacingapoleforareconductorproject,thatconsistedoftwocircuitsanda12kVtransformer.Thecrewhadalreadyspreadthefirstcircuitontop.Thecrewmenstartedonthesecondcircuitbycoveringthecenterphasewithonelinehoseoneachsideofthepole,andtworubberblanketsontheoverarmjumpers.Twolinemeninstalledtheirhoistandgripsontheoutsidephasethatconsistedof#2solidcopperwire,withplanstospliceitstraightthroughandspreadtheconductorforthepurposeofconductorremovalafterthenewcircuitisinstalled.Bothlinemendisconnectedtheoverarmjumperontheoutsidephase,de-energizingthewiretothenorth,toanopenpoleswitch,withoutsubstationapproval.Thefirstlinemanhadbothnutsloosenedonthedead-endshoe,butwashavingdifficultygettingthesockettocomeoffofthenut.Atthistime,thelinemanrolledthedead-endshoetowardhimelf(awayfromthecenterphase),inordertoremovethesocket.Byrollingtheshoe,thelinemandidnothavepositivecontrolofthejumperandthetailunexpectedlycameoutoftheshoe,swungandmadecontactunderneathoronthesideoftheportionofthecenterphasethatwascoveredbytherubberblanket,causingacircuitinterruption.Thecenterphasewireseparatedoutsideofthedead-end,andthewirefelltotheground.ThecrewstoppedworkandcontactedValleySubstationandtheirgeneralforeman.Thecrewwaitedforthetroublemantoshowupandisolatethelinebeforereceivingaclearancetomakerepairs.Othernotificationsweremadeaccordingly.Althoughthelinemancoveredtheopposingphase,theblanketwasnotwrappedcompletelyaroundthecenterdead-endconductor,whichallowedthetailoftheoutsidephasetomakecontactfromunderneathduetothelossofpositivecontrolofthejumper.

CCCI-October22,2016,District:Ontario,Acontractorcrewinstalledatemporaryomni-rupterswitchontheBallzak12kV.Thecrewopenedandclosedtheswitchafewtimestotestit.Thecrewthencutinisolatorsbetweentheirmechanicaljumpersontheswitch.WithisolatorsopenandundertheSCEtroubleman'ssupervision,thecrewopenedtheswitchusingashotgunforatemporaryhandle.Theswitchdidnotproperlybreakloadonallthreephases,causinganarcbetweentheoutsidetwobladesoftheswitchlockingoutthecircuit.Therewerenoinjuriesandwiredidnotcomedown.Theoutsideswitchoftheomni-rupterswitchfailedtosnuffthearc.Thearcgotintothemiddlephase,causingaphase-to-phasecontact.

CCCI-November7,2016,Region:OPS,Division:WN,AfourpersonOHelectricalcrewworkinginSanger,CA,on31061626_RainbowSubBlitzproject,setupbuckettruck(Unit#047016)toremoveatransformerandinstallanewDEarm.Thecrewhadthesinglephase12kvprimarylinecoveredandthetransformerde-energizedinpreprationtoreplacetheexisting4'steelarmwithan8'woodarm.TheJLoperatingthe55'bucketthreadedbetweentheprimaryandopen-wiresecondarytofirstremovethehigh-sidejumpersbeforeremovingthede-energizedtransformer.AfterremovingthejumperstheJLattemptedtoswingthebucketoutfrombetweenthewiresandre-positionontheothersideofthepole.Asthebucketwasmovedfrombetweentheprimaryandsecondaryalleysideconductorstheknuckleoftheboommadecontactwiththeprimaryandsecondarywirescausingadirectshort.TheFM(actingasandobserver)calledouttotheJLinthebuckettostop,butitwastoolate.PreliminaryCauses:Thebuckettruckwassetuponthewrongsideoftheworkarea.Hadthetruckbeensetuptotheothersideofthepolewheretherewerenooverheadlines,contactcouldhavebeenavoidedandthepolecouldhavebeencompletedsafely.

DigIn-OnJanuary26,2016,Location:Ione,CrewswereonsitetoinstallnewPGEconduitviadirectionalboring.Groundconditionsweresolidrockmakingsteeringdifficult.Afterseveralattemptspilotborewascompletedbutwasfourfeetsouthofthepre-dugexitpit.Anexistingsandbackfilledjointtrenchwasvisibleintheexitpitandonthesurfaceduetotheasphaltpatchrunningdownthecenterofthedrivewaybeforecomingtoatee20'fromtheedgeofroadway.Therewasapaintmarkoneachsideofthedrivewayalignedwiththeasphaltpatchandonedotfartherdownthedriveway.Theborecrewbrokeouttheasphaltontheedgeofthetrenchlineandpotholedthejointtrenchfromthesideexposingtheconduitsdowntodirt.Thejobforemannoticedwhatwasgoingonanddirectedaperatorandspottertotakeover.Thespottermeasuredtwofeetoffoftheexposedconduitsandwavedintheoperatorwhothenproceededtostickthebucketinthegroundandcurl,contactingsecondarycablesburiedinflexductthathadjuttedoutfromunderthesandditchlineata45degreeanglebeforealigningparallel2'westoftheasphaltpatch.Hewasstoppedbythespotteraftermovingthecable8to10".Thecrewcalledtheforemanwhocalledtheinspectoronsiteandtogethertheycheckedthemeterstoalltheadjacenthousesandfoundnooneoutofservice.UponarrivalMondaymorningitwasdeterminedthatalossofservicehadoccurredshortlyaftercrewsleftonFriday,evidencedbytheneutraljumperfromthehouseonthestreettothehouseatthebottomofthehill.Wecalledthehotlineandlatethatafternoonatroublemancameoutandrepairedthedamagedneutralconductor.HewastheguythatgotdispatchedonitFridayandwasgratefulthatwehadcalleditinandagreedthattherewereinsufficientmarks.

DigIn-OnJanuary28,2016,Location:LakeComanche,BelowisanincidentdescriptionoffacilityeventasdescribedbytheprojectForeman:TheprojectwaswalkedtoreviewtheborepathwithaDirectionalBoring,asub-contractorwentoverallpotholesdecidingonthedepththeywouldbore.Whenwelookedattheforcemaintherewerenumbersdownonthegroundat73”to78”deepreflectingthedepthofexistingutilities.ItwasDecidedtoboreinat50inches.WhilePrecisionwaspre-reamingtheyhittheforcemain.Thesewerforcemainwaspotholed7feetfromrunningline.Whywasthelinehit?Whenthesewerwasinitiallypotholeditwasrainingout.So,theGroundmanwrotethenumbersdownonpaperduetothefactmarkingpaintwouldwashawaythenumbersonaccountoftherain.Thenextsunnydaythecrewwasinstructedtogobackandvac-outthepotholesagain,verifyutilitydepthsandwritenumbersonthestreet.ThecrewverifiedtotheForemanallpotholeswereverifiedanddepthsmarked.Cometofindout,theteammissedtheforcemainandafterwefinishedthatnightIwashandedapaperwiththeforcemainpotholeanditdepthsonitfromtheinitialpotholeverificationprocess.Theteamhandledthesewerforcemainbreakprofessionallyandwerecomplimentedbythecommunityservicesutilitydistrict.Nonetheless,theutilitywascompromisedduetoanoversiteonthecrewspartanditwascompletelypreventable.

CloseCall-OnFebruary16,2016,Location:VillaParkSubstation,Awiremanforacontractorwascuttinghomeruncontrolcableawayfromthecarrotconnectioninsideacircuit(CB)pullbox.Hewascuttingacableoneatatimeandcheckingthemdeadbeforegree-tapingthem.Oncethewholecablewascutawayfromthecarrotsplice,hepulledthemintothecabletrench.Hethenremovedthecarrotwrappingandunboltedtheconnection.WhenthesingleconductorsgoingtotheCBCabinetwereexposedoneofthemwasstillenergizedandmadecontactwiththeC1andoperatedthebreakerclosed.Thewiremanimmediatelystoppedworkandcontactedthetestman.Thetestmanwasgoneatthetime,butwhenhereturnedheandthewiremantailboarded.Then,heturnedofftheDCswitches.ThetestmanhadprevioiuslylefttheDCknifeswithchesclosedinsidetheCBcabinetanddidnotswitchouttheC60relaythatsentaDCbackfeedtothecontrolcablesingleconductorcomingfromtheCDcabinettothepullbox.Thewiremanwasworkingunderthetestman'sdirectorders.Itwasstatedthatcablesthatwerebeingremovedandreplacedwere100percentde-energized,buttheywerenotverified.

CloseCall-OnApril1,2016,District:SanJoaquin,Acrewwasenergizinganewlyinstalledtransformerbankandserviceonthelinesideofthenewlyinstalled227/480vpanel.Thecutoutswereclosedonthetransformerbankallowingtheforemantocheckvoltageatthepanel.Upondetermininggoodvoltageatthepanel,thecrewproceededtode-energizethebankatthecutoutsallowingforthetestlockstobeclosedontheloadsideofthecustomer'spanelpriortore-energizingthenewtransformerbank,overheadserviceandcustomerpanel,thecustomerpanelsustainedanelectricalphase-to-phasecondition,resultinginaelectricalflashandcausingasmallfireinsidethepanel.Thecrewusedafireextinguishertoputoutthesmallfireanuponfurtherinquiryintotheincidentdeterminedthattheinsideofthecustomer'smainpaneltherewerethreeterminationsthatwerenotproperlyinstalledbytheelectrician.Thethreeconductorswerefoundtobeskinnedexposingbarecopperandbundledtogetherwithnosafeendingofcables.Thecrewimmediatelyopenedthecutoutstode-energizethetransformerbank.Therootcauseoftheincidentwasthatthethreeterminationswerenotproperlyterminatedinthecustomer'smainandwerenotsafe-ended.

ChemicalSpill-OnMay21,2016,District:Montebello,Adetioratedpolleunexpectedlybrokeandcausedanoilspill.Acrewhaduntiedthreeprimarytangentphasesfromathree-potbankpoleandsuspendedthemfromanarmandthelinksticksofa100-footbuckettruck'sjibcrane.Thecrewremovedservicesandloweredthemtotheground,leavingthepolewiththethree-potbankpoleandnowire.Afterabout10to15minutes,thepolebrokeatgroundlevelandfellona30/60linetruck'sboom.Momentafter,thepolebrokeagainjustbelowthetransformerbank,whichsentehpoletopandthree100kVAtransformerstotheground.Fortunatelytherewerenoinjuries,circuitinterruptins,orpropertydamage.Thecrewliftedthetransformersuprighttopreventnymoreoilfromspillingandcontaintedthatwhichhadalreadyspilledotkeepitfromenteringnearbystormdrains.ThecrewcalledEdisonandHazmatcrew,whocleanedtheoilspillandtesteditforPCBs.Theresultswerenegative.

Drive safely — Exercise extreme caution after a long dry spell. During a dry period, engine oil and grease build up on the road over time. When mixed with water from a new rainfall, the road becomes extremely slick. Continued rainfall will eventually wash away the oil, but the first few hours can be the most hazardous. Check your tires to make sure tire tread and pressure are suitable for driving Check your windshield wipers, too Drive slowly and cautiously, avoiding sudden or jerky movements Turn on your headlights so you can be seen by other vehicles Keep a safe distance from the vehicle in front of

SAFETYSnake Safety
As springtime and field work calls SCE crews and personnel to theoutdoors, encounters with snakes become inevitable. This is the time ofyear when snakes emerge from their winter habitats to warm up andsearch for food, as well as potential mates. Among the most dangerous
of these snakes, in our region, is the rattlesnake. Although a bite from any snakecan be painful, the rattlesnake’s venom can be life-threatening.
Field personnel need to be mindful of the possibility of encountering snakes,especially during ground disturbing activities, working in heavily vegetatedareas, or working in and around stored material or laydown yards. This topic
should be discussed during tailboards when work involves a risk of
encountering snakes.
Rattlesnake bites rarely result in death but can cause serious side effects. Mostbites occur while individuals are attempting to capture, kill or otherwise botherthe snake. Here are several things you can do to avoid an encounter with a snake.
1. Understand that snakes/rattlesnakes are NOT confined to rural areas. They have been found near urban areas, in river or lakeside parksand at golf courses. Be aware that a startled rattlesnake may not necessarily rattle before striking defensively.
2. Do not step or place your hands where you cannot see them, and avoid lifting up debris or material lying on the ground without checkingif a snake could be underneath it.
3. When working in the field, where possible, stick to well-traveled trails and avoid tall grass, weeds and heavy underbrush where snakesmay be hiding during the day. If your vehicle has been parked for a while, look under it to make sure a snake isn’t using it for shade.
4. Snakes are primarily nocturnal, moving around during early morning and evening hours. Remain vigilant during these times and whenworking in potentially dangerous areas where snakes may have taken cover during the day. This includes the shade of buildings. Also, someof our facilities are in the middle of snake habitats, so be careful when stepping over the doorstep as well. Snakes like to crawl along the edgeof buildings where they are protected on one side.
5. If you encounter a snake in the wild (especially a rattlesnake), the best thing to do is LEAVE IT ALONE. For nuisance snakes found at afacility, secure the area and call your Corporate Resources representative for removal.
6. Do not attempt to stop a snake that is leaving on its own. Ultimately, snakes want nothing to dowith you any more than you with them. They recognize you are much bigger than themand will usually retreat when given the chance.
7. Be aware of your surroundings and keep a watchful eye out for warning signs andsnakes. If you find a snake, give it space.

Summer is fast approaching, which means more insects, spiders and snakes are out and about in SCE territory—some of which have bites that can prove life-threatening even to healthy adults. This year, with a low snow pack and temperatures well above normal, some insect populations have flourished and snakes are venturing into backyards looking for water, making it especially important to take steps to look out for problematic critters. Here are some tips on what they are and how to avoid them.
• Create a physical barrier against insects by keeping your skin covered as much as possible. Wear a long-sleeved shirt, long pants, socks and a hat
• Wear light-colored clothing so as not to attract bugs
• Avoid using any scented soaps or other products since perfumes and aromatic fragrances can attract bugs
• You can create a chemical barrier against insects by applying DEET to exposed skin areas. DO NOT apply DEET to any flame-retardant clothing as this will reduce the integrity of its retardancy
• Get to know your team—do any of your co-workers have allergies to bees or oth-er insects you should know about in the event of an emergency?

SPIDERS & SCORPIONSSpiders and scorpions should always be avoided. Many spiders are harmless, but a bite from the female black widow, which the most dangerous spider found in SCE’s territory, is a medical emergency. Contrary to popular belief, there are no stable populations of brown recluse spiders in Southern California, but a relative commonly known as the desert recluse is found here. Even spiders not traditionally considered dangerous can cause painful wounds and nausea or vomiting. The bark scorpion, the most venomous scorpion in North America, is also a creature to look out for.• Black widows are commonly found outdoors in
woodpiles, rubble piles, under stones, in hollow stumps, and in rodent burrows, privies, sheds and garages. They are commonly found indoors in un-disturbed, cluttered areas in basements.
• The bark scorpion is the only common climbing scorpion. It typically lives above ground under tree bark and in palm trees and crevices of rocky cliffs. Because it can ascend slump block walls or stuc-co, this species is the scorpion most likely to enter dwellings. The bark scorpion is attracted to mois-ture around homes and may be found in stacked lumber or bricks, firewood piles, cellars, and attics.

Rattlesnakes are the only dangerous venomous snakes in SCE’s service territory. According to the California Poison Control Center, the six species of rattlesnakes found in the state account for more than 800 bites each year, and victims sometimes succumb to the injury. Most bites occur between the months of April and October when snakes and humans are most active outdoors.• Stick to well-used trails always wear appropri-
ate protective footwear and long pants. Avoid tall grass, weeds and heavy underbrush where snakes may hide during the day.
• Do not step or put your hands where you can-not see, and avoid wandering around in the dark. Check out stumps or logs before sitting down.
• Be careful when stepping over the doorstep as well. Snakes like to crawl along the edge of
buildings where they are protected on one side.• Always have someone with you who can assist
in an emergency.• Do not handle a freshly killed snake, it can still
inject venom.• All rattlesnake bites are emergencies and imme-
diate care is absolutely necessary. See APM Rule 1018 for instructions on treating snakebites.
SNAKES
JUST AS YOU NEVER ASSUME A CONDUCTOR IS DE-ENERGIZED, NEVER ASSUME INSECTS OR SNAKES ARE HARMLESS.
Numerous types of bees, wasps and hornets are native to SCE’s territory, including africanized honeybees, commonly known as “killer bees.” Large wasps known colloquially as “tarantula hawks,” which deliver one of the most painful stings of any known insect, are also found here. Your safest bet, of course, is to steer clear of them all.• Assume that all beehives contain aggressive bees.• Any rapid movement or noise may result in a bee
attack.• The removal of a bee colony should be left to
professionals. Call your T&D environmental specialist so they can arrange for appropriate bee control services to handle the removal.
• While a bee can only sting once, a bee usually is accompanied by others. Wasps and
hornets sting repeatedly.• When you find yourself near a hive, slowly raise
your hands to protect your face. Remain calm and stationary for a few seconds, and then slowly move away.
• Do not throw rocks at or spray nests with water. Avoid creating loud noises and disturbances near the nest.

PURPOSEThe purpose of this bulletin is to advise workers of how to avoid and treat work-related insect bites and stings and increase their awareness of this hazard.
OVERVIEWBites and stings are a relatively common occur-rence for people who work outdoors and in en-closed environments where bees and wasps, fire ants, insects, and arachnids (spiders, scorpions, ticks, and mites) feel at home.
Workers are encouraged to understand exposure risks, how to recognize and respond to stings and bites, and what they can do to prevent them.
Exposed arms and hands tend to be more suscep-tible to stings and bites among workers than legs and feet, which are usually protected by clothing and enclosed shoes. The head is also the most ex-posed area susceptible to insect bites and stings.
SAFETY REMINDERS
Prevention• Learn to identify flying and crawling creatures.• Take extra precautions when working outdoors
and in places typically inhabited by bugs and bees.
• Workers with a history of severe allergic re-action should consult a medical professional about carrying an epinephrine auto-injector and wearing an allergy identification band.
• Always apply insect repellent according to di-rections and wash it off with soap and water after work. Check the label, for example, certain repellents are effective against certain pests, such as chiggers.
• Wear light-colored, smooth-finish long-sleeved shirts and long pants, and enclosed footwear. Some outdoor stores sell clothing and gear pre-treated with repellent.
• Keep your body clean and odor-free; avoid wearing perfumed lotions and deodorant, af-tershave or scented hair products.
• Manage your environment by checking the area before starting work, closing windows, us-ing screens and nets, and removing any food or other attractive items. Insects and ants may be hidden in dark corners, or under wood piles, fallen logs or animal carcasses. Areas with abundant flowers attract bugs and bees.
Symptoms & TreatmentVenom from stings and bites often causes minor swelling, redness, pain, and itching. Reactions can vary from mild to severe. In rare cases, a bite or sting is potentially lethal for people who are highly allergic to the venom.
In many instances, appropriate self-care is an option for bites and stings. Seek immediate medi-cal care if the victim is known to be allergic or in response to symptoms such as sever pain, fe-ver, nausea, trouble breathing, light-headedness, balance problems, or confusion.
Avoiding and Treating Work-Related Insect Bites and Stings

First AidMost bites and stings will heal on their own with appropriate first aid and self-care. For an inflamed insect bite or sting, apply a wrapped ice pack for no more than 15 to 20 minutes an hour for the first six hours. When not using ice, keep a cool, wet cloth on the bite or sting for up to six hours. After the first six hours, if swelling is not present, warmth may be applied to the site for comfort. Over-the-counter anti-itch creams are recommended.
Do not break any blisters. If a bite becomes irri-tated, apply a non-prescription antibiotic ointment and cover it with a bandage. Look out for signs of infection, which include: increased pain, swell-ing, redness, warmth around the bite or sting, red streaks from the area, draining pus, and fever.
Bees, Wasps, Hornets
Pollen-collecting bees die after a single sting when they leave the stinger embedded in the skin. In the wasp family, which includes yellow jackets, only females have stingers, and they can sting people repeatedly. Hornets are a subset of wasps that also can sting repeatedly and can be extremely aggressive.
Multiple stings are likely to require a medical eval-uation. Recommendations for a single sting:• Immediately remove the stinger by flicking it
off or wiping gauze over it; do not squeeze or use tweezers.
• Wash the site with soap and water.• Apply ice to reduce swelling.• Non-drowsy, non-prescription antihistamines
and analgesics may be used temporarily in ac-cordance with instructions to help relieve itchi-ness, pain, and swelling.
• Calamine lotion may help relieve symptoms, but topical antihistamines have no proven clin-ical value.
Fire AntsThere reportedly are five times more fire ants per acre in the U.S. than in their native South Ameri-ca. They are found mainly in the southeastern U.S., with limited geographic distribution in New Mex-
ico, Arizona, and California.
Fire ants are typically no more than a quarter-inch long and brown to black in color. They build mound-shaped nests and are capable of delivering a one-two-punch by biting and stinging. Venom from a fire ant causes a burning sensation. Red bumps may form and become pustules within two days.
Recommended response:• Briskly sweep off ants to prevent them from at-
taching to skin with their jaws.• Check for ants under clothing.• An over-the-counter antihistamine may help re-
duce pain and swelling.

• Keep bite sites clean; pustules are a sign of infection that may need to be evaluated and treated.
Spiders
Spiders are usually not aggressive; most bites oc-cur because a spider is trapped or unintentionally touched. Venomous spiders found in the United States include the black widow (plump, black, and shiny with an hourglass-shaped red mark on the abdomen) and the brown recluse (dark, violin-shaped patch just behind the head). If the biting spider is identified as venomous, a medical profes-sional may administer antivenom or other treat-ment. Without seeing a spider, a bite may be iden-tified by telltale signs such as a round, red welt, swelling, and skin damage. Healing may take lon-ger than other types of insect bites and leave scar tissue.
Self-care for most spider bites:• Clean the bite area with soap and water.• Do not attempt to remove venom.• Apply an ice pack.• Elevate bitten extremities to reduce inflamma-
tion and swelling.• Take an over-the-counter remedy to reduce
pain; avoid medications that cause drowsiness.
TicksTicks are generally found in wood-ed, brushy or grassy areas. They burrow into skin using a barbed appendage to withdraw blood.
A tine-tipped tweezer is the pre-ferred removal tool. To remove a tick, pull upward with steady, even pressure. Twisting or jerking can cause parts to break off and remain in the skin. Af-ter removal, clean the bite with rubbing alcohol, an iodine scrub, or soap and water.
A clinic visit may be advisable if the tick is in a dif-ficult to reach spot or the bite becomes infected.
