In this technique using 99mtechnetium pertechnetatefor dynamic study of vascular “flow―pathways (bothintra- and extra-cranially), the Pho/Gamma Scintillation Camera is equipped with the Nuclear-ChicagoSuper-8/Persistence Scope.

SETTING-UP. The standard 4000 parallel-hole collimator is used. The area to be visualized includes thepatient's neck and head. With the patient in the supineposition, the Pho/Gamma detector is positionedtouching the tip of the nose. This orientation can bereadily achieved, because of the Pho/Gamma detector's positioning flexibility.

ISOTOPEANDDOSE. An intravenousinjectionof10 mC of99mtechnetiumpertechnetateisadministered,preferablyinone oftheantecubitalveins.No attemptis made for a bolus injection.

DATAACCUMULATIONANDDISPLAY.At the firstdetection of events on the persistence scope (whichdisplays data in live “fluoroscopic―fashion), thescope display is filmed with the Super-8 movie camera. Frame rate is 32 per second. Filming is stoppedwhen the recirculation phase is detected —usuallyaboutone minuteafterthebeginningofthestudy.

Then, approximately one hour later, conventionalscintiphotos are taken, in a variety of viewing positions, each representing approximately 250,000counts.

The motion-picture film is subsequently viewed withthe Super-B Analyst projector in slow, fast, or stopmotion, as necessary for evaluation.

These Pho/Gamma-generated data can also berecorded, in high-resolution digital form, on theNuclear-Chicago Data-Store/Playback Accessory oron the CDS-4096 Clinical Data System. With either ofthese system accessories, patient data can be storedand then re-played, processed, and manipulated attheclinician'sdiscretion.The resultisan increasedrangeofanalysis,yieldingadditionalqualitativeandquantitative data.

CASEHISTORY.The clinical study illustratedonthe opposite page is of a patient with the followinghistory: Male, 51 years old. Three-month history ofintermittent episodes (one to three minutes duration)of right visual-field constriction. Physical examinationnegative, except for slight blurring of right optic disc.

EVALUATION. In the selected frames from theSuper-B motion-picture film shown at left, these dinically relevant indications are seen: Frame 119, thereis no isotope flow through right carotid artery pathway (arrow); note also outline of anterior and middlecerebral artery pathways, with relatively decreasedconcentration in right hemisphere. In Frame 158 (capillary phase), block in right carotid pathway is stillevident. In Frame 204 (venous phase), delayed arterialperfusion in the right hemisphere begins. And, inFrame 490, recirculation with evident delayed arterialperfusion in right hemisphere is seen.

The static scintiphoto shown is essentially negativefor any evidence of abnormal isotope accumulation,as were a number of other scintiphotos taken followingthe Super-B study.

CONCLUSIONS. In this case, detection and localization of an abnormal “flow―pattern in the Super-Bdynamic study—butnot in the static scintiphotos—ledto a meaningful differential diagnosis. To this end, aserial arteriographic study was performed. The radiograph.selected from that study reveals complete ocdlusion (arrow) of the right internal carotid artery atthe bifurcation with the external carotid on the right.The intra-cranial problem was therefore shown to bethe result of extra-cranial pathology.

Thus the Pho/Gamma Scintillation Camera permitsthe use of a relatively innocuous, yet rapid, techniqueto produce supplementary diagnostic information.This information can provide direction for the use ofother investigative techniques and make possible amore definitive diagnosis. 0-225

Why does Picker keep refining and improving its basicrectilinear scanner (Magnascanner®500),when It alsohas a most sophisticated high-speed scanner(Dynapix®),and two exceptional cameras (DynacameralM and Magnacamerae)?

Because: despite the rapid forward thrust of progress—whichwe ourselvesaid, abet, foster and contribute to—nothingwe or anyone else has done has obsoletedthe basic rectilinear scanner. What basic scanners do,nothing does better, and few do as well. Examples?

For a small hospital starting a diagnostic radioisotope laboratory with a small patient load and amodest budget, there is nothing quite as appropriate asa scanner. Hence, four out of five nuclear medicinedepartments get started with a Magnascanner andthere are now over 2500 in use throughout the world.Similarily, a Magnascanneris a most relevant choicefor larger hospitals in need of an instrument with thehighest resolution for diagnostic confirmation. A basicscanner like the Magnascanner Is still the best deviceavailable for static-imaging applications by virtue of itsvery high resolution, large field of view, wide energyrange, contrast enhancement, wide choice of focusingcollimators, and modest cost.

None of this should imply that the Magnascanner is

an untouched island in the stream of progress.Today'sinstrument is generationsaway from yesterday's.Note:(1) maximumscan speed has been increasedfrom 200cm/mm to 500 cm/mm; (2) detector can be positionedby a control on the detector head itself, and a ratemeteron the detector head facilitates and speeds location of“hot―and “cold―spots; (3) a new color photo recording system is available in addition to black and whitephotorecording, multicolor dot recording, and Teledeltos black dot recording; (4) push button energywindow selection (in addition to manual selection) forthe most common radioisotopes used in diagnosis.And Now the Dual Magnascanner®@@Thisinstrument isessentially identical to the Magnascanner®500 exceptthat it has two separate opposed detectors which acquire information independently. AP and PA, or RL andLL rectilinear scans can be performed simultaneously.This capability minimizes the need for patient re-positioning and reduces the scanning time by half.

It should be noted thatalthough radiopharmaceuticals are notgenerally used in individuals under 18,procedures using ‘@Tc-DTPA or @Tc-Scolloid are occasionally necessary in suchpatients. The low internal radiation dosage of ‘°‘@Tc-DTPAmakes it a very satis

factory agent when scans of the kidney,brain, or blood vessels are necessary inyoung patients. The low internal radiation dosage of @Tc-Scolloid makes it avery satisfactory agent when liver orspleen scans are necessary in young patients.Radiopharmaceuticals, produced by nuclear reactor or cyclotron, should be usedonly by physicians who are qualified byspecific training in the safe use and safehandling of radioisotopes and whose experience and training have been approved by the appropriate federal orstate agency authorized to license theuse of radioisotopes.When obtaining elutions fromThchnetopeII (Thchnetium 99m) Sterile Generator,proper radiation safety precautionsshould be maintained at all times. Thecolumn containing °@Moneed not be removed from the lead shield at any time.There is a high radiation field surrounding an unshielded column. Solutions ofsodium pertechnetate @Tcwithdrawnfrom the generator should always be adequately shielded. The early elutions fromthe generator are highly radioactive. Important : Since material obtained fromthe generator may be intended for intravenous administration, aseptic techniquemust be strictly observed in all handling.The stoppers of the eluent bottle, of theelution tube, and of the collecting vial,aswellasbothrubberclosuresinthegenerator column, should be swabbed with asuitable germicide before each entry. Allentries into the generator column mustbe made aseptically with sterile needles.Only the eluent provided should be usedto elute the generator. Use a fresh milking tube and collecting vial for each elution; sufficient equipment is provided forthis purpose. All equipment used to collect or administer sodium pertechnetate00mTcmust be sterile. Do not administermaterial eluted from the generator ifthere is any evidence of foreign matter.NOTE: The Renotec Kit and theTesuloid Kit are not radioactive. However, after the eluted @“@Tcis added,adequate shielding of the resulting preparation should be maintained.Precautions : When using radioactivematerial, care should be taken to insureminimum radiation exposure to the patient (i.e., by using the smallest dose ofradioactivity consistent with safety andvalidity of data) as well as to all personnel directly or indirectly involved withthe patient. Before a test is repeated inthe same patient, the need should becarefully evaluated; this is especiallytrue in younger patients.Each elution from Thchnetope II (Technetium 99m) Sterile Generator should be

assayed before use for “ETcactivity andfor the possible presence of “Mo.Material containing more than 5 microcuriesof “Moper dose of °°‘@Tcpertechnetateexceeds Atomic Energy Commission urnits and should not be administered. Poorgastrointestinal. absorption of an oraldose of pertechnetate and resultant lowblood radioactivity levels have been observed in the postprandial state, in seriously ill patients, and in a small numberof normal, fasting individuals. Since pertechnetate is concentrated by the gastricmucosa and the salivary glands, secretions of the digestive tract are radioactiveand may cause artifacts on the cranialscan. Therefore, all possible care shouldbe taken to avoid extracranial contamination, not only for the protection of patients and of hospital personnel but alsoto avoid obtaining a falsely positive scandue to extracranial radiation. Any condition which alters the blood-brain barrieror the normal cranial vasculature maycause abnormal areas of increased radioactivity. The brain scan with sodium pertechnetate °°mTcis therefore likely to beabnormal in patients with scalp contusions or acute head injuries. Following acraniotomy, uptake of radioactivity isincreased throughout the operative field,usually for only a few weeks but in someinstances for prolonged periods. Sincecerebral radiographic techniques ternporarily affect the blood-brain barrier,brain scanning with sodium pertechnetate @@mTcshould precede cerebral angiography when possible, or should bepostponed for several days thereafter. Anegative brain scan does not rule out thepossibility of a lesion and should therefore never be considered diagnosticallyconclusive. Because the normal vascularstructures are more apparent on a @°‘@‘Tcpertechnetate scan than on a radiochiormerodrin scan, and because the choroidplexus may be visible, it is particularlyimportant to recognize the appearanceof a normal brain scan when @“Tcpertechnetate is used, in order to avoid incorrect interpretation.NOTE: The Renotec Kit and theTesuloid Kit were designed for use withthe sodium pertechnetate eluate obtamed from a Technetope II Sterile Generator. It is recommended that onlyThchnetope II be used as the source ofsodium pertechnetate with the RenotecKit and the Thsuloid Kit unless the userhas demonstrated that other sources ofU(mTc are consistently compatible andmeet the standards of Thchnetope II.
