You may receive copies of TNAI Bulletin only after submitting the complete mailing address afresh!

We have been observing for quite some time that copies of TNAI Bulletin are not reaching the properpersons for many reasons, mainly because the address in TNAI records is not got updated by the sub-scribers with their change of residence. In many instances, the subscribers furnished the Institute’s ad-dress where they last studied and it was never got changed later. This leads to massive wastages.

It has therefore been decided to follow a reverse strategy, by mailing copies of TNAI Bulletin only to thosemembers who submit their addresses again. DISRESPECT & ABUSE DURING MATERNITY CARE SERVICES

Causes and Recommendations

Women’s experiences with maternity caregivers can empower and comfort them, or inflict lasting damage and emotional trauma. A woman’s positive or negative memories of childbearing experiences stay with her throughout her lifetime. Women who experience disrespect and abuse are less likely to seek skilled health care in the future.

Studies shows that violating women’s rights during childbirth leads women to distrust health care providers and facilities. These women are not only less likely to seek out maternity care - such as postnatal and emergency obstetric care—but other health services as well, such as family planning. What Causes Disrespect & Abuse during Maternity Care Services? The contributing factors to the disrespect and abuse faced by pregnant women can be broadly categorized into three categories: Health System Work culture normalizing D&A Lack of awareness of patient’s rights, gender sensitization,

clinical empathy skills and humanism among health care providers.

Lack of communication and interpersonal skills of health care providers.

Health Care Providers’ prejudices based on specific patient attributes

Inadequate infrastructure leading to poor working environment Staff shortages leading to high stress Poor supportive supervision Weak implementation of standards and quality of care

guidelines

Individual and Community Acceptance of disrespect and abuse as ‘normal’ Poor birth preparedness

Lack of autonomy & empowerment of women Low socio-economic status of women

Policy and Governance Lack of laws, policies, enforcement and legal redress. Lack of awareness and implications of international conventions

and guidelines

Lack of accountability mechanisms for redressal Insufficient funding for maternal health care

How to Address Disrespect & Abuse during Maternity Care Services? Successful efforts to promote RMC must consider and address the systems, structures, attitudes, and behaviors that are generating D&A within a given environment. Combating disrespect and abuse requires concentrated effort across sectors Facility Level: Observe a ‘zero-tolerance’ rule for all forms of disrespect and

abuse in the facilities. Allow a birth companion to be present with the women during

labour and delivery. Training for service providers on interpersonal skills, clinical

empathy, patients’ rights, behaviour, communication and gender sensitivity.

A robust accountability mechanism which includes proper redressal mechanisms.

An enabling environment for service providers to provide RMC. This includes provision of infrastructure (bed, screens for privacy, hygiene, medical supplies, blankets etc); adequate number of healthcare personnel to lower provider: patient ratio and; supportive supervision.

Implementation of quality of care protocols, standards and guidelines such as the LaQshya - Government of India’s initiative to improve quality of care in labour rooms of public health facilities across India. Ensure effective and timely implementation of LaQshya, especially the RMC indicators.

Policy Level: Making RMC an integral component of all maternal health

policies and programs. Inclusion of RMC in training and curricula of health care

providers. Monitor and assess RMC as part of all national and state-level

quality assurance guidelines and protocols.

Ensure a good working infrastructure (beds, screens, hygiene, medical supplies, blankets and sheets) and adequate number of service providers (doctors, nurses) to create an enabling environment for health care services be given with dignity and respect

Community Level: Overcome normalization of disrespect and abuse by enabling the

community to expect and demand for RMC as their basic right. Promotion of birth preparedness among women and

communities.

Join Us: Help ensure that every woman’s right to respectful maternity care is upheld. Compiled by Tina Ravi for The White Ribbon Alliance for Safe Motherhood, India
