It is noted that since 1991 the criteria has changed and performing surgeries for weight loss in patients with a BMI above 30 is accepted more and more each day.

Obesity Surgery candidates must undergo multiple laboratory studies, and an evaluation made by a group of medical professionals including a surgeon, internist, psychiatrist, anesthesiologist, and nutritionist, who must come to a consensus regarding the candidate.

What is the Adjustable Gastric Band?

The gastric band is the most recent bariatric surgery technique, and the only one that is adjustable reversible operation for treatment of obesity. It consists of a control system of food consumption by implanting an INFLATABLE SILICONE flexible ring at the top of the stomach, so that its size is reduced to a small cavity. This is a laparoscopic surgery.The gastric band operation is performed completely as a laparoscopic surgery. The procedure requires general anesthesia and takes about one hour.

Band Adjustments

During the operation, the gastric band is placed completely flat. To modify the internal diameter of the gastric band (inflate it), saline solution is introduced into the band.

This is accomplished by inserting a special needle through the skin into the port. The first adjustment is made approximately 4 to 6 weeks after the operation. Future adjustments are programmed according to the patient’s needs and as the attending physician and his team deem necessary.

Methods

This procedure promotes weight loss by two methods:

• The first method is restriction. The band is placed at the top of the stomach and forms a small gastric pouch to limit the amount of food eaten.

• The second, is that the movement of the food through the band is slow, producing a feeling of fullness with small portions of food.

Weight Loss Expectations

Most patients who undergo this procedure lose about 70% of their excess weight in an average of 2 years. This weight loss depends on the patient’s compliance with the treatment, improved eating habits, exercise, among others.

Gastric Band Surgery Benefits

• This is a laparoscopic surgery.

• It is the only adjustable reversible surgery for obesity.

• It does not require cutting or stapling the stomach.

• It does not alter the normal function (physiology), of the stomach.• No artificial unions are created (anastomosis); therefore there is no risk of

them tightening (stenosis).• It is an AMBULATORY SURGERY.

• A future procedure can be done.

• The administration of supplements and vitamins are not required.• The time under anesthesia is shorter.

• In addition to the surgery, consistent breathing exercises should be done. Pain medication will be administered as necessary.

• The volume of fluid intake will gradually increase. A liquid diet must be followed until advised otherwise by your physician, approximately one week after returning home.

• In the first post-op visit, the surgeon will discuss with the patient any changes and continue to guide the patient in later visits.

• It is recommended that patients walk and perform light physical exercise as well as continue the breathing exercises at home after surgery.

• Pain after laparoscopic surgery is usually mild.

• You should be able to resume most normal activities after 5 days, including showering independently, driving, climbing stairs, working and exercising lightly.

• Physical therapists from Dr. Esmeral’s team will be responsible for you adjustment and return to healthy living.

ADJUSTABLE GASTRIC BAND GASTRIC BYPASS SURGERYWhat is Gastric Bypass surgery?

It is a surgical procedure that reduces ingestion and stimulates poor absorption of food, a collective procedure. Like any other bariatric surgery procedure, it is not a fat removal method nor is it cosmetic surgery.

Technique:

The procedure involves using staples to divide the stomach, creating a small pouch which has a capacity of approximately 30 ml (1 ounce), which provides the restrictive part of the surgery. Next, a part of the small intestine is given a “Y” shape by making a series of cuts and unions between the small intestine and the formed stomach. When food is digested it goes directly to the lower part of the small intestine, which completely eliminates food from passing through the stomach, duodenum, and jejunum, where most of the absorption of food takes place.

Weight Loss Expectations:

Most patients who undergo gastric bypass surgery lose between 70% and 90% of their excess weight in an average of 1 year, losing close to 95% 5 years after the surgery.

¿WHAT IS A SLEEVE GASTRECTOMY?The Sleeve Gastrectomy is a restrictive procedure, meaning that it significantly reduces the amount of food ingested by the patient. It is gaining in importance on a global level, because in addition to being fast, it is a safe and effective technique, generally very well tolerated by the patient, and it has a low complication rate.

Technique

This surgery involves sectioning off and removing two-thirds of the stomach, about 70% to 80%, creating a longitudinal reservoir with a capacity of about 100ml or 4 oz. As this procedure requires the removal of a part of the stomach, it is considered a permanent and irreversible procedure.

This is a laparoscopic surgery, performed by placing a tube into the stomach to adjust the size of the reservoir using surgical staples. Following this, approximately 80% of the stomach is removed and a suture line is placed on the edge of the stomach to reduce the risk of bleeding and drainage from the staples. In addition, two pressure tests are performed to check the size of the stomach that is left, and that there is no leakage or bleeding.

Weight loss expectations

With this procedure, it is expected that within 6 to 12 months the patient will lose at least 60% to 90% of their excess weight. This weight loss depends on the patient’s compliance with the treatment, monthly follow-up appointments with several of Dr. Esmeralda’s team members, and improved eating habits, exercise, among others.

Oral hygiene and periodontics

Oral hygiene is the daily oral care for the health of the mouth and teeth. Healthy teeth not only enable you to look and feel good, they make it possible to eat and speak properly.

It’s very important not only for your mouth, it’s really important for your entire body.

Daily preventive care, including proper brushing and flossing, will help stop problems before they develop and is much less painful, expensive, and worrisome than treating conditions that have been allowed to progress.

You should be careful with their oral hygiene habits, they can cause some reducing gums. There are usually three reasons for reducing gums, brushing the teeth too hard, bacterias or orthodontic treatment. You should be very careful with your brushing and flossing techniques. Bad techniques can cause gum recessions, that could lead to long-term tooth loss.

Fortunately gum recession is highly treatable. Surgery is only one solution in treating the type of gum recession that is caused by gum disease or orthodontic treatment. Gingival grafting is a treatment that involves grafts from neighbouring gums or the roof of the mouth onto the affected areas. This surgical procedure tends to be performed by a Periodontist and is done under local anesthetics. The treated gums will usually heal within a month. Additional sahping is sometimes carried out in order to make sue the gum looks as natural as possible.

The best prevention a patient can undertake for gum recession is to visit the dentist regularly. Any problems they may suffer can then be identified early and are then much more easily treated.

TMJ surgery“Temporomandibular Joint Replacement”

Indications for temporomandibular joint (TMJ) reconstruction include bony ankylosis, failed previous alloplastic and autogenous joint replacement, posttraumatic condylar injury, avascular necrosis, posttumor reconstruction, developmental abnormalities, functional deformity, and severe inflammatory conditions that have failed to resolve with conservative treatments.

Alloplastic joint reconstruction currently offers several advantages compared with autogenous replacements. These advantages include lack of donor morbidity, reduced intraoperative surgical time, immediate functioning, the ability to correct malocclusion (with bilateral replacement), and, most importantly, improved predictability. The most widely used autogenous graft for TMJ reconstruction is the costochondral graft. Compared with alloplastic joint reconstruction, costochondral grafting has a higher complication rate. Complications include graft resorption, overgrowth, and ankylosis. Alloplastic joint reconstruction allows for a stable platform in which retrognathia and facial asymmetry

may be corrected in a single surgical procedure.

Advances in Implantology and the technologies we use today, allow us to give our patients

implants with fixed teeth in one day. In most cases, it is no longer necessary to wait months to have fixed teeth that allow you to chew, look good and talk normally.

Dr. Luciano has been trained rigorously, and thanks to his knowledge he has refined a technique, that allows the patient to leave the clinic with fixed teeth, the very same day of the implant placement. Dr. Luciano Retana was the first specialist performing guided implant surgery in Costa Rica with The Navigator® System.

Through documentation of all cases performed with this technique, equal or better results than with conventional procedures have been achieved at a success rate of 99%.

However, not all patients are candidates for this technique, and

therefore, each patient is carefully evaluated, before any intervention, by advanced technology using 3D CAT Scanning. Traditional dental x-rays most often do not provide sufficient information on the shape and amount of bone to predict the prosthetic outcome of implant treatment.

In preparation for the 3D CAT Scan x-ray, a plastic appliance is fabricated. The patient wears this appliance during the 3D CAT Scan. This allows prosthetic information to

be incorporated on the 3D CAT Scan. Once the medical radiologist processes the scan, it is sent to Dr. Luciano for interpretation and planning. The study can be analyzed on a computer using software program called Sim/plant®. The incorporation of this technology to our practice provides us with three-dimensional information on the height, width and density of the bone, and the location of the mandibular nerve and maxillary sinus, securing the best outcome and show you how your new teeth will look even before treatment. Surgical simulations can be done on the computer, so the exact size and ideal location for the implant is known. In essence, your treatment is completed on the simulator pre-

surgically. Recent computer technology gives us a customized surgical guide, electronically constructed. This surgical guide is the link between virtual implant planning, and actual treatment, hence its immense advantages over traditional methods. Free-hand implant dentistry becomes a thing of the past.

With this method gums can often be left intact for faster healing helping to a much more comfortable post-operative stage. It is a painless dental implant treatment, providing safety, predictability, maximum comfort, and the most important, no surprises.
