SPINE SURGERY IN THE 21ST CENTURY

The technological advances in spinal surgery over the last decade have been dramatic. Many of the instrumentation systems and techniques that are used by spine surgeons today were not even de-veloped during my Fellowship training 14 years ago. Fortunately, our ability to learn and perfect these techniques has also improved with the availability of ca-daver-based training facilities and surgeon mentoring programs. Surgeons that have the desire to remain on the cutting edge of these technological advancements have a myriad of resources available to assist them in becoming proficient in perform-ing new and often more complex proce-dures. I was very fortunate to have been trained by a surgeon who foresaw the need to remain current with the many advance-ments in the treatment of spinal disorders that we have seen in the last decade. Dr.

Alan Goodrich was Chief Of Spine Surgery at the Medical College Of Georgia during my residency training. He always stressed to all his resident trainees the need to remain current with all available knowledge and techniques that can improve patient care and outcomes in the treatment of disorders of the spine. Dr. Goodrich is now in private practice in Augusta, Georgia, and continues to re-main on the cutting edge in spine technology. He is nationally known for his work on the evolving lateral interbody fusion technique, or XLIF procedure. My patients and I have benefited greatly from his continued generosity in sharing his knowledge and experience with me.

Many of the improvements in spinal surgery have centered around the development of minimally invasive surgical techniques (MIS). MIS surgery utilizes smaller incisions and specially developed retractors and instruments. Procedures that 10 years ago re-quired long hospital stays and months of painful recovery, with large incisions, now require one or two days in the hospital, with recovery in only weeks. Scars resulting from an MIS procedure are small, and postoperative pain has been drastically reduced when compared to more traditional “open” procedures. Not all patients are candidates for MIS surgery. Each case needs to be evaluated by a surgeon with experience in these often complex techniques. Extreme lateral interbody fusion (XLIF) and minimally-invasive transfo-raminal interbody fusion (MISTLIF) are two such procedures that I have utilized extensively in my practice in recent years.

XLIF has become a common technique to help patients with spinal deformity or severe degenerative lumbar conditions that have failed all types of conservative treatment. It involves a lateral approach through the patient’s side. A small skin incision can be used because of the advancements in retractor systems with high-powered fiberoptic light sources. The procedure cannot be utilized in all cases, but XLIF is performed in about one to two hours and is associated with low blood loss and quick recoveries when compared to traditional open posterior fusion. About two-thirds of the lumbar disk can be removed through the patient’s side, and a plastic spacer is placed in-between the vertebra to facilitate correction of deformity and fusion. Posterior fusion with placement of rods and screws can also be performed using MIS techniques. The newest rod and pedicle screw systems are now able to be placed through small per-cutaneous incisions utilizing similar retractors and lighting systems used in XLIF.

All surgery has potential risks. Conservative measures such as physical therapy, nonsteroidal medicines, activity modification, injections and appropriate chiropractic care should all be exhausted prior to surgery even being considered in all but the most serious spinal conditions. MIS surgery does have some disadvantages when compared to open procedures, and is not appropriate in all cir-cumstances. Potential disadvantages include increased operative time and steep learning curves for surgeons. Less visualization dur-ing surgery and higher radiation exposure for surgeon and patient are also disadvantages. The advantages, as previously mentioned, include:

1. Small incisions with less postoperative pain.2. Less blood loss.3. Shorter hospital stays.4. Quicker return to daily activities.

Patients considering a minimally invasive spinal procedure should consult with their surgeon and discuss all potential risks, goals of the proposed procedure, chance of success and expected recovery course. All appropriate nonoperative treatment options should be exhausted. While all patients are not candidates for MIS surgery, continued technological advancements are making these proce-dures much more likely to be an option in the treatment of common spinal problems in the 21st century.

We sometimes hear the term “work ethic” as it applies to the way a person performs his or her job - good or bad. The

older generation complains it no longer exists, business owners prize it in their employees, caring parents hope to instill it in their children, and “lay-abouts” don’t even know what it is. So what really is the meaning of this phrase, anyway? I would like to describe it as a “willingness to step forward and do what needs to be done, even when no one is looking”. It becomes a habit.

Over the years I have developed a desire to have a good work ethic, although I certainly didn’t start out that way. Like everything else in my life, it had to come the hard way. At age twelve, I had a paper route, delivering about a hundred papers each day after school. If I accidentally missed even one house, that customer would report the offense to the newspaper office. I was then made to ped-dle all the way back to that neighborhood to deliver the missing paper. (Sometimes a dog would run off with it, but I was still held responsible!) At that point in my life I didn’t realize those situations were building my work ethic. A few years later, when working at our family run business, the Tastee Freez, I left a sandwich out of a good customers “to go” bag. When he got home and reported the missing burger, I was made to deliver it to his home, several miles away. (At least I had finally graduated from a bicycle to a car!) It wasn’t that I wanted to do it, but there was no other alter-native if I was to keep my job. I didn’t understand it then, however, these experiences were adding to the building blocks of my character - my work ethic.

By the time I became a business owner, the work ethic was becoming a habit, something I wanted to do. One profound example, which I will always remember, involved a home my company constructed on Pinson street for Miss Willie Boyd, a retired school teacher. It was Christmas eve and I was at home with my wife and our two young daughters when Miss Boyd called me crying. A pipe had burst, flooding her kitchen with about two inches of water, and, at her age and living alone, she was helpless. I guess that work ethic, which had been developing in me over the years, “kicked in”. I drove to her home with towels and bucket, cleaned up the mess, and got home about the time my daughters were going to bed. This willingness to “do the right thing” had now become a part of my character. By the way, a good work ethic will not necessarily make you succeed in business, but it can certainly, in part, make up for some mistakes.

Every day I connect with people in our community involved with the commerce of products and services. Although there are always some exceptions, I believe the majority of this present generation does have the desire to do a good job. I see it everywhere. These people are trying to develop a good work ethic, whether for their own business or for their employer. And yes, “even when no one is looking”!

P.S.: Here’s what my high school teachers used to say about my work ethic. “He is a “model student” - a model of what not to do”! Perhaps I was a “late bloomer”.

Local Family In Need~ Fund Established For Family of Burn Victims

Two Meriwether County residents, Jennifer Weaver and Billy Reddock, remain at Grady Memorial Hospi-tal’s burn unit following a grease fire at their Greenville home. Jennifer has third degree burns over 95 percent of her body, according to her sister Connie Thompson. Billy Reddock reportedly suffered burns over 25-30 percent of his body.

Thompson, with the help of her friend Shannon Mc-Daniel, has started a fund in Weaver’s name to help with medical expenses and to provide for her three children, who are 5, 7 and 12 years old. According to Thompson, Weaver owned her own business and was the sole source of income for her family. Donations are being accepted at United Bank in Newnan, located at 61 Bullsboro Drive. Donations can be made in person at the counter, in the night drop or by mail. An account has been opened in the name, “Connie Thompson for the benefit of Jennifer Weaver.” All proceeds will ben-efit Weaver or her immediate family.

NEWNAN, Ga. (June 23, 2011)—A recent study by the Center for Disease Control reveals that 29 per-cent of Americans get less than the recommended minimum of seven hours of sleep each night. To raise awareness of the need for better sleep, Pied-mont Newnan Hospital is teaming up with the American Sleep Association to establish a local Alert, Well, and Keeping Energetic (A.W.A.K.E.) support group.

The group’s first quarterly meeting is open to friends, family members and patients with sleep apnea and will be led by board-certified physi-cian, Vijay Patel, M.D. “Understanding Sleep Apnea and its Treatment Options” will be held Tuesday, June 28 at 6 p.m. at the Sleep Center located at 20 Francis Way, Suite 150 in Sharpsburg, Ga.

The A.W.A.K.E. network has been creating awareness about sleep apnea since 1990. Groups are located throughout the country in almost all 50 states that provide a learning environment about keeping yourself in a healthy state while suffering from sleep apnea. Meetings are coordinated and guest speakers are invited to these groups and top-ics may include advice on complying with CPAP therapy, legal issues affecting those with sleep ap-nea, weight loss and new research findings.

Obstructive Sleep Apnea is something that some people have never heard of, but many peo-ple suffer from. It is estimated that 85 percent of Americans that have OSA are undiagnosed, and continuing to suffer from this disorder without proper medical attention can have severe medical repercussions.

OSA has been shown to be a primary factor in hypertension and is implicated in the develop-ment of cardiac arrhythmias. Other medical prob-lems such as coronary artery disease, strokes, as well as other heart failure and cardiac issues can arise.

Being educated and proactive about this dis-order is the primary goal of the A.W.A.K.E. net-works. With the availability of these support groups, patients have a better understanding of their disorder and the steps they need to take to better themselves.

Legal Briefs: Bill Stemberger

As trial attorneys, my partner and I are in both the State and Superior courts weekly. At times we are required to try cases in front of a jury. Today’s article deals with the jury trial process.

The first step in a jury trial is the “voir dire”, french words that mean “to speak the truth”.This is better known as jury selection. Both the plaintiff and defense attorneys have the opportunity to question prospec-tive jurors about their background and potential biases before being cho-sen to sit on the jury. Attorneys are allowed to challenge a prospective juror “for cause” if that person says or otherwise expresses a bias against the attorney’s case. Each attorney can also exercise a limited number of challenges to particular jurors for which no reason is required.

After the jury is selected they are then “sworn in” and opening state-ments are presented. Each attorney has the opportunity to discuss the facts regarding their side of the case. Anything said in the opening state-ments is not evidence, but just each party’s attempt to explain their side of the story.

After opening statements, the evidence is presented. The plaintiff ’s side presents their evidence first which is called direct examination. Once a person has gone through direct exam, the opposing party has a right to cross examine that person regarding their testimony. This process is con-tinued through each of the plaintiff ’s witnesses until the plaintiff closes. At that point in time the defendant has a right to present evidence if they so chose. If the defendant presents witnesses, the plaintiff ’s attorney then has an opportunity to cross examine the defense witnesses also.

At the end of all the evidence, the attorneys are given the opportunity to present closing arguments. Closing arguments are the attorney’s chance to persuade the jury to their side of the case by reviewing evidence and by the use of exhibits as approved by the court.

Once both the plaintiff and the defense attorneys complete their clos-ing arguments, the judge would begin reading jury charges. Jury charges are specific laws and rules a juror should follow when the jury is deciding the case. Such charges include burden of proof, definitions of negligence, credibility of witnesses, and the law applicable to the particular case being tried.

At the close of the jury charges, the jury is then sent back into the jury room to deliberate and the jury receives any documents which have been entered into evidence. After the jury deliberation and reaching a verdict, the jurors return to the courtroom and the verdict is read to the parties.

ICCA will be placing ads in newspapers, radio and running television spots this week asking people to bring in any old silver and gold coins made before 1965. Those that do bring in their coins will be able to speak with collectors one on one and have their coins looked at with an expert set of eyes. With the help of these ICCA members offers will be made to those that have coins made before 1965. Offers will be made based on silver or gold content and the rarity of the coins. All coins made before 1965 will be examined and purchased including gold coins, silver coins, silver dollars, all types of nickels and pennies, Those that decide to sell their coins will be paid on the spot.

If you are like a lot of people you might have a few old coins or even a coffee can full lying around. If you have ever wondered what they are worth now might be your chance to find out and even sell them if you choose. They could be worth a lot according to the International Coin Collectors Association also known as ICCA. Collectors will pay a fortune for some coins and currency for their collections. If they are rare enough one coin could be worth over $100,000 according to Eric Helms coin collector and ICCA member. One ultra rare dime an 1894S Barber sold for a record $1.9 million to a collector in July of 2007. While that is an extreme example many rare and valuable coins are stashed away in dresser drawers or lock boxes around the country. The ICCA and its collector members have organized a traveling event in search of all types of coins and currency. Even common coins can be worth a significant amount due to the high price of silver and gold. says Helms, even Washington quarters and Roosevelt dimes and worth many times their face value. Recent silver markets have driven the price up on even common coins made of silver. Helms explains: all half dollars, quarter and dimes made before 1965 contain 90% silver and are

sought after any time silver prices rise. Right now it’s a sellers market he said.

The rarest coins these collectors are looking for include $20, $10, $5 and $2 1/2 gold coins and any coin made before 1850. These coins always bring big premiums according to the ICCA. Silver dollars are also very sought after nowadays.

Other types of items the ICCA will be purchasing during this event include U.S. currency, gold bullion, investment gold, silver bars, silver rounds, proof sets, etc. Even foreign coins are sought after and will be purchased.

Also at this event anyone can sell their gold jewelry, dental gold or anything made of gold on the spot. Gold is currently trading at over $1,400.00 per ounce near an all time high. Bring anything you think might be gold and the collectors will examine, test and price it for free. If you decide to sell you will be paid on the spot – it has been an unknown fact that coin dealers have always paid more for jewelry and scrap gold than other jewelers and pawn brokers.

So whether you have one coin you think might be valuable or a large collection you recently inherented you can talk to these collectors for free and if your lucky you may have a rarity worth thousands. Either way there is nothing to lose and it sounds like fun!

Grantville Mosquito☛ This family needs our help: Grantville resident Con-nie Thompson’s sister was seriously injured on Sunday, June 12. Jennifer was cooking and the skillet that she

was cooking in caught fire. She called her husband in to help her and when he picked up the skillet to take it out he slipped on the

grease that had already spilled on the floor. The hot grease covered both of them. He sustained burns over 30% of his body and underwent surgery that day. Jennifer was burned over 95% of her body including some internal burns. She is in a clinically induced coma and has recently had a 5 hour surgery. She is only 29

and has three children. She was the sole provider for her family because her hus-band has just been laid off. Her family has set up an account at United Bank on

Drying PlantsMost of us who love gardening would say that there is no good

substitute for fresh plants and flowers. However, the time, work and cost of fresh flower arrangements can be burdensome. Long lasting dried arrangements can be the perfect solution. Growers can profit from the need of busy people to have more long lasting decorations for their homes. Seasonal changes offer variety as well as special holiday themes.

There are a number of methods of drying flowers and foliages, pods, cones and seed heads and branches. Air drying, water drying, sand, borax, cat liter, silica gel, alum, glycerine, or using a microwave or pressing can be used for different types of plant materials.

Air drying is done by hanging small branches of plants upside down until thoroughly dried. A dark, warm, dry place, free of humidity, with good air circulation is needed ~ such as an attic, closet, basement, barn, loft or garage. Pick the plants on a dry day around mid-day. One method is to attach bunches to coat hangers with rubber bands. Drying should take one to two weeks. Store dried materi-als in a dry, dark place.

Some plants good for air drying are: Baby’s Breath, Bells of Ireland, Cockscomb, Hydrangea, Larkspur, Statice, Blue Salvia, Strawflower, Tansy, Yarrow, Pampas Grass Plumes, Wheat, Cat Tails, Dock, Chi-nese Lantern, Honesty, Mullein, Peony, Goldenrod. Leaves such as “Silver King” Artemisia, Aspidistra, magnolia or iris foliage dry well. Okra pods, bean pods, garlic heads, lavendar, rosemary, lotus pods, sweetgum balls, wisteria pods, trumpet vine seed pods, cottonwood tree bolls, and nandina berries or china tree berries all dry easily. Pine cones and corn husks are excellent to dry this way as is sumac and Queen Anne’s lace. Some of these plant materials benefit from a spray of lacquer, hair spray, or commercial plant sealers to help preserve them longer.

Corn husks can be made into various types of flower forms. Pine cones can be cut in half to make rosette forms. Small magnolia leaves can be combined with a cone to form ‘flowers’. Artichokes can be dried and used as flower forms. Dried materials may be left natural; brushed with pastels and sealed; spray painted; or dyed with fabric dyes to achieve a softer appearance.

Dried plant materials can be com-bined in arrangements using sahara (similar to oasis) or chicken wire to hold in position. They can be used to make seasonal wreaths or wreaths and swags of herbs for the kitchen. Topiar-ies may be made using a container of choice (with plaster of paris) to hold a branch topped by a styrofoam or chick-en wire ball.

Most folks who have lived in Coweta County for any length of time surely know the name “Christo-pher”. John and his brother, Frank, have operated Christopher Brothers Land Surveyors for about as long as I can remember. They have probably measured and recorded at least 80 percent of the land boundaries around here. And anything they may have missed was certainly surveyed by their grandfather, Mr. T.Y. Mattox, many years before they came along in the business.

Diane Christopher has had a love of animals all her life, but not just dogs. She and John have traveled around the country competing in Cutting horse competition for several years. They now live on the family farm near Sharpsburg which has been in the Christopher family since the civil War.

Diane decided there may be an opportunity to make a business of her love for dogs and still stay near home. In early 2010, she and John converted a barn on their prop-erty into an overnight boarding home for dogs, and on Labor Day 2010, The Dog Barn was open for business! A few weeks ago, I visit-ed their business to get information about an article I wanted to pub-lish, which I thought would be of interest to our readers. I was im-pressed, to say the least. Both the “overnight guest” areas and the office area were absolutely clean, fresh and bright. Each dog has its own locker to hold special food, toys, needed medications and other items. There are eighteen 5x18 indoor/outdoor type “suites” connected to full size outdoor enclosures. There is also central heat-ing and A/C which is a must for any “Discriminating Canine”. Dogs are housed separately, unless they are from the same home.

Diane is especially proud of their good facility and the extra care they offer. “John and I live just a few hundred feet away”, says Diane, “allowing us to provide a safe, loving and caring atmosphere 24 hours a day”.
