On behalf of the Kentucky Dental Association Executive Board, Council on Annual Session and staff, welcome to the 2017 KDA Annual Meeting, Thursday, August 24th – Sunday, August 27th!!

Dr. Mark Moats, as Scientific Session Chairman, has worked very hard to tailor a lineup of continuing education courses that you, as members, have asked to be highlighted at the Annual Meeting. There will be more CE offerings on and around the exhibit floor space, allowing better accommodation of dentists, hygienists, assistants and administrative staff members to “do both” (courses and exhibit space) more conveniently. Mark has worked extremely hard since we are in a new location to make sure the quality of CE offerings are some of the best ever offered at an annual meeting.

On Thursday, there will be an “Opening Session” to help set the tone for member involvement and dentist-to-dentist interaction, in an effort to “bring the profession together again”, as we all work together for the overall common good of dentistry. First, on Thursday morning at 9:30 a.m., Dr.

Lee Ann Brady will speak on “Today’s Top Clinical Tips” - she is sponsored by Heraeus Kulzer & GC America. At 11:30 a.m., the 2017 William Marcus Randall Memorial Luncheon will begin & awards will be presented. Finally, at 12:30 p.m., Dr. Jim Willey, Director of the ADA Practice Institute will speak at our Opening Session.

A party has been planned for Friday night from 7:00 p.m. until 10:00 p.m., complete with food, beverage and music/entertainment. Dental students, new dentists and matured dentists (of all ages!!) will come together to celebrate our profession, rekindle relationships and start new friendships that will last for years to come. Please make plans to join us Friday evening before you head to the casino.

We hope to see everyone August 24th through August 27th. Please bring your staff and family to French Lick Resort for the 2017 KDA Annual Meeting!

On-Site RegistrationLocated right outside of the Windsor Ballroom on Level 2 of the French Lick Springs Hotel, the registration desk is conveniently open during the following hours:

As a member benefit, registration is free of charge to KDA members.

Ticketing PolicyAll attendees who pre-register for the 2017 meeting will have their badge and/or tickets mailed in advance of the meeting. You do not have to purchase tickets in order to pre-register for the meeting. All tickets and badges will be mailed to the address indicated on the order form. Please note that plastic badge holders will not be mailed and will be convenient-ly located on-site. Completed forms must be postmarked by August 11, 2017. Tickets may be purchased by MasterCard, Visa, check or money order. Fees will be assessed, up to the maximum allowed by law, for checks not honored by the bank. Tickets will be sold at the meeting depending on availability. There will be a $10 per ticket increase if purchased at the door.

Ticket payment will not be refunded after 5:00 p.m. (EDT) Tuesday, August 22, 2017.

Please note that the KDA will not replace lost tickets!

CE CreditsContinuing education courses provided by the Kentucky Dental Associa-tion are recognized by the Kentucky Board of Dentistry and most other state licensing bodies. Continuing education requirements differ from state to state. Check with your state for specific requirements. Continu-ing education credit hours are shown on each synopsis.

Board of Dentistry: Continuing education credit hours are earned by attending courses presented at the meeting. Each course synopsis and course ticket lists the hours earned for attending. Members will be provided with a ticket allowing entry to each course, as well as proof of attendance. The course ID # given at the end of the course must be recorded on the ticket for it to be valid for CE credit.

Note: It is the attendees’ responsibility to maintain these cards for their records.

Academy of General Dentistry: The Kentucky Dental Association is designated as an Approved PACE Program Provider by the Academy of General Dentistry. The formal continuing education programs of this program provider are accepted by AGD for Fellowship, Mastership and membership maintenance

credit. Approval does not imply acceptance by a state or provincial board of dentistry. Application is pending and will be retroactive to 1/1/17.

They may also have premium give-a-ways on hand to distribute to the audience at the end of their 15 minutes.

Participating vendors will distribute a sign up sheet to use for the door prize drawing at their booth.

Door prize winners must be present to win. A list of participating vendors will be posted at the Speaker Stage inside the exhibit hall.

Table Cl inic & Hobby Show

Participate in Our Annual Table Clinic and Hobby Show!You are invited to participate in the annual Table Clinic and Hobby Show during the 2017 Kentucky Meeting to be held on Saturday, August 26, 2017 at French Lick Springs Hotel.

The Kentucky Dental Association will offer three separate clinic areas. One area is reserved for clinical clinics and information, the second area is for hobbies and crafts only and the third is for research clinics. Let’s work together to make this the best year ever! There are cash awards for each area (clinical, hobby and research).

1ST Place is $200.00

2ND Place is $100.00

3RD Place is $50.00

In past years this has been a very popular and well-attended feature at the annual meeting. There is so much information to share. This would be a great office project with everyone participating or hygienists and as-sistants could work together or separately to represent your office. Also, individuals are very welcome to pres-ent their clinic. There is ample time to put together

your program. Please encourage your spouse, family members and auxiliary personnel to be a part of the fun!

Table clinics may be anything you feel would be of interest to the profession and should be able to be presented in a minimal amount of time (5-10 minutes). New and improved techniques, time saving ideas, case presentations, innovative practice tips, collectibles and crafted items represent just a few of the possibilities.

I know there are many talented people and many wonderful hobbies in our association. Let’s get together and share our recreational interests as well as enjoy this wonderful profession.

The Table Clinic and Hobby Show will be Saturday, August 26, 2017 from Noon – 2:00 p.m. You are re-quired to be present and remain with your clinic for the entire time. One six-foot table or a tack board will be provided (per clinic) – no additional space is available. Please indicate whether you require a tack board or table for your presentation on the reservation form.

The set up time is 11:00 a.m. – NOON. Please set up your clinic at least one half hour prior to the session. The clinics will be assigned a particular space as reser-vations are received. Look for your name and topic displayed on each table. For students or groups, please identify the primary participant (one person) and then name each individual participant. Students use name only, not intended degree.

No Audiovisual Equipment Will Be Provided by the KDA!

So that the committee may make the necessary prepa-rations to ensure that your clinic needs are met, it is requested that you complete the application and return it to the Kentucky Dental Association office by August 1, 2017.

Registrations are processed in the order received. Fees are payable to the Kentucky Dental Associa-tion in U.S. funds by check, money order, MasterCard or Visa. Credit card users must supply a new credit card number within 24-hours of denial notification or ordered tickets will be returned to inventory. There has been a new pathologic entity which was identified in the literature in 2003. Patients were found to have painful exposed bony areas of the jaws which occurred after simple dental procedures such as extractions. There were also reports of spontaneous exposures of bone in the jaws which were symptomatic. The common link was to the drug class of bisphosphonates. Bisphosphonates were given intravenously to cancer patients to combat the hypercalcemia often seen in multiple myeloma and metastatic cancers. This new condition was named osteonecrosis of the jaws (ONJ).

There are also oral forms of bisphophonate drugs such as Fosamax, Actonel and Boniva. Although there is a higher incidence of osteonecrosis associated with IV administration of bisphosphonates, we have seen cases of ONJ associated with oral bisphosphonates that were prescribed to treat osteoporosis.

The lecture will review what we currently know about bisphosphonates, including recommendations for dental management and prevention of ONJ. We will also discuss what is uncertain at this early stage of the new pathologic entity.

Objectives:

1. The participant will understand the new pathologic entity named osteonecrosis of the jaws (ONJ) and the link to bisphosphonate drugs.2. The participant will understand how modifications may have to be made in the treatment plan to accommodate bisphosphonate patients.3. The participant will understand the current recommendations in prevention, management and treatment of ONJ.

The biggest disappointment for patients and practitioners is failure of dental implants after investing the time and money for the treatment. The Kentucky Board of Dentistry amended the minimal requirements of dental auxiliary personnel prior to taking dental radiographs under a dentist’s direct supervision (201 KAR 8:570E). All dental auxiliary personnel must satisfy two (2) requirements before taking x-rays in a dental office:

1. Didactic - Completion of a course in radiation safety: This can be verified by providing proof of completion of the Radiography Section of the Dental Assistants National Board Examination or attending an approved continuing education (CE) course (6 hours) provided by a certified pre-senter.

2. Practical - A practicum course in x-ray technique (4 hours minimum): This is verified by a dentist’s written documentation of a student’s comple-tion of four (4) hours of instruction in dental radiography technique in the practice or by attending a four (4) hour continuing education course in radiography technique sponsored by the Board.

This course satisfies the didactic requirements of the Kentucky Board of Dentistry. The course contains both theoretical and practical clinical topics including:

• Basic principles and concepts of radiation• Exposure factors and how they affect the radiographic image• Image receptors and processing of radiographs• X-ray interaction with matter and potential biological effects• Radiation safety for both patient and operator• Principles of intraoral (bitewing/periapical/occlusal) imaging and clinical technique• Criteria for diagnostic radiographs - technique and processing error recognition and correction• Radiographic film mounting and labeling and record keeping• Principles of Quality Assurance and implementation into clinical practice• Occupational considerations including infection control principles, waste management principles and legal concerns• Extraoral Radiography

Successful completion of the course requires a 75% pass in a multiple-choice question examination administered at the end of the course. Participants who successfully complete the examination will receive a certificate of completion and after fulfilling requirement (2) above are allowed to expose radiographs on patients under the direct supervision of a dentist.

Restoration of the severely worn dentition is one of the most challeng-ing procedures in dentistry. In order to successfully restore and maintain the teeth, one must gain insight into how the teeth arrived at this state of destruction. Tooth wear can result from abrasion, attrition and erosion. Research has shown that these wear mechanisms rarely act alone, and there is nearly always a combination of the processes. Evaluation and diagnosis should account for the patient’s diet, history of eating and gastric disorders and influence of airway on the masticatory structures, along with the present state of the occlusion and its functional parameters.

Once a complete understanding of the etiology of the dentition’s present state is appreciated, a treatment plan can be formulated, taking into account the number of teeth to be treated, condylar position, space availability, the vertical dimension of occlusion (VDO) and the choice of restorative material.

This course will give an overview of factors that should be considered for these patients. It will also review treatment parameters that should be con-sidered, along with material selections that may help in long-term success with these cases.

Participants will gain knowledge in the following:

online instruction at www.leeannbrady.com. She practices in Glendale, Arizona, is a member of the editorial board for the Journal of Cosmetic Dentistry, Inside Dentistry and Dentaltown Magazine.

Patty Bonasso Byrd, RDH, BS is currently co-director of Infection Control at the Univer-sity of Louisville School of Dentistry as well as in private practice.

In dental hygiene since 1978, Patty is a graduate of the University of Louisville Dental Hygiene Program. She received her BS in Occupational Training and Development in 2002. She is an ac-tive member of the American Dental Hygienists’ Association as well as past president of KDHA.

Having an accomplished background in several dental hygiene arenas, such as clinician, national speaker, corporate educator, faculty, sales and author, she brings a wealth of experience and knowledge to her courses.

Dr. Jerry Caudill is the Kentucky State Dental Director for Avesis Incorporated, a Guardian Company. Avesis administers the dental benefits for four of Kentucky’s five MCOs administering an aggregate 1.1 million Kentucky Medicaid patients for Aetna, Humana-CareSource, Passport and WellCare. He is a graduate of the UK College of Dentistry, a Fellow and Master of the Academy of General Dentistry and a Certified Dental Consul-tant by the AADC. Dr. Caudill is a board exam-iner for the Council of Interstate Testing Agencies

Dr. Geoffrey Ball is a practicing general dentist with more than 15 years’ experience, both in private practice and government service. Dr. Ball began his career by completing an AEGD at the University of Louisville 2003. He then went on active duty for United States Air Force, including a combat deployment. Dr. Ball completed a GPR program at University of Kentucky College of Dentistry in 2009. Throughout his career Dr. Ball has been a member of multiple root-cause analysis teams responsible for

identifying what led to patient mishaps or near mishaps. Furthermore, the teams made recommendations of systems which could prevent fu-ture mishaps using SMART metrics. Dr. Ball’s passion for patient safety, in part, comes from medical mishaps that he has seen in his own family. Dr. Ball is the Past President and CE Chair for the Kentucky Academy of General Dentistry and has been involved with the Kentucky Dental Association since dental school.

Timothy M. Bizga, DDS, FAGD is a general dentist practicing in Cleveland, Ohio. His practice focuses on comprehensive care, with Dr. Bizga’s special interests in implants, cosmetics and facial aesthetics. His background in dentistry is lengthy and diverse. Once a former chairside assistant, he also worked as a dental lab-techni-cian, making his perspective unique among others in the field of dentistry. He graduated from Saint Louis University with a BA in Biology, before receiving his DDS from the University of Michigan

School of Dentistry, at which time he was the recipient of the Francis B. Vedder award for excellence in prosthodontics. He is currently a mem-ber of the American Dental Association and the Academy of General Dentistry. He is a graduating member of the Whitecap Institute, where he received advanced implant training. Dr. Bizga is a Certified John Maxwell Speaker/Trainer/Coach, a certified DISC profile trainer, clinical consultant for The Dental Advisor and member of Catapult Speakers Bureau. (CITA Board) which administers the ADEX Board exam recognized in 46 states and territories, including Kentucky. He has practiced in rural Kentucky, Tennessee and North Carolina and participated in treating Medicaid patients in all three states. Dr. Caudill also practiced seven years in Dubai, UAE and Kuwait. He was the National Dental Director of a multi-state, 120-office DSO and Special Advisor to the Academy of General Dentistry National Taskforce on Corporate Dentistry. He is a former President of the North Carolina AGD and is a member of the ADA, KDA, LDS, AGD, KYAGD, ADEA, IAO, and AADC. In addition, he is board-eligible in the International Association for Orthodontics having completed over 500 CE hours and a five-year course of study in orthodontics from the United States Dental Institute. In 1991, he was among the first general dentists ever licensed in both general anesthesia & conscious sedation in Kentucky. Dr. Caudill has lectured on Medicaid dental administration, both statewide and nationally.

Bruno de Azevedo, DDS, MS is a Diplomate, American Board of Oral and Maxillofa-cial Radiology and an Assistant Professor, Oral and Maxillofacial Radiology at the University of Louisville School of Dentistry in Louisville, Kentucky.

Dr. Alex Fleury received his Masters of Science Degree in Oral Biology and Certificate in Endodon-tics from the University of Pennsylvania School of Dental Medicine. He received his DDS Degree in 1984 from the University of São Paulo. Dr. Fleury served as Director of Predoctoral Endodontics for six years at both, Nova Southeastern University School of Dental Medicine in Florida and Baylor College of Dentistry. He currently serves as As-sistant Professor at Baylor College of Dentistry,

working with Endodontic Residents. Of particular pride is being named the recipient of the “Edward E. Osetek Award” for best new Endodontic Educator of the year by the American Association of Endodontists. In addition, Dr. Fleury is honored to have been the recipient of the “Rich-ard F. Weiss Achievement Award” presented by the Louis I. Grossman Study Club (University of Pennsylvania). He also served on the Scientific Advisory Board of the Journal of Endodontics, and as well serves as the Di-rector of Educational Resources for Real World Endodontics. Dr. Fleury is currently in the full-time private practice of Endodontics in Dallas, Texas.

Jason E. Ford, DMD, MD has been in a group practice in Oral and Maxillofacial Surgery in the Lex-ington, KY area for nine years. He is married to Lisa Ford D.M.D. and they have three children. Dr. Ford attended the University of Kentucky for undergradu-ate, dental school, medical school and his residency. He has served on the Kentucky Board of Dentistry since 2011.

Mr. Fred Frisse IV, CDT, TE is the owner of Frisse Dental Laboratory, which spe-cializes in removable prosthetics and implants. He received his Degree in Dental Technol-ogy in 1976 from Southern Illinois University. He has served the dental community for 35 years as a Certified Dental Technician and is a 10-year member of the Select Study Club and Seattle Study Club. In his free time, Fred

enjoys spending time with his sons, Fred V and Benton. He also enjoys hunting, fishing, clay target shooting and making teeth.

Susan Grammer graduated from the Univer-sity of Louisville from the Dental Hygiene Program. She received her Master’s in Education and taught at the University of Kentucky in an experimental Dental Hygiene Program that was based in Ashland, Kentucky. sisting Program, Ms. Grammer taught in the Dental Hygiene Program until last year. She is now teaching in the Radiology Department at the University of Louisville School of Dentistry and is currently Associate Professor in the Department of Oral Health and Rehabilitation. Ms. Grammer teaches all of the radiology courses in the Dental Hygiene Program, is co-course director of the DMD freshman Radiology course and teaches the laboratory and clinical portion of the DMD Radiology courses. She has also given numerous continuing education courses in Radiation Safety.

Dr. Gerald “Jerry” Grant is currently a Professor of Prosthodontics and Interim Chair of the Oral Health and Rehabilitation Department, University of Louisville, Kentucky and Immediate President of the American Academy of Maxil-lofacial Prosthetics. He recently retired after 33 years of service with the United States Navy as the Service Chief of the 3D Medical Applications Center, Department of Radiology at Walter Reed National Military Medical Center – a center he established for research, development and use of 3D printing and digital design for treatment of

Wounded Warriors world-wide. He received his D.M.D. degree from the University of Louisville, School of Dentistry in 1985, a certificate in Prosthodontics from the Naval Postgraduate Dental School, Bethesda, MD and a Masters from George Washington University in 1995, and a certificate in Maxillofacial Prosthetics from the Naval Postgraduate Dental School in 1999. He is a Diplomate of the American Board of Prosthodontics. Dr. Grant continues his area of research from the military with the University of Louisville in the validation and applications of Advanced Digital Dental Technologies, virtual surgical applications, Advanced Digital Applications in the design and fabrication of medical devices for craniofacial reconstruction, dental restoration and rehabilita-tion, and recently in bio-printing/bio-fabrication.

Dr. Leslie Horn graduated from the University of Kentucky College of Dentistry in 1991, which was the first dental class in the US that was over 50% female. She then finished her residency in 1993 and subsequently became the first female periodontist in Lexington, where she owns and operates Beaumont Periodontics. Dr. Horn is a member of American Dental Associa-tion, American Academy of Periodontology, Southern Academy of Periodontology, Kentucky

Dental Alumni Association and Blue Grass Dental Society, currently serving as Vice President. She has received several awards and honors and has been on a number of mission trips and participated in a volun-teer capacity for other service programs. Dr. Horn currently lectures at various dental schools, study clubs and other dental organizations and events.

Ayaz P. Jafri, DMD is a graduate of the University of Louisville School of Dentistry Class of 1994. Dr. Jafri also received advanced training in a General Practice Residency at the Genesee Hospital in Rochester, NY. Dr. Jafri is also a candidate for the board of Great Shape! Inc. which is the largest dental humanitarian organization in the world. He has also been a Donated Dental Services Dentist in Michigan for 17 years. Dr. Jafri is currently engaged in full-time practice with Mid-Michigan Dental Center in Owosso, MI.

Dr. Jack Kall received his DMD in 1977 from the University of Louisville School of Dentistry. For the next 25 years he served as Dental Director of the Family Health Centers, Inc., a federally-qualified community health center in Louisville, KY. Ad-ditionally, he founded Dental Health Center, his private practice focusing on biological, biocompat-ible dentistry.

In 2012, upon invitation of the Philippines’s Secretary of Health, he gave the presentation “Mercury-free Dentistry in Public Health” at the national conference: Philippines—Towards Mercury-free Dentistry

In 2013, he co-authored “International Academy of Oral Medicine & Toxicology (IAOMT) Position Statement against dental mercury amalgam fillings for medical and dental practitioners, dental students and patients”.

He co-authored a chapter about the health risks of mercury exposure from amalgam fillings in the textbook titled, Epigenetics, the Environment, and Children’s Health across Lifespans, published by Springer in 2016.

Dr. Kall is a member of the American Dental Association, Kentucky Dental Association and the Louisville Dental Society. He is a Fellow of the Academy of General Dentistry and a past President of the Kentucky chapter. He is an Accredited Master of the International Academy of Oral Medicine and Toxicology (IAOMT) and since 1996 has served as the Chairman of its Board of Directors.

Dr. Zafrulla Khan is a Diplomate of the Ameri-can Board of Prosthodontics, Fellow of the American College of Prosthodontics and American Academy of Maxillofacial Prosthetics. He received his Dental Degree from Bangalore University, India and Masters in Oral Biology from University of Louisville.

He completed his Residency in Prosthodontics at State University of New York at Buffalo, NY and Fellowship in Maxillofacial Prosthetics and Dental

Oncology from University of Texas M. D. Anderson Cancer Center, Houston, TX. He is also an Adjunct Professor at Tufts University School of Dental Medicine, Boston, MA. and a visiting Professor at KGF College of Dental Sciences and Hospital, R.G. University of Health Sciences, Karnataka, India.

Dr. Khan’s previous appointments include Director of Post graduate Prosthodontics and Chairman, Department of Prosthodontics at Univer-sity of Louisville, Dental School. He holds Active Medical Staff appoint-ments and is a member of the cancer committee at multiple Hospitals in Louisville, KY. He is an Editorial Review Board Member of several prosthodontic journals. He is an Invited Speaker at many Local, National and International Meetings. He has received various Honors and Induc-tion in Scientific Honor Societies. Ned Kivett, CFP® has spent the last two de-cades providing clients with strategies to successful-ly meet their business and personal financial goals. In that time, he has gained extensive experience, not only with pension and retirement planning, but with complex dental practice transitions, as well. As a practice integration advisor with the firm’s Indianapolis office, he leads client relationships and develops tools and processes to better assist medi-cal professionals in reaching financial freedom.

Ned served as a senior planning consultant with Hufford Advisors until that firm became part of Buckingham in 2014. Before joining Hufford Advisors, Ned worked in management and product development for a multi-national medical device company.

He holds a bachelor’s degree in mechanical engineering from Purdue University and a master’s degree from Purdue University’s Krannert School of Business. In addition, Ned has addressed a wide range of financial planning topics and dental practice transition strategies as a presenter at national dental forums, industry meetings and seminars.

Dr. George Kushner is a graduate of Temple University School of Dentistry. Dr. Kushner received his medical degree from the University of Alabama School of Medicine. He completed his Oral and Maxillofacial Surgery residency at the University of Louisville. Currently, Dr. Kushner is Professor of Oral and Maxillofacial Surgery at the University of Louisville. He is the Director of the Oral and Maxillofacial Surgery Residency program. Dr. Kushner is the author of several publications and articles and lectures extensively throughout the

country. He maintains a faculty practice in oral and maxillofacial surgery through the University of Louisville School of Dentistry.

As both, a former practicing dentist and accoun-tant, Pat Little, DDS, FAGD, CFE brings a unique blend of skill and experience to his pre-sentations, which focus on strategies for reducing financial and regulatory risks.

Pat began his dental career in private practice shortly after dental school and over the next 30 years launched two successful practices, start-ing each from “scratch.” After leaving clinical practice due to a cervical vertebrae condition, Pat completed his accounting and general business

education. While working with a dental CPA firm, he developed a deep interest in the area of fraud and embezzlement. Pat joined Prosperident as a Senior Fraud Examiner and earned the Certified Fraud Examiner credential. Through Prosperident, Pat conducts embezzlement examina-tions on behalf of dentists and advises them on matters related to fraud and embezzlement.

While maintaining his relationship with Prosperident, Pat has expanded his lecturing and consulting services to include topics related to financial integrity and accountability, data security and HIPAA compliance.

Pat is an engaging and entertaining speaker who presents well-informed and technologically up-to-date presentations while converting compli-cated (and sometimes scary) material into plain “English”.

Dr. Megan McHugh earned her dental degree from the University of Kentucky College of Dentistry in 2006. She completed her Master of Science in Orthodontics from the Univer-sity of Kentucky College of Dentistry in 2009. Upon graduation, Dr. McHugh stayed on at the University of Kentucky as an Assistant Profes-sor in the Orthodontic Department. In 2011, Dr. McHugh joined a private practice, White, Greer, and Maggard Orthodontics, in Lexington,

KY where she currently resides. She presently serves as President of the University of Kentucky Orthodontic Alumni Association.

Sanda Moldovan, DDS, MS, CNS is a periodontist and nutritionist, author and consultant on oral health, periodontics, dental implants and integrative nutrition. She is a Diplomate of the American Board of Peri-odontology, practicing in Beverly Hills, CA and Manhattan, NY, with an emphasis on full arch immediate load prostheses. She received her certificate in Periodontics and an MS in Oral Biology from the UCLA School of Dentistry

and a Certificate in Nutrition from CBNS. She is an internationally rec-ognized speaker who continues to inform the public and dentists about the advancements in dental implants and nutrition on different platforms, such as radio, TV and social media.

She has taught courses such as Implants A to Z at the UCLA School of Dentistry since 2005. She is a faculty member at Global Institute for Dental Education (gIDE), where she also performed live surgeries, as well as hand-on courses. Dr. Sanda Moldovan is an editor for the Inside Dentistry magazine and has written several articles on post-operative healing and dental implants. Dr. Mark Nation attended the University of Louisville to obtain an undergraduate degree in Bi-ology. He completed his dental training from the University of Louisville in 2000. He was chosen as ULSD Most Outstanding Graduate in 2000; won the Pierre Fauchard award for dental excellence and was also given the ACP award for outstand-ing achievement in Prosthodontic education. Dr. Nation graduated from the Advanced Graduate Residency Program in Prosthodontics in 2003. Dr. Nation has extensive experience in implant

prosthetics and surgery. He has given numerous lectures and CE cours-es concerning diagnostics, treatment planning, full mouth reconstruction, implant prosthodontics, implant surgical procedures, provisionals, sleep prosthodontics, airway and occlusion. He received his certification for Parenteral Conscious Sedation in May 2010. He is a founding member of Renew Institute: Beyond Dentistry. Dr. Nation is a member of the American College of Prosthodontists (ACP), the flagship organization for the ADA dental specialty of Prosthodontics, Academy of Osseointegra-tion (AO), American Dental Association and the International Congress of Oral Implantologists (ICOI).

Dr. Brian Nový is the Director of Practice Improvement at the DentaQuest Institute and an adjunct associate professor of restorative dentistry. He serves on the ADA Council of Scientific Affairs, acts as the chair of the CAMBRA coalition, is a member of the AGD and a clinical evaluator for the Catapult Group. The California Dental Association Foundation presented him with the Dugoni Award for “Outstanding contributions to dental educa-tion,” and in 2009, the American Dental Associa-

tion awarded Dr. Nový and his office the title, “Adult Preventive Care Practice of the Year.”

Laci L. Phillips discovered at an early age her true passion for speaking and entertaining audiences. As a founding partner at Practice Dynamics she combines her knowledge of dentistry with her passion for teams to deliver customized coaching, workshops and speaking events throughout the country.

Laci began her road to coaching as a chairside assistant while going to college to pursue a dream of one day having her own talk show. William Ken Rich, DMD, CDC, FICD, FACD, lives in Grant County, Kentucky where he began practicing dentistry in 1975, right after his gradua-tion from the University of Louisville Dental School and continues to practice, along with his two sons. He is the Dental Medicaid Director for the state of Kentucky, serves as Chair of the Dental Qual-ity Alliance (DQA), as a member of the American Association of Dental Consultants, and as Chair of

the Medicaid and SCHIP State Dental Directors Association (MSDA). He is a founding board member of the U S National Oral Health Alliance, a former trustee of the ADA, and former president of the Kentucky Dental Association.

Mr. Jeffrey A. Sexton returns to the annual meeting for the 19th year to provide an educational and highly useful session for members. He is the President and Chief Investment Officer of Silver Leaf Capital, LLC, an investment firm located in New York City, where he is also the Portfolio Manager primarily responsible for the day-to-day manage-ment of a market neutral long-short equity hedge fund based in part on his own academic research on momentum he conducted while at the University of

Chicago under the direct tutelage of Professor Eugene Fama, the 2013 recipient of the Nobel Prize in Economic Science. Mr. Sexton earned an

M.B.A. at the University of Chicago; an M.A. in Economic Statecraft from the University of Kentucky; a J.D. from the University of Louisville; and, a B.S. in Accounting from the University of Kentucky. Mr. Sexton is also a member of the Louisville, the Kentucky and the American Bar Associa-tions.

Dr. Brian Shumway is an assistant professor of Oral and Maxillofacial Pathology at the University of Louisville, School of Dentistry. He received his dental degree from The Ohio State University in 2003, followed by one year of advanced education in general dentistry at the Veteran’s Affairs Audie L. Murphy Memorial Hospital in San Antonio, TX. He then completed a residency in Oral and Maxillofacial Pathology and Master’s Degree at The Ohio State University. He is a Diplomate of the American Board of Oral and Maxillofacial Pathology and a

Fellow of the American Academy of Oral and Maxillofacial Pathology. He has an active clinical practice specializing in diagnosis and management of oral diseases and participates in the University of Louisville oral pathol-ogy biopsy service. In collaboration with The Ohio State University and University of North Carolina, Chapel Hill, he and Dr. Kushner are engaged in a clinical trial focused on using freeze-dried black raspberries to treat oral epithelial dysplasia, a precursor to oral cancer.

Dr. Michael Smith graduated from the University of Louisville School of Dentistry and pursued extensive post graduate education in dental rehabilitation in the field of occlusion, full mouth rehabilitation, TMJ disorders, sleep apnea and conscious sedation. His private practice in Phoenix, Arizona is dedicated to aesthetic, contemporary restorative and cosmetic rehabilitation. Dr. Smith instructs other dentists nationally and internationally as a lecturer and mentor for companies involved in advanced dental treatment and diagnosis in TMD,

full mouth reconstruction, and implants.

He is actively involved in numerous organizations and associations dedi-cated to the pursuit of providing patients with extraordinary care. He is currently the faculty director for Clinical Mastery and is co-founder of the Arizona Community Dental Clinic in downtown Phoenix.

Dr. Jamison Spencer is the director of Dental Sleep Medicine for the 28 locations of Lane and Associates Family Dentistry in North Carolina. He also continues to direct the Center for Sleep Apnea and TMJ, his original practice limited to sleep apnea and TMJ problems in Boise, Idaho, with the help of his two partners. Dr. Spencer has personally treated thousands of patients with sleep apnea and TMJ problems, and now focuses on helping dentists help their patients with such problems.

Dr. Spencer is the Past-President of the American Academy of Cranio-facial Pain (AACP), a Diplomate of the American Board of Craniofacial Pain, a Diplomate of the American Board of Dental Sleep Medicine, a Diplomate of the American Board of Craniofacial Dental Sleep Medicine and has a Masters in Craniofacial Pain from Tufts University. He taught head and neck anatomy at Boise State University and is adjunct faculty at the Tufts Craniofacial Pain Center in both the Craniofacial Pain residency and Dental Sleep Medicine programs. Dr. Spencer is also adjunct faculty at the University of the Pacific School of Dentistry, and the University of North Carolina at Chapel Hill. Dr. Spencer has been invited to give

lectures on TMD, Dental Sleep Medicine and head and neck anatomy throughout the United States and Canada, as well as in Central and South America, the Caribbean, Australia, Europe and India. He is a founding faculty member of the AACP’s Institute, the AACP/Tufts Dental Sleep Medicine program, and the UOP Dental Sleep Medicine mini residency. Dr. Spencer is the inventor and patent holder for the “Silent Sleep,” a temporary oral appliance FDA-approved for the treatment of obstructive sleep apnea and snoring. Dr. Spencer also recently released the Spencer Study Club, an online education, mentoring and implementation program to help dentists and their teams help more of their patients with sleep apnea and TMJ disorders.

Dr. Bethany Valachi, PT, DPT, MS, CEAS is author of the book, Practice Dentistry Pain-Free, clinical instructor of ergonomics at OHSU School of Dentistry in Portland, OR, and Doc-tor of Physical Therapy and Certified Ergonom-ic Assessment Specialist. She brings the most current & effective evidence-based education to dental professionals, worldwide, to prevent pain and extend careers. Dr. Valachi has published more than 50 articles in peer-reviewed dental

journals and offers dental ergonomic and exercise DVDs, newsletters, videos and product reviews on her website at www.posturedontics.com.

Dr. Lawrence Wallace is a board certified Oral and Maxillofacial Surgeon who maintained a private practice in Des Plaines, Illinois. He is the developer and founder of the Larell One Step Denture. He has lectured nationally and inter-nationally and has written many articles in dental industry journals. He has volunteered extensively and brought the Larell One Step Denture to the Mission of Mercy free dental clinic program across the country. He has also done multiple interna-tional mission trips. His technique is being taught at many dental schools across the country and is being used in the faculty practice at NYU College

of Dentistry. He is also a consultant to the medical industry in the field of Oral and Maxillofacial Surgery with input in the areas of TMJ, sleep apnea and dental implantology.

Dr. Jim Willey is the Senior Director of the ADA Practice Institute. Before coming to the ADA, Dr. Willey practiced general dentistry in Elburn, Illinois for 26 years.

In his previous position as the Director of the Council on Dental Practice, Dr. Willey led the interagency staff team that developed the new ADA Center for Professional Success. The Center is an interactive web portal developed for members to access practice management content, decision

support tools and other business applications.

Dr. Willey was also elected mayor of Elburn, IL for three terms, ending in 2009. As a local mayor, Jim led efforts to create a unique Veteran’s Memorial featuring a retired U.S. Army battle tank.

Jim is also a founding member of the Pritzker Military Museum and Library and a brigade color bearer of the Civil War Trust. He also enjoys collecting Lionel model train sets.

Dr. Willey holds a B.A. from Southern Illinois University; a B.S.D. and D.D.S. from the University of Illinois; and an Executive M.B.A degree from Northern Illinois University.

Dr. James Woodyard received his Doctorate of Dental Medicine from the University of Ken-tucky College of Den tistry. He then completed a residency in Periodontics at the University of Louisville School of Dentistry where he graduated with a Masters in Oral Biology and a Certificate in Periodontics. He maintains a private practice in New burgh, Indiana specializing in Periodontics and dental implants. Dr. Woodyard is a Diplomate of the American Board of Periodontology. He is a member of the American Academy of Periodon-

tology, Pierre Fauchard Dental Honorary Society, a Board member of the North American Society of Periodontists, the American Dental As-sociation, the Academy of Osseointegration, the Indiana Dental Associa-tion and the First District Dental Society. He is the director and founder of the Select Study Club, an affiliate of the Seattle Study Club system. Dr. Woodyard routinely presents lectures nationally on dental implants, cosmetic periodon tal plastic surgery and periodontal treatment.
