Testimonial Statement for Public Hearing on September 9, 2019 From Harriet Loizeaux, FNP-C

Wayne Memorial Hospital and Wayne Memorial Community Health Centers, in collaboration with the Pike and Wayne County Task Forces, created and opened a Tick Borne Disease Wellness Center in Lords Valley, Pike County, PA, located within the Pike Family Health Center located at 750 Route 739, Lords Valley, PA 18428. Phone number: 570-775-7100.

The Tick-Borne Disease Wellness Center opened on April 10, 2019 at the frequency of one day per week. By May, 2019, it was clear that we needed to be open more than one day per week. Therefore, the decision was made to increase to three days per week to provide increased availability and access to care.

The Wellness Center offers: diagnostic testing and verification ofTBD; evidence­based treatment of TBD and advice for management of symptoms for previously diagnosed patients.

Tick-Borne Disease Statistics have been collected and are attached as Appendix 1 and Appendix 2.
