November 6, 2019

Dear Parents,

We will be conducting hearing and vision screenings for the 2019-20 school year. The

screenings will be performed on all students in Kindergarten, 1st grade, 8th grade and

students receiving special education services.

A screening examination is a very general test that is designed to detect possible

problems. Vision screenings are not a substitute for an eye examination. For vision

screenings, students are shown a vision testing chart and asked to read letters or

identify symbols from a distance. If they are unable to do so, parents will receive a letter

advising you to have your child assessed by an eye doctor

(optometrist/ophthalmologist).

Hearing screenings are performed by having students listen to tones through

headphones. If they fail to respond to these tones, you will receive notification

recommending that your physician see your child and provide further guidance.

If you have a religious observance that objects to hearing and vision screenings or if

your child has been screened within 1 year and you have documentation, please notify

me by email or written note so that I can place it in the student’s medical file.

Thank you,

Jennifer Danticat RN
