• Fostering social and physical environments that support health and independence,

What are Congregate Meal Programs?

Provide affordable and nutritious meals in a group-setting.

Targets those who are unable to prepare meals on a regular basis.

Targets those who are socially isolated. Provides and encourages volunteer and leadership in

contributing to the meal program. Reasonably priced meals.

What is Meals on Wheels? Provides affordable and nutritious meals that are delivered to

individual’s home. Services can be accessed at any age. Meals are delivered by volunteers. Meals are available to individuals who are unable to prepare or

otherwise obtain meals on their own. Includes hot meals, kosher meals, larger portions, supper bags

What are Tenant Resource Programs?

Targeted to older adults living in 55+ housing blocks. Coordinators develop and coordinate a service plan for tenants of the

building or collection of buildings. Assist tenants to find services to help with their needs to support

independent living in the community. Link to services such as:

Information and referral, Advocacy, Health clinics, Grocery shopping options, Friendly visits, House cleaning referrals, Translation, Filling out forms, etc.

What is Supports for Seniors in Group Living?

Defined as housing with enhanced support servicesthat supports health promotion and independence with a goal of aging in place.

These services are for individuals (primarily seniors) who require support with instrumental activities of daily living, and not 24 hour support and supervision.

Filling out forms, setting appointments, arranging transportation, social/education/support groups, etc.

What are Senior Resource Finders?

Information and Referral Hub Formerly Community Resource Councils Help link and refer older adults 55+ to community supports, programs and

services. Services vary from community to community based on identified needs; however, some common services are E.R.I.K. (Emergency Response Information Kit) Community outreach Escorted transportation and other transportation options Yard and home maintenance referral program Networking and community outreach Congregate meal programs Educational materials and community presentations

What is the Emergency Response Information Kit?

E.R.I.K. is an Emergency Response Information Kitdeveloped by the Seniors Resource Finders (formerly Community Resource Councils).

Established in response to community concerns regarding the availability of adequate information in emergency situations.

All area emergency groups have endorsed this program. Once completed, the kit provides the necessary

information for emergency personnel to respond quickly to an individual’s situation.

Activities include: Fitness & exercise programs Leadership development Health promotion, social, recreational, cultural and educational programs Volunteer opportunities Advocacy Information and referrals

Promote, maintain and improve health and quality of life. Support independence. Encourage involvement in the community. Address the determinants of health. Provide resource information on living well.

Provincial focal point to facilitate communication, networking and planning among senior centres and raise their profiles across Manitoba.

Assists in the development of senior centres and collaborates with other senior serving organizations.

What is the Dental Program?

Home Dental Care Program Uses special portable dental equipment to provide care

for those who have difficulty getting to a dental office specifically long term care facilities.

Deer Lodge Centre Dental Program Located in Deer Lodge Centre, this clinic specializes in

dental care for older adults and welcomes older adults from any area in the city.

What is A & O Support Services for Older Adults?

204-956-6440 intake lineOffers Specialized Support Services for Older Adults including: Information and referrals - information about a wide range of services for older

adults and links or refers them to the network of programs that exist in the community.

Counseling - Some of the services available address loneliness, death of a family member, loss of health and adult-child, grandchild / aging parent relationships.

Housing – connect with a housing project coordinator and housing guide. Legal services - on matters such as wills, powers of attorney and health care

directives. Entry Program for Older Adult Immigrants - orientation for newcomers about living

in Canada; Health and aging in Canada; Canadian laws and citizenship; Housing and community information and conversation circles to practice English skills.

This Full House (hoarding) - assist older adults whose “stuff” fills their homes and limits their lives.

A & O Support Services for Older Adults

Senior Centre Without Walls - offers a unique opportunity for socially isolated older adults. Free half-hour programs are accessed through a toll-free number.

Older Victim Services - service developed in partnership with A & O and the Winnipeg Police Service. Assistance is provided to older adults who have been victims of property crime, such as a break-in to their residence, and personal crimes, such as assault or robbery.

SafetyAid – falls prevention program that gives seniors a greater sense of safety and security in their homes.

Connect Program (formerly friendly visiting) - assist socially isolated older adults living within the community in Winnipeg, connecting them to resources that will facilitate independent living.

Volunteer Opportunities - flexible volunteer opportunities available throughout a variety of programs.

A & O Support Services for Older Adults

Elder abuse services - Social Workers respond to the needs of individuals 55+ who are experiencing neglect, emotional, financial, sexual or physical abuse. Information and resources are provided to family and friends who may be concerned about an older adult.Services include: Safe Suite - Provides temporary housing for men, women and couples, 55 years

or older who are in need of a safe place to stay due to abuse or neglect and whose needs cannot be effectively met by existing abuse/crisis services.

Senior’s Abuse Support Line - is available 24-7; calls are free and confidential. Counsellors are trained to provide one-on-one support to older adults experiencing abuse.

Counselling regarding the abuse and options available Referral to appropriate community resources Consultation and referral for family members Support Groups Assistance accessing crisis accommodations and legal services, including

protection orders Information and Education

What is the Alzheimer Society?

Provides community awareness; individual, family and professional education; support programs and research funding.

First Link® is a referral program designed to link individuals and families affected by

Alzheimer’s disease or other dementias with services and support as soon as possible after diagnosis.

Formal referral by physicians and health professionals allows for proactive contact to help the family better cope with their situation.

Supports available include information, support groups, education sessions and referrals to other health resources as needed.

While physicians and health-care providers can make referrals via First Link®, individuals and their families can also contact the Alzheimer Society directly for assistance.http://www.alzheimer.mb.ca/

What is Health Services on Elgin?

Serves the Downtown and Point Douglas communities.

Nursing; Nutrition; Occupational Therapy; Physiotherapy; Hygiene; Foot care; Social Support; Social Work; Outreach; Meal Program.

Adults over the age of 65 and adults with special health needs, who require any three of the above services qualify for referral to Health Services on Elgin.

What is the Healthy Aging Resource Team (HART)?

Three teams (River East/ Transcona, Downtown/ Point. Douglas, and St. James/ Assinibioa/Assiniboine South).

Composed of a nurse and allied health professional. Healthy Aging Resource Teams are committed to:

• Promote health; • Manage chronic disease; • Prevent injury and illness; • Provide health programs; • Promote independent living; and • Work with communities
