Life Membership of Charitable Society is must

Only Active Rotarian & Spouse will join this scheme &

Age should not be more than 62 Years.

Submit Application and Membership Forms with

Nominee details & certification of Club President or

SSSS Ambassador of Club.

No cash transactions - only DD or MICR Cheques

payable at Ahmedabad will be accepted.

Membership ID Card will be issued to all the members

Financial & Administrative Year will be

1 April to 31 March

Rules & Regulations and formalities for

admission are with forms.

“श्रद्धांजलि” notice & Audited Financial report

will be send to all the members every year.

Annual General Meeting (AGM) will be called

every Year at Assembly or any Rotary Meeting

➢Life Membership Fee for

RID 3050 Charitable Society Rs. 500/-

➢Admission Fee for SSSS Rs. 100/-

➢“श्रद्धांजलि” (DFC) Deposit Rs. 800/-_______

Total Rs.1400/-

Annual “श्रद्धांजलि” (DFC) contribution will be Rs. 100/- per Death by number of Death or

minimum Rs. 500/- Every Year

SSSS CARD

DEATH CLAIM

Nominee or President of club will inform

and send Claim to Chairman/Secretary

SSSS in case of Death of the Member.

Immediately payment will be sent thru
