BackgroundDrug expenditures continue to rise due to increasingdisease burden and emergence of new drugs. WorldHealth Organization (WHO) recommended that Drugand Therapeutic Committee (DTC) be set up in health-care institutions to manage drug formularies and policiespromoting rational drug selection and use. Operations ofDTCs in MOH facilities are yet to be documented. Thisstudy was set to gain insights on DTCs particularly inexerting these roles.
Materials and methodsA mixed method of focus group discussions (FGD), surveyand semi-structured interviews was used. The FGDs pro-vided basic information on DTC operations and formularymanagement in MOH facilities. Survey questionnaireswere sent to all 136 MOH hospitals and 15 state healthoffices to obtain more detailed information. Seven keyinformants were interviewed to gain in-depth knowledge.
ResultsAltogether 118 completed questionnaires were returned(response rate: 78%). Almost all (97.5%) of the facilitieshad operational DTCs. However, they varied considerablyin structures and activities. All DTCs were multidisciplin-ary consisting of mainly department heads with 3 to 36members. There were 96 of 118 (81.4%) facilities whomaintained local drug list that are subsets to the NationalDrug Formulary. Eighty-one (68.6%) of the DTCs lacked
term of reference documents. Expected DTCs’ responsibil-ities were not clearly specified, often being assumed bypharmacy departments. The interviews highlighted thatlocal DTCs’ fundamental agenda was managing individualinstitutions’ drug expenditures, supporting the survey find-ings that drug budget and procurements being the twomost frequently discussed issues in DTC meetings. Newapplications to list new drugs into the national formularyoften not debated much during local DTC meetings. Themain reasons identified include inadequate resources toconduct drug evaluation and perceptions of overlappingroles with national level DTCs.
ConclusionsSuboptimal standardisation in structure and activities ofMOH DTCs emphasises the need for national levelcoordination of individual DTC operations in formularymanagement roles.
