Hello and welcome to the spring issue of Exchange. My name is Jessica Mulligan and I am the new Executive Director at Community Memorial Foundation. I am honored to succeed Kerry Freiberg and grateful for the 19 years of service she has dedicated to the foundation. Under her

leadership, Community Memorial Foundation has grown and generated resources to support the care our patients receive at Community Memorial Hospital. Please join me in thanking Kerry for her dedication, loyalty, and commitment to the foundation and Community Memorial Hospital. We wish her well as she continues to serve us as vice president of Community Engagement for the Froedtert & the Medical College of Wisconsin health network.

We have a special edition of our newsletter prepared for you! You will once again meet one of our grateful patients whose life has been impacted by our Cancer Center. Our spring issue is also the time when we extend an extra special thank you to the donors who supported the foundation in 2017. It has been yet another remarkable year and we could not have done it without our donors, community partners, and volunteers.

There is something very special about being a part of the Community Memorial Hospital family. I look forward to seeing you at CMH and throughout the community. Abundant blessings for a healthy and happy 2018!

Warmest Regards,

Jessica L. Mulligan

(9th Edition, 2017)

This valuable handbook features breast cancer survivors from different backgrounds sharing information about a range of topics to support self-care. Issues addressed include: The Emotional Impact of Breast Cancer; Relationship with Your Partner; Telling Your Children; Calming Your Fears; What Is Breast Cancer?; Surgical Treatment Decisions; Reconstructive Surgery; The Surgical Experience; and much, much more.

The Breast Cancer Treatment Handbook

everything in it, from start to finish. I could read as I needed to learn more, which made the entire experience so much easier. The handbook helps you understand what’s happening physically and emotionally. I could learn about side effects, and what to expect after surgery from other women who experienced it themselves. There’s even a section on relaxation and nutrition, to help you take care of yourself.”

At Julie's suggestion, Sara also took advantage of support services offered by the After Breast Cancer Diagnosis (ABCD) program. ABCD provided her with mentors, fellow breast cancer survivors, with whom she could confide in as she considered treatment and reconstruction options. On September 11, 2017, Sara underwent a mastectomy. The surgery was a success and her recovery progressed as expected.

Looking back, Sara remembers clearly how, after surgery, Julie was at her bedside with yet another gift, a specially designed camisole created for women experiencing the pain and limited mobility following mastectomy or lumpectomy. The camisoles are soft and loose fitting under the arms

where lymph nodes have been removed, and feature inside pockets to hold surgical drains. Julie also gave Sara a 'seatbelt pillow' to make her ride home after surgery more comfortable. The pillows are handmade by CMH’s Volunteers in Partnership (VIP). "You have no idea how important these are until you've been through breast cancer surgery," said Sara. "I didn't even know these kinds of things existed. And yet I'm so grateful for the camisoles and the pillow and many other items I received from Community Memorial. They made my recovery so much easier."

All funds raised by the Community Memorial Foundation directly benefit patients, like Sara, receiving care at Community Memorial Hospital. Please consider helping Community Memorial Foundation continue to help women like Sara with Breast Cancer Treatment Handbooks, camisoles, seatbelt pillows, and access to patient education. Gifts can mailed to Community Memorial Foundation or given online at communitymemorialfoundation.org.

To Our Foundation Friends:

Thanks to the support of our generous staff, volunteers, donors, businesses and foundations from across the community, Community Memorial Foundation has much to celebrate in 2017!

The CMH Foundation was proud to support Froedtert & the Medical College of Wisconsin Community Memorial Hospital’s new Garden View Unit overlooking the hospital’s peaceful Healing Garden. The Garden View opened in April of 2017 to provide comfort and care to patients and their family at the end of life’s journey in partnership with Horizon Home Care and Hospice. We are also happy to share some of the ways your gift to Community Memorial Foundation promotes a healthy community in our giving highlights.

In 2017 our signature fundraising events continued to flourish and grow support for our community hospital in Menomonee Falls . Please see our Spring Annual Report for more information about these highlighted events:

• $137,000 was raised by Community Memorial Foundation’s 27th annual Spring Splendor Gala ~ The Magic of CMH to benefit the Garden View Unit and end-of-life care at Community Memorial Hospital.

• $73,000+ was brought in by the foundation’s 19th annual Golf Classic to benefit the Froedtert & the Medical College of Wisconsin Joint and Spine Center at Community Memorial Hospital.

• $25,000+ was raised by Wheeling for Healing for the hospital’s Cancer Care Center.

• $16,000+ came from the annual Fashion Show & Luncheon to benefit the Breast Care Center.

Finally, I want to give very special thanks to Kerry Freiberg for her leadership and dedication to Community Memorial Hospital and Foundation over the last 25 years! We are proud to see her lead Community Engagement as Vice President for Froedtert Health and know she will always be a great champion and supporter of CMH and the Foundation. Thank you for your support, hard work and generosity!

Fall fashions and breast cancer awareness were center stage at Community Memorial Foundation’s Fashion Show and Luncheon on Friday, October 20 at Hilton Garden Inn Milwaukee Park Place. 210 guests enjoyed an afternoon of silent and live auctions, raffles, lunch and fashions provided by A.J. Ugent Furs, Robin’s Nest and Stan’s Fit For Your Feet. All fashion show models were breast cancer survivors who received care at Community Memorial Hospital. The event had its best year yet, raising more than $16,000 to benefit Community Memorial Hospital’s Breast Center!

Special thanks to all models, event attendees, donors and volunteers who generously contributed to this important fundraiser.

Thank you to our Evergreen Society members!

Legacy gifts support the future of Community Memorial Hospital. We thank each of our Evergreen

Society members for securing the hospital’s future for generations to come.

When you choose to include Community

Memorial Foundation in your estate plans, you

leave a legacy of enduring value. You also join the

Evergreen Society, which honors those who have

made a planned gift to the Foundation to ensure

quality care for generations to come.

Community Memorial Foundation was grateful

to add three new members to the Evergreen

Society over the last year. The Foundation was

also thankful to receive a generous legacy gift from

James H. Basler. His $1.2 million contribution

to Community Memorial Hospital’s Emergency

Department in 2016 will help make certain that

those who are critically ill or experiencing life

threatening emergencies receive lifesaving care close

to home.

A planned gift of any size to Community

Memorial Foundation can make a difference to

CMH. Planned gifts to the foundation, like other

contributions, stay at the hospital to benefit the

local community.

If you deeply value CMH’s compassionate care and

want to preserve it for the future, please consider

a planned gift. For more information, call the

Foundation at 262.257.3769.

Learn valuable information to help create or update your financial plan for today and tomorrow. Find out how you can meet your needs and create a legacy that honors your values and vision by taking part in a legacy planning seminar sponsored by Community Memorial Foundation:

Purse PowerMarch 28, 2018 • 6-8 p.m.

Estate PlanningApril 10, 2018 • noon-1:30 p.m.

Wills & TrustsJune 6, 2018 • 7:30-9 a.m.

All seminars take place at Treiber Conference Center at Community Memorial Hospital, W180 N8085 Town Hall Rd., Menomonee Falls, WI 53051. Seminars are free, but space is limited. Call 262-257-3200 to learn more and register for upcoming events.

A simple and tax efficient way to include Community

Memorial Foundation in your estate plans is to name the

foundation as a retirement plan beneficiary.

Distributions from many pre-tax retirement plans, such as

403B Plans, 401K Plans, and Traditional IRAs, are subject

to income taxes, while distributions from Roth accounts

and life insurance proceeds are not. Be smart about who

you name as beneficiaries. Since qualified charities do not

pay income taxes, when you name a charity as a beneficiary

of your retirement plan the full value of your gift can

support the purpose you designate. To name a charity as a

beneficiary, simply request a beneficiary designation form

from your plan administrator or financial advisor. You can

name Community Memorial Foundation at whatever % you

choose. Even 1% makes a difference.

If you include Community Memorial Foundation in your

legacy plans, your gift can preserve the quality of care at

Community Memorial Hospital for generations. You also

join the Evergreen Society. Contact Community Memorial

Foundation for more information. Call 262-257-3769 or

email [email protected].

Join us for a special Legacy Planning Series
