1. INTRODUCTION

This workbook is intended to be used as a companion to HPG Module 7: Outcome

Based Pathway and Reimbursement and contains activities and review questions to

support your learning. Active learning (which refers to techniques where learners do

more than simply listen to a lecture or instruction) is a very important part of

achieving the learning objectives. You will have the opportunity to hear, see,

discover, explore, and apply the information that you are learning.

Your instructor will provide you with information through lecture, demonstration,

discussion, quizzing and guided practice.

1.2 The Workbook

This workbook is composed of a series of scenario-based exercises designed to

provide you with the opportunity to systematically practice and apply the HPG OBP

OBR functionality as you learn it in the classroom.

The Instructor can choose to complete this training by demo only or hands on.

How will you use the workbook?

Your Instructor will guide you through the activities in the workbook using the client

prepared for you in Training HPG.

The instructor may modify the content and instructions to suit the needs of individual

training situations.

Note: To have HPG prepared for this training; the surgical wound pathway must be

initiated first in CHRIS by the CCAC Educator. Make sure that the Surgical Wound

pathway is the first service authorized for training patients so that it is OBR. You

may find it easier to complete the offer with verbal acceptance completed so that the

pathway is initiated and in a ready state for SPO entry.

In each exercise, you will be provided with specific information to enter in a field

when that information impacts on exercises to follow. You will not be provided with

information to be entered in every field that must be completed on a page. In this

case, you can draw from your experience and enter information that you feel is

appropriate. (Be sure not to enter any personal information)

3. NAVIGATING TO THE SPO PATHWAY DETAILS ‘ACTION’ PAGE

Outcome Reporting is available for both pathways that are reimbursed as fee for

service and those that are reimbursed based on outcome achievement.

3.1 Exercise 1 - Navigating to the Outcome Based Pathways Tab

1. Navigate to Client View in HPG, Client search, search for the client name you are

provided by your instructor, and then open the Outcome Based Pathways Tab.

3.2 Exercise 2 – Explore the Outcome Based Pathways Tab in Detail

The Pathway Details page displays the details, history, reimbursement status,

Interval and outcome status, and submitted pathway reports for the pathway that

have been submitted by the CCAC and/or SPO.

In each section on the Pathway Details page, there are a number of actions that you

can complete and/or review.

Explore the pathway Details Tab using the diagram and chart below as a guide. Do

not complete any actions at this time, rather press cancel to exit a screen without

making changes.

4. SPO ACTIVITY – ADD NEW PATHWAY

4.1 Exercise 3 – Add a New Pathway

A second pathway may be requested by the CCAC or SPO

When a second pathway is added by a SPO and authorized by the CCAC to an

existing OBP-OBR, the second pathway:

is OBP-FFS

has NO FFS visit payments released during the period the second pathway

overlaps with the OBR (until Day X)

Has FFS payment released on the start date of the warranty period

6. SPO ACTIVITY – SUBMIT PATHWAY REPORT

A single pathway report may contain a combination of different types of OBP

reporting data such as outcomes, exceptions, activity notifications/requests,

additional information, discontinue provider, or discontinue pathway requests.

You can expand and collapse each section of the pathway report independently by

selecting the section check-box, and you can enter data for one or many of the

outcomes, risk factors, activities or requests available in the report.

Resubmission is possible for any interval for which there are unmet outcomes.

The following activities will provide opportunity to practice various combinations of

data entry in pathway reports.

6.1 Exercise 5 – Submit SPO interval outcome with activity

notification and additional information.

Each Pathway outcome is identified as belonging to a single “Responsible Partner”.

The Partner is the CCAC or SPO who is responsible for achieving the Interval

Outcomes and submitting the Interval Reports by the Interval Due Date

Interval Outcomes:

define Interval Goals

contain a collection of Service Outcome Types for each Pathway Event

defines the Outcomes that are reportable in a Pathway Report and must be met

to complete the related Interval

have a status of „Complete‟ or „Incomplete‟

6.2 Exercise 6 – Submit Second Interval Report with unmet

outcomes, variance and activity request

Surgical Wound Outcome-Based Pathway second interval outcome is due 21 to 28

days based on the pathway Start Date and there must be a 20 to 30% reduction in

the wound size

6.5 Print Pathway Report

1. Select Print Pathway report button at the top of the Details section for the

Pathway to view the Outcome-Based Pathway Summary Report.

6.7 Demo 9 - Discontinue – Supervening Event

Supervening events are events which:

identifies conditions that determine that Patient is no longer eligible for the

pathway

occurs independently of (i.e. are not caused by) the pathway itself that affect

the quality of treatment

Note: For this demonstration the instructor will need to have a second patient set up

with an outcome based pathway in progress.

The SPO submits a Discontinue report as the client is moving to live with her

daughter in another city.

7. NOTIFICATIONS IN HPG

7.1 SPO Notifications

Outcome-Based Pathway notifications are system generated and sent to the SPO via

the HPG Inbox where the provider can open the notification.

Notifications are generated for the provider when certain events occur and are

grouped on a single notification when multiple actions have resulted in notification

being generated around same timeline.

There are ten types of Outcome-Based Pathway notifications:

1. Pathway Interval – Review Date Coming Due: One-time notification

generated for the pathway interval when an interval review date is approaching

2. Pathway Interval – Review Date Over Due: One-time notification generated

for the pathway interval when an interval due date has passed

3. Pathway Interval – Review Date Updated: One-time notification generated for

the pathway when the review date for an interval has been updated

4. Pathway Report – Added: One-time notification generated for the pathway

when a pathway report has been added by a user in CHRIS

5. Pathway Report – Deleted: One-time notification generated for the pathway

when a pathway report has been deleted

6. Pathway Shared Partner – Discontinued: One-time notification generated for

the pathway when the shared partner status (actor status) has been updated to

“Discontinued”

7. Pathway Shared Partner – Un-Discontinued: One-time notification generated

for the pathway when the shared partner status (actor status) has been updated

from “Discontinued” to “Active”

8. Pathway Final Payment – Authorized: One-time notification generated for the

pathway when the warranty payment status (final payment) has been updated

to “Authorized”

9. Pathway Final Payment – Reversed: One-time notification generated for the

pathway when the warranty payment status (final payment) has been updated

to “Reversed”
