PROVISION OF ASSISTIVE GADGETS FOR PERSON WITH DISABILITY

ABOUT THE SERVICE Republic Act No. 7279, otherwise known as the Magna Carta for Disabled Person, expressly provides that people with disabilities shall be provided with necessary auxiliary services that will restore their social functioning and integrate to their communities. One of the auxiliary services is provision of assistive devices for the mobility of PWD’s.

REQUIREMENT(S)

Certificate from the doctor or attending physician indicating the client’s need for assistive gadget/device.

SECURING DISCOUNT IDENTIFICATION CARD & BOOKLETS FOR SENIOR CITIZEN

ABOUT THE SERVICE Senior Citizen is entitled to twenty percent 20% discount in purchase of medicines, hospitals, restaurant, public land transportation, air and sea transportation, etc.

REQUIREMENT(S) To be eligible for a senior Citizen’s ID card, an applicant must be:

a Filipino Citizen at least 60 years old and a resident of the locality for at least 6 months

An applicant must submit the following:

A duly accomplished application form (available at the MSWDO) Barangay clearance/certificate Birth certificate 3 pieces colored 1”x1” ID picture Community Tax Certificate

When applying for replacement of lost Senior Citizen’s ID Card, an applicant must: Submit two (2) copies of an Affidavit of Loss.

SECURING SOLO PARENTS IDENTIFICATION CARD

ABOUT THE SERVICE THE SOLO PARENTS’ WELFRE ACT OF 2000 is an act that provides for the benefits and privileges of solo parents and their children. The act aims to develop a comprehensive package of social development and welfare services to solo parents and their children. In order to avail that benefits and privileges, a solo parent must secure a Solo Parent Identification Card from Municipal Social Welfare and Development Office.

REQUIREMENT(S) A barangay certificate residency in the area Documents/evidence that the applicant is a solo parent(e.g. death certificate of

spouse, declaration of nullity of marriage, medical certificate (if incapacitated) ITR or certification from the barangay/municipal treasurer In case of transferee; certification/clearance from former barangay where he/she will

surrender his/her ID issued to him/her. 2 pieces 1”x1” ID picture

ABOUT THE SERVICE The MSWD supports children and youth whose basic needs such as food, clothing, education and protection have been deliberately neglected or unattended by their parents or guardians, or who have been victims of any form of child abuse.

AID TO INDIVIDUAL IN CRISIS SITUATION (AICS) ABOUT THE SERVICE The Municipal Social Welfare and Development provides limited assistance, in cash or in kind, to individuals/families who are hampered to function normally because of socio-economic difficulties. ABOUT THE SERVICE It is a provision of unified basic health care benefits to all deserving community indigents and government employees. Coverage of Socorro Health Care Program: A. 1. Indigent residents of Socorro (those belonging to families living below poverty line,

SOCORRO SCHOLARSHIP PROGRAM (SSP)

ABOUT THE SERVICE The program is intended for those intellectually capable elementary and high school graduates who cannot afford to attend high school and college due to financial constraint. Qualifications for admission are based on the requirements set by the Socorro Scholarship Committee. This program, by the virtue of Municipal Ordinance No. 004-1994 as amended, provides for the yearly selection of eight (8) scholars for secondary and seven(7) for tertiary level.

REQUIREMENT(S)

Barangay Clearance (stating that the applicant is bonafide resident of the municipality for at least six months.

Photocopy of Form 138 (applicant has a good academic standing with a general average of 85%)

Parents with a gross income of not more than one hundred thousand pesos (P100,000.00)

SELF – EMPLOYMENT ASSISTANCE PROGRAM (SEA)

ABOUT THE SERVICE SEA – Self Employment Assistance program is the provision of Capital Assistance (tulong puhunan) or additional capital to individuals or groups who are engaged in small-scale businesses. Capital Assistance is payable for the period of one year and it can be renewed if the grantee regularly pays the amount borrowed. It is given without interest.
