Case Planning & Case Plan Implementation Application Exercises June 2017 Supervisor Led Application Exercises (SLAE) Exercises 1 - 4: Supervisors will discuss a self-assessment of strengths and opportunities for growth around both competencies with each of their caseworkers individually. As a unit, they will brainstorm strategies to celebrate success in their unit and identify ways to facilitate improved plan implementation through more effective collaboration with team members.
MiTEAM Specialist Led Application Exercise (MSLAE) Exercises 1 - 3: Practice using the case planning process to create behaviorally specific and achievable plans that incorporate the family’s voice and choice. Track and adjust plans.

Complete the Case Planning & Case Plan Implementation Measuring Fidelity Worksheets for each member of your staff.
On the worksheets, document if staff demonstrated the selected fidelity indicators while you observed them working with families or community partners.
Provide feedback to staff regarding strengths observed and potential opportunities for growth.

Compile scores and identify trends collected from your unit and document results on the Measuring Fidelity Tally Worksheets. Provide completed Fidelity Tally Worksheets to your Manager.
