• Member profile – female, in her 80’s with some form of dementia

What makes us different?

Interdisciplinary approach to care– permanent care management team members’ work together to ensure the members have the most comprehensive care to address their needs, and maintain member in their homes. 

Each Care Management  Team (CMT) consists of a:

• Nurse

• Social Worker

• Service Coordinator

Welcome/Transition Teams (WT/TT)‐ temporary care management teams for new members

• care manage members within the first 30‐60 days

• onboard the member after receiving the state approval; conduct the welcome call

• WT/TT care managers enter the first set of authorizations for members coming into plan.

SHPReferralToADHCWhy and how does SHP make referrals to ADHCs?There are times when coordinating services at home become onerous for the member and plan. The member may have skilled needs and require multiple services to improve their current level of functioning. There may not be family involvement, to assist the member with appointment scheduling and treatment follow up. In these cases we can refer to Adult Day Health Programs. 

As a MLTC and care managers, we approve ADHC attendance taking the following into consideration:

• Eligibility‐ does the member have a skilled medical need, that cannot be successfully met in the home

• ADHC vs. social day‐ all authorized services must be justifiable and medically necessaryafter close examination of member eligibility to attend ADHC, we then authorize the service. If the need isn’t medical, but social in nature then a social day care is the appropriate choice

SHPReferraltoADHCContinuedHow often do we allow our members to attend?

• We approve a schedule that addresses the skilled level of service the member would receive if at home. Example‐ if a member qualifies for three days of PT, and a nurse for pre‐pour, we would approve three days of ADHC attendance.

Can members attending ADHC have pca services as well? 

• Yes, the personal care aide can prep the member in the morning and/or provide service to the member in the evening.

• Please note: We are not allowed to duplicate services, therefore members receiving nursing, PT, OT, ST, or any other specialty treatment will not receive that service at home. There are some extenuating circumstances like a wound requiring wound care, when services may be additionally authorize

Eligibility• Must be 21 years of age or older.

• Reside in Manhattan, Bronx, Brooklyn, Queens, Staten Island, Nassau, or Westchester County.

• Be Medicaid eligible or currently on Medicaid or willing to private pay spend down to Medicaid (private pay).

• Require long term care services (nursing, therapies, home health aide, personal care, adult day health, social day care) for at least 120 days.

• Be able to live safely at home with the support of Senior Health Partners• Clients with Psychiatric/Cognitive Impairment MUST have a 2nd

signature & community back‐up

• Agree to receive all covered services through Senior Health Partners (voluntary enrollment).

Intake&Enrollment• Referrals generated by Marketing are received via fax or phone.• Intake Team

WelcomeTeamThe role of the WT is to maintain contact with new enrollees prior to service authorization. Authorize/care manage members for 1 month. The team is comprised of:

• Social Worker: • Maintains contact with enrollee/significant other prior to service  & 

reviews Initial Plan of Care.• Monitors Hospital/Nursing Home discharges prior to service.• Authorizes Specialized Services: PERS, Safe Return, Medical ID Bracelet, 

Adult Day Social & Medical.  

• Nurse:• Monitors change of health / service status.• Contacts Primary Care Providers.• Authorizes: Professional & Rehab Services: RN / OT / PT / DME.

• Service Coordinator:• Assist with telephone contacts.• Authorizes: Home Care, Transportation & some Medical Supplies.

• Is the permanent team the member is transferred to from the Welcome Team

• Team consists of:• Social Worker

• Maintains contact with enrollee/significant other prior to service  & reviews Initial Plan of Care.

• Conducts psycho‐social assessment for all new members. Provides referrals for mental health services and benefits.

• Authorizes Specialized Services: Personal Emergency Response System (PERS), 

• Adult Day Social & Medical. Coordinates Safe Return & Safe Link (cell). 

• Nurse• Monitors change of health / service status.

• Contacts Primary Care Providers.

• Authorizes: Professional & Rehab Services: RN / OT / PT / Durable Medical Equipment (DME).

• Service Coordinator • Assist with telephone contacts.

AuthorizationProcessforAutoEnrolledCases

• Auto enrolled cases come into SHP care planned by the HRA vendor agency. Services remain unchanged for 60 days to ensure continuity of care.

• The member can be assessed for increase of service in the certification period based on changes in function if needed.

• Reassessment Team will conduct an assessment within 30 days after enrollment to assess for a new plan of care

• SHP’s Transition team authorizes a five (5) month Service Plan to the current vendor providing service.

RecentchangesinMLTC• The Department  of Health (DOH) Commissioner is required to seek input from stakeholders, 

which include providers, consumers , the Medicaid Redesign Team (MRT) and the Medicaid Managed Care Advisory Review Panel. 

• 10/1/12 ‐Mandatory enrollment.

• 10/1/12 ‐ Eliminates the “nursing home eligible “ requirement .

• 11/1/12 ‐ Expanded benefit: * Consumer Directed Personal Assistance Program (CDPAP/CDPAS). 

• The DOH is authorized to seek a federal waiver to require individuals age 21 or over, who require more than 120 days of community based LTC to enroll in MLTCP.

• DOH will approve up to 75 MLTC Plans.

• * CDPAP/CDPAS provides flexibility and choice to chronically ill/physically disabled individuals.  The consumer  is responsible for hiring, training, supervising and if needed, terminating caregivers .
