Payroll Deduction Plan for any direct medical expenses paid

by Company relative to injuries sustained by employee(s)

of Company resulting from slip and fall accidents while

wearing shoes purchased from SHOES FOR CREWS.

Conditions & Exceptions

At the time of the accident, the employee must have been wearing shoes purchased 1.

through SHOES FOR CREWS voluntary Payroll Deduction Plan within six (6) months prior

to the date of the accident.

Accidents occurring due to solid obstacles on the fl oor, or on ice surfaces, in freezers, 2.

coolers, or sub-zero environments are specifi cally excluded. Acceptable obstacles

include, but are not limited to: water, grease, liquefi ed fats, and synthetic lubricants.

The maximum individual claim is up to $5,000.00, limited to 20% of your year-to-date 3.
