The safety of operations requires that all questions should be answered affirmatively by clearly ticking () the appropriate box. If an affirmative answer is not possible, the reason should be given and agreement reached upon appropriate precautions to be taken between the ship and the terminal. Where any question is considered to be not applicable, then a note to that effect should be inserted in the remarks column.Operasyonların emniyetli olmaları, ilgili kutuların açık bir şekilde işaretlenerek () bütün soruların olumlu cevap verilmelerini gerektirir. Eğer olumlu cevap mümkün değilse, nedeni belirtilmeli ve gemi ile terminal arasında alınacak uygun önlemler konusunda anlaşmaya varılmalıdır. A box in the columns ‘’ship’’ and ‘’terminal’’ indicates that checks should be carried out by the party concerned. The presence of the letters A, P or R in the column ‘’Code’’ indicates the following:‘’Kod’’ sütunundaki A, P ve R harflerinin anlamları aşağıda açıklanmıştır.

A –(Agreement). This indicates an procedure or agreement that should be identified in the “remarks” column of the Check List or communicated in some other mutually acceptable form. In either case, the signatures of both parties should be required.Belirlenmiş yöntem veya anlaşma, bu kontrol listesinin açıklamalar sütununda veya diğer karşılıklı anlaşılarak kabul edilmiş form üzerinde belirlenmiş olacaktır. P –(Permission). In the case of negative answer, the operation should not be carried out without the written permission from the appropriate authority. Olumsuz cevap halinde, uygun otoritelerin yazılı izni olmadan operasyon yapılmayacaktır.

R –(Re-check) This Indicates items to be re-checked at appropriate intervals, as agreed between both parties, at periods stated in the decleration. Deklerasyonda her iki tarafca anlaşılan süreyi geçmeyecek şekilde uygun aralıklarla tekrar kontrol edilecek maddeleri gösterir.
