Installation Instructions Complete the following steps to install the New Accounts application.

Before You Begin: New Accounts must be installed by a user with local administrator rights.

Step Action 1 Double-click Accounts Installation icon.

2 If the User Account Control dialog box prompts you to allow Accounts Installation to

change your computer, click Yes. If not, proceed to next step.



3 Click Next.

4 On the Destination Folder screen, key the AS400 Server Name (usually CSIDC*) and AS400

Server IP (usually 172.25.2.50) and then click Next.

Note: The AS400 server name must be set up on either your DNS server or on each individual computer in

<InstallDrive>:\windows\system32\drivers\ect\hosts to point to the AS400 server IP address.



5 When the installation is complete, click Finish.

Result: The New Accounts log-on screen displays.

Post-Installation Task: Migrating from BFO to PDF Forms Complete the following steps if your bank wants to begin using PDF forms when available instead of BFO forms.

Step Action 1 On each workstation where New Accounts is installed, browse to the

<InstallDrive>:\Cardinal\Accounts directory. 2 Open the AppSetup.xml file stored in a text editor. 3 Find the following section in this file and change the value of the formmode type

attribute from bfo to pdf: <form> <formmode type="bfo"/>

4 Save and close the file. Result: New Accounts is configured to present you with PDF forms rather than BFO forms whenever a PDF form is available.



Issues Addressed in This Release The following table briefly describes issues in previous New Accounts versions that version 16.1 resolves. Issues whose ticket number contains a hyperlink have more detailed information about their resolution available when you click that link.

Ticket Issue Resolution 122229 In the MPMP-LAZ-LA Account Agreement

form, the UTMA verbiage and checkbox does not populate in the Ownership of Account section even if you set up the account as a custodial account.

Corrected logic so the free-text checkbox is marked and its textbox populated with UTMA.

122230 In the MPMP-LAZ-LA Account Agreement form, the Joint Account checkbox in the Ownership of Account section is not marked even if you set up the account as a joint account.

Corrected logic so the Joint Account checkbox is marked appropriately.

124591 On form CD-AA-LAZ-IL, when you select the interest-crediting frequency, the populated text unnecessarily begins with the word credited, which causes the printed notice to repeat that word.

Removed the unnecessary instance of credited so the form reads correctly.

126245 New and changed relationship codes entered in Cardinal Core are uploaded incorrectly to New Accounts or sometimes display completely blank.

Included relationship codes in the list of records that New Accounts downloads.

126533 Employer and occupation information entered in New Accounts does not upload to Cardinal Core.

Corrected logic to allow this data to transfer to Core.

127175 Customer information does not automatically populate in the IRA-49-LAZ form.

Corrected some owner items in the BFOConversion table to allow the form to be populated.



Ticket Issue Resolution 128553 The MOSC-LAZ-IL signature card and ID-INFO-

IND forms do not populate with the following information:

• ID issuer

• ID number

• Issue date

• Expiration date

• Occupation

• Employer

Updated BFOConversion table to populate these fields with information previously entered for the account holders.

128771 Sometimes the relationship entered in New Accounts does not upload through the Platform Interface for a joint relationship on a trust account.

Added code to check the Account Type and to return the appropriate flag in the case of trust accounts.

129969 When an account has multiple customers, the default class code that set at the product level gets reset to the class code for the last customer during Cardinal Core processing. This issue occurs for both consumer and commercial accounts.

Removed code that forced the class code to change to the last customer’s class code. The product class code is now either the default for the product or blank.

133831 In the FAST-FAP/Your Ability to Withdraw Funds form, the business-hour field is not available for you to enter a value.

Opened the field for entry from users.

135240 In the Print Product Forms dialog box, the print-all buttons do not print every form in the list.

Removed a refresh command that was causing forms to print unpredictably.

135300 On the FASTFEE/Fee Schedule form, tiered fees you define in the Product Information screen do not print on the form.

Corrected logic to print tiered fee verbiage when it is applicable to the product.

136560 On the CD-AA-LAZ-LA form, the last digit of the APY value that populates in the form is rounded incorrectly.

Set APY values to display with two numbers after the decimal in accordance with government regulation.



Ticket Issue Resolution 136604 On the Truth in Savings Disclosure, the field

designed to express the rate duration you set instead populates with the crediting frequency value.

Corrected mapping between the value in New Accounts and the rate duration field in the Truth in Savings Disclosure.

141194 On the second page of the Truth in Savings Disclosure/TSD-TIME form, in the Early Withdrawal Penalty section, the will checkbox is sometimes checked even if you check the may checkbox.

This occurs because New Accounts automatically checks will whenever the Early withdrawal penalty field in product setup contains any characters.

Removed the rule that forced the will checkbox to be checked based on the value of the Early withdrawal penalty field. Users can now set a default checkbox to mark and change the value as needed when printing the form.

141919 On the Truth in Savings Disclosure, minimum-fee amounts of $1,000 and greater print without a comma to mark the thousands place. This occurs even if you include a comma in the fee amount in the product parameters.

Updated the form to mark the thousands place with a commas in all fee amounts.

144477, 145870

New Accounts returns an error when you attempt to launch a third-party check-ordering process.

Updated the check-ordering feature to call the correct function.

145809 Bank requires new Wolters Kluwer deposit forms.

(Custom Programming) Applied new form at bank.

91999 New logic in the Tiered Rates Schedule notice handles cases where the maximum amount of the highest tier is undefined. In this situation, if 0.00 is entered as the tier maximum, the notice reads:

“If the average daily balance is more than or equal to Minimum Value, the interest rate paid on your account …”

Previously, New Accounts printed the following statement (the highlighted portion is removed in version 16.1):

“If the average daily balance is more than or equal to Minimum Value but less than or equal to $999,999.99, the interest rate paid on your account …”

98447 On the Bond Rates Redemption screen, click the new Redeem Bonds button to redeem bonds. To close the form without redeeming bonds, click the Quit button. This user experience is more intuitive than in previous releases, where you must click the Quit button to redeem bonds.

98939 Added the ability to set default values for fields in the ACH Origination Agreement.

127280 Increased the length of the free-form field in the Transaction Limitations section of the Truth in Savings Disclosure.

133066, 134255

In the Safety Deposit Box Lease (SDBL-IA) form, added ability to enter the account number that will be debited when rent is due.

Ticket 127280: Increased length of free-form Transaction Limitations field The contents of the free-form field in the Transaction Limitations section of the Truth in Savings Disclosure (pictured as follows at the left) is populated from the textbox on the New Accounts screen pictured as follows at the right. The textbox now accepts up to 250 characters.

The form D-OVERDRAFT, Discretionary Overdraft Payments, is being created. Overdraft practices continue to be under scrutiny from regulators and the target of litigation. This E-Form is designed to provide most of the functionality of the menu DOVER-BRO and to specifically address concerns expressed by the CFPB and CUNA Mutual about debit authorization holds.

D-OVRDRFT-WS (DISCRETIONARY OVERDRAFT PAYMENTS WORKSHEET) Rev. 6/10/2015

The form D-OVRDRFT-WS, Discretionary Overdraft Payments Worksheet, is being created. This is the worksheet that can be used to help create the text to be entered in the Payment Order of Items section of D-OVERDRAFT or DOVERDRAFTCU.

Deleted 1200 (DIRECT DEPOSIT SIGN-UP FORM) Rev. 07/15/2010

This form was a Treasury form used as part of the “GoDirect®” program to convert recipients of paper checks for federal benefits to direct deposit prior to the legal mandate for all paper check payments to be eliminated prior to mid-2014. This program has now long since ended, and the form has not been maintained as to inaccurate phone numbers or outdated information, and is therefore obsolete. As stated on the GoDirect website.

A 'Note' is being added regarding the one rollover per 1-year (12-month) period along with a checkbox to mark if the distribution includes hard to value assets. A complete explanation of these additions is being added to the Additional Information section.

• Section 3 – PAYMENT INSTRUCTIONS Under 'Payment Method' the option for 'Mail check to me' is being removed per financial institution feedback since this is no longer a common practice. The 'Other' field can still be used to document this type of distribution method. Under 'Payment Detail' the section



that includes a field to enter the amount of earnings paid to date as part of the distribution is being removed as it not necessary to collect the amount separately. The earnings should be included in the 'Amount Requested'.

Revision ACHORIGAGRLZ (ACH ORIGINATION AGREEMENT) Rev. 4/17/2015

The “Request for Acknowledgement Entries” section on Page 2 was missing a field after the colon, we have added the field. Formats affected: BFO, PDF, SFX, UFF, IFD, MDF, Symitar Reflecting a change to the baseline threshold for unauthorized returns from 1% to 0.5% in keeping with an underlying ACH System Rules change effective September 18, 2015; (b) to add flexibility to the timing of delivery areas of the form; (c) to make the Authorized Signatures section optional; and (d) to add terms to the Security Procedures area for greater consistency with similar provisions on other Wolters Kluwer Financial Services documents subject to Uniform Commercial Code Article 4A. This form is used as a service agreement when a business/commercial account holder arranges directly with the financial institution for the ongoing origination of Automated Clearinghouse (ACH) transfers of various types. These changes are recommended, not technically mandatory, as the contract has always deferred to ACH Rules and has always anticipated ongoing changes to details like return thresholds, and users have had other means, such as use of the Additional Terms area, to address the odd case-specific scenario not accommodated by the standard options.

VMP9030 (REQUEST FOR TAXPAYER IDENTIFICATION NUMBER AND CERTIFICATION)

Revising with numerous changes throughout, to follow the IRS model form. IRS has published (as of December 18, 2014) a new "December 2014" revision of the official form W-9. The changes to the form are generally clarifications stemming from changes to the previous revision of the form, which added certain Foreign Account Tax Compliance Act ("FATCA") elements that many found confusing. The changes don't appear to affect substantive content collected on the form for most scenarios, and the changes are not apparently driven by any new law or specified date, as the IRS released the new document with no formal announcement and unaccompanied by any expression of implementation expectations. This typically means that the IRS expects usage of the new form as soon as reasonably possible.

W-9INST (REQUEST FOR TAXPAYER IDENTIFICATION NUMBER AND CERTIFICATION INSTRUCTIONS) Rev. 1/30/2015

The W-9INST, Instructions for Requester of IRS Form W-9, is being revised to match the changes the IRS made in late December 2014 to the instructions document used as a reference document for financial institutions or other requesters of IRS Form W-9 (VMP9030). The primary changes to the instructions are changes intended (presumably) to clarify requirements for any substitute forms W-9, such as the substitute backup withholding certifications found in many signature card documents. (Incidentally, this particular change will result in upcoming changes to our signature cards in 2015, which are necessary to remain technically compliant with the oddly crafted requirements of this document.) Other changes are minor, including some clarification to the instructions for completing the official form with regard to general exemption codes and FATCA exemption codes, consistent with changes made separately to the official W-9 form (VMP9030).

Kansas Status Form Change

New ETM-2-LAZ (ELECTRONIC FUNDS TRANSFER DISCLOSURE STATEMENT) Rev. 10/18/2014

• The form ETM-2-LAZ, Electronic Funds Transfer Disclosure Statement, is being added to the Deposit Module and Deposit Credit Union Module for Kansas. Kansas SB 240 repeals K.S.A. § 9-1111d, effective July 1, 2015. K.S.A. § 9-1111d establishes that the liability of a depositor for an unauthorized transaction by a machine readable instrument shall not exceed $50 unless the depositor fails to notify the bank within 4 business days after learning of the loss or theft of the device, in which case the liability shall not exceed $300. This Kansas statute is more protective of the depositor than Regulation E, which provides that liability of the



depositor shall not exceed $50 unless the depositor fails to notify the bank within 2 business days after learning of the loss or theft of the device the liability, in which case the liability shall not exceed $500, and which also provides that there may be no limit on liability if the depositor fails to notify the bank within 60 days of when the unauthorized transfer appeared on a statement (Regulation E, 12 C.F.R. 1005.6).
