Services Description GPSoC Service Name CareHomePod GPSoC Service ID CareHomePod Functionality/ Service Overview The CareHomePod is designed to assist GPs and other local providers in their provision of service to the residents of Care Homes. A mobile, portable CareHomePod (based on our SurgeryPod and HomePod) will enable health care workers in a Care Home to measure vital signs and work through protocols and questionnaires with the results recorded on a central database. The system could either be used by the patients themselves or by the health care worker on behalf of the patient. A secure video link capability (Medvivo™ Connect) will also be provided to allow the health care worker to contact additional support, for example a GP, so that further assessment and investigation can take place remotely. The results posted to Medvivo’s central database (CUI) will be analysed with alerts raised if questions are answered in a way that causes concern, or if measurements are outside pre-defined tolerances. Functionality/Service Overview The CareHomePod is designed to assist GPs and other local providers in

their provision of service to the residents of Care Homes.

A mobile, portable CareHomePod (based on our SurgeryPod and HomePod) will enable health care workers in a Care Home to measure vital signs and work through protocols and questionnaires with the results recorded on a central database. This service will implement two major protocols with particular relevance to Care Homes:

Lower Urinary Tract Infection

Tissue Viability

Our tissue viability protocol will address and improve the end-to-end process for tissue viability for what can be an onerous paper-based activity. The protocol will support the care assistant or clinician in gathering data, which will be sent and stored with the results. Alerting within the CUI system will then be used to identify patients that may not be improving and evidence for the case to be assessed.

Our standards for service and support for CareHomePod are driven by our Telecare Services Association (TSA) Platinum joint registration.Our technology enabled care services are accredited to platinum level by the Telecare Services Association (TSA), which defines minimum standards for SLAs and response.

Service contact and support will be provided through the support desk. Clients with 5 or more CareHomePods will be allocated a Medvivo Client Service Manager to provide additional service management support and escalation. Regular service reporting will be supplied by the Client Service Manager.

Availability Standard: 99.5%

The CareHomePod posts its data to a central database but should this database not be available it retains the information and will post it with the next set of data. The availability standard reflects the availability of this central database (‘the CUI’).

Availability = (total service hours - unscheduled downtime hours) / (total service hours)

This is calculated per calendar month. Service hours are from 09:00 to 17:00 Mon to Fri excluding public holidays.

Incident Fix Times Standard: 2 days

Typical fixes requiring visits are to check or replace peripheral medical devices, or where the patient needs additional support in understanding or resolving an issue. We use our own highly-trained engineers, or that of our one partner subcontractor, to ensure the quality of the service, the capability of the staff and an awareness of safeguarding issues.

Helpdesk Standard: 09:00 to 17:00 Mon to Fri excluding public holidays

Most calls can be resolved over the telephone, or by gaining remote access to the HomePod to fix the incident. Remote access also allows remote training and advice to be provided when required.

Dependencies Infrastructure dependencies

The Care Home must have Wifi and broadband a 3G/4G signal..

Clinicians at the GP Practice or other provider services will require access to the WebCUI, and optionally to Medvivo™ Connect for video contact with the Care Home.

Service dependencies

The Call Off Ordering Party will be required to validate and approve all of the pathways, patient letters, communications and training. A project initiation meeting will be arranged to do this, and is included within the costs.

Local Personnel dependencies

A half day training course will be arranged within the key clinicians or stakeholders to explain the agreed pathways, patient impacts, roles and responsibilities as well as system training of the HomePod and WebCUI. This is for up to 10 clinicians. Additional courses for clinicians or key stakeholders can be purchased.

Engagement will be required with all key stakeholders involved in patient care, though the level of detail and involvement will vary by implementation. These may include:

Medvivo provides a Long Term Condition Monitoring service, where our own specialist nurses undertake initial case management for patients, responding to alerts, making the initial contact and providing advice, then informing the patient’s other health care workers as required. This service is available but is not within the remit of this GPSoc Framework agreement.

Resource Based Pricing

Where charges contain an element of charging for staff time (e.g. training, consultancy services or bespoke activity) then such charges should be referenced to the SFIA day rates contained within the supplier details section of the catalogue together with any non-staff

components contained within the charge (e.g. training materials, room hire etc.)

There are no planned additional resource based activities associated with an implementation or deployment of the service. Any bespoke, exceptional or change activity will be charged using the following rates, which are extracted from those described in the SFIA Day Rate card.

Deployment arrangements: costs, duration and activities associated with the deployment of this product or service

Medvivo’s costs above include the cost of deployment including: Project Initiation meeting to review and approve protocols, pathways and processes Initial On-Site Training Course for up to 10 clinicians, including CareHomePod and

Exit arrangements: costs, duration and activities associated with the termination of the supply of this product or service

Our primary concern during the exit period will be the continuity of care for the patient. Medvivo will undertake a reasonable level of activity at no additional cost during the exit process, including:

Removal of users from the various applications for security purposes Transfer of electronic data to the new supplier Meeting with new incumbent to explain the data

Costs will not include the recovery of the equipment from the Care Home, but if requested will be £125 per unit, with all units and their associated equipment being returned to the client’s central location. An additional charge of £50 per unit will be payable if decontamination is also required before return.

Additional activity will be charged through the Standard rate Card provided in Appendix C, with scope and estimated being agreed in advance through change control.OtherNone
