IntroductionGeOverdose.it is a geographic information system (GIS), interfaced to a website. GeOverdose displayis on a map overdose deaths and other acute fatal eventsrelated to drug and alcohol use.
What is GeOverdose?
GeOverdose represents acute drug mortality on a map of Italy, continuously updated in real time.
What is the purpose of GeOverdose?
GeOverdose aims at creating an open collaborative effortinvolving a multi-disciplinary network: including drug and alcohol professionals, emergency services, journalists, epidemiologists, local community representatives, servicescommissioners, law enforcement agencies and even ordinarycitizens concerned.
How GeOverdose works:
GeOverdose is a database of cases. These are continuously updated by 2 professionalsworking in public treatment services (Bologna, Palermo). In addition the website collects comments and integrations from about 100 visitors (10 are the most active) provided they have evidence supporting their statements.
For example, undefined or illdefined and cases waiting for more details are kept open for corrections for 60 days.
How GeOverdose works
3 categories of events:
1) Deaths clearly linked to illegal drugs and or alcohol;
2) Deaths that are suspectedto be linked to use of illicitdrugs and or alcohol;
3) Hospitalizations for acute cases of drugs and alcoholintoxications (excludingsuicides)
Methods for Identification and collection of information:
GeOverdose collaboratingvolunteers search:
What: online, national and localnewspapers.
How: Using a pre-definedsearch strategy and automaticalerts for the last 24 hours.
Intergating these searches with weekly manual searches, usingfree-text searches.
When: Results of automaticalerts and weekly searches are introduced into the system within24 hours.
Results : Periodic reports and trend analysis are published quarterly on Dedizioni, the SITD blog http://blog.sitd.it
Limitations
• Publication bias by online and paper magazines
• Publication bias by searchengines (keywords, text searches)
• Report bias by users• Incomplete/Incorrect news• Subjective interpretations by
voluntering collaborators
• Smaller areas (newspapers) are more likely to report overdoses than capital citiesand national journals;

Opportunities
• GeOverdose is a proposed model of intersectorial collaboration;
• GeOverdose stimulates discussionand information sharing amongfront-line professionals;
• It has a potential for real-time information sharing;
• Information can be triangulatedwith other sources (drugs seizures, waste waters analysis, EWS) to suggest hypothesis and trends;
• GeOverdose can rise localcommunities awareness for promptinterventions: mobile naloxone provisions, drugs consumptionrooms, outreach units)
• ……
• Despite the fact that the overallnumbers on overdose mortalityhave never been as reassuringas they are today, there are areas of the country where the probability of dying from drugsis much higher than in others.
• These differences may be due to an infinite number of reasons, notleast the geography and dynamics of drug trafficking, buthave never been thoroughlyinvestigated.
• There are numerous signs of a recovery in the heroin market and this is inevitable, if one takes intoaccount the record numbers of production and the changes thathave occurred in recent yearsregarding trends in drug use.
• There is also a growing impressionamong public and private service operators, which has so far notbeen confirmed by official data, that the current number of 'submerged' heroin users not in contact with services (and as suchat greater risk of death) is muchlarger than in the past.
Discussion
• GeOverdose can intergate information systems that are basedon retrospective data.
• GeOverdose has a potential to provide current and up-to-date trends.
