Safety Starts with Me!!!
1. Monsoon Precautions
During monsoon heavy rainfall begins. Following check points of Monsoon precautions should be carried before onset of monsoon to prevent hazards that could result in personal injury, property damage.

1. Check all external electrical points
a. Check plug socket are mounted on water ingress proof panels.
b. Check RCCB/ELCB are installed and functional
c. Check these are covered as soon as possible.
2. Ensure the dewatering pumps for projects where excavation is done is available and are in working order
3. Ensure the material construction/ passenger hoist to be protected with G.I shed/plastic cover.
4. Ensure where a passenger hoist is inside the shaft, see to it that the water seepage inside the shaft is prevented.
5. Check for rain water logging in completed in buildings, in the terrace and staircase to prevent the water seepage in to the lift shaft.
6. Check and clean all rainwater outlets in all completed buildings, for proper drainage of water.
7.
Check and ensure the meter rooms in all the completed building. The entry of the cable should be a chamber outside/inside the meter room & it should be filled with sand & IPS to be done on top to prevent it from entering in the meter room.
8. Check the lift pits for leakages and leak proof where required.
9. Ensure the LMR windows to have chajja’s for preventing rain water entering from coming inside.
10. Ensure Tarpaulin/Plastic sheets are kept ready at temporary storage of cement near mixture location, for mixture/hoist and for slab concreting.
11. Check the entire area for lighting and all signage are illuminated.
12. Check the temporary wires/Cables for open joints and cover with plastic for water sealing.
13. Ensure during monsoon, keep the windows of the flat not handed over, closed firmly and checked every day by the security/staff at site.
General precautions:
14. Check and clear all storm water drains in and around project site.
15. Cover all open drains.
16. Keep site clean and tidy and prevent obstruction at work site.
17. Work areas, passageways, office access, walkways, or other traffic ways, stairways and all other areas shall be kept free of debris and materials.
18. Cover materials at with tarpaulin to prevent damage.
19. Keep antidote if site is infested with poisonous snakes and insects.
20. Construction materials shall be stored or placed in an orderly manner, if possible stack on sleepers.
21. All PPE`S and tools to be stored in a dry place.
22. Cover the motors, gears, wire rope drums of cranes/material hoist by plastic/PVC sheets.
23. Check and repair roofs and anchoring, wherever required.
24. Provide rain protection sheds for D.G. sets, Electrical Panels, Electric distribution boards, workers etc.
25. Ensure provision of rain coat and gumboot to all sites personal.
26. Ensure rain protection shed for welding and gas cutting operations.
27. Ensure availability of first aid facilities at site. In first-Aid station, maintain adequate medicines to treat the ailing persons and emergency services are available 24 hour.
28. Scaffolds used on soft surfaces, which may become unstable and collapse. Ensure scaffolds for working at height that the ground where scaffold is to be erected firm or preferably erect them on sole plates.
29. Restrict height work & working on mobile scaffold in rainy & gusty atmosphere.
30. Check and ensure fitness of scaffold before start of work daily by a competent person. If unfit, put danger tag.
31. Carry out mosquito control measures and disinfestations around to control mosquito-borne deceases and other epidemic deceases from local authority.
32. The Excavated pits should be barricaded by rigid material at a distance of 1M from the edge to prevent fall of person or vehicle.
33. Backfill unwanted pits.
34. If old structure/wall close to deep excavated pit exists, support adequately to prevent from collapsing. Ensure sufficient shoring is provided or the excavations are back filled and compacted.
35. Ensure de-watering arrangement is available.
36. Inform all drivers/operators not to move or park vehicle close to the excavated area.
37. Ensure regular Sewage collection and disposal at labour camp and sites to prevent foul smell and health hazard.
38. Mobile cranes should not be operated if ground condition is not firm but soft which may topple the crane.
39. Provide adequate “wata” (BUND or DYKE) to retain water over slab top & make arrangement to release water through suitable arrangement in designated place.
40. Maintain internal roads in good condition without pot-hole & soft surface.
41. Any loose material such as G.I. sheet, shuttering material, etc stored at the height should be properly secured so that it will not fly off and cause injury to any one moving/ working below.
Electric Safety precautions:

To prevent electric shock, ensure all electric connections shall be taken through electric distribution boards/boxes fitted ELCB/RCCB (30 mA cut out rating) with sound earthing. Check and ensure continuity of earthing to earth plates in all distribution boards.
43. All electric distribution boards shall be protected from rainwater with adequate shed.
44. Provide rain protection shed for D.G.sets, Electrical Panels, etc.
45. Inspect installed ELCB &check earth resistance prior to monsoon.
46. All electric installation shall be properly earthed to ground and protected from rainwater.
47. All electric connections through industrial plugs and sockets.
48. Don’t touch the electric installations in wet condition.
49. Check for water accumulation around electrical installation like Main
electric panel, Electric D.B`S, D.G.sets, etc, if required elevate the level for safe approach.
50. Ensure body earthing is in good condition in all machineries.
51. Rectify water leakage in all offices/buildings as this may cause electrical faults/short-circuit in the electrical system.
52. Never allow cable joints in water.
53. All electric cables shall be routed at height over cable trays or suitable supports and should not be trailing on the ground.
54. Check all cable joints make them water tight.
55. Poles installed for over head cable lying should rigid and stable.
56. Ensure all halogen fittings protected against water ingress through glass fittings.
57. All water pumps connections only through proper ELCB & earthing protection.
58. Lightening arrestor: Where required the condition is warranting such as tall cranes & building lightening arrestor need to be provided according to the relevant code.
Labour camp Monsoon safety Precautions:
59. Camp area shall be fenced and security lighting be installed.
60. All housing units must be of good quality and condition.
61. Accommodation units must be cleaned on a regular basis and all waste disposed of into segregated waste skips with doors.
62. Garbage and waste food collection yard should be set away from living area.
63. Regular fogging & pest control is required to prevent from epidemic diseases.
64. Clear all water drains in and around labor camp. Provide covering on open drains.
65. The camp shall have adequate drainage to eliminate monsoon rains and units shall be elevated to eliminate water ingress.
66. Secure all GI sheets to prevent from flying off in case off stormy & gusty wind.
67. Prevent contamination of drinking water.
68. All units must be electrically powered and all temporary electric connections must be routed through ELCB`s installed on distribution boards/boxes and checked regularly .Lighting is required in each unit.
69. Ensure proper cable management and proper distribution of current load to prevent fire by over loading of wiring.

Ensure all cables /wires are not laid on G.I. sheet partitions/sharp edges/holes within G.I. sheets to prevent damage to cable insulation. Route the cable on ceiling through conduits pipes for lighting, this would avoid misuse of electricity and protection against electrical shock and fire hazard. The common switching arrangement made separate and electrician will be overall control authority.

2. Lessons Learned
2.1 Leg trapped in between counter weight &metal guard
Division : CGPL
Date of Incident : 05st May’15
Type of incident : FATAL
Incident Time :15:20 Hours
Location : Boiler # 40 Lift
Description of Incident:
Boiler # 40 lift maintenance activity was planned on the 5th of MAY 2015. Victim was engaged
from M/s. Metro Elevator Company Private Limited for carrying out the maintenance activity. He
instructed the lift operator to put the lift on normal mode and take the lift to the 13th floor. While
the lift was in operation he heard the sound of the chain hitting the metal plate. To identify the
source of sound he leaned forward. The counter weight which was moving downwards hit the
victim on his back and pushed him in between the counter weight and metal guard of the
counter weight. This resulted in the victim getting crushed in between the counter weight and it’s
guard plate and subsequent amputation of his right leg.

Recommendations:
1. Implementation of “No Supervision No Work” policy to be ensured by CGPL.
2. PTW system should be meticulously implemented.
3. HIRA should be reviewed for its adequacy every year.
4. Job specific JSA should be prepared.
5. Inspection & maintenance should be carried out only by qualified & competent
persons as prescribed Chief Inspector lifts.
6. Procedure should be developed for lift operation, inspection & maintenance.
7. Legal Compliance Monitoring should be strengthened.
2.2 Conductor fell on live 22 KV line

Date of Incident : 21st March’15
Type of incident : Occurrence
Incident Time :13:30 Hours
Description of Incident:
On 21st March’15 at around 13:30 hrs. While 220 KV Kalwa Salsette line- 3, A phase conductor
pulling work was in progress, Tensioner failed and the conductor was released. Conductor fell
on live 22 KV line, due to which Line tripped on earth fault. Both Tensioner and Puller ware
provided with grounds so there were no injury to any person and no equipment was damaged.
On Further Investigation it was found that the Tensioner operator heard some abnormal noise
from machine and he tried to adjust gear lever rod. While adjusting gear lever, the gear lever
came to neutral position and machine started releasing the conductor. When operator realized
this, he tried to force engage gear in high torque position but the gear lever broke and after this
tensioner lost control of conductor and released conductor in a free fall and the conductor
landed on 22 Kv live lines.
Root Cause Analysis

Warning tag for not operating tensioner under tension condition displayed on machine but operator tried to operate after hearing an abnormal sound when the machine was in tension condition

SYSTEM FACTORS:
Operating instructions including emergency operation were not displayed on machine Recommendations:
1. Training and certification from OEM for Machine Operator along with Certificate. Only
trained and certified person should operate this machine
2. Operating instructions along with emergency operation plan should be displayed on
machine
3. Fire Services week
SS Fort Stikine Ship berthed at Mumbai Port had fire & explosion on 14th April 1944. During fire
fighting 66 fire personnel lost their life along with fire fighters from auxiliary services & port
people at large. Government has declared the week as Observance of Fire Service Week in the
fond memory of those who lost their life, to spread the mass awareness amongst people.

4. Safety training
What is training? Training means helping people to learn how to do something, telling people what they should or should
not do, or simply giving them information. Training isn’t just about formal ‘classroom’ courses.
Why provide health and safety training? Providing health and safety information and training helps to:
ensure that people who work for you know how to work safely and without risks to health;
develop a positive health and safety culture, where safe and healthy working becomes second nature to everyone;
meet legal duty to protect the health and safety of employees.
Effective training:
will contribute towards making your employees competent in health and safety;
can help business avoid the distress that accidents and ill health cause; can help avoid the financial costs of accidents and occupational ill health, such as damaged
products, lost production and demotivated staff. Don’t forget that insurance might not cover all these costs.
Who needs health and safety training? Everyone do! Whether you are an employer or self-employed, are you sure that you’re up to date with how to identify the hazards and control the risks from your work? If not, you would probably benefit from some training.
Managers and supervisors do! Managers or supervisors need to know what is expected from them in terms of health and safety, and is it expected to be delivered. They need to understand health and safety policy, where they fit in, and how health and safety be managed. They may also need training in the specific hazards of the processes and how risks are to be controlled.
Employees do! Everyone who works, including self-employed people, needs to know how to work safely and without risks to health. Like supervisors, they need to know about health and safety policy, arrangements for implementing it, and the part they play. They also need to know how they can raise any health and safety concerns. Contractors and self-employed people do! Remember, these people might not be familiar with working environment and safety systems that are put in place for regular employees.

8. Security Highlights
I. Trombay 20 Apr 2015 – Atithi program was organized at Trombay by inviting as guest for the day
Mr. Vasant Dhoble – Assistant Commissioner of Police, Mumbai. Necessary security
arrangements were made for the smooth entry exit of the Chief Guest.
13 Feb – 20 Feb – 27 Feb 2015 – Exercise ‘’Sagari Kavach’’ was conducted from 0600
hrs of 16th Apr 15 to 1800 hrs of 17th Apr 15. High alert was declared by Head – Security
for the mentioned period, all processes and security measures were adopted and followed
as per procedures mentioned during Level 3 (High Alert). Police force was deployed at all
the vital locations round the clock during the Exercise.
20 Apr 2015 - On the eve of Fire Service Week 2015, a Fire Service Week Awareness
program was conducted by Fire department for security personnel.ill on security scenario,
safety briefing and OJT training in March were conducted in Trombay.
25 Apr 2015 – Security Officers Monthly Meeting held from 0800 hrs to 1300 hrs at Security
Conference Hall, Trombay with Head – Security.
09, 11, 22 & 26 Apr 2015 – On Job Training on various security related Gadgets and
Procedures were imparted to Security personnel.
II. MPL(Maithon)
23rd Apr 2015 - KAMARDIH (Coal handling plant area) & Purni Openings were closed.
23rd Apr 2015 - Foiling theft attempt: On 3rd Apr 2015 security personnel of BSS did a
splendid job by recovering six core 30 meters cable when miscreants were trying to take it away. Other items were also recovered in this month of total value of 71,100/-.
III. CGPL Mundra:
14th Apr 2015 – On the occasion of fire service week a fire fighting and live rescue demonstration was performed near CGPL project office parking area by CGPL fire, Safety, Medical and Security teams.
PRE-BRIEFING OF SAGAR KAVACH EXERCISE INDIAN COAST GUARD: On 13th Apr’15, Pre briefing session of Sagar Kavach exercise (GJ/DD-01/15) has been carried out at Coast Guard HQ in Mundra, exercise is scheduled to be conducted on 16 & 17.04.15, Mr. Sheril T - Lead Associate- Security attended the briefing session from CGPL.
On 15th & 16th, 17th Apr’15, In view of Indian Coast Guard conducting Sagar Kavach exercise, CGPL Sea front security put on high alert.
IV. Bhivpuri (Hydro):
Indefinite hunger strike started on 16th April by the Chairman of SSSKB Sahakari Sanstha
Mr. Agaj along with others came to an end peacefully on 20th April at 2000hrs due to deft
handling of the situation by Chief-Hydro and Head - Bhivpuri with the support of
Admin/Police & Security.
V. Transmission & Distribution:
04th Apr 2015 - Security Mock Drill Terrorist attack: was carried out with the help of Kala
Chowky police station at Parel RS at 2100hrs. Total 52 Police personnel and Commandos
participated in the mock drill. The aim of this exercise was to streamline the procedures and
action to be initiated during emergency in our location.
The training workshop for security guards of T & D was conducted at Dharavi receiving
station on 26 April 15. Total 30 security personnel’s attended the workshop. The following
was covered during the training:-
(a) Security process – People, Vehicle, Material movement. (b) Turn out, Discipline, Job responsibility of security guards and supervisor. (c) How to use of communication radio sets (practical) training (d) Practical training on frisking/checking by HHMD (e) Documentation at security gate. (f) Knowledge sharing. (g) Case study incident.
