SAFETY PRACTICES FOR CONSTRUCTION WORKS DIGBOI REFINERY (AOD) 1.0 Introduction Safety practices during construction are already provided in the GCC. In addition to these provisions, safety practices followed at Digboi Refinery are being described in these documents and to be followed for all construction works. 2.0 Work Permit System: 2.1 Work permit system must be followed for carrying out any job inside the Refinery, Flare System, LPG Dispatch Unit(LDU), Effluent Treatment Plant(ETP), and New Tank Farm(NTF).Presently permit system available are Hot Permit, Cold Permit, Height Permit, Excavation Permit, Confined space Permit, Radiography Permit, Electrical energizing/ de- energizing Permit & Vehicle Entry Permit. The work permit system should be followed as per OISD-105. 2.2 Permit such as Hot Permit, Excavation permit, Confined space permit should be obtained by IOCL’s authorized person and shall be registered at Fire Station. If the permit validity has been expired and not closed, in such case new permit will not be issued to the same permit receiver. 2.3 The permit once registered at fire station or Safety Section for a period given in the permit, the works to be performed by the contractor after taking clearance from the work place area and need not to be re- registered at fire station/safety section. SAFETY PRACTICES FOR CONSTRUCTION WORKS DIGBOI REFINERY (AOD)

1.0 Introduction

Safety practices during construction are already provided in the GCC. In addition to these provisions, safety practices followed at Digboi Refinery are being described in these documents and to be followed for all construction works.

2.0 Work Permit System:

2.1 Work permit system must be followed for carrying out any job inside the Refinery, Flare System, LPG Dispatch Unit(LDU), Effluent Treatment Plant(ETP), and New Tank Farm(NTF).Presently permit system available are Hot Permit, Cold Permit, Height Permit, Excavation Permit, Confined space Permit, Radiography Permit, Electrical energizing/ de-energizing Permit & Vehicle Entry Permit. The work permit system should be followed as per OISD-105.

2.2 Permit such as Hot Permit, Excavation permit, Confined space permit should be obtained by IOCL’s authorized person and shall be registered at Fire Station. Whereas Height permit shall be registered at Safety section. All these permit shall be closed by permit receiver and returned to the registering section. If the permit validity has been expired and not closed, in such case new permit will not be issued to the same permit receiver.

2.3 The permit once registered at fire station or Safety Section for a period given in the permit, the works to be performed by the contractor after taking clearance from the work place area and need not to be re- registered at fire station/safety section. However, if the permit is extended for carrying out the work, in that case the extended permit shall be re-registered at fire station/Safety section.

2.4 Permit should be obtained by IOCL’s authorized person. Cold permit, radiography permit, Electrical energizing / de- energizing Permit and Vehicle Entry Permit record should be maintained by the permit issuer and ensure that permits have been closed and returned to the issuer.

2.5 Radiography with x-ray machine shall be carried out under hot work permit. Cold work permit may be used if radiography is done with radio-active isotopes. Radiography should be carried out when occupancy is least. Based on the maximum allowable radiation, area shall be cordoned off. Adequate warning signs on the outskirt of cordoned area shall be provided.

2.6 Contractor shall fulfill all the requirements/ conditions given in the permit before commencement of work.

2.7 No hot work including Civil jobs like chipping, excavation etc. which produces open flame/sparks ignition should be carried out in the plant area unless such work is authorized by fire & safety permit issued by IOCL(AOD)/ authorized representative. Hot work permit would be given for a period of one shift only to carry out hot work on all the working days. The permit may be extended by the issuer as and when required.

2.8 The permit should be available at the work site. The contractor should produce the same to the inspecting personnel of IOCL. The inspecting personnel can stop the work if valid permit is not available with the contractor at site or permit requirements are not fulfilled by the contractor.

2.9 No person shall be required or allowed to enter any confined space, until all practicable measures have been taken to remove any gas, fume, vapor or dust, which may be present so as bring its level within the permissible limits and to prevent any ingress of such gas, fume, vapors or dust and unless entry permit in writing has been issued based on the test carried out that the space is reasonably free from dangerous gas, vapor or dust.

2.10 Confined space entry permit is required for the protection of personnel entering a confined space such as vessels, boiler, storage tanks, large diameter piping, etc. Excavation more than 1.2 meter deep, entering on floating roof tank when the roof is more than 3 meter down from the top, space located below the ground level such as pit, drain, channel restricted entry/exist etc. also fall under the confined space.

2.11 Before start of any job in a confined space, safety demonstration to be conducted by operational personnel in presence of the permittee to the contractor personnel. During the safety demonstration, the safe ingress to and exit from the confined space shall be explained. The safe use of utilities like air, steam, nitrogen, electricity etc. with associated hazards shall also be explained to the contractor workers/IOCL employee.

2.12 In the event of emergency indicated verbally or by fire alarm, all work permits/fire and safety permit issued to the contractor and his employees will stand cancelled. The contractor’s personnel must suspend all jobs, leave the job in safe condition and move to a safe location /assembly point.

2.13 IOCL will nominate the Officer-In-Charge for co ordination with contractor in connection with work permit system till completions of the job.

2.14 Any safety related matter should be brought to the notice of Site-In-Charge. If required, the Site-In-Charge can contact fire and safety section for resolving the matter.

3.0 Safety Training to Contractors’ Workmen :

3.1 All contractors’ workmen must have to undergo safety briefings before they are issued gate passes by CISF. On completion of this briefing, individual Safety Cards will be issued to the workers by Safety Section which will be valid for six months.

3.2 Daily Tool Box talks must be conducted by the Supervisor of the contractor prior to starting of jobs at site. Attendance of workers with their signatures in such Tool Box talks must be properly recorded in a register. The register should also contain the name of the persons delivering Tool Box talks with dates along with the topics discussed.

4.0 Safety arrangement at work place:

4.1 All contractors’ employees must use safety appliances to protect against hazards as and when required under the “Work Permit System” to carry out the job safety. It will be the responsibility of the contractor to provide common safety appliances and Personal Protective Equipment(PPE) to his employees.

4.2 All the fire safety conditions laid down in Fire & Safety permit should be strictly adhered to by the contractor.

4.3 Use of bamboo scaffolding/ staging is strictly prohibited. Only steel scaffolding/staging(pipes & clamps type or cup-lock type) shall be used by the contractor.

4.4 Proper base plate supports must be provided in the scaffolding posts. The stage must be provide with hand rails and toe guards on the working platforms.

4.5 Before use of the scaffolding ,it must be certified by a competent/certified rigger and until the check, “Not Fit for Use” must be displayed in red tag. Once certified for use, the red tag should be removed and “Ready for Use” to be displayed in green tag. The green tag must be duly signed by all concerned including the rigger.

4.6 While working at height using scaffolding, safety belts with double anchor hooks must be worn.

4.7 In case of fire alarm/siren, all work must be stopped immediately. 4.8 IOCL(AOD) reserves the right to make periodic checks of the construction area at any time

to ensure compliance of safety rules. 4.9 In case the contractor does not follow IOCL(AOD) Safety regulation, IOCL(AOD) shall have

all the right to stop the work and initiate appropriate disciplinary action against the contractor.

4.10 A joint record will be maintained at the place of work for the stoppage of work. The work can be stopped by any operating personnel and he shall sign in the register maintained by the contractor but later on the record should be countersigned by an authorized officer of the owner. However for such stoppage of work, no compensation will be paid to the contractor.

4.11 Tool Box Talk should be performed by the contractor on daily before start of the job. A register should be maintained by the contractor at site where the details of Tool Box Talk along with name and signature of their entire employee to be recorded. The register will be examined by IOCL’s official at Site.

4.12 Any flammable materials like jute, dry grass, plastic tarpaulin etc. should not be kept near the hot job area.

4.13 All gas cylinders shall be properly secured in upright position. 4.14 Acetylene cylinder shall be turned and kept in such a way that the valve outlet points away

from oxygen cylinder.

4.15 Acetylene cylinder key for opening valve shall be kept on valve stem while the cylinder is in use, so that the acetylene cylinder could be quickly turned off in case of emergency.

4.16 Use flash back arrestors to prevent back-fire in acetylene/oxygen cylinder. 4.17 Valves of all cylinder shall be kept closed, when not in use. 4.18 A running water hose near the hot job is to be provided by the contractor.

5.0 Use of PPE

5.1 Contractors shall provide all safety item/equipments like helmets, protective clothing, gum

boot, hand gloves, goggles etc. required for various jobs. Corporations will provide breathing apparatus free of cost. Apart from breathing apparatus, if the contractor takes any other Safety/lifting/handing tools from Corporation, Contractor will have to pay charges as per laid down procedure of the Corporation. These may be collected from Central Engineering Godown of Refinery. In case of damages or loss of materials / equipments provided by IOCL on loan/returnable basis, same shall be recovered from the contractor’s bill at the rate decided by IOCL plus 15% surcharge thereon.

5.2 All employees of contractor shall wear safety shoes and helmet while carrying out the works at job site. No worker shall be permitted inside the refinery premises without safety shoes.

5.3 The helmet provided to the workers should be of good condition. 5.4 Welding should use proper weld shield while doing welding job. 5.5 Grinder should use proper goggles and hand gloves while carrying out grinding jobs. 5.6 Worker should use proper goggles, mask, gloves and face shield while doing shot biasing,

spray painting etc.job. 6.0 Welding Job/ Fire extinguishers/Fire hydrant water :

6.1 Fire extinguishers and running water shall be provided by the contractor during

execution of work and all the equipment shall be maintained by them in good working condition all the time.

6.2 As a precaution during welding, proper fire screen must be provided by the contractor to prevent eye injury to other personnel working within the area.

6.3 Necessary precaution and protection shall also be exercised to prevent fire and other hazards from failing sparks and hot slag from welding and cutting operations.

6.4 Welding machines should be located in non-hazardous and ventilated areas. 6.5 Fire Hydrant water should not be used for any purposes other than fire fighting.

Appropriate disciplinary action against the contractor will be initiated in case of violation. 6.6 During welding, the earthing must be done at a place/ point selected carefully & cleared

by the SIC.

7.0 Housekeeping :

7.1 The Installation site shall be completely fenced by contractor on all three sides with only one entry point for movement, as per instructions of Engineer-In –Charge.

7.2 The Contractor’s personnel should not cross the fencing and they should work only within the fenced area.

7.3 The contractor should ensure that the job area is cleaned as soon as the job is completed Good Housekeeping will eliminate many unnecessary hazards causing accidents.

7.4 All stairways, platforms and walkways must be kept clean at all times. 7.5 Make shifts of any kind is strictly prohibited.

8.0 Use of Mobile/ Portable Welding Set, Generators, Hydra & Cranes, Transportation of Cylinders :

8.1 Entry of automotive vehicles with approved flame arrestor only will be permitted for carrying materials within premises/ fenced are Entry permit for Vehicles to be collected from the authorize Area-In-Charge ,

8.2 The mobile /portable welding sets are to be tested, audited by the Electrical Engineer of IOCL(AOD)and Health Certificate to be obtained prior to use. The health certificate is to be revalidated after every six months.

8.3 All the portable generators are to be fitted with approved spark arrestors. 8.4 Helper/ Khalasi/ “Flag Man” should always accompany during heavy vehicle/ hydra/ crane

movement. 8.5 Hydra shall be utilized only for loading and unloading purpose and not for material

movement/ transportation.

9.0 Loading and Unloading of Materials at Site :

9.1 Mechanical/hydraulic loading and Unloading equipment are to be used for loading and unloading of materials at work site.

9.2 Hydra/ Lift all must never be used for shifting materials from one place to another. These are to be used for only loading and unloading.

9.3 For shifting of pipes, four-wheeled trailers (truck/tractor mounted or manual) are to be used. Pipes must be properly secured/fastened with the trailer in order to avoid fall during transportation.

9.4 Loading and unloading must be carried out under supervision with proper signals for communications.

10.0 Use of electrical equipment

10.1 The contractor will ensure that all electrical equipment used by his personnel are properly grounded and fitted with a proper plug. Use of damaged/bare cables/conductors for any machine is strictly prohibited. Cable shall be laid preferably in one piece length to avoid joints. All cable must be properly insulated.

10.2 Flame/spark/explosion proof fittings and fixtures will be provided by the contractor wherever required and/or directed by shift-in-charge/safety Engineer.

10.3 Temporary electrical panels to be tested/audited by the Electrical Engineer of IOCL(AOD) and Health certificate is to be obtained prior to use. The health certificate is to be revalidated after every six months.

10.4 ELCB(Earth Leakage Circuit Breaker) for all temporary connection must be provided. 10.5 Any earthing wire not to be connected to the structure/pipelines. 10.6 No unsafe temporary connections, naked joints/wiring etc. to be made at site. 10.7 Working in wet condition on electrical system is not permitted. 10.8 The electrical system arranged for the work is not to be over loaded. 10.9 To ensure that temporary cables are free from cuts, damaged insulation, kinks or improper

insulated joints.

10.10 All power supply cables should be laid properly and neatly so that those cannot create hindrance to persons working in the place and physical damage to the cables does not take place during construction activities.

11.0 Stress Relieving jobs Following Safety precautions must be taken during Stress Relieving(SR) jobs. 11.1 Full scale barricading of the active area of SR with caution notification on live voltage(with

grade) present. 11.2 Notification for entry in active area by only authorized persons (Name of the authorized

persons should be mentioned in the Hot Permit). 11.3 No charging of electrical line to the SR equipment till all connections, insulation

level(including the SR Equipment) are checked and certified by Electrical Section under P&U department as safe.

11.4 The SR equipment should be placed at the most possible convenient, safe and clean place. To the extent possible, clumsy area to be avoided.

11.5 The operator should be shown the electrical main switch main (With a hung board on the main switch of ‘SR Duty’ ) for immediate isolation in case of emergency.

11.6 Under no circumstances the equipment on run should be left unattended. 11.7 The health of the SR equipment and accessories are to be certified by the contractor and

to be verified by the EIC. 11.8 A Suitable size electrical insulating Rubber Mat( of required voltage grade) to be placed

for the operator of the SR equipment. 12.0 General safety norms for working at Corporation Site (Not limited to the following) :

12.1 Smoking or carrying smoking requisites is not allowed in the Refinery. 12.2 Contractor should not block roads with their materials in order to ensure movement of

fire tenders and safety equipment. 12.3 Near -miss incidents are to be reported to the respective Plant in charges /safety Cell. 12.4 Necessary advice may be sought on any of the safety matter from Fire & Safety officer and

safety Cell as and when necessary. 12.5 No spark or flame producing appliances or equipment’s including flash lights, transistors,

camera, match box, mobile phone, etc. will be allowed to be taken inside the Main Gate of the Plant. Such material, if brought, must be deposited at the Main Gate with Security personnel.

12.6 Contractor will be responsible for the safety of his personnel and property as well as the safety of property of the Corporation.

12.7 Bringing in, or being under the influence of, intoxicants or narcotics, inside the factory premises are strictly prohibited.

12.8 Transportation of empty or filled cylinders, by rolling, is strictly prohibited inside the plant areas.

12.9 Any situation likely to compromise, affect the safety of any employee or other co-workers must be promptly reported to the Area –in-Charge.

12.10 Supervisors shall develop safety awareness in the minds of all the employees working under him. He must set a good example in knowing and observing Safety rules and precautions.

12.11 Walking on pipelines, instrument cables or cable trays is prohibited.

12.12 Horseplay is strictly prohibited inside the Refinery. 12.13 It is strictly prohibited to use Naphtha of any highly inflammable liquid or corrosive

chemicals such as caustic soda or acid for cleaning purposes. 12.14 Acetylene , Oxygen cylinders must not be taken inside any closed vessel under any

circumstances, whereas LPG cylinders are not be used for gas cutting purpose. 12.15 Any injury must be reported to the Officer-In-Charge immediately and send the injured

person to hospital. 12.16 Contractor shall designate a particular person at the level of Site in Charge or supervisor

as safety in charge and shall inform user department for each contract. He will be responsible for strict adherence to safety measures.

12.17 For any violation of safety measures as detected during the course of execution of job either by the user department or by Safety Officer, Punitive action will be taken against the contractor as deemed fit by the Corporation.

12.18 In the event of an accident ,action will be taken as per prevailing statutory requirements.

NOTE :

1. Contractor shall specify a particular person to user department for each contract, who will be responsible for strict adherence to safety measures.

2. Safety clearance is to be taken by the Contractor in writing before commencement of work in the Corporation’s Standard Safety Clearance Form.

3. Contractors shall provide all safety item/ equipments like helmets, protective clothings, gum boot, hand gloves, goggles etc required for various jobs Corporations will supply breathing apparatus free of cost and Safety belt.@ Rs. 5.00 per belt per day. Apart from breathing apparatus and safety belt, If contractor takes any other Safety / lifting/ handing tools from Corporations, Contractor will have to pay hire charges as per laid down procedure of the Corporation. These may be collected from Central Engineering Godown of Refinery. In case of damages or loss of materials / equipments loaned on basis shall be recovered at market value / rate plus 10%

4. For any violation of safety measures as detected during the course of execution of job either by the user department or by Safety Officer. Punitive action will be taken against the contractor as deemed fit by the corporation

5. In the event of an accident, action will be taken as per prevailing statutory requirements. 6. In case of any work found to be unsatisfactory the contractor at his own cost including supply of

necessary materials will have to rectify the work to the satisfaction of the Corporation. No additional claim will be entertained in such case.

7. It is essential that the contractors contract the user departments for detailed explanation of the jobs content before quoting for it.

8. The tenderer should specifically state whether he is relative of any Director of our Corporation or the tenderer is a firm in which Director of our Corporation or his relative is a partner or tenderer is a private company in his Director of our Corporation is a member of Director, Declaration to be given in form’C’ attached.

9. The Earnest Money Deposit shall be refunded to the Tenderer within Six months of production of Cash Receipt issued by AOD in case the refund is not claimed within 6(Six) months the same will be forfeited.

10. Please note that all payments and receipts due to you and due from you will be rounded off to the nearest rupee. For this purpose wherever the amount to be received/ paid is less than 50 paise the same will be ignored and wherever it is 50 paise or more it will be rounded off to the next rupee.

11. Please mention the provident fund Code No. issued to you by Government Authority in favour of workers engaged by you.

12. Vehicle engaged on the contractor jobs inside Refinery must Spark Arrestor falling which the contractor will not allow to work.

13. Contractor will be liable to Sales Tax as per Assam Sales Tax Act 1947. 14. Contractor must obtain adequate Insurance coverage under the Workmen’s Compensation Act 1923

Photocopy of the Insurance Policy in respect of workman, engaged by the contractor on Corporation’s job is to be produce to the concerned department prior to commence any jobs offered to him by the Corporation.

15. Contractor must produce declaration on a nonjudicial stamp paper value of Rs. 10/- (Rupees ten only) against each contract prior to signing the Work Order/ Agreement Sample of declaration can be obtained from Contract Section

Safety, Health and Environmental guidelines to contractors

At work place the contractor should comply with the adequate Safety Health & Environmental measures as under but not limited to the following

1. Maintain work place plant, equipment, tool and machinery and organize work in such a way that as for as possible, There is no risk of accident or injury to health of Workers.

2. Unsafe situation liable to arise at the workplace is prevented as soon as possible.

3. Excessively and unnecessarily strenuous work positions and movement are avoided.

4. Material and products are used which are suitable from safety and health point of view.

5. To establish committee with representatives of works and management (contractor)for the participation of workers in ensuring safe working conditions.

6. To arrange for regular safety inspection by competent person at suitable intervals. The competent person to examine and test to ascertain the safety of working platform, construction machinery and equipment.

7. To assign workers to employment for which they are suited by their age, physique and state of health and skill.

8. To satisfy themselves (contractors) that all workers are suitable instructed in the hazards connected with their works and environmental and trained in the precautions necessary to avoid accidents and injury to health.

9. On dispersed sites and where small group of workers operate in isolation, contractor should establish a checking system by which it can be ascertained that all the members of a shift including operators of mobile equipment have returned to the camp or base at the close of the work.

10. Contractors should provide appropriate first-aid, training and welfare facilities to workers and also provide personal protective equipment and clothing.

11. Whenever two or more contractor undertake activities simultaneously or successively at one construction site , the principal contractor should be responsible for planning and coordinating safety aspects.

12. A suitable housekeeping program should be established and continuously implemented on each work site which should include provision of. a. The proper storage of materials and equipment. b. The removal of scarp, waste and debris at appropriate intervals.

13. Loose materials which are not required for use should not be placed or allowed accumulate on the site so as to

obstruct means of access to and engress from work places and passage ways. Workplace and passageways that are slippery owing to rain, oil or other causes should be cleaned up or strewn with sand sawdust, ash or the like.

14. Combustible materials used in the work-site are to be stored in a safe place inclosed metal containers Sufficient fire extinguishing equipment should be made available to prevent danger of fire.
