    HEALTH AND SAFETY POLICY

    1. It is policy of Syarikat Pembinaan LAL Sdn. Bhd. that operations shall becarried out to meet the requirement of the Factories and Machinery Act (1967)

    and Regulations. And that our operations be carried out only under the legal

    requirement.

    2. To ensure that this policy is effectively enforced LAL will formulate and

    implement a comprehensive policy in respect of safety at work and thus.

    Detailing of individual and collective responsibilities throughout themanagement structure.

    3. Within the framework provided by this policies all employees have a part toplay in maintaining the highest standards of safety and achieving full

    compliance with legal requirement. The basic need is for every employee to

    care for himself. Colleagues and the public.

    4. There will be joint consultation on health and safety matters with employee

    representatives through safety committee at all office and site locations.

    5. LAL is responsible for ensuring that all its employees receive the necessary

    training to carry out their duties in a safe manner and any employee charged

    with particular responsibilities for safety can fulfill those duties andobligations.

    6. This policy forms that basic for a reduction in hazards and an improvement insafety at work. Its effectiveness will come form the individual and combined

    efforts of us all to make it successful.

    STATEMENT OF INTENT

    The Policy objective will be achieved through the action of all employeesin the Company in accordance with the general responsibilities detail of

    the following page. The Policy shall be brought to their attention by the

    following means:-

    a. On introduction, it shall be issued on a personal basic to staff

    employees.

    b. Site personnel will receive a copy of Syarikat Pembinaan LAL Sdn.

    Bhd. Site safety booklet containing the appropriate policy details

    are shown.

    c. All new employees shall received induction training at which time

    they shall be instructed in the policy requirements and any specific

    hazards associated with the work under taken.

    d. It shall be displayed on Safety Notice Boards in Main and Site

    Offices.

    We ask you become familiar with the policy in the shortest possible timeand to implement it during all working operations.

    HEALTH AND SAFETY POLICYStatement of Policy

    The health and safety policy defines the companys general objectives.Organization and arrangements in the field of health and safety. The

    Companys Health and Safety Policy has been designed to ensure:-

    a. The prevention of injury to and impairment of health of all persons

    affected by Company operations.

    b. The Observance of the requirement of the Factories and Machinery

    (1967): The Factories and Machinery (Building Operation and

    Works of Engineering Construction) (Safety Regulations 1986); theOccupational Safety and Health Act 1994 and Regulations and all

    other, governing our operations.

    c. The adoption of safe working practices and standards.

    d. The establishment of an organization with clearly defined

    responsibilities for carrying out the policy and monitoring its

    effectiveness.

    e. The promotion of joint consultation in Health and Safety.

    f. The revision of the Health and Safety Policy regularly.

    SECTION 1 - SAFETY INFORMATION

    1 The Project Managers must make available the following Acts / Rules &Regulations at sites for references.

    SECTION 2 - GENERAL SAFETY

    1. HEAD PROTECTION

    a. Generally, all construction sites are required to wear safety helmets.

    The Project Manager however, will decide and establish those areas oractivities where there is risk of head injury and where the wearing of

    safety helmets is compulsory.

    b. The areas of activities will, if practicable be clearly displayed with

    appropriate safety signage, or otherwise brought to the attention of all

    employees or persons concerned about the prevailing safety hazards.

    c. All employees and visitors who may be in an area of risk of head injury

    will be issued with safety helmets and given clear instruction to wear

    them. A record of issuance of helmet shall be maintained.

    d. The Project Manager must ensure that safety helmets are issued to

    employees that are required to wear them. The supervisors will ensurethat the helmet regulation is complied. Any employee who fails to wear

    helmets will be issued a Notification of Breach on Health and Safety

    Rules and Regulations (SAF 2).

    e. Safety helmets must be maintained in serviceable condition. Damagedsafety helmets must be replaced. However, willful damage loss of

    safety helmet will be debited from the employees pay.

    2. FOOT AND HAND PROTECTION

    a. The Project Manager must ensure that all employees, workers and

    visitors who may be at LAL construction site shall wear appropriatefoot and hand protection at designated areas at all times.

    b. Suitable and appropriate foot wear shall be worn by everyone at a site toensure that any injury is prevented.

    c. At water clogged or muddy area, appropriate water boots shall be worn.

    d. For electrical works, especially when working with high voltages,

    appropriate insulation boots and gloves shall be worn to avoid any

    possible electrical hazard.

    e. Suitable hand protection shall also be provided to workers who may beexposed to hand injury when handling substances / materials which are

    hot, corrosive, sharp or rough.

    f. Gloves shall not be worm when working or handling machinery with

    moving parts, e.g. electric saw, as gloves can be caught and trapped and

    consequently injure the hands.

    3. EYE PROTECTION

    a. The Project Manager must ensure that adequate and suitable eye

    protections are made available for use by operatives for the following

    operations:-

    (i) Grinding of metal

    (ii) Cutting or breaking of glass, hard plastic and metal

    (iii) Cutting, grinding and hacking of concrete, fire clay, plaster,

    stone or brick

    (iv) All welding operations

    b. It is the duty of every person to use the eye protection whilst employed

    on any of the processes listed above.

    c. Employees must take reasonable care of the eye protections provided

    and not willfully misuse them and must report immediately to his

    supervisor the loss or defect of the eye protection supplied.

    d. It is the responsibility of site management to ensure that the eyeprotection devices are properly controlled and recorded.

    SECTION 2 - GENERAL SAFETY

    4. RESPIRATORY PROTECTION

    a. Where employees and workers are subjected to respiratory hazard in a

    work place, the Project Manager shall ensure that adequate protectionbe provided to safeguard the health and safety of the employees and

    workers concerned.

    b. Appropriate types of respiratory protective equipment shall be providedto workers who are required to work in confined spaces and areas where

    toxic gases or other contaminants like dust particles may be presentabove permissible level.

    c. Respiratory protective equipment shall be cleaned and disinfected

    before reissue them to other workers in order to avoid infection.

    d. Employees required to use the respiratory equipment shall be trained to

    wear them properly. They shall also be briefed on its uses andlimitations.

    e. Factory and Machinery (Mineral Dust) Regulations 1989 stipulate the

    obligation if employers and the duties of employees exposed to dusthazards.

    5. FACE PROTECTION

    a. Where site operations give rise to the danger of injury from airborneparticles, hazardous spray, molten metal or hot solution, then adequate

    face shields and neck protection shall be worn.

    b. Where necessary and practicable, the face shield and neck protection

    shall be provided by the Management.

    6. HEARING PROTECTION

    a. All employees and workers shall be protected against any hearinghazard that may exist at any construction site.

    SECTION 2 - GENERAL SAFETY

    b. The Project Manager shall ensure that no body is exposed to a noiselevel beyond the Permissible Exposure Limit (PEL) as stipulated in first

    schedule of Factories and Machinery (Noise Exposure) Regulations

    1989. The action level of noise pollution is 90 dB(A).

    c. When the action level is detected for location where workers are

    exposed continuously for at least 8 hours, appropriate noise expose

    reduction or hearing protection shall be provided. The hearingprotection makes use earplugs, earmuffs, ear defenders or other suitablenoise attenuation devices.

    d. Where first action level is detected appropriate earplugs or muffs ordefenders with adequate attenuation shall be provided to workers

    employed in this area. If the second action level is reached,

    administrative control and / or engineering control shall be instituted asfar as is reasonably practicable.

    7. SAFETY BELTS, LIFELINES

    a. Where any person is required to work at a place from which she / hewill be liable to fall a distance of more than 10 feet (3 meters), means

    shall be provided to ensure her / his safety and such means shall, where

    practicable, include the use of safety belts or lifelines.

    b. The Project Managers shall ensure that the fall arrest devices like safetybelts, lifelines are provided to all employees and workers who are

    required to work at height which will be liable to fall more than 10 feet

    (3 meters) or the landing area is hazardous.

    c. The worker shall inspect every safety belt and lifeline before use. Any

    slight damage to the safety belt or lifeline must be reported to the

    supervisor for its replacement. Soiled or damaged safety belts orlifeline must be segregated or destroyed.

    8. MANUAL LIFTING AND HANDLING

    a. A person shall not be employed to lift, carry or move any load so heavyas to be likely to cause injury to him / her.

    b. Six major points to observe when lifting:-

    (i) Back straight

    (ii) Chin in

    (iii) Arms close to body

    (iv) Feet slightly apart and slightly outward

    (v) Grip load with palms, not first fingers

    (vi) Bend knees lift with the legs

    c. General points to observe

    (i) Beware of slippery surfaces

    (ii) Size of job, look out for jagged edges

    (iii) Clear the walkway

    (iv) If team work, clear instruction must be given by one team leader

    (v) Special care shall be taken when moving gas cylinders or roundobject

    d. Unloading materials

    (i) Use mechanical loader for loading / unloading heavy and

    awkward objects

    (ii) Use a platform to reduce the distance between lorry and ground

    when unloading manually

    (iii) Ropes and chains used must be strong enough for the job and

    inspected to ensure in good condition

    (iv) Ensure you have enough men to do the job

    9. SAFETY NETS

    a. Where there is danger of falling objects at the external wall of a

    building and any opening where workers may fall into. Project

    Managers shall ensure where practicable, that safety nets are installed toprevent any accident caused to people working below and accidental

    fall of workers working above.

    b. All safety nets must be of good condition/repair. They must be properly

    secured during installation. Periodic inspection must be carried out to

    ensure their security.

    10. SAFETY SIGNAGE

    a. Safety and security signage and notices are very important tools fornotifying and warning people of the danger and hazards that are

    prevailing at the construction site. This preventive measure is one ofthe best strategies in reminding safety awareness.

    b. The Project Managers shall ensure that adequate and relevant safety and

    security signage/notices are conspicuously displayed to optimize their

    effectiveness.

    c. The signage shall ideally be in pictorial and in appropriate languages

    namely Bahasa Malaysia, English and Chinese.

    11. WORK OVER WATER

    a. The danger of falling into water, being carried away by currents or

    drowning is prevailing when working over or adjacent to water. The

    Project Managers, therefore, are necessary to take the followingprecautions.

    (i) Working platforms must be properly secured and level to avoidtripping.

    (ii) Toeboards and handrails must be fitted to prevent dislodgementin high winds and swell conditions.

    (iii) Ladders must be securely lashed to prevent movement.

    (iv) Safety equipment like safety net, belt, helmet, life jacket or

    buoyancy aid must be made available where necessary and wornwhen working over water.

    (v) A rescue boat must be made available where necessary andmanned by competent person who is a qualified first aided and

    has sufficient knowledge of river / sea conditions of that area.

    12. SAFETY IN CONFINED SPACES / SEWERS

    a. It is the responsibility of the Project Manager to ensure that a safesystem of work is adopted and that all necessary action / investigation

    has been carried out prior to entry into a confined space, manhole or

    sewer. The Safety Officer must also be informed prior to the

    commencement of work in such areas.

    b. He must ensure that:-

    (i) Permit of work system must be implemented and strictly

    enforced;

    (ii) Atmospheric testing is carried out before entry and periodicallyduring work activity. Record of tests must be kept;

    (iii) All necessary protective / emergency equipment is readilyavailable and in good working condition;

    (iv) All personnel involved are suitable trained in the use of theequipment;

    (v) The activity is properly supervised. Ensure no man is working

    alone in the confined space / sewer / manhole;

    (vi) A lifeline shall be required to be attached to the worker in the

    confined space and another man standby outside at the other endof the line for emergency withdrawal.

    13. WORK ON SCAFFOLD (ERECTION & REMOVAL)

    a. Project Managers must ensure that all scaffold erection Supervisorsemployed on site for the erection of scaffold and falsework are DOSH

    certified. Where this is not the case, a letter of competency shall beobtained and the matter referred to Safety Manager / Office.

    b. The erection and removal of scaffold shall comply with factory and

    machinery (Building operation and work of engineering construction

    safety) Regulations 1986 part X.

    c. Do not remove any tube or fitting or otherwise interfere with the

    scaffolding in any way. Alterations shall only be done by an authorizedperson.

    d. Do not use scaffold under erection or whilst being dismantled and a redtag. DO NOT USE must be prominently displayed.

    e. Keep ladder access and walkways clear. Report any defects as soon as

    possible.

    f. Do not overload a scaffold. Maximum loading of scaffold platform

    must be clearly indicated.

    g. Materials shall never be thrown, tipped and dropped from scaffolds(other than into a purpose-made rubbish chute).

    h. Never climb up or down scaffolding. Install staircases or use a ladder.Which must be secured and extended 1.0m above landing platform.

    i. Aluminium access ladders must not be used in locations where theymay come into contact with electricity.

    14. EXCAVATION WORK

    a. Before commencing any excavation work, it is important to know the

    location of the services and that suitable and sufficient support materialare available. Excavations shall be treated as temporary works and the

    following basic safety procedures must be complied:-

    (i) Use proper gangways for crossing excavations.

    (ii) Do not stand or walk on struts. Use a proper ladder access.

    (iii) Do not interfere with or alter support work unnecessarily unlessauthorized to do so.

    (iv) Ensure that manholes are covered adequately, protected and

    signaged.

    (v) When vehicles are working near edges of excavation, chocks

    shall be positioned to prevent tipping vehicles overriding or

    falling in.

    (vi) Do not work in an excavation over 1.2m deep unless it is

    properly supported or battered, or in solid rock.

    (vii) Never work beyond the supported sides of an excavation.

    (viii) Working area above and alongside excavations shall be keptclear of objects and loose material. Soil heaps must be kept well

    clear of sides of excavation (at least 1m).

    (ix) When timber is used for supports ensure that it is free fromdefects.

    (x) Ladder being used as a means of access and egress must alwaysbe placed in the supported section of the excavation.

    (xi) Be careful of slipping when using ladders, especially under wetconditions. All ladders must be tied and extended 1.0m above

    landings.

    (xii) A competent person must inspect the excavation and its supportfacility regularly to ensure its integrity.

    15. MOBILE SCAFFOLD SAFETY

    a. Mobile work towers shall only be used and moved on surfacessufficiently firm and level to avoid instability.

    b. Where work towers are to be erected on suspended floors, ensure thatthe bearing capacity of the floor is sufficient for the load to be imposed.

    c. On soft or uneven ground and unstable surfaces, a temporary foundation

    or track shall be provided to facilitate the erection and moving of thework tower.

    d. Work towers must be erected by competent persons.

    e. Prior to erection, check to ensure that the:-

    (i) Tower components and construction are in good condition.

    (ii) Castor wheels and swivels for effective rotation.

    (iii) Brakes and locking devices for effective operation.

    f. Prior to use, check to ensure that the:-

    (i) Tower is firm and stable.

    (ii) Horizontal braces and the platform are level.

    (iii) Outriggers are set correctly and secured.

    (iv) Base plates / castor wheels, including outriggers, are fully in

    contact with the ground.

    (v) Castors are lockable.

    (vi) Spigot and socket joints are secured.

    (vii) Bracing is properly located.

    (viii) Boards are properly supported / cleaned.

    (ix) Guardrails and toeboards are adequately provided.

    g. For stability the height of working platform must not exceed:-

    (i) 3 times minimum base width for towers outside.

    (ii) 3.5 times minimum base width for towers in enclosed buildings.

    (iii) Provide ties to permanent structure wherever possible and for alltowers exceeding 10m in height. Where ties are not applicable,

    ballast weights, guy ropes and or ground anchors shall be used.

    h. Loading shall not exceed SWL designed for and clearly displayed on

    tower.

    i. Materials must be hoisted from within the effective base area.

    j. Towers must never be moved with men and / or materials on theplatform. Apply force at or near the base. Do not tow with a vehicle.

    k. Report defects to site management immediately.

    l. Towers must be inspected every 7 days and recorded.

    16. WORKING AT HEIGHTS

    a. Where work cannot be done from the ground or other permanent

    structure, scaffold must be provided. Where it is impracticable to erectscaffolds, safety nets and / or safety harnesses must be used.

    b. Persons who are physically fit, have good eyesight and experiencedshall be employed and under the controlled of an experienced

    supervisor.

    c. All working platforms and access ways from which a person can fallmore than 3m must be fully boarded and be provided with guardrails

    and toeboards.

    (i) For working platform, for footing only shall be minimum

    635mm wide or minimum 860mm wide when used for deposit

    of materials. The thickness of plank shall be 32mm (supportedat 1m spacing), or 38mm thickness (supported at 1.5m spacing)

    and 50mm thickness (supported at 2.6m spacing).

    (ii) For guardrails installation, the toprail will extent at least 900mm

    above the top surface of the platform. The midrail will be fixedat about 690mm.

    (iii) For toeboards installation, the height must be at least 200mm

    above the top surface of the platform.

    d. Safety harness shall be worn and securely attached to a fixed anchorage

    where scaffolds are not provided. The lanyard shall restrict the fall to

    only 1.8m.

    e. The use of static anchor points are preferred, however an inertia reel

    harness can be used where additional vertical movement is required,provided there is no danger of persons being injured by swinging intoobstruction etc.

    f. Where work is carried out over or adjacent to water, buoyancy aidsshall be worn in addition to the provision of suitable rescue equipment.

    g. Due regard must be taken of prevailing weather conditions, i.e. strongwind, thunderstorm, rain, wet condition etc. Lightning arrestor shall be

    installed if the working is as is the highest point in the vicinity.

    17. ROOF WORK

    a. Roof work shall only be undertaken by persons with the adequateknowledge, experience and resources to carry out the work safely.

    b. Before any work or access on a roof, fragile material shall be identified,and precautions to be implemented. Crawling boards and / or roof

    ladders must be provided and use where the roof is liable to collapse

    under a persons weight and the sloping angle with a pitch over 10.

    c. Where persons can fall more than 3m from the edge or through an

    opening in the roof, parapet wall, catch platform, safety nets, guardrailsand toeboards must be provided. In circumstances where the work is of

    short duration and / or the provision of guardrails and toeboards is

    impracticable, safety harness must be used. Suitable anchorage points

    must be provided.

    d. Where workers are employed on roof of fragile material through which

    they could fall more than 3m, suitable ladders, crawling boards orwalkways shall be provided. Appropriate demarcation and warning

    signage shall be displayed to warn workers. Where it is reasonably

    practicable, catch nets shall be installed.

    e. Hazards resulting from adverse weather conditions must be anticipated

    and appropriate precautions taken e.g. lightning arrestors.

    f. Rubbish / materials must not be thrown down from a height where theyare liable to cause injury. Use a chute or lower materials in a safe

    method. The rubbish / materials shall not be accumulated on rooftop.

    18. ROADWORK SAFETY

    a. The danger of working on and or adjacent to live carriage ways is

    obvious and requires additional safety precautions to be taken by allemployees. The following are the minimum safety measures that shall

    be taken:-

    (i) Maintain a safety clearance between the moving traffic and the

    workers using demarcation devices.

    (ii) High visibility clothing and safety helmets shall be worn byworkers at all times.

    (iii) Clearly define traffic lanes to ensure smooth traffic flow.

    (iv) Keep workers within designated area and keep alert.

    (v) Vehicles / mobile plant to be fitted with rotating amber beacons

    and safety signage where applicable.

    (vi) Appoint Banksman / traffic controller to control vehicle /

    pedestrian movement.

    (vii) Install appropriate safety, warning and traffic signages atstrategic locations.

    19. COMPRESSED GAS CYLINDER

    a. Storage

    (i) Full and empty cylinders shall be store separately;

    (ii) Oxygen and combustible gas cylinders shall not be storedtogether;

    (iii) Acetylene and Propane cylinders shall be store upright. Oxygencylinders, shall be stored and used upright and all cylinders shall

    be secured to avoid falling over;

    (iv) Gas cylinders shall be stored away from main activity areaproperly protected from heat and away from combustible

    materials;

    (v) Display NO SMOKING, DANGER and COMPRESSED

    GAS signage;

    (vi) Suitable fire extinguishers must be made readily available at

    storage area.

    b. Handling

    (i) Do not allow cylinders to drop or come into violent contact with

    one another. Do not roll cylinders on the ground;

    (ii) Use only rope / belt slings for lifting and never attach it to

    cylinders valve or gauges;

    (iii) Cylinders shall be placed in trolleys or tied in the upright

    position to a solid structure to avoid damage.

    c. Usage

    (i) Pressure regulator must be attached to the cylinder valve andflash back arrestors installed before putting into use;

    (ii) Pressure regulators shall not be turned to the maximum but onlyto the required pressure;

    (iii) Avoid directing the flame on cylinders or workers;

    (iv) Soap water shall be used to check any leakage;

    (v) Closed the valves tightly when not in use;

    (vi) Oxygen shall not be used as a substitute for compressed air;

    (vii) Suitable fire extinguisher must be made readily available at

    work area.

    20. SAFETY IN PRESSURE TESTING

    a. Pressure testing may be carried out using a liquid, (hydraulic testing),

    by using gas, steam or air (pneumatic testing). Hydraulic testing is byfar the safer and preferred method.

    b. It may be necessary to contain the vessel under test inside a blast pit orretaining wall. Any enclosures must be capable of containing flying

    materials in the event of a vessel failure.

    c. Account must be taken of the proximity of any public areas, such asroads and buildings.

    d. The vessel must not be subjected to any form of hammer test whilstunder pressure, and the vessel shall not be approached for close

    inspection until after the test pressure has been reduced to a safe level.

    21. WELDING AND CUTTING

    a. Whenever and wherever welding or cutting or any hot work is carried

    out, the following minimum safety measures must be ensured:-

    (i) Keep working area tidy and free from flammable materials;

    (ii) Goggles of the approved type must be worn to protect the eyes

    from ultraviolet and infrared rays which are particularly harmfulto the eyes. Respiratory protection shall be applied to ensure

    toxic gases fumes or metallic dusts produced during welding and

    cutting are not inhaled;

    (iii) Take care when working in congested areas, use screens where

    possible;

    (iv) Warn persons working below / near you. Hot slag is dangerous.

    Cordon off the area affected to avoid injury;

    (v) Ensure there is adequate ventilation. Suitable enclosure for

    welding operation to prevent sparks or hot slags from striking

    combustible materials;

    (vi) Check hoses / cables regularly and before use, ensure thepressure gauges are serviceable;

    (vii) Do not use the equipment other than for the purpose intended;

    (viii) Suitable fire extinguisher must be made readily available for

    welding operations or hot work.

    b. Are welding utilizes AC or DC supply, in either case the following

    minimum safety must apply:-

    (i) Ensure cables are correctly connected and adequately earthed;

    (ii) Keep cables clear of water or oil as short circuiting or ignitioncould occur due to high power;

    (iii) Do not allow traffic to pass over cables. If this is unavoidablethen the cables must be protected;

    (iv) Disconnect from power source before joining cables or when notin use;

    (v) Never use damaged cables;

    (vi) Use are welding screens where possible.

    c. Oxyacetylene

    (i) Cylinders (whether filled or empty) must be used in an upright

    position and securely fastened to prevent falling over. Emptyand filled cylinders must be stored separately and away from

    prolong exposure to direct sunlight or heat;

    (ii) Keep cylinders away from welding / cutting operations;

    (iii) Ensure hoses do not cause tripping hazard or allow traffic to

    cross over;

    (iv) Keep cylinders free form oil and grease;

    (v) Ensure hoses and connections are leak proof.

    (vi) Do not use regulators with broken gauges. Replace broken

    gauges as soon as possible;

    (vii) Use correct tools, as provided, when changing cylinders or

    turning on / off;

    (viii) Check valves and flash-back arrestors must be installed.

    22. FUEL, OIL AND LIQUID STORAGE

    a. Diesel storage tanks shall be mounted on suitable elevated platforms.The area must be cleared of vegetation and a spillage bound wall shall

    be erected.

    b. Flammable oil or liquids shall be stored in lockage steel containers and

    the doors kept close with appropriate warning signs displayed.

    c. No smoking will be allowed and appropriate safety / warning signage

    displayed.

    d. Appropriate fire extinguishers shall be positioned at such storage. Abucket of sand shall also be made available for emergency or any

    cleaning up of spillage.

    23. WORKING IN THE VICINITY OF UNDERGROUND ELECTRICAL LINES

    a. General

    (i) Live underground electrified cables are dangerous and can causeserious injury if damaged. Extreme care must, therefore, beexercised at all times;

    (ii) In an area such as town, city and industrial estates, it must beassumed that live cables are present beneath any footway or

    carriage way.

    b. Preparation

    (i) Work must be planned and notice must be given to the public

    utilities where excavations are planned in footway or carriageway;

    (ii) Obtain from the local Electricity Board plans showing positionsof their mains, but do not trust accuracy of actual placement

    fully as it may be outdated;

    (iii) It must be remembered that only mains are shown. Servicecables to property, street lamp etc., are not likely to be indicated.

    If a plan does not show a cable, it does not follow.

    (iv) Take care to avoid breaking through concrete or other hard

    surfaces onto soft materials with a power tool;

    (v) Keep a continuous watch for signs of cables as work progresses,

    with periodic checks with the CAT;

    (vi) Live cables can occur at any depth and may not be protected. If

    the end of the cable is found, check to ensure that this cable isdead and out of use;

    (vii) If a cable is found embedded in concrete, do not break the

    concrete away until the Electricity Board confirms it is safe to

    do so:

    (viii) If a cable is exposed, do not touch it or use it as a step or

    handhold. Inform your supervisor immediately if the cable hassuffered any damage, however slight. In this case, isolate the

    area and keep everybody away from the cable until it has been

    made safe.

    24. WORKING IN THE VICINITY OF OVERHEAD ELECTRIC

    LINES

    a. General

    (i) Contact with overhead electric lines can be lethal with supply as

    low as 240 volts. Overhead electric lines are not normallyinsulated, and if contact or near contact is made with them by

    crane, jib, scaffold pole, ladder or similar object. The risk of

    fatality or severe shock or burns to any person in the immediate

    vicinity is eminent;

    (ii) Similar dangers can also exist in movement of abnormally high

    loads on roads crossed by overhead lines.

    b. Pre-planning

    (i) Pre-planning safe working procedures is essential. Specific

    warnings / notices will be necessary if vehicles, plant or

    equipment etc. are installed in the vicinity of overhead lines.

    In this case, the Electricity Board would be approached foradvice of the safe working distances of the overhead electric

    lines.

    (ii) In areas where, in the opinion of the supervisor, there may be

    special hazards or difficulties, then he must use the electricitycable drawings as a guide and survey the site to locate any

    cables prior to work commencement;

    For this purpose a Cable Avoidance Tool (CAT) must be used,

    but be reminded that this will only indicate live load carrying

    cables. The presence of street lighting, illuminated signs,bollards, traffic signals, and the like shall be looked for. Noteshall be taken of the fact that every fronting property will have

    an electricity supply somewhere. Any cables located shall be

    clearly marked on the surface of the footway or carriage wayusing fluorescent paint (aerosol), wooden pegs on grass or

    unsurfaced areas;

    (iii) Accidents have occurred where cables are embedded inconcrete. If cables are suspected of being embedded in

    concrete, the Electricity Board shall be informed with a request

    that they examine the situation and, if necessary, cut off thesupply prior to excavations.

    c. Excavation

    (i) Before commencing work, the supervisor must have a copy of

    the cable plan and must check any marked positions of cables inrelation to the excavation proposed. A further check must alsobe carried out using the CAT (Cable Avoidance Tool);

    (ii) If there is any doubt in your mind concerning the presence oflive cables, inform the supervisor at once before continuing with

    excavation;

    (iii) If possible, use trial holes when excavating near suspected

    cables to establish their exact location;

    (iv) Use hand digging methods wherever practicable in theimmediate vicinity of the indicated line of cables;

    (v) Do not use mechanical excavators or power tools within 0.5metres of the indicated line of any cable, unless consent of the

    Electricity Board has been obtained;

    Officer be consulted for advice on what additional precautions

    will be required.

    (vi) Access for plant and materials and the working of plant shall be

    under the direct supervision of a responsible person appointed toensure that safety precautions are observed.

    (vii) Plant, equipment, or tools that could reach beyond the safe

    clearance limit shall be modified by the addition of suitable

    physical restraints so that they cannot reach beyond the safeclearance limit.

    (viii) When work has been carried out on a structure with aconsequentially reduced safety clearance, the Electricity Board

    and Safety Officer shall be consulted about proposed working

    methods. The Safety Officer is the responsible person forensuring the observance of safety precautions on the work is

    carried out.

    NOTE: The use of insulating guards for jibs and proximity warningdevices is NOT advisable.

    25. WORKING IN THE VICINITY OF GAS PIPES

    a. General

    (i) Damage to gas pipes can result in uncontrolled gas escapes that

    may be dangerous. These occurrences can cause expensivedisruption of work and inconvenience to the public. The

    commonly use cast iron gas pipes are very similar in appearance

    to cast iron water pipes. Various materials are used for gas

    pipes and services. If any cast iron pipe is uncovered, it shall betreated as a gas pipe, as gas pipes installation is increasing at a

    steady pace in Peninsular Malaysia.

    b. Safe working practices

    (i) Gas pipes must be located by hand-digging before mechanicalexcavation. A mechanical excavator must not be used within

    0.5 metres of a gas pipe.

    (ii) Where heavy plant may have to cross the line of a gas pipe

    during construction work. The crossing points shall be suitablyreinforced and the number of crossings shall be kept to a

    minimum and clearly identified signaled.

    (iii) No explosive shall be used within 30 metres of any gas pipe

    without prior consultation with the local Gas Authority.

    (iv) Where it is proposed to carry out excavation work, piling orboring within 15 metres of any gas pipe, the appropriate

    authority shall be consulted.

    (v) When welding or other hot works involving naked flames are

    to be carried out in close proximity to gas plant, and the

    presence of gas is suspected, the appropriate Authority must be

    contacted before work commences to check the atmosphere.

    (vi) Even when a gas free atmosphere exists, care must be takenwhen carrying out hot works in close proximity to gas plant inorder to ensure that the area is safe.

    (vii) Particular care must be taken to avoid damage by heat or nakedflames to plastic pipes or the protective coatings on other gas

    pipes.

    c. Actions on leakage of gas

    (i) Immediately clear off all personnel from the immediate vicinity

    of the gas leakage.

    (ii) Inform the Fire Department by telephone (Tel: 994) and the gas

    supplier and policy if necessary.

    (iii) Prevent any approach by the public, prohibit smoking, and

    extinguish all naked flames or other sources of ignition for aleast 15 metres from the leakage. Carton off the area if possible.

    (iv) Assist fire personnel, police where necessary.

    26. WORKING ON OR ADJACENT TO RAILWAYS

    a. Work on or adjacent to railways presents special hazards, high speed

    locomotives, high voltage electrical conductors and signalingequipment. It is, therefore, most important to consult with the railway

    authorities (KTM Berhad) at an early stage.

    b. If there are electric lines over site, near the site boundaries, or overaccess roads to the site, it is essential that the Electricity Board shall be

    consulted without delay in order that the proposed work can bediscussed and full time given for the line to be diverted or disconnectedor for other precautions to be taken.

    c. General Precautions

    (i) Where there is no work or passage of plant under the electric

    lines, barriers are required to prevent close approach. Barriersshall be erected parallel to the overhead lines and ideally more

    than 6 metres distance from it. The distance may depend upon

    the voltage of the overhead line and advises sought from

    Electricity Board. The barriers shall be surmounted by colouredbunting which forms an additional warning. If access is only

    possible from one side then a barrier on that side will be

    sufficient.

    (ii) Where it is necessary for plant to travel to and fro under

    overhead lines, the area where vehicle they may pass shall beadequate and preferably not more than 10 metres wide. This

    passageway shall be clearly defined by the use of fencing

    barriers and goal posts shall be of rigid construction and of a

    non-conducting material, distinctly marked in order that they

    may clearly identified.

    (iii) Warning notices shall be provided on each side of thepassageway advising people of the hazard and giving the cross

    bar clearance to warn drivers to lower their jibs etc. To give

    crane drivers sufficient time to lower the jib before reaching thegoal posts, warning notices are required prior to reaching the

    goal posts. Goal posts may consist of tensioned steel plastic

    coated rope and shall be erected on both sides of the line at the

    distance not less than 12 metres from the lines. The increaseddistance allows for the possibility of ropes being stretched by

    cranes. Where steel ropes are used, they must be effectivelyearthed and shall be fitted with coloured bunting.

    (iv) If it is essential for work to be carried out beneath the overhead

    lines and they cannot be diverted or made dead, it will benecessary to take precautions in addition to those noted above.

    It is essential that both the Electricity Board and the Safety and

    instruct employees and sub-contractors on the routines and safesystems of work which must be followed when working

    adjacent to railways.

    d. Early notification of work of this nature must be given to the SafetyOfficer prior to starting work.

    e. Permit to Work shall be implemented.

    27. FIRE PRECAUTION FOR SITE, OFFICE, COMPOUND OR

    STORE

    a. Major causes of fire on site are due to:-

    (i) Electrical faults or overloading of circuit;

    (ii) Misuse or faulty installation of compressed gasses;

    (iii) Misuse or poor sitting of heating appliances;

    (iv) Improper disposal of smoking materials;

    (v) Indiscriminate burning.

    b. The office areas and compounds shall be properly maintained of its

    vegetation and housekeeping.

    c. Offices shall be well-spaced to allow for fire separation and access for

    emergency services.

    d. Installations of high fire risks shall be sited away from other buildingsor sited down wind.

    e. Adequate and appropriate fire equipment must be strategically locatedand properly signaled.

    f. Compressed gas must be stored in isolated compound protected andappropriate signage displayed.

    g. Fire plan must be made known to all occupants and practices to becarried out to ensure systematic and orderly evacuation in the event of

    emergency.

    28. FIRE EXTINGUISHER REQUIREMENT

    a. Supply of fire extinguishers is the responsibility of Project Manager orthe nominated person. As a general guide, the following shall be borne

    in mind:-

    (i) One fire extinguisher for every 25 sq.m of office space.

    (ii) In cabins or kitchens dry powder or CO type extinguishers are

    recommended.

    (iii) For switch rooms, offices with electrical equipment,

    photocopiers, computers, etc., CO type extinguishers isrecommended.

    (iv) For petrol / diesel, foam or dry powder type extinguishers andsand are suitable.

    (v) For LPG and compresses gasses, use dry powder typeextinguishers.

    b. All fire extinguishers must be serviced by authorized centre once every

    12 months and that a signed and dated certificate issued by Fire

    Department is attached to each extinguisher.

    29. DEMOLITION

    a. Preparation for demolition must include the following as a minimum

    requirement:-

    (i) Any demolition shall be properly planned and coordinated to

    ensure a safe and systematic execution.

    (ii) Site inspection shall be carried out by the Project Manager and /

    or Safety Officer prior to demolition operation.

    (iii) Unstable parts of the building shall be made safe by shoring,

    bracing or other means of support. Glass fixtures on doors and

    windows are to be removed together with loose objects and

    projecting parts.

    30. ELECTRICITY

    a. The following are the minimum precautions and responsibilities to

    be carried out:

    (i) The permit to work procedure shall be implemented for work on

    line conductors / installation / equipment.

    (ii) All work on electrical systems shall have a tag in and tagout procedure.

    (iii) A specialist electrical company / contractors or trained

    technician shall be employed on any electrical work be it

    installation, operation, maintenance and testing.

    (iv) Adequate training on safety and supervision on work shall be

    provided.

    SECTION 2 - GENERAL SAFETY

    (v) All electrical wiring, termination shall be properly and

    effectively carried out.

    (vi) Broken / damaged wiring / socket / plug are not allowed to be

    used until proper repair / replacement is effected.

    (vii) Adequate and proper personal protecting equipment shall beprovided and used by all working / near the electrical

    installation.

    (viii) Adequate earthing shall be provided for all electrical

    installations.

    (ix) All electrical installations portable equipment should be

    inspected on monthly intervals.

    (x) All wiring shall be raised and supported above the floor unless

    suitable protection / waterproof type is used.

    (xi) Elevated power lines must have sufficient vertical clearance andprominently identified.

    b. Demolition Equipment

    (i) When excavators and bulldozers are used for demolition,

    consideration shall be given to the nature of the structure.

    (ii) If swinging weight is used for demolition, a safety zone of at

    least two times the height of the building or structure shall be

    maintained around the points of impact. Swinging weight shall

    be controlled to hit at specific direction.

    (iii) Scaffolds used for demolition operations shall be independent ofthe structure to be demolished, to ensure their stability.

    c. Catch Platforms

    (i) Catch platforms shall be provided along the exterior walls so as

    to prevent danger from falling objects.

    (ii) Catch platforms shall be inclined so that the outer edge is higher

    than the inner edge and shall be at least five feet wide and

    closely planked. It shall be able to withstand live load of 120lb /ft from falling object.

    d. Demolition of Walls and Floors

    (i) The walls shall be demolished floors by floor beginning at roof

    level and working downwards.

    (ii) If walls are pushed over, all the workers shall stay on the safe

    side and be kept at a safe distance.

    (iii) The workers demolishing walls that are thin or structurally weak

    shall be provided with scaffolding.

    (iv) When floors are to be demolished, the area immediately

    underneath shall be fenced off and warning notice displayed.

    Nobody shall be allowed to enter it.

    (v) Power on all the mechanical and electrical services shall be shut

    off. If it is necessary to maintain electric power or other service

    lines, they shall be sufficiently protected against damage.

    (vi) The area around the building to be demolished shall be

    sufficiently fenced off and warning notices posted at convenientplace.

    (vii) Workers shall not be working at different levels unless prior

    arrangements are taken to ensure the safety of those at lower

    level.

    e. Demolition Operation

    (i) All demolition operations shall be under the direct supervision

    of an experienced engineer / supervision.

    (ii) Demolition operation shall start from the top to bottom of the

    structure.

    (iii) Care shall be taken not to demolish any parts which would

    destroy the stability of other parts of structure.

    (iv) No demolition activities are allowed under strong windy

    conditions, as this would cause collapse of already weakened

    structure.

    (v) To keep down dust, water shall be sprayed at suitable intervals.

    (vi) If buildings are demolished by explosives, the relevant RBMsafety requirements shall be complied with.

    (vii) Keep the public and unauthorized employees at least 30m awayfrom the area.

    (viii) Remove walls by picking them apart. Working scaffolds shallbe supported independently of the wall.

    (ix) Remove all debris promptly and barricade the dump.

    1. VEHICLE SAFETY

    a. Licensed Operators

    No person shall be permitted to drive a vehicle without a license

    authorized and / or issued by the local government or other regulating

    authority for the particular type of vehicle concerned.

    b. Inspection and Maintenance

    Motor vehicles including construction equipment shall be checked dailyby the operators for the satisfactory operation of brakes, clutches, horns,

    light etc. All site vehicles and machinery must be fitted with

    appropriate fire extinguishers.

    c. Prohibited Vehicles

    Vehicles which are not properly maintained / unsafe or which do not

    have a proper vehicle entry pass shall not be allowed to enter the project

    site.

    d. Speed Limit

    The maximum vehicular speed within the project site shall be 20km/h.However, it shall be reduced due to proximity to work area, dangerous /

    confined access or slippery roads.

    e. Refueling

    Refueling within the project site shall only conducted in designated

    areas and combustible materials. Fire extinguisher must be readily

    available while refueling / at fuel storage area which are away from heatsource.

    f. Operating Rules

    The following operating rules shall be strictly adhered to by allpersonnel:-

    (i) Driving under the influence of drugs or alcohol shall be strictlyprohibited.

    (ii) No overtaking shall be allowed unless the front vehicle operator

    has full knowledge and allows it.

    (iii) When reversing any construction equipment or other large

    vehicles a signalman shall be employed. Ideally a reversingalarm shall be fitted.

    (iv) All traffic signs shall be strictly observed.

    (v) At all blind corners, the operator shall stop and ensure the safety

    of other traffic and pedestrians before proceeding.

    (vi) All vehicles shall proceed along the correct side of the road in

    compliance with the local regulations.

    (vii) The number of persons, including the operator, allowed in a

    vehicle shall not exceed the intended seating capacity of the

    vehicles.

    (viii) No person shall be allowed to ride with arms or legs extended

    outside any vehicles, not to ride on running boards, fenders or

    on top of loads.

    (ix) No person shall be permitted to get on or off a vehicle whilst it

    is still in motion. Vehicle shall be brought to a complete stopand hand brakes operated before loading or unloading.

    g. No Parking Areas

    No vehicles shall at any time park:-

    (i) In front of fire extinguishers or other installed fire-fighting

    appliances.

    (ii) In hazardous work area.

    (iii) In the vicinity of road construction, crane work or erection

    work.

    (iv) Within five (5) meter from the corners of road and building.

    (v) Within other designated restricted areas.

    h. Parking

    The following regulations with regards to the parking of vehicles shall

    be strictly observed by all personnel:-

    (i) Vehicles shall be properly parked in the designated parking lots.

    (ii) Ignition keys shall be removed from all vehicles when parked.

    i. Other Regulations

    The following additional regulations shall also be strictly adhered to:-

    (i) When hauling long materials such as angles, steel bars etc. theextended portion of the materials shall be clearly marked with a

    red flag and at night a reflector and / or a red light shall also be

    attached thereto.

    (ii) Smoking shall be prohibited when driving a vehicle or operating

    a plant / machinery.

    (iii) Vehicle entry procedures shall be strictly observed.

    k. Bicycles and Motorcycles

    (i) Bicycles and motorcycles shall only be parked in the designated

    parking lots.

    (ii) Only personnel holding a valid motorcycle license shall be

    allowed to ride a motorcycle.

    l. Accident Reporting

    (i) All vehicle accident at site shall be promptly reported to SafetyOfficer for investigation as specified in the accident reporting

    system. The scene of an accident shall be left untouched unless

    the injured person needs immediate rescue or there is apossibility of further damage to equipment or facilities.

    (ii) Any contact required with the local police shall be made by orwith the consent of the Project Manager.

    m. Violation of Traffic Regulation

    (i) To help to maintain a high level of traffic safety, a warning letter

    or penalty shall be given to any personnel who violates thetraffic regulations contained herein.

    (ii) Personnel who have repeatedly violated these traffic regulations

    shall be prohibited from entering the project site or otherwisedecided by the Project Manager.

    2. CRANE OPERATION

    a. Cranes shall only be operated by persons who are qualified by training,

    and / or certified competent by experience and authorized to operatecranes in a safe and proper manner.

    b. No person under 21 years of age shall operate a crane.

    c. No crane shall be used unless the valid certificate of fitness (PMA)

    from JKKP has been obtained.

    d. Cranes shall only be used for the purposes to which they have been

    designed for.

    e. All cranes shall be properly maintained and the operator shall be fully

    conversant with safety procedures. A record of maintenance shall be

    maintained.

    f. All lifting tackle used with the cranes shall be properly maintained,

    inspected, tested and records maintained.

    g. The safe working load (SWL) of any crane must NOT be exceeded.The safe load indicator must NOT be altered in any way.

    h. Signals to crane operators must only be given by a trained banksman

    who is conversant with Standard Signals for Crane Operators. Under

    no circumstances shall the operator lift or move a heavy / large loadwithout a banksman.

    i. No load shall be left suspended from a crane over prolong period orwhen the operator is not in attendance.

    j. Only qualified and authorized person shall make any modifications oralterations to any crane.

    k. Cranes must only be used to firm and level ground. Steel plates must beused for soft ground.

    l. Outriggers must be fully extended and the pads based on a firm and

    level surface.

    m. Only the operator shall be on or near the crane during any liftingoperation. Never allow passenger to ride on the machine. No person,other than the operator shall be within the slewing / swing area.

    n. No crane shall be used in a confined area without the use of suitablebarriers to ensure no persons encroach into the slewing area.

    o. No load shall be lifted unless it is properly supported and secured toprevent it from slipping or becoming dislodged.

    p. Register of servicing will be maintained weekly by the operator.

    3. BANKSMAN / SLINGER

    a. It is the responsibility of the Project Manager to ensure that only trained

    banksmen / slingers are appointed for lifting operations. A register of

    banksman / slinger shall be maintained.

    b. Banksman shall:-

    (i) be readily identifiable; i.e. coloured helmet or reflective jacket.

    (ii) be over 18 years of age and a responsible person.

    (iii) be conversant with the standard signaling procedure to Crane

    Operators.

    c. Slinger shall:-

    (i) ensure that the lifting equipment is suitable for the task.

    (ii) ensure the load is secured, balanced and correctly slung.

    (iii) ensure that any defective equipment must be identified and

    segregated and is returned to store.

    4. FORKLIFT OPERATORS

    a. Operators must be trained, experienced and certified to operate the

    forklift assigned to him.

    b. Ensure the machine is serviceable and properly maintained at all times

    and report any defects immediately.

    c. Never allow workers to ride on the forklift.

    d. The safe working load (SWL) shall be clearly indicated. This must

    never be exceeded.

    e. Always load with the mast vertical or tilted slightly backward.

    f. Have forks spaced as widely as practicable to give maximum stability.

    g. Ensure load does not obscure drivers view on movement direction. Ifnecessary, use banksman.

    h. Be aware of plant and personnel in the working area. Sound horn to

    warn others.

    i. Avoid sharp obstacles.

    j. Travel with load in the lowest position possible. Do not raise or lower

    load while traveling.

    k. Always travel at a steady speed. Do not brake or turn violently.

    l. Do not pass under power lines unless within goal posts, or banksmanis used.

    m. The machine must always be parked in a manner so as not to endangerother site users. When leaving the machine the forks shall be lowered

    to the ground, the hand brake applied, the engine stopped and ignition

    key removed.

    5. DUMPERS, TRACTORS AND TRAILERS

    a. Nobody is permitted to ride on any dumper, tractor or in any trailer

    except where proper seats for passenger are installed and safety ofpassengers ensured.

    b. Only licensed drivers of 18 years of age and over are permitted to drive

    dumpers and tractors. The names of these persons shall be registered bythe site management.

    c. The operator must ensure that the machine in his charge is maintainedin a serviceable condition, and must notify site management of any

    defects.

    d. The drivers of all vehicles must have received adequate training andcertified competent and record kept by site management.

    e. Machines shall not be placed at or near any excavation unless safetychocks and banksmen are used.

    f. Whilst loading a dumper by means of an excavator, the engine must beturned off, gear level set in neutral and the hand brake applied. The

    driver of the dumper must stand clear until the loading operation is

    completed.

    6. EXCAVATOR OPERATORS

    a. Ensure that excavator in your charge is serviceable and properly

    maintained at all times and record maintained, report defects

    immediately.

    b. Check ground conditions before commencing work. Find out position

    of underground services in working area.

    c. Never carry passengers or riding on plant or use for lifting and loweringworkers.

    d. Be aware of other persons and plant in your area. Use banksmen whereand when necessary.

    e. Always lower bucket to ground when excavator is left unattended,

    switched off the machine and removed the key.

    f. Never stockpile materials too close to the machine. Always leaveplenty of room between machine and stockpile for safe passage of sitepersonnel.

    g. When used in confined spaces, never slew without a trained banksmanbeing present.

    h. An excavator may only be used for lifting under the followingcircumstances:-

    (i) When a crane is not available or impracticable due to site

    accessibility, poor site conditions and for short duration usesonly.

    (ii) Must only be used for lifting in the immediate vicinity of an

    excavation with which that machine is involved.

    (iii) Must never exceed its safe working load when lifting.

    (iv) Never attempt to lift any load into a trench until you are sure

    that all personnel are clear of the area.

    (v) Never lift or lower a load if your view is obstructed unless under

    the guidance of a trained banksman.

    (vi) When lifting, always ensure that the method of fixing lifting

    tackle is safe and secure. Never use open hooks attached to thebucket or the bucket teeth.

    7. BATCHING PLANTS

    a. Batching plants must only be operated with proper written approval

    from DOSH. A copy of PMA must be retained at the plant. Batching

    plants shall only be used by, or under the supervision of persons whoare qualified by training or by experience.

    b. Operators must be authorized by the management and be fully

    conversant with the manufacturers operating and safety procedures.

    c. All guards and maintenance access gates must be properly secured or

    locked when plant is operating. Appropriate safety measures must be

    taken and signages must be posted.

    d. The operator cabin must be locked at all times when unattended andsignaged No entry to unauthorized person.

    e. Unauthorized person is NOT allowed to enter the batching plant areaand appropriate notices posted at the entrance.

    f. No person must enter the aggregate storage area when the scraper orloader is in operation. The scraper or loader operator may only be

    contacted via the batcher operator.

    g. All main switches must be isolated and security keys removed before

    any maintenance or repairs are carried out. Emergency isolationswitches must be installed at strategic locations for the purpose of

    emergency shut off.

    h. Running adjustments must only be carried out by qualified technician in

    the presence of experienced batcher supervisor.

    i. Vehicular traffic must follow the instructions of the batcher

    management.

    j. All access ladders and maintenance platforms must be properly guardedand kept clear at all times.

    8. PASSENGER HOISTS

    a. Passenger hoists must only be erected by the competent person inaccordance with statutory regulations.

    b. Passenger hoists, must be registered with the Department of

    Occupational Safety and Health and PMA certificate issued by them

    before operation.

    c. The hoist must not be used until an overload and droopiest and

    thorough examination has been carried out by a competent andauthorized person. Thereafter, the hoist must be examined every 6

    months and a record kept. A copy of PMA certificate must be retained

    on site and a photocopy displayed at the passenger hoist cabin.

    d. Hoists must be inspected WEEKLY and proper records maintained.

    The following areas must be undertaken in this inspection:-

    (i) Bases and jacks

    (ii) Ties in position and security

    (iii) Mast condition and fixings

    (iv) Gates, operable and signs in position

    (v) Hoist tower condition

    (vi) Mesh condition

    (vii) Ropes condition

    (viii) Platform condition

    (ix) Mechanical condition of hoist

    9. HAND OPERATED POWER TOOLS

    a. The following rules must be complied by all operatives when using

    power tools:-

    (i) Electrical tools must be properly connected and earthed usingapproved apparatus and methods.

    (ii) All power tools must be thoroughly inspected before every use

    to ensure its join together and safety.

    (iii) Faulty tools must NEVER be used and shall be segregated and

    tagged.

    (iv) No tools shall be left out in the open and unattended at site, they

    must be stored in a clean, dry place.

    (v) Only persons skilled in the use of power tools shall be permitted

    to use them.

    (vi) Ensure that the correct tools is being used for any particularoperation.

    (vii) Only competent persons shall effect any repair or adjustment toany power tools.

    b. Pneumatic Power Tools

    (i) All spindles, drive shafts, fan blades, fan belts, etc., oncompressors must be guarded. Likewise, any dangerous part of

    any tool driven by air pressure.

    (ii) All joints on air lines must be made with properly matchedconnections. Improvisation shall never be permitted.

    (iii) Air jets must never be directed at any person and shall never be

    used for cleaning clothes and both.

    (iv) When an airline is being used for blowing out, then all personnel

    in the immediate vicinity, including the operator, must weargoggles.

    (v) The air supply to an air operated tool must be switched off when

    the tool is left unattended, before it is disconnected, when it is

    being moved or when it is being repaired.

    (vi) Before a pneumatic tool is used, the airline shall be inspected to

    ensure that it is free of splits and holes.

    c. Chain Saws

    (i) Chain saws must only be operated by a competent person

    authorized by the site management.

    (ii) And defective or unsafe machine must be withdrawn from useimmediately and reported to site management.

    (iii) All guards must be securely fitted and safety devices tested

    before use.

    (iv) No person shall operate a chain saw in a completely isolated

    area.

    (v) Chain saws must be kept sharp, and the sharpening carried outby a competent person only.

    (vi) Suitable protective clothing / equipment shall be worn when

    operating a chain saw.

    (vii) The chain saw must be kept in store after used.

    10. ABRASIVE WHEELS

    a. Nobody is permitted to mount abrasive wheels unless he is certified

    competent. A list of certified competent persons must be maintained on

    site. Where as certified competent person has failed in installing awheel / disc correctly, his competency shall be revoked.

    b. Only trained persons are authorized to use abrasive tools.

    c. Where an operative hold a certificate issued by another organization,

    the operative shall be re-tested and re-certified by SPLALSB before

    being employed on the job.

    d. Guards shall be fitted and kept in position. Under no circumstances areabrasive wheels to be used without guards. Floor stand and bench

    mounted abrasive wheels must be guarded for at least of the

    circumference.

    e. Use only correct tools issued by the supplier for changing wheel / discs

    and ensure that correct wheel / disc is fitted for the operation.

    f. All abrasive wheels shall be stored properly so that they are notdamaged.

    g. Checks must be carried out to ensure the output of the power course forair and electric tools is compatible with the equipment in use.

    h. Adequate and suitable eye protection must be worn by any person using

    an abrasive / cutting wheel.

    11. CARTRIDGE TOOLS

    a. Cartridge tools must only be used by a trained and approved person. A

    list of approved persons must be maintained on site.

    b. The manufacturers representative will train any person if requested todo so.

    c. Only nails and cartridges manufactured for that make of tool must be

    used. Case hardened nails must not be used.

    d. Tools must be maintained, serviced and kept in good order.

    e. Goggles and ear defenders must be worn by operators.

    f. Splatter guards must be kept in position.

    g. Tools shall be armed ONLY when required for immediate use. It shallnot be stored with a cartridge in it.

    h. The material and its opposite side into which fixings are mounted must

    be checked prior to the operation to ensure that persons are not likely tobe injured by flying fixings or penetration of material.

    i. Tools must be kept in box when not being used, even for short period.

    The cartridge must be securely stored separately and under strict control

    by a responsible person.

    j. Extreme care must be taken when using these tools and they mustNEVER be pointed at any person or issued for any purpose other than

    that for which they are intended.

    k. The cartridge tool and all spent / unspent cartridges must be returned to

    the site office / store when work is completed.

    l. The operator must ensure that the tools are maintained in a satisfactorycondition and its maintenance to be carried out by qualified technicianand recorded.

    m. Except for certain specialist applications, only low velocity indirectacting type tools may be used.

    12. CIRCULAR SAWS

    a. All bench type circular saws must be rigidly secured to prevent

    dangerous movement.

    b. Circular saws and other woodworking machinery must only be operated

    by competent persons of 18 years and above.

    c. The area surrounding the circular saw must be kept clear of materials,

    scrap, oil, etc. at all times.

    d. Suitably constructed and readily identifiable push sticks must always be

    used to avoid persons fingers going near the saw blade.

    e. Any defects on the machinery or any unsafe conditions must bereported immediately.

    f. A guard that covers both sides of the blade must be kept as close aspossible to the surface of the materials being cut, and at least covering

    the root of the teeth. Where squared stock is being fed into the saw by

    hand, the guard must not be more than 12mm from the upper surface ofthe materials being cut.

    SECTION 3 - VEHICLE, MACHINERY & TOOL SAFETY

    g. The riving knife must always be positioned as closed, as possible to the

    saw blade, and the gap between the two must not be more than 12mm at

    the level of the table.

    h. Where the underside of the saw blade is exposed and can be reached byany person, it must be fitted with suitable constructed guards.

    i. On any machine that can accommodate a blade of more than 450mm,

    the table must be such that the distance between the delivery end of the

    table and the up-running part of the saw blade is not less than 1200mm.

    j. No cutting of any rebate, tenon, mould or groove may be carried out ona circular saw.

    k. No ripping operations may be carried out unless the teeth of the saw

    blade protrude through the materials being cut.

    l. Circular saws must not be used for cross cutting logs etc., unless thematerial is held in a gripping device on a traveling table.

    m. Suitable eye and ear protection must be worn by the operator where

    appropriate.

    n. Appropriate safety signages must be posted.

    o. The emergency stop control of all saws must, at all times, be maintained

    in effective working order, and where an emergency stop becomes

    defective, the machine shall be taken out of commission until these

    repairs have been executed.

    SECTION 4 - MEDICAL FITNESS / FIRST AID

    1. MEDICAL FITNESS

    a. It shall be the general rule that any person who is either physically or

    mentally unfit shall NOT be allowed to work at the project site.

    b. All personnel shall be medically fit to perform their work, and shalladhere to all statutory rules and regulations and any other procedure or

    guidelines for medical surveillance or monitoring of industrial hygiene.

    2. FIRST AID FACILITIES AT PROJECT SITE

    a. RBM shall endeavor to provide a qualified / trained first aider at project

    site for the administration of first aid.

    b. In order that the first aid treatment is readily available, all the necessary

    first aid equipment, including sufficient number of first boxes shall be

    made available.

    c. RBM shall ensure that adequate number of first aid boxes andequipment are strategically located and shall be checked and

    replenished on a periodic basis or when contents are substantially

    consumed.

    3. FIRST AID EMERGENCY PLAN

    a. Emergency plan for injury shall be formulated to ensure speedy

    removal and transportation of victim to the nearest medical treatmentcentre / hospital.

    4. FURTHER MEDICAL TREATMENT / HOSPITALIZATION

    a. The first aider or the senior management staff shall decide whether theinjury is such that further medical treatment and / or hospitalization is

    required.

    b. If further medical treatment and / or hospitalization is required, the

    transportation arrangement shall immediately be made available to send the

    injured to the nearest clinic or hospital without undue delay.

    SECTION 5 - HYGIENE AND WELFARE

    1. INTRODUCTION

    a. SPLALSB values the well being of the staff and as such shall endeavor

    to provide, where it is reasonably, practicable, the hygiene and welfare

    facilities for the staff and workers, including the subcontractorsengaged.

    2. DRINKING WATER

    a. An adequate supply of wholesome drinking water from an approvedsource shall be provided to the site for the consumption of the workers.

    b. Where the supplies of water that is unfit for drinking shall beconspicuously indicated by notice NOT FOR DRINKING.

    c. Drinking water if required to be stored, shall be kept in rust proof and

    non-toxic containers that are covered to prevent contamination. It shall

    be dispersed through taps or other hygienic method.

    d. Untreated water, if required for drinking shall be tested and certified

    safe by the approved establishment.

    3. TOILET FACILITIES

    a. Adequate toilet facilities shall be provided for the staff and workers at

    convenient places including canteen, site office, accommodation andwork areas.

    b. Toilet facilities shall be operated for each sex where practical andpossible.

    c. Water flush toilets connected to sewage systems shall be provided

    wherever and whenever possible.

    d. Toilets shall be so constructed as to provide adequate privacy and afford

    protection against the weather and construction hazards.

    e. The toilets shall be supplied with adequate water and toilet paperswhere necessary / possible.

    f. Toilet facilities shall be kept in a h
