Reshaping Care for Older People The Change Fund The Change Fund is a major Scosh Government iniave, established in 2011 for a four year period. The programme aims to shiſt the balance of care from acute and instuonal sengs to community based and preventave approaches. £80m is being invested in this iniave across Scotland during 2012-13 to bring about lasng changes in the way we care for older people. The funding targets Scotland’s 32 local authority areas and requires a partnership approach to transform the care of older people from a reacve to a preventave model and from instuonal care to care at home or in a homely seng. There are four key partners involved in each local Change Plan - the NHS, local authority, independent care sector and Third Sector Interfaces, each of whom negoate, agree and sign off Change Plans for their area. Change Plans represent a unique opportunity for partners to work together across services and sectors, to idenfy the most effecve ways to achieve the outcomes that will make a real difference to the way that people experience ageing in Scotland in years to come. Reshaping Care for Older People

What’s RCOP all about?Scottish people are living longer and remaining healthier into old age. By 2033 life expectancy is projected to rise to 81 years for men and 85 years for women. While this is good news, it presents significant challenges. A plan is now needed to meet the health needs of our ageing population and ensure the infrastructure to support this. Government consultation has explored how improved health and care services can be delivered for Scotland’s older people over the next twenty years.

• Support people to look after their own health• Loosen dependence upon institutional care• Enable people to stay active and engaged within

their communities

Reshaping Care for Older People

GCVS Role and Progress to DateGCVS, on behalf of Glasgow’s Third Sector Interface, has negotiated and signed off Glasgow’s Change Plan in Partnership with NHS GG&C, Glasgow City Council and Scottish Care.

To ensure a shared approach to this work, we have engaged and consulted with key third sector partners who are represented on a Third Sector Reference Group (TSRG). This group is part of the governance structure for the Change Plan.

The TSRG is chaired by GCVS and reports bi-monthly to the Glasgow Interface Third Sector Executive Committee and to the RCOP Change Plan Strategy Group. A list of the key partners represented is provided on page 4.

In addition to our role in the governance of the Change Plan we also aim to ensure the meaningful involvement of the third sector in the programme and to research the contribution of the third sector to this agenda and potential ways this can be grown.

Over the past year, the TSRG has recommended a number of key priorities including the need to:

• Research the role the sector plays in RCOP

• Develop funding opportunities to support the third sector to build capacity & bring forward demonstration projects

Details of progress in these areas is given in the rest of this bulletin.

The New Third Sector Transformation Fund The RCOP Transformation Fund will launch in July 2012 . Organisations will be able to access funding of between £20,000 and £50,000.

Specific investment will go towards projects and activities that can support Change Fund strategic priorities and can:

• Demonstrate maximised social capital

• Work to coproduction delivery models

• Deliver preventative and anticipatory services

• Build on existing effective service models

• Work in partnership to scale up proven effective

services in a wider geographical or thematic area

Development FundFor the next three years GCVS has secured a recurring fund of £50,000 to build the capacity of key third sector partners to engage in RCOP, demonstrate their expertise and be supported to:

• Fulfil key representational roles

• Lead on development and co-ordination

• Promote RCOP

• Consult with constituencies to build intelligence

• Implement policy and best practice

• Share information and engage in partnership

activities.

Activities Funded in 2010-11A range of RCOP activities have been funded to date including:

• Development of a RCOP consultation mechanism for Carers

• Support to enable community controlled housing association research and demonstrate the contribution of housing-led solutions to support independent living

• Pathways for Change events (supporting lifelong learning for older disabled people)

• Information sharing and consultation lead by TS partner organisations with their members

• Support to strategic partners to increase capacity to attend RCOP events, fulfil representation roles within RCOP structures and feed in expertise and knowledge

Reshaping Care for Older People

Third Sector MappingOur research is being undertaken to gather evidence & demonstrate to health planners in the city, the third sector’s contribution to RCOP. Building upon existing Infobase intelligence, the research aims to:

• Scope the current reach of third sector services

• Identify the capacity and potential to grow more localised, community-based services

• Capture examples of good practice & opportunities for replication

• Explore & establish options for effective monitoring and evaluation

Since Jan 2012, over 200 face to face interviews have taken place, capturing information on 300+ groups & organisations contributing towards RCOP agenda. Final interviews are currently being held if you wish to participate please email [email protected]

Third Sector Mapping Final ReportThe report will be available later in June and will be accompanied by a series of case studies highlighting some existing good practice, innovative approaches and a range of developments being pursued.

Emerging ResultsThe research has been particularly valuable in raising third sector awareness of RCOP and of the organisations we interviewed:

88% work directly with older people

30% are aware of their current activitiescontributing towards RCOP agenda

52% recognise potential to contributetowards RCOP in coming years

We are now starting to identify key trends and themes in relation to service users, identified need, service demand, outcomes, volunteer involvement, capacity to develop & much more.

These will be shared the at the forthcoming RCOP event.
