1 DEFINITIONS
1.1 Inspection Body (IB)
Body that performs inspection.
1.2 Inspection
Any physical activity related to ensuring that an equipment/item, in its entirety and at agiven location or environment, meets the specified design and operating Standards and issafe to operate for a specified period. The period is defined by the fact that theequipment/item should remain always the same.
1.3 Safe Working Limit (SWL)
Maximum load (mass) an equipment/item may safely operate under a particular
configuration.
1.4 Inspection Certificates
Certificates issued by a DAC accredited Inspection Body, to indicate compliance of anequipment/item with safety requirements and its fitness for use,
1.5 Inspection Report
When the equipment/item does not comply with the requirements of the relevantstandards, an Inspection Report shall be issued, the full description as to why theequipment/item failed the inspection.
1.6 Lifting Equipment
Any lifting machine, driven by manual or mechanical power that is able to raise, lower orsuspend loads, and includes the supporting structure and all plant, equipment and gearused in connection with such a machine, but excludes continuous mechanical handlingdevices (i.e. conveyors), such as but not limited to: -
(a) Cranes (tower, mobile etc),
(b) Wall / Pillar Cranes, Derricks, Swing Jibs and Davits,
(c) Runway Beams, Monorails, All Pad Eyes, Gin Poles and Gin Wheels,
(d) Winches, Hoists (air and electric), Crabs, Telfer Hoists,
(e) Pull Lifts, Trolleys,(f) Elevators, Lifts and Escalator,
(g) Forklifts, Self Loader and Side Booms,
(h) Lifting Jacks (pneumatic or hydraulic).

1.7 Lifting Accessories or Loose Gears
Any item used to connect a load to the lifting equipment but which is not in itself a part of
the load or the equipment, such as:(a) Chains and Wire Ropes,
(b) Chain, Wire Rope and Webbing Slings,
(c) Rings, Links, Hooks, Shackles, Eye Bolts, Swivels, Blocks, Snatch Blocks,
(d) Beam Clamps and Plate Clamps,
(e) Lifting Beams / Spreader Beams.
1.8 Scaffolds/temporary works equipment
Elevated temporary working platform used to support worker, materials and tools duringconstruction work. It is being used to work at heights more than a worker places on the
same platform and when not possible to perform works with other safe mans such as:
(a) Working platform,
(b) Ladder access tower,
(c) Loading bays,
(d) Free standing,
(e) Mobile access tower,
(f) Any scaffolding structure used as temporary supporting structure.
1.9 Pressure Equipment
Vessel, piping, safety accessory or pressure accessory. Where applicable, pressureequipment includes elements attached to pressurized parts, such as flanges, nozzles,coupling, support, lifting lugs, etc.
1.10 Non-destructive Testing (NDT)
It is a wide group of analysis techniques (testing methods) used in science and industry toevaluate (identify defects) the properties of a material, component or system withoutcausing damage
1.11 Fairground and Amusement Equipment
Device: any arrangement of equipment through or on which a guest moves, where thedesired effect is primarily achieved by virtue of the guest’s self -powered motion or anyother system that is not covered by amusement rides.
Ride: an arrangement of structural or mechanical elements (or both), which has as itsprime function the provision of movement of patrons in some controlled manner such thatthe patrons are not necessarily required to move themselves to obtain the desired effect

1.12 Personal Protective Equipment
Any device or means intended to be worn or held by a person for protection against one
or more hazards to his health and safety.
1.13 Related Authority
Regulatory bodies such as Dubai Municipality (i.e. Public Health and Safety Department,Building Department and Environmental Department), Dubai Police, Dubai Civil Defense,Department of Civil Aviation, Dubai Ports Authority-Ports Customs & Free zoneCorporation and Jebel Ali Free Zone Authority (JAFZA).

2 SCOPE
2.1 Inspection Services:
2.1.1 This document is applicable to all Inspection Bodies that are certifying the variousequipment used in construction, manufacturing, marine, and other related industries. Thisdocument is used as a supplementary accreditation criteria along with ISO/IEC 17020international standard.
2.1.2 Scope-specific supplementary requirements were prepared for Inspection Bodies tocomply with, as stipulated in the following table:

2.2 Subcontracting:
2.2.1 Where the Inspection Body subcontracts certain specialized activities temporarily for partor all of its inspection activities for unforeseen extra work load or any other reasons theremust be identifiable member(s) of the management personnel sufficiently knowledgeablein those technical activities being subcontracted, to be able to:
(a) Define the problem adequately to enable the subcontractor to offer appropriateservices, personnel and equipment;
(b) Choose an appropriate subcontractor and to assess its technical competence (e.g.
methods, personnel and facilities);(c) Interpret the results supplied by the subcontractor and relate those results properly
to the service originally requested or problem originally defined.
2.2.2 If the Inspection Body subcontracts any part of its DAC accredited activities covered inthe scope of this document, the subcontractor must be accredited by DAC.

3 GENERAL REQUIREMENTS
3.1 The Inspection Body applying for accreditation as per this program must have amanagement system, which includes the following as minimum:
3.1.1 Proper documentation of its policies, procedures and operations in accordance with thedocumentation requirements of ISO/ IEC 17020:2012 “Conformity assessment — Requirements for the operation of various types of bodies performing inspection”, therelevant standard of inspection and/or test methods, according to which it is to beaccredited, as well as any additional requirements set by DAC here within this documentand other related documents, as stipulated in clause 2.1.2 of this document.
3.1.2 Facilities properly equipped with the equipment and instruments appropriate for the typeand range of inspections under accreditation as minimum.
3.1.3 Suitable and qualified technical and administrative staff in the Inspection Body.
3.2 The Inspection Body shall prepare work program/plan for its activities with a frequencysuitable to its nature of work.
3.3 The Inspection Body shall prepare an approved list of authorized inspectors (howeverprepared), and it shall include the following (as minimum):
(a) Name of inspector;
(b) Scope of authority;
(c) Date of authorization; and
(d) Sample signature.
The list shall be prepared in a way to allow traceability of when inspectors’ scope ofauthority was initially granted and when it was updated; thus, keeping records ofold/obsolete authorization lists.

4 ACCOUNTABILITY AND INTEGRITY
4.1.1 The Inspection Body shall have clear rules for the fees charged for the inspectionservices and for the issuance of the Inspection Certificate/Report; and the terms ofpayments for each. Fees charged by Inspection Body shall be for the inspection servicesand not for the sake of issuing an Inspection Certificate/Report; the fee shall bechargeable even if a certificate is withheld.
4.1.2 Professional Judgment that is included in the certificate will form part of the assessmentand will be subject to accreditation.
4.1.3 If the inspection commissioned by the client could not be carried out in full or in part, awritten notification to that effect shall be given to the client.
4.1.4 When an Inspection Body undertakes an inspection of any equipment/item, inspectors
are required to be capable of making identification of all types of findings in theequipment/item under inspection.
4.1.5 The Inspection Body shall use Checklist forms containing all the requirements of relevantinspection standards prepared internally and approved by DAC. The checklist forms shallcontain the results of evaluating the inspection methods. The Inspector must sign in thechecklist after recording all necessary information.
4.1.6 If the equipment/item found do not comply with the requirements of the relevantStandards, and, therefore, Inspection Body declines to issue an Inspection Certificate, aseparate document named Inspection Report shall be issued.
4.1.7 If the equipment/item is found unsafe and can result in an imminent danger, the
Inspection Body shall advise the owner to cease use of the equipment/item and reportthis incident immediately to the related authority (where applicable).
4.1.8 The Inspection Body must have the ability to identify the inspected equipment/item (i.e.by Serial Number and Model Number, or some form of Registration Number). If theequipment/item, and some of their critical components, cannot be identified, theInspection Body shall consider this as a defect in the equipment/item and shall report itimmediately to related authority (where applicable).
4.1.9 The re-inspection has to be performed by the same Inspection Body for equipment/itemthat failed the inspection. In case the client did not respond back for the re-inspection, forthe given time in the inspection report, the related authority shall be immediately
informed.

4.2 In case of an accident involving the equipment/item inspected by the accredited
Inspection Bodies, and, as a result of the official investigation, it was determined that theaccredited Inspection Body was responsible, DAC shall immediately suspend theInspection Body’s Accredited related scope and exclude the inspector who performed theconcerned inspection from the list of authorized inspectors. A special assessment will beimmediately carried out by DAC related to the Management System and technicalcompetence of the Inspection Body under suspension and, subsequently, relevantclauses of DAC-REQ-01 related to suspension & withdrawal of accreditation will beapplicable.
In case the accident involved serious injuries or was fatal, DAC Director reserves the rightto withdraw the accreditation for the related scope with immediate effect.

5 CURRENCY OF CERTIFICATES OF SAFETY
5.1.1 After an Inspection Body has completed an inspection and found the inspectedequipment/item in conformance with the requirement, the Inspection Body shall issue an
Inspection Certificate.
5.1.2 The Inspection Body shall produce an Inspection Certificate to fulfill the clients’ needs,the related authority requirements and the applicable clauses of the relevantcode/standard.
5.1.3 The certificate shall be signed by the authorized Inspector, or by any other appropriateauthority assigned by the Inspection Body, who has performed the inspection.
5.1.4 Computer-generated (electronic signature) is allowed on the certificates/reports, subjectto provision of adequate evidence for the integrity (access and control) over thoseelectronically generated signatures.
5.1.5 An Inspection Certificate shall be issued with a nominal life as specified in the relateddocuments, as stipulated in clause 2.1.2 of this document. Each Inspection Certificatemay be subject to evaluation as determined by the relevant authority and whenever anequipment/item is involved in an accident.
Moreover, the validity of this certificate is based on the equipment/item being maintainedand operated in accordance with all of the recommendations made by the manufacturer,and not suffering any damage. Therefore the Inspection Body shall only issue anInspection Certificate when it is satisfied with the results of the inspected equipment/item.
5.1.6 The designated signatories shall only be authorized by the Inspection Bodies to sign theirown Inspection Certificates and Inspection Reports. The designated signatory mustassume responsibility for the technical validity and accuracy of all information contained inthe Inspection Certificate and Inspection Reports.

6 INTERNAL MANAGEMENT SYSTEM AUDITS
6.1 The Inspection Body’s internal Management System audit program for the on-site audit of
inspectors shall be designed so that, within each cycle of the audit program, at least oneinspector is witnessed/evaluated thoroughly on-site. The program shall also ensure thateach of the inspectors engaged in inspection is assessed at least once within a period of3 years for each of the fields in which they are authorized. The chief/senior Inspector(however named) must co-sign the checklist whenever he makes verification visits to thesite.
6.2 The audit program shall ensure that where inspections are managed from locations otherthan a central location (e.g. Branch Offices), including those located overseas, the auditprogram encompasses these different locations in a systematic way over the 3 yearsvalidity period of the accreditation cycle.
6.3 The records of the internal Management System audits produced must be in such a waythat will enable the Inspection Body to verify the previous inspections. Inspection Bodiesshall describe this point in their Management System.

7 INSPECTION CERTIFICATE/REPORT
7.1 The Inspection Certificate/Report shall include the following information as a minimum
(points marked with asterisk* are only applicable for Scaffolding):1. Designation of the document, i.e. as an Inspection Report or an Inspection
Certificate, as appropriate.2. Identification of the document, i.e. date of issue and unique identification (i.e.
Inspection Certificate/Report number).3. Identification of the issuing Inspection Body Name and address of the Inspection
Body issuing/endorsing the certificate.4. Identification of equipment/item Owner‘s/Contractor‘s name and address. 5. Description of the inspection work ordered.6. Date(s) of inspection and type(s) of inspection.7. Information on where the inspection was carried out (i.e. the address of the premises
at which the Inspection was made).8. Manufacturer or supplier of equipment/item name and address.9. Equipment/item unique identification number and SWL.10. Identification of the component(s) inspected.11. Where applicable, any reservations or restrictions (limitation) on the use or
maximum capacity of the equipment/item.12. When applicable, information on what has been omitted from the original scope of
work.13. Applicable Reference Standard/Code and/or brief description of the inspection
method(s) and procedure(s) used, mentioning the deviations from, additions to, orexclusions from the agreed methods and procedures.
14. Identification of measuring and testing equipment.15. If any part of the inspection work has been subcontracted, the results of this work
shall be clearly identified.16. The Due date of next Thorough Inspection.17. When applicable, the due date of next Proof Load Test (please refer to the specific
requirement, as stipulated in clause 2.1.2 of this document).18. When applicable, details of any major repairs/alterations carried out on the
equipment/item, provided that the client inform Inspection Body‘s inspector of anymodifications or structural repairs.
19. When applicable, details of latest inspection performed including any NDT ifpossible.
20. Measuring units (for Loads/Weights, for Lifting Equipment, Lifting Accessories andScaffoldings; or Capacity/Volume, for Pressure Equipment, NDT, Fairground and
Amusement Rides/Devices, Personal Protective Equipment…etc.) shall be ineither/both Metric kilograms or pounds (for Lifting Equipment for Lifting Equipment,Lifting Accessories and Scaffoldings), or either/both Metric System (m3) or ImperialSystem (gallon) (for Pressure Equipment and NDT).

21. The results of the inspection including a declaration of conformity and any defects orother non-compliances found (results can be supported by tables, graphs, sketchesand photographs).
22. A statement that the inspection results relate exclusively to the work ordered or theobject(s) or the lot inspected.23. A statement that the Inspection Certificate/Report shall not be reproduced except in
full without the approval of the Inspection Body and the client.24. Names, signature and designation of the staff member(s) who have performed the
inspection.25. Name, signature and designation of signing authority of Inspection Body (if different
than the inspector who performed the test).26. Clearly defined Liability Clause of the Inspection Body.27. [Optional] Information on environmental conditions during the inspection, if relevant.28. [Optional] Where applicable, and if not specified in the inspection method or
