1. The Schools Division Office hereby reiterates Regional Memorandum No. 005, s. 2018regarding the additional requirement for applications of governmentpermit/renewal/recognition of private schools.

2. The said additional requirement shall be included in the scanned documentaryrequirements submitted online.

· 3.. For information and reference, see the attached Regional Memorandum including the

template of the LIS and EBEIS Compliance Form.

4. Immediate dissemination of and strict compliance with this Memorandum is earnestlydesired.
