Driver DT Foreman MMR (PAMA) Hauling, Km 1 + 500 Hauling DT MMR 902 and DT-UTSG 709 traveled from portsite to Pit. When DT UTSG 709 entered the constriction of Km 1.5 the unit stopped in order to gave an opportunity to the another unit (PJM 09) that was in loading direction. at that moment, MMR 902 failed to brake and finally hit the rear vessel of UTSG 709 in front of him.

Operator HD operator HD Group Leader PAMA Non Hauling, Disposal 203 IUP JMB Three units were parked facing to the east, where the HD 3151 was positioned to the left at the end, the right was HD 3202 and then HD 2539. When HD 3202 was about to start moving forward into the loading point at panel 10, the operator felt the unit was slipped and moved to the left before finally the rear left of his vessel hit the right side rearview mirror of HD 3151 which were parked next to him, it cause it broken.
