The Queen Elisabeth II (QE II) Hospital is a single storey building with basement originally built in 1979 in the city of Grande Prairie, Alberta. It provides space for the QE II Hospitals laundry, kitchen, shipping and receiving departments etc. and is provided with a basement and service tunnels connected to the main hospital building. The building provides ~6000 m² of space including the service tunnels. No major renovations or additions have been reported at time of assessment.

General Summary:

Structural Summary:There were no structural drawings provided at time of assessment and the foundations were concealed. It is assumed that the foundations are comprised of cast-in-place (CIP) concrete wall foundations with spread and pad footings. The building frame consists of CIP concrete columns and beams at basement and structural steel columns and structural steel beams at main level. The basement level and service tunnels are provided with a CIP concrete slab on grade and the main floor slab is composed of metal decking and a concrete topping. The roof is provided with structural steel roof trusses and open web steel joists supporting a metal deck. Work is anticipated for foundation and cast-in-place concrete floor repairs and floor construction firestopping within the next 5 years. The buildings structural elements are in overall acceptable condition.

Envelope Summary:The exterior is clad with a mixture of pre-finished vertical and horizontal metal siding, and brick veneer at the base. Pre-finished aluminum windows are only provided at the west side of the building. Framed pre-finished aluminum sloped glazing are provided on the sides and the roof. Skylights are pre-finished units. Pre-finished aluminum storefront doors in pre-finished aluminum frames and painted metal utility doors, set in painted metal frames are provided throughout the building. The roof was reported to be built-up-roofing (BUR) and was not assessed due to extensive snow coverage. Sloped sections of pre-finished metal roofing are provided at both the basement and main levels. Work related to the joint sealers, storefront doors, roofing and skylights are anticipated within the next 5 years. The buildings envelope is in general acceptable condition.

Interior Summary:The administration area, the laundry and some storage areas are provided with resilient sheet flooring. The mechanical, storage areas and tunnels are provided with sealed or painted concrete flooring. Quarry floor tiles are installed in some of

the washrooms, kitchen and food storage areas. Carpet flooring is provided in the administration area. The building is provided with painted gypsum board and concrete masonry unit (CMU) interior walls. The exterior walls in the basement level are painted CIP concrete walls. Ceramic wall tiles are provided in some of the washrooms and in some areas of the kitchen. Washrooms are provided with pre-finished metal toilet partitions. Interior doors are painted metal, with commercial grade hardware, set in painted metal frames. The stairwells and rated walls are provided with painted metal fire doors, set in painted metal frames. The ceilings are mostly painted metal exposed structure or painted gypsum board walls. Some pre-finished T-bar grid with lay-in vinyl tiles are provided in the laundry and kitchen areas. Work within the next five years is anticipated for visual display boards, fabricated compartments, painted concrete floor finishes, resilient flooring, carpeting, acoustical ceiling tiles, fixed casework, blinds and hydraulic freight elevators. The interior is in overall acceptable condition.

Mechanical Summary:The building is provided with stainless steel sinks in the staff break rooms and in the kitchen and laundry areas. Floor mounted stainless steel service sinks are installed in the housekeeping rooms. The domestic water distribution piping was reported to be mostly type L copper with some type M sections. The waste piping is a mixture of cast iron and ABS plastic piping. The building is supplied with hot and chilled water as well as stream via the piping from the power plant. The washrooms are provided with vitreous china flush valve toilets and counter mounted enamel iron or wall hung vitreouschina lavatories. Wall mounted vitreous china urinals with flush valves are installed in the males washrooms. Four air handling units provide heat and make-up air throughout the building with supplemental hydronic unit heaters and fan coil units in mechanical/electrical, laundry and building entrances. Painted spiral and sheet metal ducting is provided. A nitrous oxide and oxygen system throughout the building is locatedin the building. Electronic thermostats and a building system control controls temperature and major building systems. A wet sprinkler system is provided for the building along with a chemical suppression system in the kitchen hoods. ABC type fire extinguishers are located throughout the building. Repair cost are anticipated for the steam lines in the next 5 years. Lifecycle replacement cost are anticipated for the stainless steel and service sinks, the shower valves, the drinking fountains, the washroom fixtures, backflow preventers, the medical gas systems, refrigeration compressors and HVAC equipment within the next 5 years. The buildings mechanical systems are in overall acceptable condition.

Electrical Summary:The building is provided with electrical power via underground lines from an utility owned exterior transformer. An SKV 6000 amp Federal Pioneer electric switch gear and a central distribution panel are provided in the main electrical room. Several secondary distribution panels are provided throughout the building. Two Allen Bradley motor control centers are provided in the building mechanical and electrical rooms (1 each). Motor starters for various pieces of equipment are spread throughout the building. The entire building is provided with copper wiring in conduit. Interior lighting is mostly provided by fluorescent T-8 light fixtures and ceiling mounted high pressure sodium light fixtures. Some interior incandescent light fixtures are provided throughout the building. The buildings exterior is provided with wall-mounted HP Sodium light fixtures controlled by a photo cell. Emergency lighting is provided via built-in emergency light fixtures connected to the emergency power generator located in the power plant. The building is provided with fire pull stations, alarm bells, and smoke and heat rise detectors. Lifecycle replacement costs are anticipated for the secondary power distribution panels, the interior fluorescent light fixtures, the fire alarm system, security access and video surveillance systems. The electrical systems in the building are in overall acceptable condition.

We have provided an allowance for the excavation, repair to water proof membrane, sealing the crack with epoxy on both sides and replacement of sections of cast-in-place slab-on-grade. The cost of repairs will be based on the results of the Study.

The service tunnels and the basement level are provided with a CIP concrete walls. Some cracking is present. Some older water leakage was observed; however it was dry at the time of assessment. The interior elements are in overall acceptable condition.

The loading docks are provided with telescoping blast proof overhead doors.

Due to extensive snow coverage, a full roof assessment was not possible. The roof was reported to match the adjacent power plant building and therefore is assumed to be a built up roof (BUR) with pea gravel for UV protection and concrete walk pads.

Sloped prefinished metal panels cover portions of the upper and basement levels.

Pre-finished aluminum framed sloped skylights are provided for the building's basement level.

The Prime Audit Firm is Golder Associates Ltd. The evaluation date was January 13, 2014 Site description: The primary site access is via 98 Street running west of the site. However, secondary access is possible via 96 Street which runs east of the site. The site is mostly covered by other building or asphalt paved driveways and parking lots. Grassed areas, medium to large shrubs and conifer and deciduous trees are located throughout. The entire site is relatively flat.

The Prime Audit Firm is Golder Associates Ltd. The evaluation date was January 13, 2014 The building documentation was provided by Alberta Infrastructure. The service building is a building providing one storey with basement of service space including the laundry, kitchen and shipping/receiving facilities for the QE II Hospital. It was constructed in 1979.
