boring was constructed starting on June 13, 1989 and in August 7, 1989. The final test was conducted on 1989 with the temporary casing set at a depth of 80 open hole to 120 feet. Water quality tests were

as drilling progressed and the results are summarized 1.

Upon final cleaning and testing, a 4 inch diameter PVC casing was installed as shown in figure 2. As indicated in figure 1, the ocean quality water was not struck until a depth of 80 feet. A complete chemical analysis of the water at that depth is enclosed and is nearly 100% sea water.

The pumping test of August 5, indicated a specific capacity of 73 gpm/ft. of drawdown (138 gpm with a draw down of 1.9 feet). This would result in a draw down of about 3 feet at a pumping rate of 200 gpm (the estimated rate needed for make up water). An adequate supply of salt water can be developed on site below elevation minus 70 feet. Attached is the driller's log.

In accordance with the Department of Land and Natural Resources Administrative Rules, Section 13-168, entitled "Water Use, Wells, and Stream Diversion Works", your application to construct and test Well No. 4126-01M within Tax Map Key: 2-1-08: 89 is approved subject to the following conditions:

1. The Division of Water and Land Development (DOWALD), Geology-Hydrology Section, shall be notified at 548-7619, before any work covered by this permit commences.

2. The well shall be used for monitoring and testing only.

3. The following shall be submitted to DOWALD, P.O. Box 373, Honolulu, Hawaii 96809, within 30 days after completion of the well:

a. Well Completion Report.

b. Final elevation (referenced to mean sea level) by a Hawaii­licensed surveyor.

c. As-built sectional drawing of the well.

d. Plot plan and map showing the exact location of the well.

e. Driller's logs, geologic logs, and salinity logs.

f. Periodic reports of monitoring and testing results.

4. The applicant shall comply with all applicable laws, rules, and ordinances.

5. This permit may be revoked if work is not started within six months of date of issuance or if work is suspended or abandoned for six months.
