Resident Portfolio

Overview The Portfolio is a tool used to track your progress in the program and submit specific forms to the Residency office. To access your Portfolio, go to https://one.uwmedicine.org/sites/psychres/Pages/Portfolio.aspx If you have any technical issues contact [email protected]. If you have any questions on the portfolio contact [email protected].

Forms The following forms are completed and submitted exclusively through the Portfolio.

• PGY1 Supervision Level Assessment • Evaluation of PGY3 and PGY4 Supervision • Individual Learning Plan (ILP) • Clinical Skills Examination (CSE) • Personal Psychotherapy • Scholarship Elective

To start a form, select the form on your homepage and follow the instructions. Forms that are completed and/or signed by someone other than you (faculty, advisor, etc) can be assigned to that person by you in your Portfolio. They will receive an automated email with a link to complete and/or sign the form. You will receive an automated email confirmation when there is a change in the status of a form (e.g. your advisor signs, your signature is needed, the form has been accepted by the Residency office).

Check My Status & Checklist To check the status of submitted or pending forms, from the homepage, go to:

To see a full checklist of forms expected to be completed each year and their status, go to Check My Status and click the “Edit” button under My Portfolio Checklist at the top of the page.

Check My Status Check My Status shows the status of pending forms. Once forms are complete, they will only show under your checklist.

Your Portfolio Checklist The checklist shows all the forms assigned to you by year and their status.

1. To access your checklist, go to Check My Status from your homepage, then click the “Edit” button under My Portfolio at the top of the page.

2. Scroll down to Resident Portfolio and you will find each form assigned by year and their status.

a. Note that the CSE and Personal Psychotherapy can be completed at any time during Residency but must be completed by the time you leave the program so they are listed under “Portfolio Forms Due During PGY-4

Individual Learning Plan The Individual Learning Plan should be completed collaboratively between the Resident and advisor.

1. To start a new ILP, go to “Individual Learning Plan” in your Portfolio.

2. Complete the form and submit it to your advisor for their review and signature.

3. After your advisor signs the form, you will receive an automated email from [email protected] with a link to review, sign and submit to the Program Director for final review. Before signing please carefully review for any changes your mentor may have made!

Clinical Skills Examination

1. To start a new CSE, either: a. go to “Clinical Skills Examination (CSE)” in your Portfolio and complete only the top

section of the form, check the box for “Are you assigning this form to an examiner?” and submit for the examiner to complete.

b. Or, your examiner may start the form through the Residency intranet. They may refer to the Faculty guide for directions.

i. Please assign the form to them yourself if they are unfamiliar with the system.

2. After the examiner completes and signs the form, you will receive an automated email from [email protected] with a link to review, sign and submit to the Program Director for final review.

Personal Psychotherapy

1. To submit your Personal Psychotherapy form, go to “Personal Psychotherapy” in your Portfolio.

2. Review the attestation, enter your name at the top of the form, then sign, date and submit to the Program Director for review.

Scholarship Elective The Scholarship Elective form should be completed collaboratively between the Resident and mentor.

1. To start a new Scholarship Elective form, go to “Scholarship Elective” in your Portfolio.

2. Complete the form and submit it to your mentor for their review and signature. Before submission please ensure the site and mentor are listed, as well as the dates of the rotation.

3. After your mentor signs the form, you will receive an automated email from [email protected] with a link to sign and submit to the Program Director for final review. Before signing, please carefully review for any changes your mentor may have made!

PGY-1 Supervision Level Assessment

1. To start a new PGY-1 Supervision Level Assessment form, either: a. go to “PGY-1 Supervision Level Assessment” in your Portfolio, complete only the top

section of the form, and submit for the evaluator to complete.

b. Or, your evaluator may start the form through the Residency intranet. They may refer to the Faculty guide for directions.

i. Please submit the form to them yourself if they are unfamiliar with the system.

1. To start a new PGY-3/PGY-4 Supervision form, either: a. go to “Evaluation of PGY/PGY4 Supervisory Capability” in your Portfolio, complete only

the top section of the form, and submit for the evaluator to complete.

b. Or, your Resident evaluator may start the form through their Portfolio. They may refer to the “Evaluation of PGY-3/PGY-4 Supervisory Capability (Evaluator)” section of this guide for directions.

2. After the evaluator completes and signs the form, you will receive an automated email from

[email protected] with a link to review, sign and submit to the Program Director for final review.

Evaluation of PGY-3/PGY-4 Supervisory Capability (Evaluator)

1. Start a new Evaluation of PGY-3/PGY-4 Supervisory Capability form by either, o The trainee may start the form in their Portfolio by assigning you as their evaluator. You

will receive an automated email from [email protected] with a link to complete and sign the form;

o Or, you can start the form by going to the form in your Portfolio and entering the trainee’s info.

2. Complete the form, then sign, date, and submit. The form will return to the resident for signature before going to the Program Director for final review.

Other Uploads The Other Uploads section can be used to store anything relevent to your training. This may include publications, presentations or awards. This is also where the Residency staff will store your log summaries and record of training.

1. To add a document, select the file with your name.

2. Then, either select the folder you would like to add a document to or select “new document” to add a document to the main folder.

3. Choose file and upload.
