_________ ____________ __ PROGRAM PROPOSAL Rebuilding Healthy Communities Recipient: Direct Relief , Santa Barbara, CA, USA Offerer: Health Futures Foundation, Inc.  (HFI), Quezon City, Philippines Subject: For the construction of barangay  health stations (BHS) and the implementation of community-based health and wellness programs (CBHWP) in three Haiyan-affected municipalities in Eastern Visayas  Areas: (1) Marabut , Samar (2010 est. pop. 15,000); (2) Quinapondan, Eastern Samar (2010 est. pop. 14,000); (3) Salcedo, Eastern Samar (2010 est. pop. 20,000) Background Super-typhoon Haiyan devastated most of the communities in Eastern Visayas, leaving thousands of people dead, millions displaced, and billions of damage in public and private infrastructures. The Samar and Eastern Samar provinces have particularly suffered due to the pre-disaster socioeconomic conditions and poor health indicators. The super-typhoon has left health systems in affected communities crippled. Health infrastructures from community BHS to large district hospitals have been flattened; supply chains of medical supplies and medicines have been disrupted; and health care providers face personal traumas and the influx of emergent post-disaster morbidities. Public health servicing have been particularly hit and in the three towns covered by this proposal, the health systems stand in a dire need of support. Health rehabilitation, while but one of the few aspects of post-disaster rebuilding and reconstruction, is paramount  if the right to health be affirmed. Goal and Objectives The Program pursues the impact objectives of zero maternal deaths; zero infant mortality; 80% or greater reduction of under-5 child mortality rates; 100% detection, sputum conversion rates, and cure of tuberculosis patients.  These impact objectives are in line with the Philippine Department of Healths Universal Health Agenda of full health security for the communities through 100% PhilHealth health insurance coverage; availability and accessibility of health professionals; delivery of public health services; utilization of digital health information systems; good health governance; and effective referral systems. The Program is also aligned with national and local initiatives for the fulfilment of the Millennium Development Goals in the Philippines, particularly its goals in reducing maternal mortality, improving child health, and eliminating tuberculosis, malaria, and other endemic diseases. Our service delivery objectives also focus on holistic wellness, health promotion, disease prevention, and identification and treatment of community members with hypertension, coronary artery disease, diabetes, and chronic renal diseases important, but often overlooked, chronic morbidities in poor communities. Services The BHS strategically serves as catchment areas, catering to the health needs of thousands of community members. Basic health services delivered by these BHS, through trained, competent, and tenured health professionals and barangay health workers include health promotion and medical care. These Program services include capacity building of health workers; maternal and child health; child nutrition; prevention and control of diseases; sexual and reproductive health; community mental health; community dental and oral health; prevention and control of non-communicable diseases; acute health care; provision of essential medicines; integration of traditional medicine with conventional medicine; and provision of basic laboratory and diagnostic services. This holistic wellness paradigm complements conventional public health concentration on curative care.
