Inpatient treatment with premixed insulin resulted in similar glycemic control but in significantly higher frequency of hypoglycemia compared with treatment with basal-bolus insulin regimen in hospitalized patients with diabetes

An insulin regimen with basal, nutritional, and correction components is the preferred treatment for patients with good nutritional intake

Change in Practice

QUESTION: ARE PATIENTS (INPATIENTS AND OUTPATIENTS) ALLOWED TO OBTAIN COPIES OF THEIR RESULTS OR MEDICAL RECORD? ANSWER: Yes, under the HIPAA Privacy Regulations, patients have the right to obtain a copy of their medical record or test results. Provide patients or patient legal representative HIPAA Form # 17; “Authorization for Use or Disclosure of PHI” prior to obtaining a copy of their medical record or test result. A valid authorization form requires all items highlighted in yellow to be completed. Forms are located under “Shared Files Folder”, ‘HIPAA Resources”. Guidelines for completing HIPAA Form # 17;

Patient: o Complete all items highlighted in yellow on the first page; o Sign and date the form

Hospital employees complete the following section; o From the first page:

Information Released By; initial and date o From the second page:

Office Use Only Information Released

o Place HIPAA Form # 17 in the patient chart (or scan directly into Power Chart or send to HIM for scanning)

State and Federal laws require covered entities to document and track all PHI that is released to patients for Non-TPO reasons. PHI = PROTECTED HEALTH INFORMATION = Patient name, room number, address, phone number, diagnosis, SS#, etc. TPO = Treatment, Payment or HealthCare Operations.
