Torrance Memorial’s Bioethics Committee Is Here to Help You

Torrance Memorial Medical Center has a Bioethics Committee that considers ethical issues arising during the care of patients and provides education to caregivers, patients and families on ethical issues in health care. The goals of the committee include responding to requests for biomedical ethics consultations for patients of Torrance Memorial Medical Center and maintaining a forum for discussion and review of consultations and bioethical concerns. Mission Statement & Goals: The Torrance Memorial Medical Center Bioethics Committee acts as a resource for the medical center employees, medical staff, volunteers, patients, families and the community regarding bioethical and patient rights issues. The Bioethics Committee meets its mission by:

Providing guidance and expertise in the area of bioethics and patient rights Assessing medical center performance regarding patient rights issues Reviewing and evaluating medical center policy and procedures that address the areas of bioethics and

patient rights Development of training programs for medical staff members, employees, physicians, volunteers and the

community Provision of a consultation program for issues or concerns in the area of bioethics or patient rights Education and communication regarding bioethical issues and patient rights for the South Bay community

Committee Membership: The committee is a multidisciplinary group that includes members of the medical staff, members of the medical center staff and administration, lay community representatives, clergy, ethicists, and attorneys.

How the Bioethics Committee Can Help: Because health care decisions can be complex and difficult to make and disagreements can arise among individuals involved in a patient’s care, the committee can help by taking into account what the law says, what our values are and how similar issues were resolved in the past. Committee members help:

Decision Makers – When family members disagree about what to do, when members of the care team have different opinions or when the patient lacks the ability to make his or her own decisions the committee acts as a consultant and facilitator but the actual decision is still left up to the patient or the patient’s legal representative or family.

Health Care Providers – Health care providers depend on policies and procedures to protect the patient’s right to decide and the committee reviews and comments on proposed new guidelines, helps keep existing policies current and identifies issues or situations that may need to be addressed in the future.

Situations for Which Committee Assistance May Be Needed: Early recognition of the following situations and early intervention by the Bioethics Committee may help to avert serious conflicts which may arise concerning a patient’s care:

Expressed disagreement regarding management of the patient among surrogates or caregivers Objection by a surrogate to an Advance Directive on file Staff, patient or surrogate request for early intervention Failure to follow a patient’s Advance Directive Absence of an end-of-life management plan in a critical patient Disagreement regarding end-of-life management between a competent patient and others who seek to speak

for the patient Unique patient or surrogate beliefs that may indicate a need for committee support or intervention Patient or surrogate expressions of uncertainty about end-of-life wishes Failure to provide follow-up information and assistance for the completion of an Advance Directive after a

request by a hospitalized patient How to Obtain Assistance from the Bioethics Committee: When you recognize the presence of one or more of the situations listed above, call the Medical Staff Services Department at 310-517-4616 to alert them. They will then notify the Chair of the Bioethics Committee or his/her designee who has the discretion to call on one or more members of the committee, or the committee as a whole, to provide consultant support, usually within 48 to 72 hours. Requests for assistance may come at any time and from any individual who is concerned about an existing or potential dilemma for a patient, including a caregiver, case manager, nursing supervisor, patient representative, volunteer or family member. If the attending physician did not initiate the referral to the Bioethics Committee, he or she will be notified by the Chair of the Bioethics Committee or a designee that a concern has been identified and referred to the committee. CURES Update

Medical Executive Committee Approvals

On January 8, 2016, the Department of Justice (DOJ) released the upgraded Controlled Substances Utilization Review and Evaluation System, also referred to as CURES 2.0. A new streamlined registration process has been implemented that allows prescribers to request and receive approval to access CURES entirely online. Existing CURES users do not need to re-register; however, you will need to confirm your account with DOJ and update security information the first time you access CURES after January 8. The Torrance Memorial Integrated Physicians ACO (Accountable Care Organization) has entered into a transformational partnership with four other health systems in Southern California and the Boeing Company to offer a customized health plan available to Boeing’s nearly 37,000 Southern California employees, retirees and their families. Additional health systems partnering in this health plan, called the MemorialCare Health Alliance, include MemorialCare Health System, UC Irvine Health, PIH Health and Monarch HealthCare. Boeing’s partnership with these health systems is the first of its kind, direct-to-employer contract in California, and one of only a few in the country. Following passage of the Affordable Care Act, new ACOs have been formed to better coordinate care for Medicare patients who have traditional Medicare coverage. On April 1, 2012 Torrance Memorial Medical Center launched its ACO, becoming one of the first in the country to set up such a program. This new partnership was developed to further expand network choices and access across the broad geography where Boeing employees live and work. Boeing chose these health systems as its trusted and exclusive ACO partners in Southern California after carefully assessing a highly competitive field of respected health systems. Because the majority of Boeing employees live throughout the greater South Bay, Long Beach and Orange County communities, areas well-served by Torrance Memorial and its partners, many Boeing employees are already accustomed to and comfortable with the network’s primary care and specialty care physicians. Since Boeing is one of the largest employers within our marketplace, this ACO contract was vigorously pur-sued in order to provide our physician partners the best opportunity to retain their existing Boeing patients and to attract new patients. This partnership puts Torrance Memorial Integrated Physicians ACO at the forefront of a transformation in our industry.

Full-Text Databases

In January 2004 Torrance Memorial's Medical Library began a subscription to EBSCOhost Web that provides access to full-text journals and allows clients to link to full-text from journal citations within National Library of Medicine's PubMed (MEDLINE) database. Most of the full-text journals covered are available in PDF format. Databases to which the Medical Library subscribes include: CINAHL Plus with Full Text is a robust collection of full text for nursing and allied health journals, providing full

text for more than 760 journals indexed in CINAHL. This authoritative file contains full text for many of the most used journals in the CINAHL index - with no embargo. Content covers nursing, biomedicine, health sciences librarianship, alternative/complementary medicine, consumer health and 17 allied health disciplines. CINAHL Plus with Full Text is the core research tool for all areas of nursing and allied health literature. Full text coverage dates back to 1937.

Cochrane Database of Systematic Reviews contains full text articles, as well as protocols focusing on the effects of health care. The reviews are highly structured and systematic, with evidence included or excluded on the basis of explicit quality criteria, to minimize bias. Data for the more than 4,800 records is evidence-based medicine and is often combined statistically (with meta-analysis) to increase the power of the findings of numerous studies, each too small to produce reliable results individually.

Health Business Elite provides comprehensive journal content detailing all aspects of health care administration and other non-clinical aspects of health care institution management. Topics covered include hospital management, hospital administration, marketing, human resources, computer technology, facilities management and insurance. The database contains full text content for more than 570 journals.

MEDLINE Complete provides authoritative information on general health and medicine, nursing, pharmacology, dentistry, genetics & genomics, veterinary medicine, the health care system, pre-clinical sciences, and much more. It is the world's most comprehensive source of full text for medical journals, providing full text for over 2,000 journals indexed in MEDLINE. This wide-ranging file contains full text for many of the most used journals in the MEDLINE index - with no embargo. With coverage dating back to 1809 and full-text back to 1865, it is the definitive research tool for medical literature.

Nursing & Allied Health Collection: Comprehensive is designed for nursing and allied health professionals, students, educators and researchers. This database provides full text for nearly 400 journals, including nearly 300 peer-reviewed titles, covering the areas of nursing, biomedicine, health sciences, consumer health and allied health disciplines. Nearly all full text titles included are indexed in CINAHL. Full text information dates as far back as 1975.

Psychology and Behavioral Sciences Collection is a comprehensive database covering information concerning topics in emotional and behavioral characteristics, psychiatry & psychology, mental processes, anthropology and observational & experimental methods. It also provides particularly strong coverage in child and adolescent psychology and various areas of counseling. This is the world's largest full text psychology database offering full text coverage for more than 530 journals.

HIPAA Tip

QUESTION: MY FRIEND IS A PATIENT AT TMMC. CAN I LOOK UP HER ROOM NUMBER OR LOCATION ON MY COMPUTER SYSTEM (CERNER)? I WILL NOT ACCESS ANY CLINICAL INFORMATION, JUST THE ROOM NUMBER SO I CAN VISIT HER.

ANSWER: NO, it is a HIPAA Privacy violation and violation of hospital policy. Accessing, looking up or viewing any patient information for personal reasons (Non-TPO) is a HIPAA violation regardless of your relationship with the patient (friend, family or co-worker). When planning to visit a patient (friend, family or co-worker) you will need to check with the Information Desk in our hospital lobby for the location and/or patient room number. You will need the patient full name (first and last name) to obtain the information. We are required to protect the confidentiality and privacy of all patients. Accessing any patient information including the location of a patient for personal reasons is a violation of the patient’s privacy. State and Federal Regulations mandate covered entities (TMMC) to protect patient PHI.

Protecting PHI is everyone’s job; PHI is NOT everyone’s business.
