    Using va rious HIE use c ases suc h a s those from the Dep a rtment of De fense (DoD) a nd theSoc ial Sec urity Ad ministrat ion (SSA), a p ane l of sta keho lde rs from various sub -do mains w illexplore in d eta il the issues of privac y and sec urity for HIEs includ ing the NHIN. The p anelmembers will share their perspectives on the primary legal agreements governingpa rticipa tion in HIEs, the role of p rivac y officers, c onsume r emp ow erment in ma nag ingthe d istribution of d a ta , and the spe c ific te c hnologies availab le in the CONNECT solutionto supp ort the p rivac y officer and the consumer.

    The p anel w ill also d iscuss p rivacy, sec urity and c onsent in the c ontext of sec ond a ry usesof data, and explore the considerations for developing a National Infrastructure for

    Translationa l Resea rch. The resea rc h d isc ussion w ill be b ased on a mo del architec ture(DARTNet) tha t m ee ts the basic d esign a nd use case sc ena rios as out lined by the NHIN,including standardizing data storage, addressing patient privacy concerns, using de-centralized databases, enabling strong institutional autonomy concerning data sharing,allowing va riable levels of d a ta acc ess, and de fining a self-sc a ling architec ture.

It is a volunta ryresearch association of primary care physicians who collaborate on studies to describe and improveupon the phenom enon of p rima ry ca re p rac tice . The AAFP NRN assists in rea lizing o vera ll strateg ies forac hieving improved prima ry c a re fo r the na tion.     NHIN An Overview

    The Nat ionw ide Hea lth Informa tion Network (NHIN) is a set of sta nd ards, services and policies tha t

    ena b le sec ure he alth informa tion excha nge over the Internet. The NHIN will provide a founda tion for theexchange o f hea lth IT ac ross d iverse entities, within comm unities and ac ross the count ry, helping toac hieve the g oa ls of the HITECH Ac t. This c ritica l pa rt of the na tiona l hea lth IT agend a w ill enab le hea lth

    information to follow the consumer, be available for clinical decision making, and support appropriateuse of hea lthca re informa tion b eyond direc t p atient c are so a s to imp rove po pulation health.

    The NHIN Work Group , pa rt of the Hea lth IT Policy Co mm ittee , is c urrent ly deve lop ing rec om mend ationsfor extending the secure exchange of health information using NHIN standards, services and policies tothe b roa dest a udienc e po ssible. Activities of the NHIN Work Group and Health IT Polic y Co mm ittee c anbe found at http://healthit.hhs.gov/policycommittee.

    A g roup of fed eral ag enc ies, loc al, reg ional and sta te-leve l Hea lth Informa tion Exchang e Orga nizat ions(HIOs) and integrated delivery networks, formerly known as the NHIN Cooperative, has been helping todeve lop the NHIN sta nda rds, services and policies. Tod ay, these o rga niza tions a re dem onstrating livehea lth informa tion excha nge throug h the NHIN Limited Prod uc tion Exc hange . By the end of 2010, it isexpec ted that app roxima tely a do zen e ntities will be sec urely sharing live he alth informa tion as pa rt of

    this Excha nge.

    Based on initial rec om me nd ations from the NHIN Work Group , a ne w initiative , the NHIN Direc t Projec t, isbeing launched to explore the NHIN standards and services required to enable secure healthinformat ion excha nge at a mo re loc al a nd less com plex level, such a s a p rimary ca re p rovide r send inga referral or care summary to a local specialist electronically.     Moving forward, the NHIN will continue evolving to meet emerging needs for exchanging electronichea lth informa tion sec urely over the Inte rnet. This evo lution will be d riven by em erging tec hno logy,users, uses, and policies.

In this role he is c harge d with building an interop erab le, p riva te a nd sec urena tionwide hea lth informa tion system and supp orting the w idespread , mea ningful use of hea lth IT.

    CONNECT An Overview

    The C ONNECT solution w as built b y fed eral agenc ies in response to their need to sha re he a lth da ta

    among themselves and with other levels of government and the private sector using the NationwideHea lth Informa tion Net wo rk (NHIN).

    Rather than have each federal agency independently build its own NHIN-compliant gateway solution,they b and ed tog ethe r through the Fed eral Hea lth Arc hitect ure (an E-Gov initiative) to b uild CO NNECT.The CONNECT projec t tea m b roug ht tog ethe r more tha n 20 fed eral agenc ies to d efine project needs, itdeveloped the solution, demonstrated its viability for connecting federal and non-federal healthorganizat ions, and it ma de the solution a va ilab le to the public in less than a yea r.

    Dr. Sreedhar Potarazu, MD MBA, will present an overview on the promise of Health Information Exchangefor facilitating administrative efficiencies, clinical innovation, and payment reform, and explore howtha t vision is beg inning t o b e rea lized today. Dr. Pota razu will d isc uss how HIE can ena b le pa yers,providers and even employers to see not only improvements in the quality and efficiency of caredelivery, but the development of consumer-directed programs and services that are beginning totransform the w ay peo ple think ab out their hea lthc are.

    Dr. Sreedhar Potarazu, MD MBACEO of VitalSpring Tec hno log ies

    Dr. Sreedhar Pota razu is an ac c laimed op htha lmologist a nd entrepreneur rec og nized as aninternationa l visiona ry in the business of m ed icine. He is the found er and CEO of Vita lSpringTec hnologies Inc ., a privat ely held enterprise softwa re c ompany foc used on p roviding e mp loyers withap plic ations to em po we r them to b ec ome more sop histic ated purc hasers of hea lth c are.

    Dr. Pota razu rec eived his me d ica l deg ree from The Geo rge Washington University and subseq uentlycompleted his internship in surgery and residency there as well. He completed fellowships inneuroophthalmology and glaucoma at the esteemed Bascom Palmer Eye Institute at the University ofMiami in Florida .

    Dr. Potarazu practiced seven years in the Washington, D.C. area, during which time he was on thefaculty of the Wilmer Eye Institute at Johns Hopkins University. While practicing, he earned an MBA atJohns Hopkins w ith a foc us on he a lth informa tion techno logy. It was his thesis tha t ultima tely served asthe basis for the o rig ina l business p lan o f Vita lSp ring . Dr. Pota razu is w ide ly rec og nized as an industrylea de r in hea lth c are and healthca re tec hnolog y and is a sought-after spe aker on the top ic .

    He currently resides in Maryland with his wife and two daughters. Dr. Potarazu is also involved in thepromotion of fine arts and education to the underprivileged. He is the author of a well-publicized book Ge t O ff The Dime The Sec ret of Cha ng ing Who Pays For Your Health Ca re. Dr. Pota razu, has raisedove r 30 million d ollars for VitalSpring, w ithout t he help of any institutiona l or venture c ap ital investors.     Dr. Steffensen rec eived his me d ica l degree a t the University of Oklahoma Hea lth Sc ienc es Center. Hecompleted an internship in Internal Medicine at the National Naval Medical Center in Bethesda wherehe a lso received his reside nc y training in c linica l Neurolog y. Following residenc y, Dr. Steffensen was thecommand Neurologist at Naval Hospital Pensacola where he was the sole provider for returningwo unde d wa rriors with trauma tic brain injury.

    Using va rious HIE use c ases suc h a s those from the Dep artment of Defense (DoD) a nd the Soc ial Sec urityAd ministrat ion (SSA), a p ane l of sta keho lde rs from various sub -dom a ins w ill explore in deta il the issues ofp rivac y and sec urity for HIEs includ ing the NHIN. The panel mem bers w ill sha re their pe rspec tives on the

    primary legal agreements governing participation in HIEs, the role of privacy officers, consumerempowerment in managing the distribution of data, and the specific technologies available in theCO NNECT solution to support the p rivac y officer and the c onsume r.

    The p anel w ill a lso d isc uss p rivac y, sec urity and c onsent in the c ont ext of sec ond ary uses of d a ta , andexp lore the c onsidera tions for develop ing a Nat iona l Infrastructure fo r Translationa l Resea rch. Theresea rc h discussion will be based on a mo del architec ture (DARTNet) tha t meets the b asic d esign anduse case scenarios as outlined by the NHIN, including standardizing data storage, addressing patientprivacy concerns, using decentralized databases, enabling strong institutional autonomy concerningda ta sha ring, allow ing variab le levels of d a ta acc ess, and de fining a self-sc a ling architec ture.

    An accomplished investigator, Dr. Paces research has focused on practice reorganization, practice-based research methodology, electronic data collection techniques and patient safety. He serves asthe p rincipa l investigat or, c o-investigat or, or in an a dvisory ca pa c ity to a numb er of hea lth informa tiontechnology, behavioral change, and disease specific projects funded by the National Institutes ofHea lth (NIH), the Ag enc y for Hea lthcare Resea rch and Q ua lity (AHRQ) and seve ra l found a tions. Dr.Pace s most rec ent p rop osa l to the NIH for Transforma tive Resea rch is foc used on d eve lop ing a ndimplementing a national infrastructure for translational research.

    Dr. Pace is the principal architect for a network of EHR enabled primary care practices, the DistributedAmbulato ry Resea rc h in Therap eutic s Netw ork (DARTNet) using both tec hnolog y and sha red lea rning tohelp the m transform tow ards a Patient C entered Medic a l Hom e (PCM H). DARTNet is a federatednetwork - a collection of databases that reside in multiple member practices and that are linked

    Dr. Russler is vice p resident of Clinic a l Informa tics for Orac le. His c urrent role a t O rac le inc ludes c rea tingstrategies for healthcare standards, healthcare training, and new product development in healthcareintellige nc e a nd hea lthca re informa tion excha nge . Dr Russler also p layed a key role in the d evelop mentof the HL7 Reference Informa tion M od el for HL7 Version 3 a nd is past mem ber and Trea surer of t heBoard of HL7. He gives frequent tutorials on HL7 requirements definition and modeling and the use ofHL7 standards in the IHE document sharing architecture. Most recently, he has been contributing to thede velopme nt of tec hnica l spe c ifica tions for the NHIN.

    Previously, Dr Russler joined Mc Kesson (then HBOC ) in late 1995 and bec ame vice p resident o f c linica ltechnology, specializing in clinical data repositories and J2EE portal development in healthcare. During

    this period , he w as c o-cha ir of the HL7 Patient Care TC, which introd uc ed the HL7 Ca re Prov isionDomain along with Care Record Messages and models for problems, allergies, care plans, goals, and

    CCD do c uments. While c o-ed itor of the IHE Pa tient Ca re Coo rdinat ion TC, the first IHE CDA d oc ume ntspe c ific ations were introd uce d , and the C DA tem plateID mode l wa s invented.

    Prior to McKesson, he focused on primary care including private family medicine, hospice medicine,long-term care medicine and acute care medicine. Dr Russler held various administrative positionsranging from clinical to information systems, including associate administrator of hospital medical clinicsand informa tion system s.

    Dr Russler earned bachelor and medical degrees from the University of Wisconsin in Madison, WI, andtrained in family prac tice a t the University of Wisc onsin Depa rtme nt of Family Medic ine and Prac tice.
