RESEARCH ACTIVITY
My aim is to undertake clinical and basic science research at a level of international excellence.
I am almost unique in the UK as a clinical academic who combines a large volume clinical
practice involving more than 1000 operations per year, with renowned research in the fields of hip
and knee implant and joint preservation surgery.
My area of research has high value for patients, healthcare providers and industry because the
implantation of orthopaedic devices is increasingly exponentially (in 2012, >1 million hip and 1
million knee replacements were performed worldwide and 80,000 of each in the UK). There is
considerable commercialisation potential in the field of better functioning and longer lasting hip
replacements over the next 20 years. The global market for hip and knee replacement was worth
$11.4 billion dollars in 2008, and this is estimated to increase by 673% for knee and 173% for hip
implants by 2030. Revision surgery is becoming more common and more complex and can be
minimised through better joint preserving surgery at a young age, good implant design,
appropriate surgery and patient selection. My research will continue to be aimed at deferring the
need for joint replacement, optimising function after joint arthroplasty, increasing the longevity of
orthopaedic implants, evaluating the outcomes of patients with these implants, and reducing the
number of failure that require revision surgery. This will benefit our patients and the UK
economy.
I have spent the last 15 years setting up a variety of outcome modules and outcome studies after
ACL reconstruction, total hip and total knee replacement as well as revision surgery at University
College London Hospitals and the Princess Grace Hospital.
I have set up collaborations with anesthetic, ITU and neuropsychology colleagues. At present
there are studies ongoing into risk assessment and risk scoring in joint arthroplasty patients, into
expectations both in disease specific situations and in relation to surgical intervention for sports
knee reconstructions as well as arthroplasties and revisions, into quality of life prior and after
surgery and functional measures both in primary and revision joint replacement.
I have instituted a programme of data collection within integrated care pathways, data bases and
studies that have allowed us to collate and follow-up a number of cohorts in order to study sports
injuries, knee and hip reconstructions and joint replacement outcomes and we are in the process of
evaluating psychometric data, inflammatory markers and functional tests in the early post-
operative period to correlate with medium term and longer term outcomes.
I have published on the use of the POMS scoring system looking at peri-operative morbidity and
its impact on the economics of hospital admission and hospital stay.
I am now in a position to look at quality of life measures and functional measures in relation to
primary and revision joint replacement and compare these.
Professor Fares S Haddad Page 7 I have treated over 150 patients with peri-prosthetic infections which have been very carefully
recorded and data based and have been treated according to a standardised protocol which will be
ready for publication within the next year. This has included a prospective randomised study of
static versus mobile spacers for infected TKR. I have introduced an algorithm for single stage
revision for infection that has been adopted internationally.
I have set up research focused on knee joint preservation. We have selected cohorts for over ten
years of clinical details, imaging, operative details and scores on patients with acute knee injuries
and their subsequent treatment, including meniscal repair, meniscal resection, meniscal
replacement, ligament reconstruction and osteotomies. I now have a large set of cohorts that we
are starting to analyse in an effort to understand knee joint preservation and, ultimately, the impact
of sports’ injuries, and the associated surgery on subsequent osteo-arthritis. These cohorts and
their analysis, in collaboration with other centres, will form part of the ARUK national centre for
sport and osteo-arthritis.
I have set up a system for embedded data collection at the ISEH whereby all patients coming
through will have body composition analysis, provide basic demographics and all the exercise
equipment and testing equipment will collect data so that we can analyse changes over time.
I have used research funds obtained to set up an RSA imaging system to look at the musculo-
skeletal system before and after surgery and evaluate new implants. I have also set up an imaging
modality within the ISEH that allows us to under fluoroscopy and understand joint kinematics
better than we have done in the past. We have also set up a biomechanics laboratory in ISEH that
will allow us to evaluate outcomes functionally as well as by questionnaire and PROMS.
The system that I have set up collates cohorts of certain phenotypes with solid datasets for future
interrogation. I have developed a unique referral practice for pelvic avulsion injuries and a large
referral practice for injuries and early osteo-arthritis of the hip and knee to specialist clinics and
will allow the analysis of these cohorts and associated data sets in order to determine significant
biomarkers for healing, functional recovery and disease progression, and to help us to clarify
appropriate interventions for these problems.
I fund and supervise eight research Fellows at any one time , each of whom has their own project
within my research theme, which will continue into higher degrees or be passed on to the next set
of Fellows. These include:
The evaluation of failing implants, including metal on metal hips, corrosion, trunnion
failure and the evaluation and introduction of innovative prostheses.
The use of stem cells and coatings to enhance implant osteo-integration with particular
reference to complex revision surgery and amputation prostheses.
The prevention of biofilm formation and infection in orthopaedic surgery with linkage to a
unique and massive clinical experience in the management of infected patients. I am also
leading a subgroup in the first international consensus meeting which will be held in
Philadelphia this year to look at the global impact of orthopaedic infections.
Evaluation after any ligament injury, analysing large cohorts of injured knees, innovative
ways of measuring outcomes after ACL surgery and providing the basis for analysis for a
national knee ligament registry.
Professor Fares S Haddad Page 8 Methods of evaluating new implants as they are introduced into the market, particularly for
younger patients. I have developed novel scoring systems that do not have the ceiling
effect. We are now using treadmills that allow uphill and downhill walking and simulated
stair-climbing to look at outcome in a more critical way and differentiate for active
patients. Linked into RSA methodologies, this should allow us to have greater clarity
about how close we are to achieving normal function for our arthroplasty patients.
Peri-operative inflammation after orthopaedic interventions and its impact on short and
long outcome. I have two Fellows looking at the inflammatory response to orthopaedic
surgery, and the factors that can influence it.
My unit has been at the forefront of the introduction of hip preservation surgery. I have a
large collection of joint preservation procedures and have piloted novel software to look at
the impact collision at the time of impingement. The analysis of this cohort in association
with colleagues elsewhere in the sport and osteo-arthritis centre will help us to define the
role of joint preservation surgery.
I have a team analysing national joint registry data in orthopaedics, looking at outcomes in
relation to variables such as case mix complexity, BMI and length of surgery.
I run a number of prospective randomized studies comparing different implant types and
implant philosophies.

I compiled and edited a book on orthopaedic trauma for undergraduates and junior doctors
that was published in 2008 and has a wide circulation.
I was chairman of the undergraduate cross-campus UCL Orthopaedics and Rheumatology
Module until 2012.
I introduced very successful special study modules in Orthopaedics and Sports at UCH /
UCL
I am programme director for the region including day to day management of 20
Orthopaedic ST3 trainees and above.
I am Associate Director of Education at UCLP and am responsible for Surgical Specialties,
I teach on regional and national hip, knee and sports courses
I am the tutor and organiser of the Sports MSc and Performing Arts MSc at UCL
I chair and run the yearly London Hip and London Knee Meetings that have >400
delegates
I am on the Editorial Board of Bone & Joint Journal, J Arthroplasty, Annals RCS Eng,

I run an MSc in Sport & Exercise Medicine and Health which has grown every year and now
includes distal learning.
I oversee the only MSc in Performing Arts in the UK

OTHER ENABLING
I developed a theatre model that was adopted as the gold standard by the UCH McKinsey review.
The average orthopaedic operating day typically comprised 4 major cases; by using 2 operating
theatres, a fast turnaround and cohesive working, I undertake 10-12 major cases including a large
number of complex tertiary referrals. This provides an excellent training model for juniors who
see large patient numbers and do more as their experience increases.
I have generated grants/bursaries for 8 research fellows at any one time, a research physiotherapist
and a research secretary.
I have used personally generated grants and industry funds to appoint research fellows who
augment our clinical service at UCH - we have more than doubled throughput in my clinics and
dealt with the reduction in junior hours.
I have developed an outcomes research programme for the evaluation of functional outcomes in
hip / knee surgery particularly in active patients. I run multicentre studies of hip and knee
replacement outcomes including the development and evaluation of modern techniques such as
navigation, resurfacing and novel bearing surfaces. I have led the introduction of PROMS in
Orthopaedics at UCH.
Professor Fares S Haddad Page 48 I developed the London Hip Meeting in 2001 and chair it yearly. It has had > 400 delegates since
2010. The London Knee Meeting has followed a similar pattern. I am an invited speaker at many
international meetings on the basis of recent research.
I have developed interdisciplinary meetings with microbiology and radiology.
I have embedded outcomes measures in daily practice such (eg ICPs) to improve data collection
I have designed a number of modern implants such that UCH's profile in Hip and Knee surgery
has been raised.
I put in the proposals for an Institute of Sports, Exercise and Health at UCH and gained
Department of Health Funding - £10Million – for the ISEH at 170 Tottenham Court Road.
I run basic science studies in collaboration with the bioengineering unit at the Institute of
Orthopaedics and the Institute of Human Performance.
I run multicentre studies of hip and knee replacement outcomes, including the development and
evaluation of modern techniques such as navigation, resurfacing and modern bearing surfaces.
I developed a novel cable system for periprosthetic fractures.
I have a new revision hip stem that started trials in 2009, a new knee replacement that started
production in mid 2010, and a new hip system that is currently in clinical trials. I have generated a
large database of infected joint replacements and periprosthetic fractures that allows for regular
presentations and publications and put me at the forefront of these areas.
I am an invited speaker at many international meetings on the basis of recent research. This
includes presentations at the prestigious American Academy and Current Concepts Meetings.
