The Segway brand name is on the charger above these wires. The battery packs were also sold separately. The black battery packs measure about 8 inches long, 2 inches wide and about 0.8 inches high. Model number CP556150-1 including all serial numbers, and model number CP556150-2 with serial number range Z120102 through Z120512 are included in this recall. The model and serial numbers are printed on the white battery label. The notebook computer’s model number is printed on a label on the bottom of the notebook.

Fujitsu has received three reports of the battery packs catching fire, including two in Japan and one in China and causing fire damage to rugs, bedding, a desk and other furniture. No injuries have been reported.

Consumers should immediately turn off their Fujitsu notebook computer, remove the battery pack and contact Fujitsu for a free replacement battery pack. Consumers can continue to use their Fujitsu notebook computer without the battery pack by plugging in the AC adapter and power cord.

The recall involves Zak Designs 26-ounce plastic water bottles. The 10 inch tall water bottles have a flip-top spout on a twist-off cap with colored inner plastic straws in clear, blue, gray, green, light purple or red. The water bottles were sold exclusively at Target stores in a “Back-to-College” promotion. The water bottles have popular characters on the front and come in a variety of colors including clear, gray, navy blue, pink, teal blue, gray with colored caps in red, blue pink and yellow. Characters include Captain America, a Batman design, Minions, My Little Pony, Spiderman, Star Wars aircraft, a Superman logo, Teenage Mutant Ninja Turtles, Thor and Wonder Woman. Mold number “14158” and “Zak Designs®” are embossed on the bottom of the bottles in the recall. Bottles with a black inner straw and black twist-off cap are not included in the recall.

DESCRIPTION

WASHINGTON, D.C. – The U.S. Consumer Product Safety Commission (CPSC) and Bed Handles Inc., of Blue Springs, Mo., are reannouncing the voluntary recall of about 113,000 adult portable bed handles. When attached to an adult’s bed without the use of safety retention straps, a handle can shift out of place creating a dangerous gap between the bed handle and the side of the mattress. This poses a serious risk of entrapment, strangulation and death.

Three women died after becoming entrapped between the mattress and the bed handles. They include an elderly woman, age unknown, who died in an Edina, Minn. assisted living facility; a 41-year-old disabled woman who died in a Renton, Wash. adult family home; and an 81-year-old woman who died in a Vancouver, Wash. managed care facility. No additional deaths of injuries have been reported.

The recall involves adult portable bed handles sold by Bed Handles, Inc. from 1994 through 2007 that do not have safety retention straps to secure the bed handle to the bed frame to keep the bed handle from shifting out of place and creating a dangerous gap. The Original Bedside Assistant® (BA10W) and the Travel Handles™ (BA11W) have a white handle with white poles that go under the mattress. The Adjustable Bedside Assistant® (AJ1) is gold in color and has a black cushioned foam handle. The bed handles are intended to assist adults with getting in and out of bed by giving them a bar to grip. Bed Handles, Inc. and the model number are printed on a white label on the bed handles.

They were sold by home health care stores, drug stores and medical equipment stores nationwide and in home and health care catalogs from January 1994 through December 2007 for about $100. They were manufactured in the United States.

HAZARD:

In one incident, an adult male suffered a broken finger, a wrist injury, a shoulder injury and abrasions. No injuries were reported in the other two incidents.

The pacifier clips have eight multi-colored beads threaded through a narrow satin ribbon that attaches to a pacifier on one end and a plastic D-ring on the other. The clip and beads are about six inches long. The garment clip has the following number code on the back of the plastic clip: 3/31/14, 08/08/14, 12/01/14, 14/30/09, 15/02/09, 25/04/14.
