■ Simplify Treatment With the Inclusive Tooth Replacement System, there are no missing components. For the day of surgery, each package features an Inclusive® Tapered Implant of the prescribed length and diameter, a prosthetic stent to help guide the fixture’s location from a restorative perspective, and brand new case-specific drills with which to prepare the osteotomy. This makes for an easy tray setup for the surgical staff, and reduces preparation time. Once the implant is seated, the same package contains both a custom temporary res-toration and a custom healing abutment, facilitating immediate non-functional provisionalization or a more conservative healing option, as indicated. The temporary can be cemented or screw-retained, with either choice more efficient than fabricating a custom provisional chairside. The remainder of the Inclusive Tooth Replacement System package can be delivered to the restorative doctor, who will use the custom impres-sion coping to capture the complete gingival anatomy post healing, and submit a prescription for the final custom abutment and crown — at no extra cost.

Clinical Workflow

STEP 1: Following a complete diagnostic evaluation, submit full-arch impressions and bite registration to the lab, along with an Inclusive Tooth Replacement System Rx denoting tooth num-ber, shade, soft tissue depth, and the desired implant size.

STEP 2: With the case information, the lab will fabricate a pros-thetic stent for use during implant placement, along with a cus-tom healing abutment, a custom temporary abutment, and a BioTemps provisional crown. A matching custom impression coping will also be included.

STEP 3: For the day of surgery, you will receive in a special case box the implant, surgical drills, and prosthetic guide. Following implant placement, deliver the custom healing abutment or im-mediately temporize with the custom temporary abutment and BioTemps provisional crown.

STEP 4: Upon completion of the healing phase, simply remove the custom healing abutment (or custom temporary) and place the custom impression coping to precisely capture the gingival anatomy with the impression.

STEP 5: From your final impression, the lab’s CAD/CAM techni-cians will design and produce an Inclusive Custom Abutment and the definitive BruxZir Solid Zirconia or IPS e.max crown for final delivery.

STEP 6: Seat the final custom abutment and crown. The precise nature of the digital design process should minimize the need for chairside adjustment. The patient is now ready to enjoy a new smile!

DescriptionThe Inclusive Screw-Retained Hybrid Denture is a proven replacement for re-movable prostheses worn by edentulous patients, offering excellent predictability, esthetics, and function. A custom acrylic framework is fabricated on your mas-ter model work and then scanned. The scanned data of the framework and the individual implant platforms are trans-ferred to a design station where the one-piece bar framework is finalized. After design, it is milled from titanium for a pre-cise, passive fit. This bar is incorporated into the acrylic processing of the denture tooth setup.

Clinical Workflow

STEP 1: Preliminary Impression

Take preliminary impression of edentulous arch.

STEP 2: Jaw Relation Records and Shade Selection

Obtain jaw relation records. Take impressions of opposing dentition and current denture. Select shade and mold of denture teeth.

STEP 3: Denture Setup Try-in

Try-in denture setup. Evaluate CR, VDO, occlusion, esthetics and pho-netics. Alter setup as needed. Document with photos.

STEP 4: Implant Verification Jig and Final Impression

Seat implant verification jig sections and lute together. Take final im-pression.

STEP 5: Implant Bar/Final Setup Try-in

Seat and verify fit of the milled implant bar. Seat final setup over the bar. Verify CR, VDO, occlusion, esthetics and phonetics.

Limited Warranty—Prismatik Dentalcraft, Inc.—Inclusive® Dental ImplantsPrismatik Dentalcraft, Inc. (“Prismatik”), is the manufacturer of Inclusive implants and related Inclusive brand dental abutments. Prismatik and Glidewell Direct hereinafter are referred to collectively as “Glidewell.” For a period from the original purchase date of seven (7) years, Glidewell will at its option replace or refund to the original purchaser (“user”) the purchase price of any Inclusive implant that is returned due to defects in material and manufacture. Glidewell will replace without charge any Inclusive implant that fails to osseointegrate, or fails following osseointegration, for a period of seven (7) years after initial placement. Glidewell will also replace the prosthetic abutments and/or attachments placed on the Inclusive implant at the time of failure, according to the war-ranty applicable at the time for patient-specific lab-fabricated custom dental devices (see, i.e., the current Rx for said lab), which in pertinent provides as follows:

Crowns, Bridges, and Dentures

Glidewell will repair or replace implant restorations fabricated by Glidewell Laboratories without charge for the cost of materials and workmanship, or refund the original price paid, at the lab’s option, as follows:

• Screw-retained titanium abutments (excluding abutments with angulations greater than 20 degrees), up to twenty (20) years.

• Porcelain-to-metal, all-porcelain, all-metal (excluding mutually opposing implant-supported full-arch bridges), single-unit resin composite crowns, implant bars, and screw-retained zirconia abutments (excluding abutments with angulations greater than 20 degrees), up to seven (7) years. Transition Crowns and Bridges® up to two (2) years.

• Dentures and partials, including screw-retained dentures but excluding immediate dentures and partials, up to one year if the failure is due to defects in materials or workmanship.

Inclusive Restorative Components on Non-Inclusive ImplantsIf an Inclusive abutment is placed on a non-Inclusive implant and the implant and/or restoration fails to osseointegrate or fails fol-lowing osseointegration, Glidewell will replace without charge any component needed for retreatment for a period of seven (7) years. The non-Inclusive implant will be replaced with an Inclusive implant, if needed.

Limitations on Warranty ProgramLaboratory restorations not fabricated by Glidewell Laboratories are not covered by this warranty. Glidewell shall not be liable for any incidental or consequential damages, whether foreseeable or not, caused by defects in the product or dental devices produced using said product. User is responsible for determining the suitability of the product for user’s application. If this product is defective within the warranty period, user’s exclusive remedy and Glidewell’s sole obligation shall be replacement or refund of the purchase price of the product. For replacement or refund under this warranty, the original purchaser shall send the product at its own ex-pense, postage prepaid, to Glidewell Direct, 18651 Von Karman Ave., Irvine, CA 92612. THIS WARRANTY IS EXCLUSIVE, AND IS IN LIEU OF ANY OTHER WARRANTIES, EXPRESS OR IMPLIED, INCLUDING BUT NOT LIMITED TO WARRANTY OF MERCHANT-ABILITY OR WARRANTY OF FITNESS FOR A PARTICULAR PURPOSE. Glidewell’s sole obligation and the buyer’s sole remedy for any breach of warranty shall be as set forth above. The buyer agrees that no other remedy (including, but not limited to, incidental or consequential damages for lost profits, lost sales, injury to person or property, or any other incidental or consequential loss) shall be available.
