PROCEDURES AND INFORMATION NEEDED TO PROCESS APPLICATION OF MODIFICATION/REPAIR FOR UNFIRED PRESSURE VESSEL.

A. Documents needed:

1. Application letter.

2. Reasons for modification/repair of unfired pressure vessel

3. 3 sets of technical drawings (as built drawing)

4. 2 sets of procedures of repair/modification

5. 1 set of design calculation of modification/repair area

6. 2 set of approved welding procedure specification (wps)

7. Material certificate.

8. PMT certificate and a copy of and/or approval letter

9. Latest inspection report on the vessel

B. Contents of application letter:

1. Name and address of company

2. Name and telephone no. of contact person

3. Reference letter and date of application

4. Subject of application (modification/repair)

5. Signature

C. Contents of repair/modification procedure.

1. Detail procedure of repair/modification

2. Attach together with sketch of modification/repair.

3. Non destructive test involved.

4. Any hydrostatic test must be witness by DOSH Inspector.

5. List of approved welders.

6. Name of approved repairer by DOSH
