the chamberlain group designs, develops, and

manufactures accurate anatomy

for surgical and interventional

training, research, device

development, sales and marketing.

• Pioneers in mimetic tissue

• Suppliers to teaching hospitals,

regional hospitals, medical device

manufacturers, pharmaceutical

companies, and researchers in over 50

countries

• Applications for cardiothoracic,

vascular, GI, reproductive, urological,

pulmonary & general surgical training

and research

• Custom integrated design,

prototyping, and manufacturing

• Physical models built directly from

patient CT & MRI datasets.

• Proprietary polymer formulations

look, feel and respond like living

tissue -- and can be standardized.

• A greater acceptance of mimetic

tissue as a result of the better

tissues we produced.

• Product line spans all levels:

Simple suturing to complex

procedural instruction, starting with

medical school & residency and

continuing for the practitioner’s entire

career.

• Working with clinicians to meet the

clinical objectives of training

our capabilities

Surgical Sam is the world’s first beating

heart, breathing and bleeding high

fidelity team trainer for pediatric

surgery.

• Developed with Boston Children’s Hospital

• Promotes pediatric surgical team training

• Allows the critical steps of surgery to unfold

in the clinical environment

• The full OR team fully immerse in simulation

• Optimizes performance, safety and outcomes

• Pressurizable, cannulatable, suturable

• Lifelike tissue enhances the immersive

experience

development of Surgical Sam

• Began in 2010

• Developed to provide Boston Children’s

with a simulated patient mannequin for

insurer-supported team training

• Designed in 3D CAD

• Able to share images of Sam before we

ever built him

Our work, by its nature, is iterative,

building on our earlier developments

for our latest creative endeavor.

Prior reference projects for Sam’s

development included:

• Transoral Trainer (EGS)

• Fetus in Utero (University of W. Ontario)

• Perfusion Beating Heart (German Heart

Center Berlin DHZB)

Challenges included making lifelike infant

tissue both appropriately strong and delicate.

Surgical Sam is modular.

• “Chassis” designed with CT and general surgery modules

• CT module complete, GS module in final prototype stage

• Potential to develop additional pathologies and scenarios

appropriate for team training or procedure simulation: septal

defects, Tetralogy of Fallot, ECMO cannulation

• Simpler corollary products may be used in conjunction for skills

acquisition, i.e. pediatric bowel, small vessels, etc.

what’s coming in pediatric

simulation?

• Developing sufficiently lifelike

mannequins and tissue for immersive

realism

• Validating the experience

• Getting buy-in from clinicians and

educators

• Proliferating the technology so that

it’s accessible

• Building curricula around its use

• Pursuing its deployment as a tool for

high stakes assessment

FULL SCALE, POINT OF CARE

SIMULATION TRAINING FOR PEDIATRIC

CARDIAC

SURGERY OPERATING ROOM TEAMS

• in situ OR team training • ECMO team training in

ICU and cath lab • ECMO skills training

BCH/CRICO OR Team Training:

Approach/Aspects

• In-situ Simulation in BCH Operating Room • Benefits

– Engaging

– Realism: “Feels Natural”

– Full Native Teams

– Systems Probing

• Limitations

– Cost

3 scenarios:

Case #1: ICU, postop bleeding patient

focusing on communication of critical information prior to

operative intervention

Case #2/Part 1: In OR, unstable bleeding patient emergent reexploration and cannulation for

cardiopulmonary bypass. Involves all team members,

highlights checklist.

Case #2/Part 2, acute oxygenator failure on bypass

– Focus on critical language with surgeon and

anesthesia.

– Bypass circuit designed to be able to simulate

numerous parameters.

Dedicated CT trainer

• native heart beat

• Ability to cannulate

• Simulate bleeding

• Transition to cardiopulmonary bypass
