– An individual who joins the eHR programme in order to share his / her health record to other parties via the eHR sharing system

Healthcare Recipient – Index

• Index

– Contains a list of personal identity and demographic data of Healthcare Recipients for

• Accurate and unique identification of each Healthcare Recipient

• Linkage of eHR record

Healthcare Recipient – Data Quality

• Data Quality

– Enrolment of a Healthcare Recipient, or change of a Healthcare Recipient’s identity data must be supported by identity document

– Clinical data sent to the eHR would be rejected if the identity data of Healthcare Recipient do not match with those in the eHR Healthcare Recipient Index

Healthcare Recipient Data

• eHR Healthcare Recipient data includes:

– A system generated eHR Healthcare Recipient identifier – ‘eHR Number’

– Identity data: HKIC number, name, sex, date of birth, identity document number (according to one’s identity document)

– Demographic data: e.g. address, phone number

– Substitute Decision Maker (SDM) and contact person information

– Birth data: e.g. birth hospital and birth identifier

– A pre-defined set of PMI data that will be subject to sharing in the eHR system
